    Registration is the FIRST step for a family

    desiring Early Head Start/Head Start services.

    From registration we obtain students and

    families to meet and maintain our required

    funded enrollment.

    Application- the primary from providing information

    needed to determine eligibility

    Completed Application- all information has been

    provided to determine eligibility

    Family Count- number of family members living in the

    home

    Family Member- all persons living in the

    same household who are

    (a) supported by the income of the

    parent/guardian of the child enrolling

    or participating in the program AND

    (b) related to the parent/guardian by blood

    Homeless- children lacking fixed, regular, and

    adequate nighttime residence,

    Determine childs age

    For Head Start the child must be at least 3

    years of age on September1, 2011

    For Early Head Start the child must be under

    the age of 3 on September 1, 2011

    Determine Childs Status

    If new to the program, family must sign-in andcomplete entire application

    If currently a FULL-TIME ESE student, familymust sign-in and complete the entireapplication process

    If a returning student (currently in programand age eligible for next year) DONOT sign-in.

    Have parent(s) join you at interview table

    Explain purpose of interview:To clarify information in application packet and to

    identify any other information that may be needed

    Review documents copies

    Return to parents all originals EXCEPTimmunization and physical forms.

    Review language survey form. Be sure it iscompleted, signed, and dated.

    Review all other forms. Be sure they arecompleted, signed, and dated by the parentand you if applicable.

    If incomplete, have parent finish form with

    you.

    Any entries you make on the form(s) should

    be in red ink with your initials and the date.

    You are to complete gray areas.

    You are to be sure family provided information oneducation level, employment status (FT, PT, etc.),occupation, and relationship of household members toparent.

    Use comments section to clarify information, make notes ifnecessary.

    Make sure that the applicant and YOU sign and date theapplication form.

    Be sure to complete the Request for Information formlisting ALL information still needed.

    Section I: Identifying Information

    Need full name of parent/guardian with marital

    status.

    Need complete LIVING and MAILING addresses

    with zip codes.

    Need phone numbers all you can get.

    Need the information requested on form for each

    of the household members

    In gray area below each members name indicate

    application status:

    NA for non-applicant child and adult

    Appropriate number of symbol for EHS/HS, etc. foreach child applying (or pregnant woman applying)

    Program site in which they reside( TRJ, BES, ETC.)

    If the childs birth certificate lists a father and

    that father is living in the home, he is a family

    member regardless of marital status to the

    childs mother.
