GENERAL INFORMATION Southern School of Beauty, Inc. is located at 140 West Main in Durant, Oklahoma 74701. Owned and operated by Dale White. Licensed by Oklahoma State Board of Cosmetology, 2401 NW 23rd Street, Suite 84, Oklahoma City, Oklahoma 73107. PH. (405) 521-2441. Accredited by the National Accrediting Commission of Career Arts and Science. NACCAS is located at 4401 Ford Avenue, Suite 1300, Alexandria, VA 22302 PH. (703) 600-7600. www.naccas.org Approved by the Choctaw Nation of Oklahoma for the training of students in Cosmetology. Approved by the Vocational Rehabilitation Department to train students in Cosmetology. The school is located in the center of town surrounded by an array of businesses including fashion shops, general merchandise, antiques shops, restaurants etc. The school currently consists of 2500 square feet floor space and is comfortably air conditioned and heated. The school is well vented and adequately lighted. Free parking is provided for all students. MISSION STATEMENT: To train students to be highly skilled technicians in the field of Cosmetology, Arts & Sciences. Train students to be knowledgeable of business management and to include the professional ethics and attitude needed to be successful. To be aware of all the avenues that is available to the student in the field of Cosmetology. To educate highly skilled professionals in cosmetology and related fields sufficient to meet the needs of area salons and professional facilities. To provide opportunities for continuing education for both the student of cosmetology and the professional seeking to expand their educational goals. To provide consumer services performed by skilled students.

DESCRIPTION OF THE INSTITUTIONS GENERAL FACILITIES AND EQUIPMENT: Classroom - The classroom will accommodate 25 students at any one time. It includes TV, VCR, and a variety of educational tapes, blackboard, slides, and a library that is well stocked.

Clinic Floor - Has 15 styling stations, 15 hydraulic chairs, 8 hair dryers, 4 shampoo bowls and chairs, and 4 manicure tables.

Facial Area - contains 1 facial chairs and adequate storage space for up to date facial supplies.

Lounge Area - Contains tables and chairs to seat students for lunch or break time. A microwave and refrigerator are available for student use.

Restrooms - Facilities are available for both men and women.

Offices - General administration, counseling, bookkeeping and enrolling of students.

Dispensary Room - Equipped with sink supplied with hot and cold running water, cabinets containing necessary supplies for clinic floor.

EDUCATIONAL OBJECTIVE Southern School of Beauty Inc. is devoted to the training of competent Cosmetologist so they may enter into and be successful in the Cosmetology Profession. Students will be taught all basic knowledge of Cosmetology in preparation for the Oklahoma State Board Examination, which will earn a license to work in the state of Oklahoma. Southern School of Beauty Inc. goes beyond simply ensuring that graduates are able to pass their licensing exam. We strive to make certain each student is prepared for success and professional growth in the work force. This means: 1. That the student develops the skills necessary for technical proficiency. 2. That the student has a thorough knowledge of the biology of hair and skin, and the chemical processes used in Cosmetology. 3. That the student has an understanding of business management, professional ethics and attitudes needed to succeed. As part of our educational objectives, Southern School of Beauty Inc. is ready to assist students with job placements.

ADMISSION REQUIREMENTS Individuals interested in attending the Southern School of Beauty, Inc. are invited to tour the school. All potential students must have a personal interview with the Director. Applicants must be seventeen years of age or older and of good moral character. The school admits as regular student(s), (1) high school graduates, and (2) holders of high school graduation equivalency certificate. Students must provide proof of education, age and other

required documentation. Students must be at least 17. , beyond the age of compulsory attendance

in the State. The school’s admissions policy does not admit students under the Ability to Benefit

Criteria

Southern School of Beauty does not recruit students already attending or admitted to another school offering a similar program of study New classes are scheduled to begin every Tuesday of each month.

PROCEDURES: Admission’s personnel or an assignee guides each student through the entire facility as a means of familiarizing them with class room accommodations, equipment, facilities, introduces them to faculty noted in the school’s organizational chart as well as students etc. The introductory tour is intended to put everyone as ease and demonstrate a healthy learning environment. Admission requirement documents are reviewed and where applicable documents are copied and become part of the student’s permanent file. All admission documents must be provided prior to a student becoming enrolled.

GRADUATION REQUIREMENTS

Students must complete all required hours specific to the course identified in the enrollment agreement, met all financial obligations according to enrollment agreement(in the case of mitigating circumstances, other options, terms, fee waiver etc., may be exercised) and have completed all class project assignments and completed all with a passing score. Upon successfully completing all requirements that are stated in enrollment agreement and catalog and have an overall average of 70 % students are awarded a certificate of completion.

CREDIT FOR PREVIOUS PROFESSIONAL HOURS

Students with accumulative hours from other schools will be accepted, when those hours are approved by the Oklahoma State Board of Cosmetology. The student's record will reflect the full credit as allowed by the Board. 5

COUNSELING AND PLACEMENT

Counseling is provided for students during the entire enrollment period to assist them in obtaining and achieving their educational goals. Efforts are made by the school to place students in desirable positions upon graduation. If desired, the school will seek to schedule interviews with salons in the area of the city where the students desires to work. Placement Assistance is provided, but employment cannot be guaranteed. Students are informed that depending on the chosen salon in this area that compensation as a percentage average is 50 to 60 and space rental is 40 to 50 dollars weekly. PHYSICAL DEMANDS

There are both mental and physical demands required in these professions. Some of the health hazards are: A. Standing for long periods of time B. Inhalation of products C. Irritation through skin absorption D. Sitting for long periods as a manicurist To prevent these health hazards from becoming serious you will be taught safety and sanitation measures throughout the program as follows:

SAFETY MEASURES Students are encouraged to wear protective gloves while performing chemical services. Students are encouraged to wear professional supportive shoes and hosiery. Students are encouraged to wear eye and face protection while using nail equipment that might harm the eyes. After each student has completed their required hours, they are then required to take both a written and practical portion of the State Board of Cosmetology examination.

RIGHT TO PRIVACY To insure the accuracy and confidentiality of student records: 1. Information about an individual is released only on written instruction of the student or parent/guardian if the student is a dependent minor. The information will be provided within 5 working days. 2. Student information is kept in separate locked files. This information is available to the respective student, qualified school personnel and authorized agencies. 3. Students and their parent, if the student is a dependent minor, are guaranteed access to their cumulative records upon written request. A staff person must be present during the review, and interpretation of student records. Original records may not be removed from the school. Copies of documents are available for .15 cents per page of copy. 4. Complete records for each student are maintained showing amounts of money paid by students, the amount due from each tuition, materials and services. 5. These records of all payments made by the students are protected from theft, tampering, fire or other disasters.

RECORD RETENTION All records are maintained for a period of 3 years. 6

INSTRUCTIONAL METHODS

Southern School of Beauty Inc. has designed its program and its curriculum to achieve basic methods and techniques. It is anticipated however that the students in this program achieve the proficiency in the art and practice of Cosmetology. Students who follow sound training principles afford them the opportunity to assist with on-hand experiences until the levels of competencies are met and the criteria for such instructions with them judged, graded, and recorded. Instructional methods and curriculum are broken down to individual time periods allotted to allow instructors ample time to deliver proper techniques in practices and sufficient time for theory and testing in order for students to move comfortable from one subject matter to the next, providing all criteria are met in that subject.

INSTRUCTION AND MATERIAL

A major part of the student’s education is devoted to learning and mastering essential basic skills for treating the face, scalp, hair, and nails. In performing these operations the student will also learn the importance of clear, orderly, and systematic habits of workmanship and good student habits. Our role is to insist continually of high standards of accomplishment. Excellence of method and instructional materials as well as on-hand supervised experience itself in the selection of materials on hand at all times for each and every student regardless of handicap, if any. All updated journals, text, self-help books, and audio-visuals are cataloged for immediate usage by any student at anytime. The more we stress the use of materials, the more effective the instruction will be and will definitely attain the specific objectives and desirable outcome that the student so desires to achieve at that point. Genuine interest in supplying good materials to the students will further act as stimulus for much greater student interest and school activity.

APPLYING FOR FINANCIAL AID

All students must: 1. Complete the Free Application for Federal Aid (Pell Grant) www.fafsa.com 2. Complete the Verification process, if required, within 90 days. 3. Supply to the institution the students/spouse and/or parents tax returns or a non-tax Filing statement. 4. Supply any additional documentation to the institution that is requested in order to determine aid. When all of the requested documentation has been completed and processed, the institution will determine the amount of assistance available to the students. Need is determined by an assessment of the students/spouse/parent income and assets. The Expected Family Contribution is determined through the Federal Needs Analysis System.

METHOD OF DETERMINING AVAILABLE FUNDS The school through an automated system currently uses EDExpress software to process applications for federal funds. Analysis is performed for the student while they wait providing each student with an EFC noting the amount of expected funds a student may expect to receive.

STUDENTS FINANCIAL ASSISTANCE The student is notified of his/her expected funds by the Financial Administrator. The student is further apprised of the following: A. Date, amount hourly requirements for each disbursement B. How funds will be credited C. Satisfactory progress must be maintained as defined by the School D. A brief description of the program

FEDERAL PELL GRANT Students must complete an ED approved financial aid application, upon completion the data is entered by the financial aid administrator and processed electronically. Pell Grant eligibility is determined by an assessment of the student/spouse/parents income and assets. The result is called the Expected Family Contribution (EFC) and schools use this figure to determine the award. Congress determines the maximum amount that may be disbursed for a given year. This is a grant and does not need to be repaid.

For more information on financial aid see Mr. Dale White or Mrs. Kim White in the school office. The school office is open Tuesday through Friday from 8:30am to 5:00pm.

ATTENDANCE, HOURS AND ABSENCES State Board requires that you be present to receive academic credit. You will not be given credit for the hours you are not present. You will be charged $5.00 per hour for any additional training hours beyond contract ending date. School hours are from 8:30am to 5:00pm Tuesday through Friday. There will be one hour lunch break Tuesday through Friday. Saturday 8:30 am to 2:30pm with no lunch. Students must sign in on an official sign in sheet provided by the school. Any student arriving more than ten (10) minutes late for school will not be allowed to attend theory class and will not sign in until theory class has finished. Students are expected to call before class begins if they are unable to attend school or will not be on time. Continued tardiness or improper notifications of absence from school my result in suspension and/or termination. The state law allows 8 hours training daily. Hours are totaled by the month. A copy of these hours may be reviewed upon written request by student or authorized persons at any time.

SCHOOL CALENDAR - LEGAL HOLIDAYS The school is open 12 months a year. School holidays are: New Year's Day, Memorial Day, 4

July, Labor Day, Thanksgiving Day, and two days for Christmas. School may also be closed one day per year for Teacher's Training.

EMERGENCY CLOSURE The school will try to notify students by telephone, local television and/or radio should it be necessary to close during an emergency.

POLICY REGARDING CHARACTER AND CONDUCT All applicants must measure up to high standards regarding character and integrity. Good morals are required and students will be asked to leave if his/her conduct is unbecoming or in discord with the rules of the school. We try to maintain a friendly atmosphere; therefore we will not tolerate gossip, foul language or trouble-makers. 8

CAMPUS SECURITY The following provides information required under Public law, 34 CFR 668. Student Assistance General Provision, Campus Security. This report is to inform you of the procedures to follow in reporting emergencies and crimes on campus. The Chief Executive Officer, all Instructors, and Staff will serve as Campus Security Personnel. Any and all suspicious activities, crimes, and other security problems should be reported immediately to an available Staff Member. Written reports will be made when appropriate and kept in the school files. When it is deemed, necessary by School Personnel, outside help, law enforcement etc., will be called in. School Officials will cooperate in ever way possible with any investigation conducted by these entities. Please note that School Policy prohibits use, sale or possession of illegal drugs on School property or at school functions, alcohol violations or drinking by underage persons will not be tolerated. DRUG POLICY To ensure a DRUG-FREE school, all students/employees are given notice that the unlawful manufacture, distribution, dispensing, possession or use of a controlled substance is prohibited in the school. If a student/employee is found to be in non-compliance with this notice, said student/employee will be terminated immediately. This school is also a smoke free facility. ANTI-DRUG ABUSE ACT CERTIFICATION MARCH 18, 1989 The student understands that as a condition of eligibility to attend this school, and as a condition of eligibility to receive Financial Aid, he/she must remain drug free. The student also understands that as a condition of eligibility to attend this School if he/she does engage in the unlawful manufacturing, distribution, dispensation, or possession of a controlled substance during their enrollment, he/she will be dismissed from school and not be allowed to return. The student has been informed of the penalties for the use of drugs. The students have received written information on the Federal penalties and section for illegal possession of a controlled substance. The students have received information on the use and effects of a controlled substance. The student understands that if he/she needs help while attending school for the abuse of a controlled substance, they may go to the owner or director for help without recourse. The school will assist the student in getting the counseling they need. 9

SATISFACTORY PROGRESS POLICY This policy is the same for all enrollees enrolled in the same program, without regard to whether the student receives Title IV assistance or funds the program by another means. In order to be considered to be making satisfactory progress towards a diploma, a student must maintain specified grade averages as well as proceed through the program at a pace leading to completion within the maximum time frame within 150% of the program. For purposes of monitoring satisfactory progress, each program is broken down into segments. Students receive a numeric grade in both theory, clinic work (practical practices), and attendance. Grades will be monitored (evaluated) at the end of each period called segment (ie. one month), and the student advised if their overall progress is falling behind. Specifically, factors which will be evaluated for academic progress are practical skills, hands on and written and theory test grades, practical practice assignment sheets, and your attendance average. An overall academic average of 70 percent is required for graduation. The following represents the equivalencies of the grades assigned. A.............................90-100 B.............................80-89 C.............................70-79

Below 70 is failing Students are permitted to take any failed exam over once with the second grade substituting for the first even if it is lower. Students who do not achieve a 67 cumulative attendance average or who's cumulative attendance falls behind a pace leading to completion within the maximum time frame established by the Department of Ed. (ie. 1.5 x contract period) will be placed on probation for a period of one month. The student will be considered to be making satisfactory progress while on probation and may receive any financial aid due. Failure to raise the average by the end of the probationary period will mean the student is no longer considered to be maintaining satisfactory progress and any Federal Student Financial Aid funds will be terminated and the student is subject to suspension or termination. However, students may make written appeal for one extra probationary period if they can demonstrate that the causes of the previous poor performance have been eliminated and that their performance during the probationary period showed improvement. Title IV Funds will be reinstated immediately upon demonstration by the student of having met the standards for making satisfactory progress. In order for a student to be considered to be making satisfactory progress as of the midpoint of the course, they must meet both the attendance and academic progress requirements on at least one evaluation by the midpoint of the program.

PROGRESS REPORTS: (ALL PROGRAMS) Attendance (based on cumulative attendance percentage) and academic progress are evaluated monthly and are graded as part of the 500 and 1000 hour progress reports (330 and 660 hour progress report for the Instructor's program). Factors evaluated for academic progress are - Theory Test grades, Work Book grades, Practical Practice sheets, Practical Skills and Attendance average.

APPEAL PROGRESS The student may appeal the determination of satisfactory progress and the termination or suspension of any assistance to the Director based upon extenuating circumstance. The appeal must be in written form and will be reviewed and considered by 2 staff/administrator persons. The student will be notified in writing within 5 working days of the decision made by the school administrators. The decision is final and may not be appealed. In such cases, the Director (& staff) may determine that the student is making satisfactory progress towards his/her diploma despite the failure to conform within the time frame or minimum grade averages. Students who cannot maintain satisfactory progress on a full time schedule may need to consider a part time schedule based upon previous pattern of attendance. 10

RE-ADMISSION POLICY / RE-ENTERING Students may reapply to re-admit to the institution after being dismissed after waiting a period of three months. Such students may be enrolled on a probationary status. With respect to financial aid, if ay, a student must re-establish satisfactory progress within 30 days, maintaining a minimum 70% average and meet attendance requirements for that period before financial aid will be re-instated. This procedure applies only to dismissals caused by lack of satisfactory progress and will never be granted more than once. It does not apply to voluntary withdrawals. Temporary interruptions in training (such as leave of absence, temporary suspension, etc.) have no affect on a student's satisfactory progress upon return (ie. your satisfactory progress evaluation remains the same as when you left). Note: Program incompletes, repetitions, and non-credit remedial programs do not apply to this school and have no affect upon the satisfactory progress standards. Re-entering students will be charged at the current tuition rates for newly entering students. Amounts paid during their first period of enrollment will be credited to the account. If the student re-enters within twelve months of the withdrawal, the enrollment fee will be waived.

SATISFACTORY PROGRESS PROCEDURES:

Implementation of SAP practices and procedures is a task jointly shared by teaching staff and other administrative personnel. Records of satisfactory progress are compiled by the immediate teaching staff; evaluation of compiled data is reviewed and assessed by other personnel in accordance with the school’s policy to ascertain satisfactory progress and or failure to achieve satisfactory progress. Notification to the student of his/her SAP status is the responsibility of the immediate instructor, in cases where a student fails to achieve satisfactory progress a member of the administrative staff assist in the process to ascertain that appropriate remedial actions are adopted and implemented.

GRADING Test grade records are kept on each student and become part of the permanent Student Transcript. Students are graded on written (theory), practical work, and attendance. Practical project sheets are assigned to determine progress and advancement of the student in the techniques and skills of cosmetology in their clinic work. The student is given a grade based on the number of projects (clinic practices) satisfactorily completed. Seventy percent is passing on both the written and practical scores. Students are counseled, as necessary regarding grades, attitude and appearance and will receive a written progress report after completing 1/3 of their program hours and again after two thirds completion. The student's final overall average is the cumulative average of their theory test grades, work book grades, practical practice grades, and attendance. PROGRAM FORMAT The program format would include but not be limited to reading assignments, video/audio, lecture, and hands on demonstration, evaluations, and written and oral assignments.

PROGRAM OBJECTIVES AND PERFORMANCE OBJECTIVES Upon completion of the 1500 Clock Hour Program, the student shall have acquired the technical abilities and the academic theories in compliance with the requirements of the Oklahoma State Board of Cosmetology. REFUND POLICY Southern School of Beauty Inc. defines enrollment time as the elapsed between the actual start date and the last physical date of attendance by the student. Southern School of Beauty Inc. maintains a cancellation and settlement policy that provides a refund of all or some money paid by the student if: * If the student cancels in writing not later than midnight of the third business day after date signed on the contract by the perspective student regardless if student has actually started training, school will provide a full refund minus the application fee. Cancellation date is determined by postmarked date on written notification or hand delivered date to school office Saturday, Sunday and schools holidays are not considered business days. * If the contracted enrollment of the student was procured as a result of a misrepresentation made in the advertising of promotional materials of the school or by the representative of the school. * IF an applicant is not accepted for enrollment by the school. The school will provide a full refund minus the application fee. If the student withdraws from the course of training or is terminated from the course before completion of our return of funds policy, school will provide a full refund minus $50.00 application fee. If payment is collected before beginning but after cancellation period 3 business days student does not begin the school will not retain full portion of the money paid for, however school will retain $50.00 application fee. After 3 days school will retain $100.00 enrollment fee as well as the $50.00 application fee. * If students who enroll and begin classes, the schedule to tuition adjustments will apply for all students according to the current Title IV department of education return of funds policy and the institutional. Both instituional and Title IV tuition Adjustments are required for refund calculations for students who receive Title IV funds. * If any monies due to a student withdrawing, terminated from or not returning from being absence shall be paid no later than the 30th day of the official withdrawal or cancellation date.

* If when a student withdraws for an appropriate reason unrelated to the students academic states the student receives a grade of incomplete and may re-enroll in the above said course during the 48 month period following the date the student withdrew and complete remaining semester study units and hours. * If in case illness or disabling accident, death in the immediately family, or circumstances beyond the students control the school will consider settlement for mitigating circumstances. * If the school will make a good faith effort to return the funds but if unable to locate the student the school shall provide upon request documentation of the effort to locate the student. * If the school closes, the school and Oklahoma State Board of Cosmetology shall attempt to arrange for students of the closed school to attend another private beauty school. If the student cannot be placed in another school the student’s tuition shall be pro-rata refund. It is in the intent of the instruction to always be in compliance with local state and federal regulations. * If it should it become necessary to make changes in the future it will be in the form of an addendum to be added to this contract. Changes will only reflect terminology as used in Federal Regulations. Changes would have no effect on fees stated in the agreement. This contract represents changes in refund terminology of the Department of Education, October 7, 2000. Cancellation of the course subsequent to a student's enrollment shall (1) provide a full refund of all monies paid, or (2) provide completion of course. * Payment refunds are applied as followed: Loan Fees, Pell Funds second, Private funds third. * A student on a leave of absence notifies the school that he or she will not be returning. The date of withdrawal shall be the earlier of the date of expiration of the leave of absence or the date the student notifies the institution that the student will not be returning. * Any monies due a student who unofficially withdraws from the institution shall be refunded within 45 days if a determination by the institution that the student has withdrawn without notifying the institution. To determine unofficial withdrawal, the school monitors student’s attendance monthly.

Collection Policy: Failure to make payments when due constitutes a breach of contract and Southern School of Beauty Inc., reserves the right to demand the unpaid balance, payable immediately and to file a legal suit against the student. The student hereby agrees that if court action is necessary said student will be solely responsible for all costs incurred in court action and attorney fees against said student. Return of Title iv calculations will be performed for all students withdrawing before having completed the program as contracted, calculation to be performed in accordance with federal regulations. 19

RETURN OF TITLE IV FUNDS

AND

REFUNDS POLICY

When you apply for financial aid, you sign a statement that you will use the funds for educational purposes

only. Therefore, if you withdraw before completing your program, a portion of the funds you received may

have to be returned. Southern School of Beauty will calculate the amount of tuition to be returned to the

Federal fund programs according to the policies listed below.

A full refund will be made to any student who cancels the enrollment agreement contract within 72 hours

(until midnight of the third day, excluding weekends and holidays) after the enrollment contract is signed

and a tour of the facility has been made by the prospective student.

“Official” Voluntary Withdrawal

A student is considered to be “Officially” withdrawn on the date the student notifies the Director of

Financial Aid in writing or orally, of intent to withdraw. The date of the termination for return and refund

purposes will be the earliest of the following for official withdrawal:

1. Date student provided official notification of intent to withdraw, in writing or

orally shall be the official date of withdrawal.

2. The date the student began the withdrawal process, according to Southern School of Beauty

attendance records. A student is allowed to rescind his notification in writing and continue the

program. If the student subsequently drops, the student’s withdrawal date is the original date of

notification of intent to withdraw.

Upon receipt of the withdrawal information, the Southern School of Beauty will complete the

following:

1. Determine the student’s last date of attendance as of the last recorded date of

academic attendance on the school’s attendance record;

2. Two calculations are performed:

a. The students ledger card and attendance record are sent to First Advisors to

review the calculation of Return of Title IV funds the student has earned,

and if any, the amount of Title IV funds for which the school is responsible.

Returns made to the Federal Funds Account are calculated using the

Department’s Return of Title IV Funds Worksheets, and are based upon the

payment period.

b. Calculate the school’s refund requirement (see school refund calculation):

3. The student’s grade record will be updated to reflect a grade of incomplete.

4. Southern School of Beauty will return the amount for any unearned portion of the Title

IV funds for which the school is responsible within 45 days of the date the official

notice was provided.

5. If applicable, the Southern School of Beauty will provide the student with a letter

explaining the Title IV requirements:

a. The amount of Title IV assistance the student has earned. This amount is

based upon the length of time the student was enrolled in the program and

the amount of funds the student received.

b. Any returns that will be made to the Federal program on the student’s behalf

as a result of exciting the program. If a student has completed more than

60% of the payment period, he/she is considered to have earned 100% of the

Federal funds received for the payment period. In this case, no funds need to

be returned to the Federal funds.

c. Advise the student of the amount of unearned Federal funds and tuition and

fees that the student must return, if applicable.

6. Supply the student with a final student ledger card with the outstanding balance due

to the school and the available methods of payment.

7. A copy of the completed worksheet, check, letter and final ledger card will be kept in

the student’s file.

In the event a student decides to rescind his or her official notification to withdraw, the student

must provide a signed and dated written statement that his/her is continuing his or her program of

study, and intends to complete the payment period. Title IV assistance will continue as originally

planned. If the student subsequently fails to attend or ceases attendance without completing the

payment period, the student’s withdrawal date is the original date of notification of intent to

withdraw.

Unofficial Withdrawal

Any student that does not provide official notification of his or her intent to withdraw and is

absent for more than ten consecutive unexcused absences, no more than thirty days absence

comprised of excused and unexcused absences, does not resume attendance at the end of an

approved leave of absence, fails to maintain satisfactory academic progress, fails to comply with

the school’s attendance and /or conduct policy, does not meet financial obligations to the school,

or violates conditions mentioned in Southern School of Beauty Standards, will be subject to

termination and considered to have unofficially withdrawn.

Within one week of the student’s last date of academic attendance, or failure to return from a

LOA as determined by the attendance record, the following procedures will take place:

1. The admission’s office will make three attempts to notify the student regarding

his/her enrollment status;

2. Determine and record the student’s last date of attendance as the last recorded date

of academic attendance on the attendance record;

3. The student’s withdrawal date is determined as the date the day after 10 consecutive

absences, or if student did not return from a LOA, the withdrawal date will be the

date the student did not return.

4. Notify the student in writing of their failure to contact the school and attendance

status resulting in the current termination of enrollment;

5. Southern School of Beauty calculates the amount of Federal funds the student has

earned, and, if any, the amount of Federal funds for which the school is responsible.

6. Calculate the school’s refund requirement (see school refund calculation);

7. The student’s grade records are updated to reflect a grade of W.

8. Southern School of Beauty director will return to the Federal fund programs any

unearned portion of Title IV funds for which the school is responsible within 45 days

of the date the withdrawal determination was made, and record on student’s ledger

card.

9. If applicable, the Southern School of Beauty will provide the student with a refund

letter explaining Title IV requirements.

a) The amount of Title IV aid the student has earned based upon the length of

time the student was enrolled in the program and the amount of aid the

student received.

b) Advise the student in writing of the amount of unearned Title IV aid and

tuition and fees that he/she must return, if applicable.

10. Supply the student with a final student ledger card showing outstanding balance due

the school and the available methods of payment.

11. A copy of the completed worksheet, check, letter, and final ledger card will be kept

in the student’s file.

If tuition is collected in advance of entrance and if after expiration of the 72 hour

cancellation privilege the student does not start, not more than $100 shall be retained by

Southern School of Beauty. Students are required to purchase books, supplies and

equipment at the beginning of the program. According to Southern School of Beauty refund

policy, once these materials are purchased, no refund will be made. (Please refer to the

Return of Title IV Funds Policy and the State Refund Policy for treatment of these items

per Federal regulations.)

A full refund will be made to the student if he/she:

1. Is not accepted by the school;

2. Was enrolled by misrepresentation in advertising, Southern School of Beauty

promotional materials, or representation by the owner or Southern School of Beauty

representative; or

3. Was enrolled in a course of instruction that is discontinued by Southern School of Beauty

and prevents student from completing the course.

Refunds will be made within 45 days after the effective date of termination.

IF STUDENT WITHDRAWS, TWO CALCULATIONS ARE PERFROMED:

1. The Return of Tile IV funds ( To determine amounts earned from the Federal

programs) and

2. The State Refund Policy – to determine the amount of institutional charges earned.

Southern School of Beauty will adjust student’s charges to take into account

repayments of Title IV funds that Southern School of Beauty was required to make.

(See refund Policy below)

RETURN OF TITLE IV FUNDS

Only the Title IV programs are to be included in this calculation. Southern School of

Beauty participates in the Federal Pell Grant and Direct Loan Programs. Students

receiving Title IV funds will be subject to these regulations.

The amount of Title IV aid earned is based on the amount of time student spent in

academic attendance, and the total aid received; it has no relationship to student’s

incurred institutional charges. Because these requirements deal only with Title IV funds,

the order of return of unearned funds do not include funds from sources other than the

Title IV programs. Title IV funds are awarded to the student under the assumption that he/she will attend

school for the entire period for which the aid is awarded. When student withdraws, he/she

may no longer be eligible for the full amount of Title IV funds that were originally scheduled

to be received. Therefore, the amount of Federal funds earned must be determined. If the

amount disbursed is greater than the amount earned, unearned funds must be returned.

Up through the 60% point in each payment period the required pro-rata calculation is used to

determine the amount of Title IV funds student has earned at the time of withdrawal. After the

60% point in the payment period, student has earned 100% of the Title IV funds.

Southern School of Beauty measures progress in Clock Hours, and uses the payment

period for the period of calculation.

The Calculation Formula:

Determine the amount of Title IV aid that was disbursed plus Title IV aid that could have

been disbursed.

Calculate the percentage of Title IV aid earned:

a) Divide the number of clock hours scheduled to be completed in the payment

period as of the last date of attendance in the payment period by the total

clock hours in the payment period.

HOURS SCHEDULED TO COMPLETE

TOTAL HOURS IN PRIOD = % EARNED

b) If this percentage is greater than 60%, the student earns 100%.

c) If this percent is less than or equal to 60%, proceeds with calculation.

Percentage earned from (times) Total aid disbursed, or could have been

disbursed = AMOUNT STUDENT EARNED.

Subtract the Title IV aid earned from the total disbursed = AMOUNT TO

BE RETURNED.

100% minus percent earned = UNEARNED PERCENT

Unearned percent (multiplied by) total institutional charges for the period =

AMOUNT DUE FROM THE SCHOOL.

If the percent of Title IV aid disbursed is greater than the percent unearned

(times) institutional charges for the period, the amount disbursed be used in

place of the percent unearned.

If the percent unearned (times) institutional charges for the period are less

than the amount due from the school, the student must return or repay one-

half of the remaining unearned Federal Pell Grant.

Student is not required to return the overpayment if this amount is equal to or

less than 50% of the total grant assistance that was disbursed /or could have

1. Repay the overpayment in full to Southern School of Beauty. Sign a

repayment agreement with the Department.

Post-withdrawal disbursements A post-withdrawal disbursement, a type of late disbursement, is Title IV aid that was not disbursed before a student withdrew, but which the student has earned based on a Return of Title IV Funds calculation.

Order of return of Title IV funds

A school must return Title IV funds to the programs from which the student

received aid during the payment period or period of enrollment as applicable,

in the following order, up to the net amount disbursed from each source:

Unsubsidized Federal Stafford loans.

Subsidized Federal Stafford loans.

Unsubsidized Direct Stafford loans (other than PLUS loans).

Subsidized Direct Stafford loans.

Federal Perkins loans.

Federal PLUS loans.

Direct PLUS loans.

Federal Pell Grants for which a return of funds is required.

Academic Competitiveness Grants for which a return of funds is required.

National Smart Grants for which a return of funds is required.

Federal Supplemental Educational Opportunity Grants

PAYMENT METHOD: Upon enrollment, the tuition is due and payable in full. However, the student, at the school's option, may pay an enrollment fee (identified on the Enrollment Agreement), and the balance may be paid in monthly installments as defined in the Enrollment Agreement.

CREDIT BALANCE All funds from any source disbursed on student’s behalf will be credited to student’s school expense account until the entire contract balance and associated fees if applicable are paid in full. I hereby give Southern School of Beauty permission to hold any credit balance on my account for budgeting school expenses. Additionally, in the event student withdraws prior to having completed the course of study described in the enrollment agreement student authorizes the school to return federal funds exceeding school cost to applicable sources. Student understands that this can be rescinded in writing at any time.

TUITION Payment of enrollment fee will reserve you a place in the next available class. Enrollment fees do not apply toward tuition cost.

GENERAL SCHOOL POLICIES

1. Proper conduct is required of all students at all times. Those students whose attitude and actions reflect negatively on the reputation and welfare of the school or its students may be terminated. 2. No one is to leave the school building without permission from an Instructor. No one may leave a patron with a chemical service in progress without making arrangements with an Instructor. Breaks and lunch periods are arranged around clinic assignments and every consideration is extended to patrons. 3. All students are allowed a sixty (60) minute lunch and two (2) ten (10) minute breaks Tuesday through Friday. No lunch break on Saturday. All other time is to be spent pursing Cosmetology studies. The office is off limits unless accompanied by a staff member. 4. Students are not allowed to smoke, eat, chew gum, or have anything to drink in the main work area, or classroom. There are sections of the school that are designated for these purposes. After finishing lunch, each student will clean up their own trash, and with cooperation from all students, we will keep a clean school. 5. Students may not be called out of the classroom or away from a customer to use the phone. Telephone is not for personal use. 6. Students will be assigned services on the public on a non-discriminatory basis. Students are not allowed to refuse services on customers and must take all services assigned to them by the Instructor. 7. Students may have their own hair styled and treated with permission from the Instructor. Students may receive cut, styles, and shampoos at no cost. We want our students to be well groomed at all times. Services such as color and permanent waves and conditioners for hair will carry a small charge to cover the cost of the product. Students desiring to charge their services may do so at the office and these accounts must be in full each day. 8. No students will be allowed to remove supplies from the school, such as rollers, clips, lotions, perm rods, etc. 9. Students will be assigned a daily clean-up detail. No one will be accepted in the school who Refuses to do this detail. These details will alternate periodically. Any student who refuses to do their assigned details will be sent home and not allowed back in school until he or she can Accept the responsibility assigned to them. 10. Each student is responsible for their own station, to keep it clean and neat at all times. The Mirror is cleaned on a daily basis, soiled towels placed in area designated, hair swept after each client and all supplies returned to where they belong. 11. All students are required to keep and complete a notebook of notes taken classes, results of tests and outlines. This notebook is not to benefit the school; it is for the benefit of the student. (Students will supply their own notebook). 12. The school is not responsible for your personal property, and lockers are provided for your Personal use. A duplicate key or combination must be left in the school office with the Instructor. You will be responsible for your kit and all of your working tools. Kits are issued with built-in locks and keys. School property remains at the school if the contract is breached.

13. There is to be no use of alcohol, drugs, or related substances in the school, or the parking lot and no one will be permitted in the school while under the influence of any such substances. It Is the responsibility of a student to report a suspected breaking of this rule to the Instructor of the school. 14. Any student caught in the theft of school property, or the purposeful defacing of any school property may be terminated and/or prosecuted. 15. Each absence and tardiness must be documented. The forms to record and certify your absence are at the front desk. Should you be tardy or miss a day, this form must be turned into the office, before you sign in. 16. Student will need to wear scrubs, color of their choice, with shoes that are enclosed (example) no sandles or shoes of any kind showing the toes. Instructors will wear business dress attire with enclosed shoes. 17. No talking or texting on cell phones out front on clinic floor. 26

OKLAHOMA STATE EXAM REQUIREMENTS: Upon completion of the program of study the student may submit a $15.00 examination fee to make application for the Oklahoma State Board Examination. The State Board will issue a temporary permit for the student to work under the supervision of a licensed cosmetologist operator until the next regularly scheduled examination is held. Upon notification of successfully completing the examination, a license is issued in accordance with the State Board Rules and Regulations and Licensing procedures. Passing grades for the State Board Examinations are as follows: Basic program 75% Instructor's program 85% Manicurist program 75% DIPLOMA/CERTIFICATE Upon successfully completing all requirements associated with the program identified in the enrollment agreement students are awarded a certificate of completion.

GRIEVANCE PROCEDURE

Southern School of Beauty has adopted a grievance procedure that incorporates due process standards, which provide for prompt and equitable resolutions of complaints from students, staff, program participants, and clients, etc. The intent being to address adverse issues promptly and to resolve matters before they reach grievance magnitude. The following procedures are to be followed should one find it necessary to file a grievance against Southern School of Beauty Inc. The grievance must be submitted in written form to Southern School of Beauty Inc. administrators. Upon receipt of a grievance institution administrators and staff will review the issues and provide the complaint with a written response within five (5) working days. The decision of the administrators and staff will be final. If resolution is not achieved between the complainant and Southern School of Beauty the complainant is directed to file the written grievance with the licensing agency: Oklahoma State Board of Cosmetology, 2401 NW 23
