He wasevenfeaturedin the Livonia Observer once for growing a 350-pound pumpkin in his backyard. Unfortunately, his con- gestive heart failure has impeded hisability to garden. In April, DeMatteiswas treated at St. Mary Mer- cy Hospital in Livonia forchestpainandintro- duced to Cathy Ponder, RN. Ponderserves as the full-timein-houseTran- sition Coach through the new Care Transitions Program.Together, they identifiedgardening as an achievable, person- algoal forDeMatteisand mapped outstepsto get him back to doing what he loves. “Throughcoach- ing, we help thepatient identify theirgoals, and theytakeownership,” Ponder explained. “This in turn increasestheir confidence and comfort levelsandthey’llbegin to takecareof themselves.” Most goals aresimple: Beableto walktotheir mailbox.Plant flowers. Go camping. Dance at the senior center. PlayBin- go. Eat a slice of apple pie with icecream. With 56 percent of patients over the age of 65, St. Mary Mercy hasembracedthe pro- gramdesigned to encour- age vulnerable older patients and theircare- giversto asserta more activeroleduringcare transitions. Theprogram is targeted at a popula- tion of patients identified to beat increased risk of experiencing readmis- sion tothehospitalwith- in 30 days.
