    troduction to the Report

he following report contains a site profile and summary of non-conformities identified during an onsite assessment commissioned by theliance for Bangladesh Worker Safety (Alliance) and conducted by a third-party Qualified Assessment Firm (QAF). The assessment wasnducted against the Alliance for Bangladesh Worker Safety Assessment Protocols (APs) and Fire Safety and Structural Integrity Standard, hich is harmonized with the factory assessment guidelines developed by Bangladesh University of Engineering and Technology (BUET) for e Bangladesh National Tripartite Plan of Action (NTPA). The goal of the Alliance process is to provide clear and practical technicalquirements by which Bangladeshi Ready Made Garment (RMG) Factories producing for Alliance members may be consistently and fairlyaluated for fire, structural, and electrical safety in a non-duplicative manner. Each assessment will prompt action plans that will be used by

MG factories to systematically and sustainably improve safety conditions for garment workers. Beyond tracking and reporting on actioneps taken in a transparent manner, the Alliance organization and its members will seek to further support factory improvements throughchnical assistance, training, implementation support for functional Worker Committees, and in some cases financial assistance and wagepport for workers if factories are closed for remediation.

he contents of the report do not constitute a guarantee of compliance with the applicable laws, the Alliance Standard or the absolute or ntinued safety against fire, electrical and/or structural integrity issues that may lead to injury or loss of life. The report is designed to providenon-exhaustive summary of risk issues, based on a limited sampling and duration of time onsite by the named QAF. Neither the QAF nor e Alliance can certify or guarantee the quality, outcome, or effectiveness of actions taken in response to the report.

    ASSESSMENT FINDINGS

    Structural System Design

Question: Are credible structural design documents available for review and kept on site?

    Priority Level: Medium

    Non-Compliance Level: 2

Description: Credible structural design documents are available for review and kept on site, with the exception of the design report, as per BNBC 1.9.1.1.

Source of Findings: Document Review: Credible structural design documents are properlydocumented except design report.

    Suggested Plan of Action:

Have a qualified structural engineer prepare credible as-built documentsbased on the requirements of Part 8 Section 8.19 of the Alliance Standard.

    Suggested DeadlineDate:

    24 Jul 2014

Standard: Alliance Standard Part 8 Section 8.19 Required Structural Documentation for New and Existing Factories

Question: Is a clear and redundant load path to resist lateral loads provided?

    Priority Level: Medium

    Non-Compliance Level: 2

Description: The structural configuration is moment resisting regular structure up to 4thfloor and from 5th to 9th floor it is flat slab structure with edge beam. Hence, the lateral load path is apparent only for up to 4th floor, not for 5th to 10th floor.Further information (design report) is required to understand the redundancyagainst lateral loads.

Source of Findings: Visual Assessment: Visual inspection shows the lateral load path is apparentonly for up to 4th floor.

    Suggested Plan of Action:

Have a qualified structural engineer prepare design report.

    Suggested DeadlineDate:

    24 Jul 2014

Question: Have provisions been made in floors or decks for a concentrated load (such asheavy equipment, water tanks, stored materials, etc) applied at a locationwherever this load acting upon an otherwise unloaded floor would producestresses greater than those caused by a uniform load?

    Priority Level: Medium

    Non-Compliance Level: 2

Description: There are eight water tanks with a capacity of 5000 ltr each on the toilet zoneof the roof which are not shown in the structural design. These tanks act asconcentrated load on the roof slab.

Source of Findings: Visual Assessment: There are eight plastic water tanks found on top of theroof.

    Suggested Plan of Action:

Engage a qualified structural engineer to confirm and document that provisionshave been made to accommodate concentrated loads. If provisions have notbeen made, have a qualified structural engineer develop a remediation plan.

    Suggested DeadlineDate:

    24 Jul 2014

Standard: Alliance Standard Part 8 Section 8.13 and 8.14

Question: Are Certificates of Occupancy available for review?

    Priority Level: Low

    Non-Compliance Level: 1

Description: Certificate of Occupancy is not available for review.

Source of Findings: Document Review: Occupancy certificates are not found in the submitteddocuments.

    Suggested Plan of Action:

Provide Certificates of Occupancy for review.

    Suggested DeadlineDate:

    24 Jul 2014

Standard: Alliance Standard Part 8 Section 8.3 Preliminary Structural Assessment

    Structural System Construction

Question: Have all areas of needed maintenance, including areas with efflorescence, dampness, standing water on rooftops, and corrosion been addressed.

    Priority Level: Medium
