Legislation Overview

The Caregiver Advise, Record and Enable Act ensures hospital patients can designate a family caregiver

and requires hospitals to offer that caregiver instruction

and demonstrations of medical tasks they will be expected

to provide for their loved ones at home, such as

administering multiple medications, dressing wounds, and

operating medical equipment.

Main Tenets of the Legislation

The opportunity to designate a caregiver

• “A hospital shall provide each patient…with at least one opportunity to designate at least one caregiver…”

The notification of movement to the caregiver

• “A hospital shall notify the patient’s designated caregiver of the patient’s discharge or transfer to another hospital or facility…”

The instruction of a caregiver in all after-care tasks

• “Prior to a patient’s discharge…the hospital shall consult with the designated caregiver regarding the caregiver’s capabilities and limitations and issue a discharge plan that describes a patient’s after-care needs at his or her residence.”

wounds taken care of

Our Solution:

As her father’s designated caregiver, Carla is notified

by the hospital that he is being discharged. She

speaks with the nurses about her father’s care plan

and signs up to receive care messages which will

include medication, prescription refill, and follow up

appointment reminders, instructional videos with detail

on how to properly clean his wound and change the

dressing, potential infection signs to look out for, and

secure chat access to medical professionals who can

answer her questions in real-time to ensure Andrew

stays healthy and Carla knows how to provide him

with the best care.
