For succeeding confinements for the same cause occurring within 30 days from each other, the3-day minimum confinement will no longer be required. Pre-existing conditions shall be covered after one year from effective date of employees insurance. Pre-existing conditions are those:

    (a) For which the insured received medical advice, consultation, or treatment; or(b) Whose signs or symptoms are evident, or should have been evident to the insured, even if the insured did not seek medical advice, consultation, or treatment for it prior to the effective date ofthe policy or its last reinstatement, if any.Condition means any type of illness, specific injury, disease or infirmity including all underlying or related conditions and any manifestation thereof, whether in one or more than one body system.

    Guarantees a lump sum cash benet in case

    the employee is diagnosed with any of the 35 coveredcritical conditions including but not limited to Cancer,Stroke, Heart Attack and Kidney Failure.

    Life Insurance

    Guarantees a lump sum cash benet paid to thedesignated beneciary in case of the employees death.Advances a portion of the lump sum cash benet to theemployee in case of Total & Permanent Disability.Advances half of the life insurance benet in case theemployee is diagnosed with a Terminal Illness, as denedwith a life expectancy of less than twelve (12) months.

    Accidental Death & Disablement

    Doubles the amount of life insurance benet in case theemployees death is caused by an accident. A portion ofthe benet is paid in case of total or partial disablement.
