Scrotal pain, also called testicular pain or orchialgia, is common in young boys. There are many conditions that may cause scrotal pain. In most cases, the pain can be treated without surgery. It is always important, however, to visit a doctor if a boy has sudden and severe scrotal pain. Some conditions need immediate treatment. SYMPTOMS & TYPES Common causes of scrotal pain include testicular torsion, torsion of the appendix testis or epididymitis. Scrotal pain can also be caused by the onset of puberty. Referred pain in other areas of the body such as the bowel or bladder is another source of pain that is felt in the scrotum. Testicular torsion is a twisting of the spermatic cord. A testicle receives blood from the spermatic cord. When the cord becomes twisted, the blood supply can decrease or be completely stopped. If not treated quickly, the blood loss to the testicle can cause tissue damage. Testicular torsion is most common in boys between the ages of 12 and 16, but it can occur at any age. Boys with testicular torsion have very intense scrotal pain that involves the entire testicle, starts suddenly, and does not improve. They can also have swelling of the scrotum and nausea or vomiting. If a boy experiences these symptoms, take him to an emergency room immediately. Surgery may be needed. Just like the appendix in the abdomen, the testicular appendix has no function within the body. This condition is most common in boys between the ages of 7 and 12. If the testicular appendix becomes twisted, it can cause sudden scrotal pain. It is important to have this diagnosed quickly, as the pain is similar to testicular torsion. Epididymitis is an inflammation of the epididymis. The epididymis is a coiled, tube-like structure that stores sperm. It sits on the back and top of the testicle within the scrotum. The inflammation can be caused by an infection or other irritation. Symptoms of epididymitis include scrotal pain, redness and swelling which usually start gradually. Scrotal pain due to puberty can be normal part of growing up. This type of scrotal pain, also called pubertal orchialgia, is intermittent. It can be dull and achy or much sharper. The pain usually will settle over time. Scrotal pain can be referred from another area in the body as well. For example, abdominal pain from constipation can cause dull scrotal pain. DIAGNOSIS & TESTS Your provider may be able to diagnose the cause of the scrotal pain based on symptoms and physical exam. It is important to know the intensity of pain, when it started, and if there are additional symptoms like swelling or fever. The physical exam includes inspection of the abdomen and pubic area including the penis and scrotum. An ultrasound or X-ray may also be needed to confirm the reason for the pain.

TREATMENT & CARE Epididymitis and torsed appendix testis can be treated without surgery. The pain and swelling will improve over time with rest, scrotal support, and help from mild over-the-counter anti-inflammatory pain medications. The pain usually improves within 1 month.

Testicular torsion is a true emergency. It requires surgery to untwist the blood vessels and bring blood flow back to the testicle. Without this surgery, there is a risk that the testicle may die. It is important to have any scrotal pain promptly evaluated by a health care provider.

Dolor en el escroto, también conocido como dolor testicular u orquialgia, es común en jóvenes varones. Existen muchas afecciones que podrían causar dolor en el escroto. En la mayoría de casos, el dolor puede tratarse sin cirugía. Sin embargo, es siempre importante acudir al doctor si un joven tiene dolor escrotal repentino y severo. Algunas afecciones necesitan tratamiento médico inmediato.

SYMPTOMS & TYPESLas causas comunes de dolor en el escroto incluyen torsión testicular, torsión del apéndice testicular o epididimitis. El dolor escrotal puede también ser causado por la llegada de la pubertad. Torsión testicular es la torcedura del cordón espermático. El testículo recibe sangre a través del cordón espermático. Cuando el cordón se tuerce, el suministro de sangre puede disminuir o eliminarse por completo. Si no es tratado rápidamente, la falta de sangre al testículo puede ocasionar daños al tejido. Torsión testicular es más común en jóvenes de 12 a 16 años de edad, pero puede ocurrir a cualquier edad. Los jóvenes con torsión testicular presentan dolor escrotal intenso que involucra al testículo completo, empieza repentinamente y no mejora. Ellos podrían también tener inflamación del escroto y nausea o vomito. Si un joven presenta estos síntomas, llévelo a la sala de emergencias inmediatamente. Cirugía podría ser necesaria.

Torsión del apéndice testicular es la torcedura del apéndice testicular, lo cual es una pequeña pieza de tejido situada arriba del testículo. Así como el apéndice en el abdomen, el apéndice testicular no tiene ninguna función en el cuerpo. Esta afección es más común en jóvenes entre 7 y 12 años de edad. Si el apéndice testicular se enreda, esto puede causar dolor escrotal repentino. Es importante ser diagnosticado rápidamente, ya que el dolor es similar al de la torsión testicular.

Epididimitis es la inflamación del epidídimo. El epidídimo es una estructura similar a un tubo enrollado el cual almacena esperma. Este está situado dentro del escroto, arriba y atrás del testículo. La inflamación podría ser causada por infección u otra irritación. Los síntomas de epididimitis incluyen dolor escrotal, enrojecimiento e hinchazón lo cual en la mayoría de casos comienza gradualmente.

El dolor escrotal debido a la pubertad puede ser parte normal del crecimiento Este tipo de dolor escrotal, es también llamado orquialgia de la pubertad y es intermitente. Puede ser un dolor persistente y tedioso o más agudo e intenso. El dolor usualmente se resuelve con tiempo.

DIAGNOSTICOS & EXAMENES Su proveedor medico podría ser capaz de diagnosticar la causa del dolor escrotal basándose en los síntomas y el examen físico. Es importante conocer la intensidad del dolor, cuando comenzó y si existen síntomas adicionales como inflamación o fiebre. Torsión testicular es una emergencia verdadera. Esto requiere de cirugía para desenrollar los vasos sanguíneos y restaurar el flujo de sangre hacia el testículo. Sin esta cirugía, existe el riesgo de que el testículo pueda morir. Es importante que cualquier dolor escrotal sea evaluado pronto por un proveedor médico.
