room. The clientrefused to engage with Serco staff stating that he would harm himselfifanyone

attempted to enter the room. At approximately 2300hrs D/AC on call!I^^rwasnotified

of the Barricade/Sit-In incident(,-9CG73M). 2 x Life Without Barriers staffremained allhedoorto

rooms. At approximately 2255hrs LWB staffadvised Operations Manager^. that client

S. 47F(I) had deliberately cut himself(Self Harm Actual, -9CH905). Operatons Manager

dispatched CSM F^tonand CSO N. .--ht0 ,o0m 3, .LWB weretalkinglothe

client through a gap between the wardrobe and the door. It was observed that the bunk bedhad

been pushed againstthe internalside of the wardrobe, preventing entry by pushing overthe

wardrobe. At approximately 23.0hrs Operaiions Manager^.. returned to the room after

briefing D/AC and Serco Senior Management. Clients' 47F(I) ands.s. 47F(,)

, friends o1S- 47F(,) were at the room. The friends were apparently assistings. 47F(,)In

attempting to gels- 47F(I) to stop self haming and to remove the barricade. The conversationwas monitored by Operations Manager. ^ through LWB interpreting. It was observedIhat

there were what appeared to be toiletpaper with smears of blood stuck 10 the interior side ofthe

windowto room 31. Operations Manager. ^. contacted D/AC on callll^^rtoadvise

her of the situation and the threats of and whatappeared to be aclualself-harm. D/AC ...advised that use offorce was only to be used if the clientwas to continue self-harmingbehaviour.

Serco Senior Management were contacted at approximately 2320hrs and provided with asituation

update and advised of the further potential risk to the client illhe situation was riot resolved, At

approximalely 2330hrs Operations ManagerÎwas advised by LWB that the clientwould CUI

his own throatifanyone tried to enter the room. S- 47F(,) was advised by operations Manager^ that ithe refused to open the curtains 10 allow line of sight to verify his medical status.

Ihen Serco would have no choice but to resort to forcing entry to the room. Clients' 47F(I)refused

to one side which allowed CSO Singh to push the wardrobe overto allow access to the room

throughthedoor. Operations Manager^-^land CSO ^ then used necessary and

reasonable force 0-9CH90Z)to restrain the dient and prevent him from continuing self-harm.

arrived as per procedure with a reported self-liarm (PROMIS numbers- 47F(,) Iaftertheambulance service rang to advise Ihatlhere would be a 25 minute waitfor an ambulance.Police

did notinierview'. '"(') arthis time. At approximately 0010hrs Operations Manager^'Icontacted the D/AC on callwith a situation update. At approximately 0012hrs the ambulance

arrived onsite. They assessed clients' 47F(I) and prepared him fortransporl. OperationsManager

I. ^n was treated forthe wounds 10 his arm and hand. The injuries sustained requiredminimal

trea"nent and paramedics advised that he go fortesting for blood borne conlagions as theblade

that cut him was the same as the client used to self-harm. At approximately 0021hrs the dient

departed in the ambulance with the NT Police and 4 Serco staff (dientrated as High Risk dueto the

incident). Napproximalely0040hrs Operations Manager ^', conlacled the SercoNational

Duty Manager to give averbal update. At approximately 0100hrs room 31 was secured wilhbunk

amved at approximately 1943hrs. ClienlS- 47F(I) was taken to the Ambulance and departedsite

with the NT Police at approximately 2000hrs for Royal Darwin Hospital.
