1 Introduction
This document describes the design of the Florida Safe Families Network (FSFN), which
supports a transformation of the Case Planning module.
The Case Plan is a FSFN tool used for all children that enter out of home placement or
participate in protective services offered through the Department of Children and Families
(DCF). Case Plans identify goals and objectives that are to be achieved to support and verify the
safety and well-being of all children involved.
The functional design of the Case Planning module represents changes to FSFN resulting from
the transformation of the departments Child Welfare business processes to facilitate the safety of
children and families, and support the following goals of the Child Protection Transformation
and SACWIS Compliance Project:
• Improve quality and consistency of decision making
• Eliminate redundant processes and re-work
• Gain efficiencies through enhanced technology
The Case Plan is a written document that specifies tasks or activities required to be completed
by parent(s), caregiver(s), child(ren), and the case worker to support safety and permanency for
the child(ren) for whom the case plan is created. The Case Plan also identifies services to assist
the family and provides specified time frames for completion of noted task. The FSFN Case
Plan follows the child(ren) from the delivery of services through dependency, foster care, and/or
the termination of rights of the parents.
The FSFN Case Planning tool is comprised of two components: Case Plan Worksheet and
Judicial Review Worksheet. These components are made up of numerous tabs and pop-up pages
to support documentation of needs, services, and activities that have been put in place to support
and verify the safety, well-being and permanency of the child(ren) for whom the plan has been
designed.
The Case Plan Worksheet page captures information pertaining to all planning activities and
includes all case participants involved. The Judicial Review Worksheet will capture additional
information such as summary of the child(ren) who in care needs, protective capacities of the
caregiver, and desired overall placement outcomes.

2.1.1 Page Overview
Page Navigation
The FFA-Ongoing/Progress Update Selection pop-up page is launched when creating a Case
Plan Worksheet page from Create Case Work or Case Book. Before a new Case Plan Worksheet
page is created, a FFA - Ongoing or Progress Update must be selected. This page is also
launched from the Case Plan Worksheet by clicking the Update Link hyperlink in the Case
Information group box.
The FFA-Ongoing/Progress Update Selection pop-up page allows the user to select the
appropriate FFA-Ongoing or Progress Update that is either “Pending” or “Approved.” The FFA-
Ongoing/ Progress Update Selection pop-up page contains a repeating table that contains
pending and approved FFA-Ongoing or Progress Update Pages associated to the Case. The user
can select only one radio button at a time. The Continue button is enabled only after a radio
button is selected. Upon clicking Continue, the system launches a new Case Plan Worksheet
Page, pre-filling with the data from the selected FFA-Ongoing or Progress Update page. Upon
clicking Close, the system returns the user to the Desktop or Case Book, whichever is applicable,
without creating a new Case Plan Worksheet page.

Selection
Contains a list of all “Pending” and “Approved” FFA-
Ongoing and Progress Update pages for the Case.
Fields Select Column displays radio buttons to select the specific FFA-
Ongoing or Progress Update.
Household Name Column displays the Household Name currently captured
on the FFA - Ongoing or Progress Update.
FFA - Ongoing ID Column displays the FFA-Ongoing ID.
Progress Update
ID
Column displays the Progress Update ID.
Worker Name Column displays the name of the worker that created the
FFA-Ongoing or Progress Update.
Type Column displays the "Type" of record: FFA-Ongoing or
Progress Update.
Approval Date Column displays the Approval Date of the FFA-Ongoing
or Progress Update.
Status Column displays the status of the FFA-Ongoing or
Progress Update: Approved” or “Pending” status.
Buttons Continue User continues to the Case Plan Worksheet page after
selecting the applicable FFA-Ongoing or Progress Update.
Close Closes page without creating a new Case Plan Worksheet
page (if launching the page to create a new Case Plan
Worksheet) or closes page without applying a new FFA-
Ongoing or Progress Update (if launching the page to
apply a pending or approved FFA-Ongoing or Progress
Update to the Case Plan Worksheet).
2.1.3 Background Processing
• Upon attempting to create the Case Plan Worksheet page, or when updating the Case Plan
Worksheet page, the FFA – Ongoing/ Progress Update Selection page displays only
“Pending” and “Approved” pages for selection. Not Approved FFA – Ongoing or Progress
Update pages will not be displayed.
• The column named “Household Name” will display the Household Name currently captured
on the FFA - Ongoing or Progress Update when accessing the selection pop-up page via any
of the following workflows: creating an FFA - Ongoing; creating a Progress Update; creating
the Case Plan Worksheet; or updating the "link" on the Case Plan Worksheet.
• Upon attempting to create the Case Plan Worksheet, at which point the worker must select
the appropriate FFA - Ongoing or Progress Update to link to the Case Plan Worksheet, FSFN

will look to determine if a Case Plan Worksheet already exists for the selected FFA -
Ongoing or Progress Update. If so, FSFN will display a soft edit stating:

2.2 Case Plan Worksheet Page-Participants/Family Change Strategy Tab
2.2.1 Tab Overview
The Case Plan Worksheet page captures information regarding the case planning activities. It
includes the case participants involved, visitation plans, summary of child in care needs, the
protective capacities of the caregivers, and desired outcomes. The Case Plan Worksheet page is
created by selecting “Case Plan Worksheet” under the Assessment and Planning drop down in
the Create Case Items group box on the Create Case Work page. When creating a Case Plan
Worksheet, the user will be asked to select a FFA-Ongoing or Progress Update. The Case Plan
Worksheet page can also be created from the Case Actions group box on the Case Book page.
Existing pages can be accessed in edit mode from the Desktop Outliner>Case>Assessments and
Planning (or view only mode if from Search). Existing pages can be accessed in the Person Book
or Case Book from the Work group box by selecting Assessments and Planning as the Work
type. If Person Book or Case Book is launched from the Desktop the page will be in edit mode.
If launched from Search, it will be in view only mode.
When the Case Plan Worksheet page is created the system will ask the user to select an FFA-
Ongoing or Progress Update. The system will pre-fill data on the new Case Plan Worksheet page

with data from the selected FFA-Ongoing or Progress Update. The Case Plan Worksheet page
cannot be created without selecting an applicable FFA-Ongoing or Progress Update page. After
the initial creation of the Case Plan Worksheet page, the user must refresh the Case Plan
Worksheet page to obtain any updated data from the FFA –Ongoing or Progress Update page
and any other applicable pages. The Case Plan Worksheet page is updated at critical junctures or
designated intervals throughout the lifecycle of the Case during the Departments involvement
with the family. The user can select the Update Link hyperlink to launch the FFA-Ongoing /
Progress Update Selection pop-up page; this enables the selection of any Pending or Approved
FFA-Ongoing or Progress Update to pull in updated information to the Case Plan Worksheet.
Once a new FFA-Ongoing or Progress Update has been selected, clicking the Save button on the
Case Plan Worksheet page will save the newly updated information.
The Participants/Family Change Strategy tab contains the involved case participant’s information
such as children, adult(s) (In a care giving role), and family support network person(s) who are

Task Summary hyperlink that is associated with any participant in the Parent/Legal
Guardian(s)/Other Adult Household Members in a Caregiving Role group box, the system will
launch a Case Plan Task Summary pop up page in view only mode. The Participant/Family
Change Strategy tab also contains narrative of the Family Change Strategy which includes the
Danger Statement, Family Goal, Ideas, and Potential Barriers information from the FFA -
Ongoing or Progress Update.
In the event the user performs the Update Link functionality, upon saving the FFA – Ongoing
page, these same fields will automatically pre-fill with the most current information documented
on the selected FFA – Ongoing or Progress Update.
Note that anywhere the term “notifications and templates” is referenced, notifications is a
technical term that is similar to templates, the key difference being that notifications pre-fill
everything from the FSFN system, do not have any user-entered fields, and are not saved to the
FSFN database. Therefore, regardless of when the template is launched it will pull in all the most
current, up-to-date information from FSFN.

Case Plan Tasks for the associated participant. The Case Plan Task Summary Pop Up page is
displayed by clicking on the ‘Task Summary’ hyperlink next to the Case Participant on the
Participants/Family Change Strategy tab. The hyperlink will ONLY be displayed once the Case
Plan Worksheet has been successfully saved for the first time and is only displayed in the
Adult/Legal Guardian(s)/Other Adult Household members in Caregiving Role group box.
The Case Plan Task Summary Pop-Up page will be displayed in view only mode at all times. All
necessary task details will be displayed on the page and include but is not limited to:
Parent/Adult name; Outcome Achievement; Family Goal; Service level; Who the Providers are
and their Contact Information. This information will pre-fill for each Task captured on the
associated Case Plan Worksheet where the Task has that participant identified in the "Who" drop
down. The Visitation Schedule will pre-fill and display for the Parent/Adult name that appears

o The Service Authorization page will not be deleted, but instead the link between the

2.14.1 Page Overview
Before a new Judicial Review Worksheet Page is created, a Case Plan Worksheet must be
selected. This is a new page. The Case Plan Worksheet Selection pop-up page allows the user to
select the appropriate Case Plan Worksheet. The Case Plan Worksheet Selection pop-up page is
launched when creating a Judicial Review Worksheet page from the Create Case Work page or
Case Book page. The selected Case Plan Worksheet will be associated with the newly created
Judicial Review Worksheet page. The system will use the data from the selected Case Plan
Worksheet to pre-fill the Judicial Review Worksheet page.
If no Judicial Case Plan Worksheets exist for the FSFN Case, the group box will not contain any
records and the Continue button will be disabled. The user will only have the option to close the
pop-up page and not be able to continue with creating the Judicial Review Worksheet page. The
Continue button is enabled only when at least one case plan worksheet is selected. Upon
selecting the Continue button, the system launches a new Judicial Review Worksheet page pre-
filling from the selected Case Plan Worksheet page.

The Judicial Review Worksheet page is created by selecting “Judicial Review Worksheet” under
the Assessment and Planning drop down in the Create Case Items group box on the Create Case
Work page. When creating a Judicial Review Worksheet, the user will be asked to select a Case
Plan Worksheet to pre-fill appropriate fields on the Judicial Review Worksheet page. The
Judicial Review Worksheet page can also be created from the Case Actions group box on the
Case Book page.
The Date Modified indicates the last date the Judicial Review Worksheet page was last modified
and saved. When successfully saved, the system will automatically populate the current date into

Date Modified. Upon Selecting Refresh button, the system refreshes the Judicial Review

The Participants tab contains the information about the case participants included in the Judicial
Review Worksheet. This tab captures the various household members including Children;
Parent/Legal Guardian(s)/Other Adult Household Members in Caregiving Role; and Family
Support Network that are involved in the Judicial Review Worksheet. Upon creation of the
Judicial Review Worksheet page, persons included in the selected Case Plan Worksheet page
will pre-fill the appropriate sections from Participants and Family Change Strategy/Resources
tabs. There must be at least one participant in each group listed to save the Judicial Review
Worksheet page. The user can add the child participant using the insert button. Upon selecting,
the system will launch the Participant Selection pop-up page and list all active FSFN Case
participants. Each participant is displayed with an associated check box for multi-selection. The
user has the capability to remove participants by selecting the Remove hyperlink.
