The transition to clerkships is a five-week experience designed to introduce andprepare third-year students for clinical rotations prior to the commencement of longitudinalintegratedclerkships.Thiscourseemphasizesthestrengtheningofcompetenciesinhistory-taking,physicalexaminationskills,problemdifferentiation,diseasepreventionandmanagement, aswellaswritten communication andoral presentation skills – including, but not limited to, presentinginformation on rounds, writing notes and orders, team leadership, and communicating withcolleagues,patients,andpeers.Inapplyingthesebasicskills,studentsdevelopanunderstandingoftheimportanceofcomprehensive,patient-centered,andlongitudinalcare.Studentsspendoneafternoonperweekininteractivedidacticsessions,“ClerkshipSchool,”insmallgroups, focusing on common patient problems, communication, developing clinical skills andthoughtprocessesinvolvedinevaluatingpatients.
§ Strengthenphysicalexamandproceduralskills.§ Apply basic concepts learned in the Foundations Phase to further critical thinking skills,
particularlywithregardtodataanalysisanddifferentialdiagnosis.§ Perform and refine communication and presentation skills, patient interview techniques,
physical examination and history-taking skills in preparation for active clerkshipexperiences, includingclinical reasoningandother factors that facilitateunderstandingofclinicalhealthcaresystems(system-basedpractices).
§ Develop reflective practices and abilities to communicate with colleagues, peer, andpatients.
§ Fosterpersonalaccountability,altruism,humanism,self-awareness,aswellasself-directedandlife-longlearning.
Upon completion of the Transitions to Clerkship course, students begin longitudinal integratedclerkships. Longitudinal integrated clerkships (LIC) are central components of the clinicaleducationphaseofthecurriculum.Studentsparticipateinthecomprehensivecareofpatientsandengageincontinuouslearningrelationshipswithpatientsandtheirattendingcliniciansovertime, all the while acquiring core clinical competencies across multiple disciplines: anesthesia, emergencymedicine, familymedicine, internalmedicine,neurology/neurosurgery, obstetrics andgynecology, pediatrics, psychiatry, and surgery. LIC’s utilize the continuity of relationshipswithpatients, faculty preceptors, and settings over an extended period of time, combined with alongitudinal developmental pedagogy, to foster an environment of caring and learning,responsibility,andaprofessionalsenseofduty.Studentsgainfirst-handperspectivesofproviders’decisionsandroles,aswellasthestructureandfunctionofhealthcaredeliverysystems.LICsalsoprovide students with exposure to socioeconomic factors that may impact patient’s access oraversion to care within the context of the community, and the influence of care continuity onimprovedpatientsatisfaction, decreasedhospitalizations, emergencyroomvisits, andpreventativeservices.

§ Performanddocumentacompleteandfocusedmedicalhistory,physicalexam,andmentalstatusassessmentinavarietyofmedicalspecialties.
§ Demonstrate the ability to interpret common laboratory tests, such as: blood and urinetests, electrocardiogram, andvarious imagingmodalities, and formulate findingsgatheredfrompatients’historiesandphysicalsintomeaningfuldiagnosticinformation.
§ Manage competing clinical priorities and navigate complex health care systems as amemberofaninterprofessionalteam.
§ Retrieve,analyze,andsynthesizerelevantandcurrentdataandliteratureusinginformationtechnologiesandlibraryresourcestoproposeclinicaldecisionsbasedonresearchevidence,clinicalexpertise,andpatientvalues.
§ Developcross-disciplinaryand interprofessional communicationskillsessential topatientcarethroughoutallspecialties.
§ Communicate effectively with patients, their families, interprofessional teams, andcommunitystakeholdersthroughverbal,non-verbal,andwrittenmeansofcommunication,respectingthedifferencesinbeliefsandbackgroundsamongpatientsandstudents.
