For Plan Year 2016 it was the consensus of the Benefits Committee to:

1. Discontinue the current wellness program2. To increase:

1. Life Insurance and AD&D to 1X annual pay-maximum of $100,000

2. LTD at 60% compensation to $6000 with a 90 day waiting period

3. To maintain 2015 rates for 2016 pending embedded benefits
