Culture Safe culture = informed culture

Enforcement policy encourages a safety reporting culture

Quality risk management based on quality information

Reporting systems fundamental to good management

Reports analyzed in context of contributing factors to determine root cause

Analyzing individual reports in isolation can lead to misinterpretation

Civil Aviation Issues Reporting System Integrated management

system IMS operational by April 2007 followed by independent evaluation

IMS is a risk based management process identical in concept to the SMS but more

IMS entrenches reporting culture into the fabric of the organization

    CAIRS launched in May 2005

  • RDIMS # 2082466 6

Safety Management Systems SMS demands the proactive reporting

    of all hazards

Expect an increase in reports in operators system, in CADORS and in CAIRS

Example: more reports of precautionary engine shutdowns but few reports of engine failures

Hazards identified and addressed before they develop into more serious occurrences

Positive sign that safety reporting culture is taking hold
