Learning Disabilities Profile 2013 Somerset Introduction Health profiles are numbers which help people who plan health services. They come as reports for local areas. Our health profiles are for the areas of Local Authorities that run Adult Social Services. They are used by planners in health services and social services. They are also interesting for self-advocates and family carers. They try to show How many people have learning disabilities, How healthy they are, How much health care they get, How well social services are looking out for them. They show how your area compares with others. Good numbers for all these things are hard to find. The numbers on the website are our best attempt.     Revisions to Profiles

    Since last year, there have been a number of changes to the Learning Disabilities Profiles.

    Firstly, we removed the following indicators:

    11. Admission rate for psychiatric ambulatory care sensitive conditions in people with LD14. Identifying people with learning disability in psychiatric in-patient statistics15. Persons admitted psychiatric specialty for challenging behaviour

    While the data does not identify individuals, some areas had low numbers that were not appropriate to use. We know from the [A&T and other specialistinpatient care for people with LD in the Count-Me-In census] (http://www.improvinghealthandlives.org.uk/gsf.php5?f=16342&fv=17542) that manyadmissions occur in the independent sector. As such, the small numbers taken from hospital episode statistics could have potentially misleadingresults.

    Secondly, due to the administrative changes in accessing hospital episode statistics we are unable to update the following indicators at present:

    Schools are very aware of children who have particulardifficulties in learning. Every term they report to theDepartment for education about all children who havespecial educational needs. This indicator shows thenumber of children in every thousand who have autisticspectrum disorders. These children have a combinationof difficulties with verbal communication, interacting withother children or adults. They often also have particularfocus on specific interests and find it difficult to engage inother subjects. The school census covers all pupilsenrolled in state-funded primary, secondary or specialschools. A formal medical diagnosis is not required,assuch these numbers may not reflect those seen in datafrom medical sources.

The school census covers all pupilsenrolled in state-funded primary, secondary or specialschools. They say what sort of needs thechildren have. There are four levels of learning difficulties:specific difficulties (like dyslexia), moderate learningdifficulties, severe learning difficulties and profound andmultiple learning difficulties. The indicator shows the numberof children in every thousand who have severe learningdifficulty. These children have serious difficulty inparticipating in ordinary school programs without support.Many have limited communications and self help skills. Theschool census covers all pupils enrolled in state-fundedprimary, secondary or special schools. A formal medicaldiagnosis is not required,as such these numbers may notreflect those seen in data from medical sources.

These children havevery severe difficulty in learning combined with physical orsensory disabilities. They require a high level of adult supportfor both learning and personal care needs.     8. Proportion of eligible adults with a learning disability having a GP health check

    People with learning disability have more difficulty thanothers in recognising ordinary health problems andgetting treatment for them. Each year GPs are supposedto offer regular health checks to make sure importantproblems are identified and treated. PCTs report to theDepartment of Health how many people there are on GPpractice lists known both to their GP and the local socialservices department to have a learning disability. Theyshow how many of these have had a health check in thelast year.

    9. Median age at death

    People with learning disabilities die at younger ages thanother people. This indicator shows the midpoint of theages of all of the people who have died and whose deathcertificates show that they had learning disabilities. It is alittle incomplete because often, doctors do not recordlearning disabilities on death certificates if they consider ithad no relationship to the person's death.

    10. Emergency hospital admissions as % of total

    Ideally, people who need treatment in hospital should beadmitted before their illness reaches a critical stagewhere they have to be admitted as an emergency. Whereillnesses are left to a late stage, this often involves moresuffering on the part of the patient and poorer outcomesof treatment. Emergency admissions are also lesssatisfactory as less advance planning is possible andpatients are commonly seen by more junior staff. Lack ofadvance planning is particularly relevant for people withlearning disability where it is helpful if staff can makereasonable adjustments in anticipation.

Admission rate for non-psychiatric ambulatory care sensitive conditions in peoplewith LDFor some types of illness, once a diagnosis is clear, carecan usually be managed without patients going intohospital. Hospitalisation is usually unpleasant for patientsand expensive for services. Three types of physicalhealth condition common in people with learningdisabilities are more likely to cause admission to hospitalif ordinary care is not provided well. These are gastric-oesophageal reflux disorder (GORD), epilepsy andconstipation.

    13. Identifying people with learning disability in general hospital statistics

    Hospitals can only make appropriate reasonableadjustments for people with learning disability if theyknow they need to. Learning disability is alwaysimportant, as adjustments will always be required tomake sure these patients understand fully what ishappening and what they are consenting to, and thatdoctors and nurses take their learning disability intoaccount in assessing symptoms and progress. Recordingthis statistically is also important to the hospital first so itcan check reasonable adjustments are being made andsecond because in many cases hospitals get paid morefor treating people who have a learning disability, asmore care is needed.

    This indicator shows the extent to which Local Authoritiesare providing day care services for people with learningdisabilities known to them. To be included in thedenominator data people must have had a review in thelast 12 months, whereas to be in the numerator theyneed only be using services. All people who are usingcouncil services should have had a review, however theymay not all have received their review before the datacollection period started and so were not counted, insome cases this has resulted in LAs having morenumbers of people using services than are known (orhave had their review). In 2011-12 the system wasaltered so that people who are eligilbe for their reviewduring the collection period by included in thedenominator counts, whether they have received theirreview or not.

    Paid work provides recognition of a valued social role,useful day time occupation, important socialopportunities, and in a few cases a helpful level offinancial reward.     The Minister for Care Services mandated that data becollected in 2010-11 on the number of alerts and referralsto adult social care safeguarding teams.
