1.1 PROJECT INFORMATION
A. Notice to Bidders: Qualified bidders are invited to submit bids for Project as described in this Document according to the Instructions to Bidders.
B. Project Identification: St. Michael’s School Boiler and HVAC Replacement. 1. Project Location: 310 E 2nd St, Pinconning, MI 48650.

E. Project Description: Project consists of removal of existing steam boiler and heat exchanger, installation of new heating hot water boiler, control valves and controls for the existing hot water unit ventilators, and (2) forced air rooftop units to serve the 1950’s era wing.
F. Construction Contract: Bids will be received for the following Work:
1. General Contract (all trades).
1.2 BID SUBMITTAL AND OPENING
A. Owner will receive sealed bids until the bid time and date at the location indicated below. Owner will consider bids prepared in compliance with the Instructions to Bidders issued by Owner, and delivered as follows:

B. Bids will be thereafter privately opened.
1.3 BID SECURITY
A. Bid security shall be submitted with each bid in the amount of 5 percent of the bid amount. No bids may be withdrawn for a period of 60 days after opening of bids. Owner reserves the right to reject any and all bids and to waive informalities and irregularities.
1.4 PREBID CONFERENCE
A. A prebid conference for all bidders will be held at St. Michael School on May 27, 2016 at 10:00 a.m., local time. Prospective bidders are required to attend.

1.5 DOCUMENTS
A. Online Procurement and Contracting Documents: Obtain access after May 25, 2016 by
contacting Architect. Online access will be provided to prime bidders only.
1.6 TIME OF COMPLETION
A. Bidders shall begin the Work on receipt of the Notice to Proceed and shall complete the Work
within the Contract Time. All work shall be inspected and completed prior to August 29,2016.
1.7 BIDDER'S QUALIFICATIONS
A. Bidders must be prequalified by Owner.
B. Bidders must be properly licensed under the laws governing their respective trades and be able
to obtain insurance and bonds required for the Work. A Performance Bond, a separate Labor
and Material Payment Bond, and Insurance in a form acceptable to Owner will be required of

1.1 INSTRUCTIONS TO BIDDERS
A. Instructions to Bidders for Project consist of the following:

requirements of the Instructions to Bidders.
1.2 ARTICLE 2 - BIDDER'S REPRESENTATIONS
A. Add Section 2.1.3.1:

requirements of authorities having jurisdiction and has properly included in the submitted

1.4 ARTICLE 4 - BIDDING PROCEDURES
A. 4.1 - Preparation of Bids:

a. 4.1.9 - Owner may elect to disqualify a bid due to failure to submit a bid in the
form requested, failure to bid requested alternates or unit prices, failure to
complete entries in all blanks in the Bid Form, or inclusion by the Bidder of any
alternates, conditions, limitations or provisions not called for.
4. Add Section 4.1.10:
a. 4.1.10 – Holy Trinity Parish is a tax exempt entity. There should be no sales tax.
B. 4.3 - Submission of Bids:

modifications or withdrawal of the Bid.
b. 4.4.2.2 - Owner will consider modifications to a bid written on the sealed bid
envelope by authorized persons when such modifications comply with the
following: the modification is indicated by a percent or stated amount to be added
to or deducted from the Bid; the amount of the Bid itself is not made known by the
modification; a signature of the authorized person, along with the time and date of
the modification, accompanies the modification. Completion of an unsealed bid
form, awaiting final figures from the Bidder, does not require power of attorney
due to the evidenced authorization of the Bidder implied by the circumstance of
the completion and delivery of the Bid.
D. 4.5 - Break-Out Pricing Bid Supplement:

1.5 ARTICLE 5 - CONSIDERATION OF BIDS
A. 5.2 - Rejection of Bids:

a. 5.2.1 - Owner reserves the right to reject a bid based on Owner's and Architect's
evaluation of qualification information submitted following opening of bids.
Owner's evaluation of the Bidder's qualifications will include: status of licensure
and record of compliance with licensing requirements, record of quality of
completed work, record of Project completion and ability to complete, record of
financial management including financial resources available to complete Project
and record of timely payment of obligations, record of Project site management
including compliance with requirements of authorities having jurisdiction, record
of and number of current claims and disputes and the status of their resolution, and
qualifications of the Bidder's proposed Project staff and proposed subcontractors.

prescribed Form of Agreement is presented to the Awardee for signature, the Awardee
shall execute and deliver the Agreement to Owner through Architect, in such number of
counterparts as Owner may require.

2. 9.1.2 - Owner may deem as a default the failure of the Awardee to execute the Contract
and to supply the required bonds when the Agreement is presented for signature within

executed Agreement, the date of commencement of the Work shall be the date of the
executed Agreement or the date that the Bidder is obligated to deliver the executed
Agreement and required bonds to Owner.
4. 9.1.4 - In the event of a default, Owner may declare the amount of the Bid security
forfeited and elect to either award the Contract to the next responsible bidder or re-
advertise for bids.

Dear Mr. Grzegorczyk,
TEK Environmental & Consulting Services, Inc. (TEK) was contracted by Wimsatt Building Materials to
conduct limited asbestos bulk sampling and material identification from within St. Michael’s Church
(School), located at 225 S. Jennings Street, Pinconning, Michigan. The sample collection was conducted
in an effort to determine the presence or absence of asbestos containing materials within the subject
property prior to any restoration/reconstruction activities. Mr. Tyler Lenling of TEK collected the samples
on March 4, 2016.
Six (6) bulk samples (including layers) of the suspect asbestos containing materials were collected on
March 4, 2016, for the analysis of asbestos content. The samples were submitted to APEX Research,
Inc., a 3rd Party analytical laboratory for Standard analysis and analyzed by Polarized Light Microscopy
(PLM) Method EPA 600/R-93/116 for asbestos content.
Laboratory results indicate that asbestos was detected in the materials sampled during the limited
inspection procedures. A material that is positive for asbestos means that asbestos was present in the material
in the amount of greater than 1% by weight. Please refer to the table below for sample location and
laboratory results. A copy of the laboratory analysis report is attached for your records.

ASBESTOS REGULATORY REQUIREMENTS
The following list highlights the primary federal regulations governing the asbestos abatement industry.
OSHA 29 CFR 1926.1101 Construction Industry Standard for Asbestos - Covers asbestos
exposure in work involving: (a) demolition or salvage of structures where asbestos is present, (b)
removal or encapsulation of materials containing asbestos, (c) construction, alteration, repair,
maintenance, or renovation of structures, substrates, or portions thereof, that contain asbestos, (d)
disposal, storage, transportation, containment of and housekeeping activities associated with asbestos
or products containing asbestos.
OSHA 29 CFR 1910.134 General Industry Standard for Respiratory Protection – Purpose proper
use, fitting instructions, maintenance, care and limitations of respirators. Requires development of
standard operating procedures and a written respirator program.
EPA ASHARA Model Accreditation Plan (MAP) - Expands TSCA Section 206, specifically the
training and accreditation requirements, to persons performing asbestos work in public and
commercial buildings. Applicable to anyone who inspects for ACM, designs or conducts a response
action with respect to friable ACM.
Department of Transportation (DOT) 49 CFR 171-178 - Includes requirements for classification of
materials, packaging, hazard communication (package marking, labeling, place carding, and shipping
documentation), transportation, handling and incident reporting.

removal methods, procedures to be used to comply with the requirements of the regulation, and

CONCLUSIONS AND SURVEY LIMITATIONS
TEK recommends that any removal of the confirmed asbestos containing Spray Textured Coating on
Plaster Wire Lathe be performed by a State of Michigan licensed and trained asbestos abatement
contractor. Future renovation activities may expose unidentified materials. If a newly identified suspect
material is found, the material should be sampled prior to disturbance or assumed asbestos containing and
not disturbed until proper remediation can be performed if not previously sampled and proven asbestos
containing.
TEK inspected the property as thoroughly as possible without impacting the exterior envelope of the
building systems. TEK has attempted to investigate the existing conditions within the subject property
using standard professional procedures and industrial hygiene practices. This regulated building materials
sampling is intended to identify materials associated with planned renovation processes. Regardless of
the thoroughness of any survey, it is possible that some materials were inaccessible during inspection
