Available all on one campus, our services include skilled nursing care and short-term rehabilitation, assisted living and adult day programs.

Skilled Nursing CareThe Elant at Goshen Nursing Care Center provides a variety of medical, clinical, social, and support services to address the needs of residents and their families. Comfortable living arrangements and a skilled, caring, highly trained staff ensure the highest quality care is delivered to each resident, giving families the reassurance and peace of mind they need. Short-term respite care stays can also be arranged for individuals when caregivers need temporary relief to refresh and reenergize.

Palliative CareFor those who need diagnosis-related palliative care or end-of-life care, Elant at Goshen has a variety of resources available to meet the needs of both the resident and the family members providing support care.

Rehabilitation Services And Sub-acute CareWhether the challenge is recovering from illness, injury, or surgery, Elant at Goshen offers a full spectrum of state-of-the-art therapeutic rehabilitation services for adults 18 and older. Our comprehensive rehabilitation programs including occupational, physical and speech therapy are specifically designed to help in recovery so you can get back to living an independent life as quickly as possible.

Highly trained, educated, and experienced, our therapists provide innovative care utilizing the most successful techniques. Our therapists are an integral part of the patients’ interdisciplinary team which includes physicians, nursing staff, dieticians, social workers, and activity leaders.

Assisted LivingThe Elant at Goshen Adult Home, a licensed adult care facility, provides seniors who wish to maintain their independence, but may need some help in doing so, a gracious and caring home. Residents enjoy companionship, stimulation and specialized care in comfortable living residences with features designed for comfort and safety. Short-term respite stays are also available.

Adult Day ProgramsThe Elant at Goshen Adult Day Programs offer options for those individuals living in the community who may benefit from additional supervision, socialization, activities and/or medical assistance. These programs are designed to support caregivers who may not be home during the daytime hours or may need a period of respite.

The Daybreak Program provides an entertaining, stimulating and supervised group environment including companionship and a variety of activities.

Daybreak activities include (but are not limited to):• Baking and gardening• Exercise and games• Music and crafts• Day trips to and from various events and locations

The Adult Day Health Program is designed to meet the daytime health care needs of the disabled, the chronically ill and seniors who require medical assistance.

The Program Provides:• Professional nursing care• Case management by a licensed social worker• Rehabilitation services by highly trained

occupational, physical and speech therapists• Nutritious meals and snacks, and personalized

nutritional counseling • Entertaining and stimulating activities in a supervised

environment• Transportation to and from community activities

The first of its kind in the Hudson Valley region, the Elant Medical Practice provides patients and residents the medical care and attention they need from our own physicians and experienced nurse practitioners. These health care providers are Elant employees whose sole focus is administering care exclusively to Elant patients. Our residents and patients find comfort in being treated by our devoted, on-site medical team.

To promote wellness and to treat those suffering from an illness or injury, the Elant Medical Practice offers a full spectrum of quality care and services. With a devoted and consistent staff, our residents and their families enjoy greater accessibility to physicians and nurse practitioners and an opportunity to build relationships with our medical team.
