LDI - 7 teaching staff with a Full Time Equivalency of 5.72 .5 Secretary 1.5 Educational Assistants FMNI Resource Room – 1 Educational Assistant and 1 Child and Youth Worker supplied by Curve Lake First Nation 1 Student Retention Counsellor (3 days per week) 1 School Board Counsellor (one day per week (Tuesdays) between both schools) 1 Technical Services Specialist (T.S.S.)

Asset Narratives include the following:

• Site Summary – a brief summary of the Site systems.
• Limitations – a summary of the scope of work and the Tactical Planning Window.
Building Elements listed are only the ones that require replacement in the next 5 years; their condition is Critical if failed orrisk of imminent failure is observed, or Poor if it is not functioning as intended with significant repairs within the next two (2)years, or Fair if normal deterioration and minor distress is observed requiring repairs within three (3) to five (5) years.
2011-2015 Cost and Year information is a snapshot from the assessment and cannot be edited in TCPS.
2011-2015 Priority is the value of the Event priority calculated when the assessment data was imported in TCPS andstored in this read-only field.
Estimated Cost and Fiscal Year are values that can be edited at any time by end users.
Event Priority is a field populated with labels like Urgent, High, Medium and Low based on the Event Priority Value. Thisvalue is calculated based on the Element Type and Element Condition.
Photos are provided at the event level: old photos are suffixed with the world “Old”, new photos are suffixed with the dateof assessment.

1. Architectural & Structural Executive Summary
Information was gathered and observations were made by Pretium Anderson Toronto Inc. on August 06, 2013.
Lakefield District Secondary School is located at 71 Bridge Street, in Lakefield, Ontario. The original building wasconstructed in 1952. The first addition was constructed in 1957. The second addition was constructed in 1966. The thirdaddition was constructed in 1971. The total building size is 8,165 square meters. The building is a 2-storey structure

without a basement. The original building is likely constructed on concrete spread footings, where foundation walls are amixture of concrete block masonry and poured concrete. Information on the type and depth of the footings is not available.The educational facility houses a library, a gymnasiums cafeteria, the workshops (electrical shop, machine shop, autoshop and the home economics classrooms), storage and change rooms, service rooms, student and staff washrooms,main office area and classrooms.
The building super structure is comprised of metal or wood roof decking supported by open web steel joists over concreteblocks. The exterior walls are brick walls.
No construction drawings were available for review at the time of the assessment. A Tar and Gravel roof membraneassembly system is present on the roofs.
The following is a list of architectural improvements that have been undertaken since the previous assessment in 2003:
- Phased re-painting to the school interior in 2003, 2004, 2006, 2008 and 2011;- Accessibility upgrades in 2004 and 2005;- Fencing renewal in 2007;- Partial lockers renewal in 2007 and 2009;- Science room retrofit in 2008;- Partial roof replacement in 2010 and 2013;- Partial window replacement in 2011; and- Partial asphalt pavement renewal (walkway and asphalt tracks) in 2012.
2. Mechanical Executive Summary
The primary source of heat within the building is hydronic, with a central plant and hot water piping distribution network.
Heating water that is provided by two Viessmann Vertomat VSB89 condensing boilers, with Weishaupt gas fired burners,is distributed throughout the school to linear fin tube wall mounted radiation units, wall and recessed convectors, unitheaters, and the heating coils of air-handling units.
Four Engineered Air LM series air-handling units with hydronic heating coils, located in mechanical rooms, provide heatingand ventilation for the gymnasium, change rooms, and the technical wing workshops. Two roof mounted Engineered Airseries DJ series gas fired air-handling units provide the heating and ventilation for the library and the cafeteria. Two CarrierHJ series packaged roof-top air-handling units provide air conditioning for the main administration offices and the secondfloor staff room. Dedicated ducts extend from the air-handling units to the areas being served and air is delivered to thevarious spaces through ceiling mounted diffusers.
Twenty two extract fans of various types, sizes and makes provide the general extract ventilation forclassrooms, washrooms, workshops, storage areas, change rooms, the kitchen and custodial areas.
All HVAC equipment is controlled by means of a Delta building automation system (BAS) and electronic control devices.
Two gas fired hot water heaters provide the domestic hot water for the facility. Both units are located in the main boilerroom and are interconnected.
• Unit 1 is an A.O. Smith, Model #BtH-199-100, with a capacity of 379 litres.• Unit 2 is a Bradford White, Model #EF100T199E3N2, with a capacity of 238 litre.
Hot and cold water services with respective sanitary drainage are provided throughout the school.
Dry chemical fire extinguishers and hose stations are provided for fire protection for the building.
There are no elevators or stair lifts in the school.

3. Electrical Executive Summary
High voltage electrical supply at 4160 volts is supplied to the facility from the Ontario Hydro distribution grid to two, on site,Board owned, 450 KVA transformers where it is reduced to 208/120 volts for general distribution. Transformers are locatedin the main electrical room in addition 3 and the sub-electrical room in addition 2. The two main switch panels in the schoolare each rated for 1,600 amps. Two separate feeds, 600 amp and 200 amps are supplied to the adjoining LakefieldIntermediate school.
The base building lighting system is comprised of suspended, recessed and surface mounted fluorescent lighting fixtureswith T8, 32W cool white lamps and electromagnetic ballasts. The emergency lighting consists of fixtures hard-wired tocentral battery packs. We understand that they are tested monthly and reported to be in operational working condition.Individual Energy Star labeled LED exit signs are provided at designated locations throughout the facility. Exterior lightingis provided around the perimeter of the building, and we understand that adequate illumination is provided in the majorityof required locations. Exterior lights are mainly controlled by the building automation system. Individual classrooms andcommon areas are equipped with manual control switches.
The fire alarm system, which jointly serves both the Lakefield Intermediate and the secondary schools is based on aSimplex 4100U fully addressable panel. The main panel is located in the secondary section of the facility. Alarm pullstations, smoke/heat sensors and bells with strobe lights are strategically located throughout the school.
The building is equipped with a motion sensing security system with key pad activation.
The school is fully wired for information technology and internet access.
4. Site Summary
The site area was reported to be ha. Site features include asphalt paved parking lot, driveways, asphalt pavedwalkways, concrete walkway, flower beds and flower boxes, trees along the property lines, and a chain link fence alongthe property lines. Paved and landscaped playgrounds including a running track, a baseball diamond and a soccer fieldare located to the south side of the property. The asphalt paved parking area is located to the west side of the property.
Definitions for Energy Checklist
Energy audit report: An ASHRAE Level I energy audit report was completed within the last three years.
Energy efficient boiler: The energy efficient boiler provided is a condensing boiler installed within the last five years or isenergy star rated.
Energy efficient domestic hot water heater: The energy efficient domestic hot water heater provided is direct or powervented natural gas fired or has an electric heat coil.
Energy efficient recovery system: The building is provided with a Heat Recovery Unit (HRU).
Energy efficient HVAC pumps and fan motors: The energy efficient HVAC pumps and fan motors are reportedly providedwith a variable frequency drive.
Energy efficient interior lighting: The provided interior lighting is controlled by motion sensors or building automation

system and/or the interior light fixtures are provided with T8 or T5 fluorescent lamps and electronic ballast.
Building Automation System: The building has a comprehensive Direct Digital Control (DCC) automation system to monitorand control the mechanical system.
Energy efficient faucets: Approximately 50% of the lavatory faucets are provided with aerators and motion sensors.
Energy efficient urinals and toilets: Approximately 50% of the urinals and toilets are provided with a low flow flush valve(less than 1.6 gpf)
Definitions for Accessibility Checklist
Designated parking space: The provided designated Barrier Free Accessible parking space is a minimum 2,400 mm wideand is clearly marked with an accessibility sign.
Path of travel to the main entrance door: The provided accessible path of travel from the designated Barrier FreeAccessible parking space to an accessible building entrance is a minimum 910 mm wide and includes curb cuts and ramps
Designated entrances: The provided designated Barrier Free Accessible entrance is a minimum 850 mm wide to allow amobility device, clearly marked with an accessibility sign and is provided with an automatic door open device.
Path of travel to all floors/elevations: The Barrier Free Accessible path of travel is provided with either an accessible rampor a vertical transportation device where a floor or an elevation difference exists.
Elevator: The provided Barrier Free Accessible Elevator has the following: clear audible communication indicating floorsand up/down direction; doors, which open long enough and a minimum 900 mm wide; and a control panel, which isprovided with Braille and an emergency call system and where the top is at a maximum height of 1,400 mm above floor.
Instructional spaces entrance doors: The instructional spaces are provided with an entrance door which is a minimum of850 mm wide.
Fire policy and fire safety plan: Fire policy and fire safety plans are reportedly in place for the evacuation of people withdisabilities.
Fire alarm system with strobe and audible signals: Fire alarm system is reported to include strobe lights and audiblesignals
Communal washrooms: There is a Barrier Free Accessible washroom stall, which is a minimum of 1,500 x 1,500 mm, inthe each boys and girls washroom on each accessible floor.
Designated washroom: A designated Barrier Free Accessible washroom is provided on each floor, and is equipped withthe following: an automatic door open device; grab bars; emergency call button; lever handle or motion sensor faucets;and a lavatory, where an insulated knee space is provided and the height of lavatory top is a maximum of 815 mm abovethe floor.

Limitations
This report has been prepared to meet the Ministry of Education (EDU) objectives for the Condition Assessment Programfor Educational Facilities in Ontario. The purpose of the Condition Assessment Program was to assess the current physicalcondition of the schools and associated site features, and to validate information currently contained in the online capitalrenewal database software Total Capital Planning Solution (TCPS).
The validation of data was limited to a five year period, which is defined as the current assessment year plus four years.Information contained in the database beyond this period was not validated or reviewed.
The provided event costs are intended for global budgeting purposes only. The event costs were adjusted to includeregional factors and were based on an approved unit cost list. Actual event costs for the work recommended may differsince the event costs can only be determined after preparation of tender documents, which would consider: specific designconditions, site restrictions, effects of ongoing building operations and construction schedule. The approved cost thresholdfor the Condition Assessment Program is $ 10,000.
Barrier Free Accessibility and Energy Conservation Measures assessments were limited to a preapproved checklistpresented on Page 2. The assessment of portables (classrooms not integrated with the building envelope), solarphotovoltaic panels, other solar energy collectors, wind turbines, sheds, less than 45 sq.m., play-equipment/structures,score boards, goal posts and flag poles, fire extinguishers, decommissioned swimming pools, window coverings,black/white boards, benches, gymnastic equipment and the appropriateness of room space were excluded from the scopeof work. Information related to these components contained in the database was not updated to reflect condition observed.Information related to events which are either planned or in progress, and currently locked were not updated.

1. Architectural & Structural Executive Summary
Information was gathered and observations were made by Pretium Anderson Toronto Inc. on August 06, 2013.
Lakefield Intermediate School is located at 71 Bridge Street, in Lakefield, Ontario. The original building was constructed in1975. The total building size is 3,090 square meters. The building is a 1-storey structure without a basement. The originalbuilding is likely constructed on concrete spread footings, where foundation walls are a mixture of concrete block masonry

and poured concrete. Information on the type and depth of the footings is not available. The educational facility houses alibrary, a gymnasium, a music room and art room, a home economics room, storage and change rooms, service rooms,student and staff washrooms, main office area and classrooms.
The building super structure is comprised of metal or wood roof decking supported by open web steel joists over concreteblocks. The exterior walls are brick walls.
No construction drawings were available for review at the time of the assessment. A Tar and Gravel roof membraneassembly system is present on the roofs.
The following is a list of architectural improvements that have been undertaken since the previous assessment in 2003:
- Roof replacement in 2004 and 2007; and- Window replacement in 2009.
2. Mechanical Executive Summary
The primary source of heat within the building is hydronic, with a central plant and hot water piping distribution network.
Heating water that is provided by two DeDietrick GT 339A gas-fired near condensing boilers, each with a maximum ratedoutput of 1,226 MBH is distributed throughout the school to linear fin tube wall mounted radiation units, wall and recessedconvectors, unit heaters, and the heating coils of four air-handling units. Three Dunham Bush and one McQuayair-handling uniut provide heating and ventilation to the library, music room, computer room and the gymnasium. Theair-handling units that serve the library and the gymnasium are located in the rooftop mechanical room. The units servingthe computer and music rooms are located at high level in storage areas adjacent to their respective service areas.De-stratification fans were present within the gymnasium. These fans serve to mix air within the large volume gymnasium.
Nine extract fans of various types, sizes and makes provide the general extract ventilation for washrooms, storage areas,change rooms and custodial areas.
There is no air conditioning in the school.
All HVAC equipment is controlled by means of a Delta building automation system (BAS) and electronic control devices.
One A.O. Smith power-vented gas-fired water heater, Model #PTV100-110, installed in 2008 with a storage capacity of371 litres, provides the domestic hot water for the facility. Hot and cold water services with respective sanitary drainageare provided throughout the school.
Dry chemical fire extinguishers and hose stations are provided for fire protection for the building.
There are no elevators or stair lifts in the school.
3. Electrical Executive Summary
The electrical systems are fully functional and, in general, are in fair condition.
Lakefield Intermediate school is provided with one 600 amp and one 200 amp unmetered 208/120 volt, 3 phase electricservice supplied directly from the adjoining Lakefield District Secondary School electrical distribution panel.
The base building lighting system is comprised of recessed and surface mounted fluorescent lighting fixtures with T8, 32Wcool white lamps and electromagnetic ballasts. The emergency lighting consists of fixtures hard-wired to central batterypacks. We understand that they are tested monthly and reported to be in operational working condition. Individual EnergyStar labeled LED exit signs are provided at designated locations throughout the facility. Exterior lighting is provided around

the perimeter of the building, and we understand that adequate illumination is provided in the majority of requiredlocations. Exterior lights are mainly controlled by the building automation system. Individual classrooms and commonareas are equipped with manual control switches.
The fire alarm system, which jointly serves both the Lakefield Intermediate and the secondary school, is based on aSimplex 4100U fully addressable panel. The main panel is located in the secondary section of the facility. Alarm pullstations, smoke/heat sensors and bells with strobe lights are strategically located throughout the school.
The building is equipped with a motion sensing security system with key pad activation.
The school is fully wired for information technology and internet access.
4. Site Summary
The site area was reported to be 7.64 ha. Site features include asphalt paved parking lot, driveway, asphalt paved walkways, a concrete walkway, flower beds and flower boxes, trees along the property lines, and a chain link fence along the property lines. Paved and landscaped playgrounds including a running track, a baseball diamond and a soccer field are located to the south side of the property. The asphalt paved parking area is located to the west side of the property.

Energy audit report: An ASHRAE Level I energy audit report was completed within the last three years.
Energy efficient boiler: The energy efficient boiler provided is a condensing boiler installed within the last five years or isenergy star rated.
Energy efficient domestic hot water heater: The energy efficient domestic hot water heater provided is direct or powervented natural gas fired or has an electric heat coil.
Energy efficient recovery system: The building is provided with a Heat Recovery Unit (HRU).

Building Automation System: The building has a comprehensive Direct Digital Control (DCC) automation system to monitorand control the mechanical system.
Energy efficient faucets: Approximately 50% of the lavatory faucets are provided with aerators and motion sensors.
Energy efficient urinals and toilets: Approximately 50% of the urinals and toilets are provided with a low flow flush valve(less than 1.6 gpf)

Limitations
This report has been prepared to meet the Ministry of Education (EDU) objectives for the Condition Assessment Programfor Educational Facilities in Ontario. The purpose of the Condition Assessment Program was to assess the current physicalcondition of the schools and associated site features, and to validate information currently contained in the online capitalrenewal database software Total Capital Planning Solution (TCPS).
The validation of data was limited to a five year period, which is defined as the current assessment year plus four years.Information contained in the database beyond this period was not validated or reviewed.
The provided event costs are intended for global budgeting purposes only. The event costs were adjusted to include

regional factors and were based on an approved unit cost list. Barrier Free Accessibility and Energy Conservation Measures assessments were limited to a preapproved checklistpresented on Page 2. The assessment of portables (classrooms not integrated with the building envelope), solarphotovoltaic panels, other solar energy collectors, wind turbines, sheds, less than 45 sq.m., play-equipment/structures,score boards, goal posts and flag poles, fire extinguishers, decommissioned swimming pools, window coverings,black/white boards, benches, gymnastic equipment and the appropriateness of room space were excluded from the scopeof work. Information related to these components contained in the database was not updated to reflect condition observed.Information related to events which are either planned or in progress, and currently locked were not updated.
