Registration is the FIRST step for a family
desiring Early Head Start/Head Start services.
From registration we obtain students and
families to meet and maintain our required
funded enrollment.

For Head Start the child must be at least 3

For Early Head Start the child must be under

If new to the program, family must sign-in andcomplete entire application
If currently a FULL-TIME ESE student, familymust sign-in and complete the entireapplication process
If a returning student (currently in programand age eligible for next year) DONOT sign-in.

You are to complete gray areas.
You are to be sure family provided information oneducation level, employment status (FT, PT, etc.),occupation, and relationship of household members toparent.
Use comments section to clarify information, make notes ifnecessary.
Make sure that the applicant and YOU sign and date theapplication form.

Need phone numbers all you can get.

member regardless of marital status to the
childs mother.
