Factors in health-care-worker EVD infections

•inconsistent recognition/triage of Ebola patients;•overcrowding of hospitals/clinics;•limitations in physical layout of hospitals;•lack of training in PPE and inadequate supply of PPE; and,•limited supervision of and adherence to infection control.

Chronology of the EVD outbreak

A 2-year-old boy, who died in December 2013

in the village of Meliandou, Gueckedou Prefecture, Guinea was the index case of the current EVD epidemic.

His mother, sister, and grandmother then became ill with similar symptoms and also died.

People infected by those initial cases spread the disease to other villages.

Early cases are misdiagnosed as other diseases more common to the area.

"I remember working on SARS and being scared, but this is much worse than SARS. It is just spreading faster and faster and faster."He is not the only person so concerned. As Dr Nabarro visited recently the White House , the former US Central Command commander, retired General Charle Zinni, said the the threat from Ebola outweighed that of the Islamic State.

"I would say Ebola is the greater global threat,“ I think ISIS certainly is a regional threat. It can create problems in our country and other countries, but it's not an existential threat. Ebola can become an existential threat on a global level."

• The NICD EMLU was deployed in Sierra Leone, Freetown as part of WHO-GOARN EVD international outbreak response in West Africa.

EMDL capacitation by NICD

• NICD will continue to provide technical support until the termination of the EMDF operation.

• The technical support will include further guidance and ad hoc consultation via telecommunication and if required additional NICD staff deployment for on-site assistance.

• Supply of reagents, consumables, and PCR machines if still required.

• Support in maintenance and repairs of biosafety devices.

• After termination of the EMDF operation the CEZD-NICD staff will conduct a full decontamination of the building and assist in its re-designing for the TB Reference Laboratory function.

Lessons• Investments being made in South Africa in building diagnostics and

surveillance capacity for formidable pathogens, including establishing of BSL4 laboratory facility and the development of mobile deployment capacity are important contributions to South African efforts in being prepared to respond and control outbreaks of dangerous pathogens, and be able to support other African countries.

• Mobile deployment capacity should be supported by emergency budget which could be rapidly operationalized, available on the scene of outbreak, and be able to effectively address changing needs.

• Field laboratory capacity requires continuous training and adequate staffing.

• Field laboratory testing capacity require further improvements.

CONCLUSIONS• Laboratory confirmation of EVD suspected cases was essential in the successful control

of EVD outbreak and minimizing its further geographic spread.

• Deployment of the NICD EMDF in SL significantly contributed to international efforts in bringing the outbreak under control.

• The Western Urban Area of Sierra Leone, where the SA NICD EMDF operates, remained a hotspot of the EVD epidemic for months.

• During the EVD crisis in the capital of Sierra Leone, for weeks the NICD EMDF was the only Ebola diagnostic capacity to respond to overwhelming demand for EVD diagnosis.

• The Ebola field molecular laboratory became a valuable site for training of South African scientists on the scene of Ebola outbreak - a life experience.

• This initiative became a nucleus of a rolling deployment that has produced much-needed medical specialists trained in the field of highly dangerous pathogens and constitutes the largest Ebola outbreak response in the history of the NICD and South Africa on foreign soil.

CONCLUSIONS

• The national staff became fully integrated into all aspects of functions and operation of the laboratory. Capacity development and training of local staff enabled successful hand over of the laboratory to the Sierra Leone MoHS.

• The NICD/SA role in the provision of Ebola diagnostic services was acknowledged by Sierra Leone MoHS, African Union, UN agencies, including WHO, UNICEF, WFO, IAEA, CDC, many other national and international Ebola outbreak response partners, and the South African parliament itself.

• At the time of Public Health Emergency of International Concern, selflessness, compassion, solidarity, dedication ,professionalism and extraordinary efforts have further elevated the standing of the NICD as an important and unique public health resource in Africa.
