Brief HistoryFounded in 1942 by Dr. Peter Sammartino and his wife,Sylvia (Sally), Fairleigh Dickinson University has grown intothe largest private university in New Jersey. More than 12,000students from 44 states in the nation and 80 other countriesare enrolled on the University’s two campuses in northernNew Jersey and its overseas campuses in Wroxton, England,and in Vancouver, British Columbia, Canada. Beginning as atwo-year junior college, Fairleigh Dickinson expanded to afour-year curriculum in 1948 to fill the need for higher edu-cation in northern New Jersey. In 1954, the first graduateprogram, a master’s degree in business administration, wasoffered, and Bergen Junior College was purchased as a sec-

ond campus in Teaneck (Metropolitan Campus). In 1956,Fairleigh Dickinson gained University status and, one yearlater, the 178-acre Vanderbilt-Twombly estate was acquired toserve as the Madison location (Florham Campus).FDU became the first American university to own a campus inEngland when it acquired Wroxton College from Trinity Col-lege, Oxford University. Opened in 1965, Wroxton College offers American students an array of graduate and undergrad-uate programs as well as an enriching cultural experience.Formerly a 13th-century abbey, Wroxton College is now abeautifully restored and modernized Jacobean mansion.Recognizing that the student profile on most U.S. campusesis changing dramatically, the University’s New College ofGeneral and Continuing Studies, now known as Anthony J.Petrocelli College of Continuing Studies, was formed inApril 1998 to address the special educational needs of non-traditional students, including adult learners.In 2000, the Board of Trustees adopted a newly focused mis-sion to prepare students for world citizenship through globaleducation.The Vancouver Campus, which opened in 2007, offers un-dergraduate students one of the few opportunities to earnan American college degree while studying in Canada. Be-cause more than 30 percent of Vancouver’s residents hailfrom China and Southeast Asia, Vancouver Campus is partic-ularly attractive to international students from China, South-east Asia and Pacific Rim countries, but it is open to studentsfrom around the world.Each of the University campuses has developed a uniquecharacter and vitality. Students benefit from studying at acomprehensive University that offers a wide range of pro-grams and courses, but they also enjoy an atmosphere ofwarmth and personal attention usually found at much small-er institutions.Today, FDU offers nearly 100 degree programs at the associ-ate, baccalaureate, master’s and doctoral levels. Its widerange of offerings, coupled with the depth and expertise ofits faculty, has Fairleigh Dickinson University poised to servethe citizens of New Jersey and beyond.

Florham CampusThe Florham Campus, located in Morris County, N.J.,bridges the towns of Florham Park and Madison. The cam-pus consists of 178 acres, part of Florham, the former coun-try estate of early 20th-century socialites Florence Vanderbiltand Hamilton Twombly. Florham is a contraction of the firstnames of the couple. The campus has been in existencesince 1958.The Florham Campus is primarily a residential campus, sup-ported by a strong student life program. The campus has ac-tive fraternities and sororities, diverse student activities anddynamic Division III and intramural athletic programs. Resi-dent undergraduate students are joined in class by under-graduate commuter and adult and certificate students.Along with graduate students, they utilize first-class educa-

tional resources that are either retrofitted into the campus’sdistinctive and appealing original structures or set in state-of-the-art buildings, several constructed within the last decade.

Historic Landscape and ArchitectureFrederick Law Olmsted, renowned designer of New YorkCity’s Central Park, planned Florham’s landscape. The sur-viving principal buildings, designed in the Georgian Revivalstyle by the firm McKim, Mead and White in the 1890s, havebeen adapted for educational uses. The 100-room mansion(now Hennessy Hall), centerpiece of the campus, is the workof Stanford White and is similar in design to the 17th-cen t-ury addition to King Henry VIII’s Hampton Court Palace in England. Most of the interior decoration, such as thestaircases and fireplaces, are of Italian marble, worked bycraftsmen brought from the same country. In addition toclassrooms and offices, the mansion holds a chestnut-pan-eled billiard room (now Hartman Lounge) and the magnifi-cent Lenfell Hall drawing room.

NatureThe campus lies on a picturesque ridge; the terminalmoraine of the last major glaciation of 10,000 years ago. A diverse collection of specimen trees adds interest andbeauty to the scenery. Many large white oaks, some morethan 150 years old, shade some areas, and “Fossil Tree Hill”has fine specimens of the ginkgo and the dawn redwood(metasequoia). Wildlife is protected on the grounds, and deer,wild turkey and particularly groundhogs are frequently seen.More than 100 species of birds have been listed for the cam-pus. The Periodical Cicada (magicicada septendecim) makes aspectacular appearance every 17 years, but is not due againuntil the summer of 2030.

In BriefThere are more than 2,600 undergraduate and 1,500 gradu-ate students attending the Florham Campus. Minority stu-dents comprise about 10 percent of the student population.Sports include men’s Division III programs in baseball,basketball, cross country, football, golf, lacrosse, soccer,swimming and tennis; women’s Division III programs inbasketball, cross country, field hockey, golf, lacrosse, soccer,softball, swimming, tennis and volleyball; and intramuralsports in aerobics, basketball, bowling, flag football, golf,karate, racquetball, softball, street hockey, tennis, Timex fit-ness week and volleyball.Three out of every four freshmen live in campus housing.First-year students generally are assigned housing in theTwomblys, two traditional-style residence halls with doublerooms and a common bath. Upperclass students live in TheVillage, Park Avenue Residence Hall, featuring apartment-style suites, and the Rutherford Residence Hall.Campus activities include an active Greek community withnational social fraternities and sororities, as well as a nation-al service fraternity. The campus theater produces student-

written and directed plays, as well as student productions ofmajor plays.The Academic Support Center is housed in the MonningerCenter for Learning and Research, offering easy access tofree tutoring, word-processing support and special seminars.

Visitors to campus also would not want to missthe Italian and Clowney Gardens located along the sides andback of Hennessy Hall.The Roberta Chiavello Ferguson and Thomas George Fergu-son Recreation Center includes a gymnasium with three full-size basketball courts and seating for 2,000 spectators, anelevated jogging track, eight-lane competition swimmingpool, racquetball courts, weight rooms and fitness center. The John and Joan Monninger Center for Learning and Re-search includes the campus library and sunny Hassan Or-angerie, which originally provided the Florham estate withfresh citrus fruit through much of the year. The center hasextensive holdings supportive of the campus academic mis-sion and interlibrary loan arrangements with the other FDUNew Jersey campus and with library resources nationwide.The Science Building was adapted from the former garageand stables of the estate.Rutherford Hall, named for the University’s first campusand the newest Florham Campus residence hall, opened inSeptember 2003. Approximately 300 upperclassmen enjoyspacious double bedrooms with private bathrooms.The Stadler, Zenner, Hoffmann-La Roche Academic Build-ing features 20 state-of-the-art classrooms wired for comput-ers and offering interactive-television capabilities, seminarand conference rooms, a psychology laboratory and a stu-dent/faculty lounge, which includes a Nathan’s snack bar.

Preparing Expert Professionals BeyondMedication SafetyThe professional graduate curriculum within the programwill include classroom, online, laboratory and practice expe-

rience courses to educate the next generation of leaders inpharmacy to embrace the future. Students completing thedoctor of pharmacy degree program at FDU will demon-strate competencies in the following areas:

Foundational Knowledge: graduates will possess knowledge,skills, abilities, behaviors and attitudes necessary to apply thefoundational sciences to the provision of patient-centeredcare.

• LEARNER: Develop, integrate and apply knowledge fromthe foundational sciences to evaluate scientific literature, ex-plain drug action, solve therapeutic problems and advancepopulation health and patient-centered care.

Essentials for Practice and Care: graduates will possess theknowledge, skills, abilities, behaviors and attitudes necessaryto provide patient-centered care, manage medication-use sys-tems, promote health and wellness and describe the influenceof population-based care on patient-centered care.

• CAREGIVER: Provide patient-centered care as the medica-tion expert (collect and interpret evidence, prioritize, for-mulate assessments and recommendations, implement,monitor and adjust plans and document activities).

• MANAGER: Manage patient health care needs usinghuman, financial, technological and physical resources tooptimize the safety and efficacy of medication-use systems.

• PROMOTER: Design prevention, intervention and educa-tional strategies for individuals and communities to managechronic disease and improve health and wellness.

• PROVIDER: Describe how population-based care influ-ences patient-centered care and influences the developmentof practice guidelines and evidence-based best practices.

CurriculumThe curriculum will combine classroom learning enhancedby the use of advanced technology, online and practice ex-perience courses to increase active learning and providetime for managing learning opportunities. Courses that pro-vide integration of topics in basic sciences, pathophysiology,pharmacology, pharmaceutics, therapeutics and patient carewill be the foundation of the curriculum. Courses that in-clude topics to advance a student’s knowledge and skills inhuman behaviors, communication, informatics, data analyt-ics, medication safety, leadership and individualized patientcare are at the heart of the FDU curriculum.

Students are required to demonstrate competency in the fol-lowing 11 domains before they are eligible to progress to Ad-vanced Pharmacy Practice Experiences (APPEs):

1. Patient safety;2. Basic patient assessment;3. Medication information;4. Identification and assessment of drug-related problems;5. Mathematics applied to pharmaceutical calculations,

compounded medications, dose calculations and applica-tions of pharmacokinetics;

6. Ethical, professional and legal behavior;7. General communication abilities;8. Counseling patients;9. Drug information and literature research;10. Health and wellness — public health; and11. Insurance/prescription drug coverage.Students who are unsuccessful in their first attempt todemonstrate competency will be provided an opportunityfor an additional attempt. Students who do not successfullypass assessments after the two allotted attempts will be pro-vided an individualized plan for remediation. Following re-mediation, students will again be assessed for proficiency.

FDU AccreditationFairleigh Dickinson University is accredited by the Commis-sion on Higher Education of the Middle States Associationof Colleges and Schools and licensed by the New JerseyCommission on Higher Education.

Pharmacy AccreditationFairleigh Dickinson University has been a member of theMiddle States Council for Higher Education since 1948 andwas last reaffirmed by this organization in 2016, with a 10-yearreaffirmation offered during the last review conducted. InMay 2009, Fairleigh Dickinson University leadership filed aSubstantive Program Change report to Middle States re-questing authorization to grant the doctor of pharmacy degree. In September 2009, the University was granted au-thorization to offer the doctor of pharmacy as one of its degree offerings.

Simultaneously, the University leaders sought approval fromthe state of New Jersey through its educational governingbody, the New Jersey Presidents’ Council (NJPC) for ap-proval to award the doctor of pharmacy degree. An applica-tion was filed with the Academic Issues Subcommittee of theNJPC in April 2009 and was subsequently approved by theAcademic Issues Subcommittee and then by the NJPC atlarge during their June 2009 meeting.

Oath of Student PharmacistsThe revised Oath was adopted by the American Associationof Colleges of Pharmacy (AACP) House of Delegates in July2007 and has been approved by the American PharmacistsAssociation.

Background/Security ChecksAdmissions Candidates and Admitted StudentsCertiphi Screening, Inc. has been selected by the AmericanAssociation of Colleges of Pharmacy (AACP) as its partner inthe provision of a centralized student-screening program forits member institutions. As a member of AACP, Certiphi pro-vides customized background-screening services solely to col-leges, universities and health care organizations nationwide.AACP has developed a centralized student background-check program collaboratively with Certiphi® that is de-signed to streamline the background-check process for bothschools and students; to lower the cost to student pharma-cists; and to help experiential-practice sites ensure patientsafety and compliance with state law and Joint Commissionstandards regarding student screening.

The AACP student background-check program will runthrough PharmCAS, AACP’s centralized application systemand allows the school to select the background-check infor-mation needed. The school works collaboratively with Cer-tiphi to upload the pharmacy program students into theCertiphi database for background checking. Backgroundchecks will be requested from the candidate upon admis-sion. Results will be submitted directly to the pharmacy pro-gram.

The criminal background check will review a person’s crimi-nal history seven years prior to the date of application. Thecheck will include the cities and counties of all known resi-dences. The following criteria will be included:

• Social Security number validation• Felony convictions• Misdemeanor convictions or felony adjudications involv-

ing crimes against persons (physical or sexual abuse)• Misdemeanor convictions related to moral turpitude

(prostitution, public lewdness/exposure, etc.)• Felony deferred adjudications for the sale, possession, dis-

tribution or transfer of narcotics or controlled substances,and involving crimes against persons (physical or sexualabuse)

Before entering the pharmacy program, admitted studentswill be required to sign a statement of acceptance of theterms and conditions of this Background/Security Checks

and Drug Screening Policy. Students may not attend classesuntil this statement is signed.

Certificate of Physical ExaminationA certificate/proof of a physical examination performed annually between April 1 and April 30 is required for all students in alignment with experiential contract site require-ments. The student will incur the cost of all physical exami-nations, and certification of physical examinations will needto be submitted to the experiential education staff by in-structed deadlines.

Dress CodeStudents in the pharmacy program must maintain a neatand clean appearance befitting students attending a profes-sional school. Therefore, attire should convey a professionalappearance in classes, laboratories or on a pharmacy-relatedexperience. Refer to the course syllabus for specific details.

Students must wear their white clinical jackets during labsand at experiential sites unless otherwise instructed by theirpreceptor and as requested by administration or faculty dur-ing school-related events.

Employed students are not to wear their FDU white lab coatduring work hours at their respective places of employment.

Electronic and Other Mobile DevicesThe University recognizes that students possess electronicdevices with video, camera or voice-recording capabilities. Insupport of each individual’s reasonable expectation of priva-cy, the copyright and intellectual property laws, the use ofthese cellular phone features by FDU students must be inconjunction with expressed consent. Students are expresslyforbidden to video, use camera or voice recordings withoutthe express consent of the subject(s) being photographed orrecorded. Any student whose use of his/her electronic devices violatesanother’s reasonable expectation of privacy or produces anymedia as a result of the cellular phone capabilities withoutexpress consent may be found in violation of this policy. Vio-lations of this policy may lead to disciplinary action.

EmailAll entering students are issued a University email account.Important college and University information will be sentusing these email accounts; therefore, it is important for stu-dents to check their accounts at least once each day. Any offi-cial University business being sent by email will be sent to thestudent’s FDU email address. Students must use their FDUemail address for email correspondence within the University.Students are expected to maintain professional behavior inall email correspondence. All students must comply with theUniversity’s acceptable use of computer and email.

Food in Academic SettingsOther than drinks, no food is allowed in classrooms unlessprior approval is given. Food and drinks are not permittedin laboratories.

IdentificationStudents must have identification cards with them at alltimes. Identification will be issued by the University and mustbe available while on campus and during IPPEs and APPEs.

Medical ExemptionsIf the student is claiming an immunization exemption relat-ed to a pregnancy or other medical contraindication, thestudent must provide documentation from his/her healthcare provider indicating the reason and time limitations thatthe vaccination is medically contraindicated. This exemptionwill be reviewed at the beginning of each semester. A med-ical exemption may result in delayed rotation placementand/or graduation.

Leave of AbsenceA leave of absence allows students to interrupt their gradu-ate studies if necessary. A leave of absence is granted for onesemester with the privilege of renewal for one more consec -utive semester. A request for a leave of absence for the cur-rent semester must be received prior to the last day ofclasses. Request for a leave of absence must be approved inwriting by the assistant dean of student affairs or his/her de-signee. (Information on procedures for requesting a leave ofabsence is available from student affairs.) Students wishingto renew their leave of absence must also do so in writing.

Students who have been granted a leave of absence will bemaintained as students in good standing during the semes-ter in which a leave of absence is taken but will be discontin-ued from graduate study unless the students register thesemester following a leave of absence or request and isgranted an extension of the leave of absence.

Photography, Recordings and Video TapingStudents wishing to make recordings of lectures in classshould consult respective course syllabi. Permission torecord classes and lectures may be granted with the inten-tion as a supplement to the educational process and are nota substitute for class attendance, class readings, assignmentsand projects. They may not be used as a basis for challeng-ing student assessments.

In order to facilitate student learning, recordings will bemade of classes that meet in lecture halls and other roomswithin the school. Regularly scheduled lectures and classmeetings will be recorded. Class recordings will not be dis-seminated and will be made available to students on-site onlyin the case of course remediation.

Social Media and Personal WebsitesThe Doctor of Pharmacy Program recognizes that teammembers may use social media to participate in discussions.If an individual chooses to identify himself/herself as a phar-macy student while participating in social media discussions,he/she must do so within the guidelines of this policy.

However, students should be aware that the improper and in-appropriate use of social media can pose tremendous risks toboth the Doctor of Pharmacy Program and the University,including:

• Infringement of third-party intellectual property rights,• Harassment,• Privacy violations and• Potential damage to reputation and brand.Social media refers to the external online tools used to sharecontent, profiles, opinions and experiences. Social mediatools include, but are not limited to:

• Professional networking sites (e.g., LinkedIn, Plaxo);• Social networking sites (e.g., Facebook, MySpace, Bebo,

Friendster);• Video- and photo-sharing websites (e.g., Flickr,

YouTube);• Micro-blogging sites (e.g., Twitter);• Personal websites and blogs; and• Forums and discussion boards (e.g., Yahoo! Groups,

Google Groups, Yelp).Disclosure of any confidential material, including but notlimited to: academic course content, student or faculty per-sonal information, patient records or other information inviolation of Health Insurance Portability and AccountabilityAct (HIPAA) policy (for a better understanding of HIPAApolicies and regulations, see the appropriate documentswithin the Doctor of Pharmacy Program). If a student is un-sure of whether a potential posting is in violation of this pol-icy, consult the Office of Student Affairs before makinginformation public.

Guidelines for Posting and Reposting Material Be honest. If students are blogging about their work at FDU,they should use their real names, identify their roles withinthe school and be clear about their roles. Do not pretend tobe an acting pharmacist. Also, students should state thattheir opinions are their own and do not represent those ofthe Doctor of Pharmacy Program or Fairleigh DickinsonUniversity.

Do not share confidential information.

Write what they know. Just by identifying themselves as studentpharmacists, they are creating perceptions about their expert-ise. Be sure that all content associated with students is consis-tent with their work and the Doctor of Pharmacy Program.

What students write is their responsibility. Participation on be-half of the school is not a right but an opportunity, so treat itseriously.

Think before posting anything. If students are about to pub-lish something that makes them even the slightest bit un-comfortable, take a minute to review these guidelines anddetermine what needs to be changed, then fix it.

Use discretion when posting on social media platforms. Anysocial media access at the student’s site should be conductedin accordance with policies and regulations set forth by thesite itself. It is recommended that no social media updatesbe conducted while in the classroom or on site.

Respect everyone. Do not post anything that would be foundoffensive or intimidating to others, including but not limitedto ethnic slurs, sexual innuendos, discriminatory comments,insults, threats or obscenity.

Defer to media relations when needed. Defer to appropriatesources when needed. If students are asked to speak on be-half of the school or University, consult the Division of Stu-dent Affairs prior to making a formal comment.

Student GrievancesIf a student has a grievance concerning issues other thangrades that he/she wishes to pursue, the following protocolshould be followed:

• If the grievance involves another student, please refer tothe Code of Student Rights, Responsibilities, and Conduct,found in the Student Handbook, which can be accessed onlineat www.fdu.edu/pharmacy/current-students.• If a student wishes to report a complaint about an employ-ee of the University, faculty or staff or a University vendor,the student should report this complaint to the dean of theSchool of Pharmacy & Health Sciences. If the student wishesto report a grievance involving an employee of the Universi-ty, faculty or staff or a University vendor specifically relatedto a violation of University policies and procedures, harass-ment and/or discrimination, please contact the University’shuman resources department at 201-692-2704.

American Council on PharmaceuticalEducation (ACPE) Complaint Policy Students in the Doctor of Pharmacy Program wishing to filea complaint can do so at any time. It is recommended, how-ever, that students make every effort to solve problems infor-mally, using this guide as a reference.

The ACPE has an obligation to assure itself that any institu-tion which seeks or holds a preaccredidation or accredita-tion status for its professional program(s) conducts its affairswith honesty and frankness. Complaints from other institu-tions, students, faculty, or the public against a college orSchool of Pharmacy & Health Sciences, including tuition

and fee policies, and as related to ACPE standards, policiesor procedures, shall be placed in writing in detail by thecomplainant and submitted to the ACPE office. The com-plaint shall be submitted to the institution for response. Re-quests for confidentiality shall be respected to the extent anysuch information is not necessary for the resolution of thecomplaint.

The Executive Director shall, based upon the complaint, theresponse and information from such further investigationdeemed necessary, promptly determine the facts surroundingthe issue, determine the validity of the complaint and resolvethe issue; provided, however, where the Executive Directordeems it necessary or appropriate, the matter shall be consid-ered at the next regular meeting of the Council. The timeframe for resolution is generally within six months. A recordof complaints regarding a specific college or School of Phar-macy & Health Sciences, including student complaints re-ceived or made available, is kept for consideration on file atthe Council office. Such record of complaints are consideredduring scheduled evaluations or a special evaluation, as thecase may require.The procedure shall provide for treatment of complaints ina timely manner that is fair and equitable to all parties. Thecomplainant shall be advised of the decision or action assoon as possible. When the ACPE has cause to believe thatany institution with which it is concerned is acting in an un-ethical manner or is deliberately misrepresenting itself tostudents or the public, it will investigate the matter and pro-vide the institution an opportunity to respond to the allega-tions. If, on the basis of such investigation, after notice tothe institution and opportunity for institutional response,the ACPE finds an institution has engaged in unethical con-duct or that its integrity has been seriously undermined, theACPE will either:Request that the institution show cause, within a stated timeperiod, why adverse action should not be taken, or In extreme cases, immediately discontinue its relationshipwith the institution by denying or withdrawing pre-accredi-tation or accreditation status.A complaint against a college or a School of Pharmacy &Health Sciences must be related to the standards or the poli-cies and procedures of ACPE and must be submitted in writ-ing to the Executive Director. Under existing practices,when a complaint is received, it is submitted to the collegeor school affected for response. If, thereafter, based uponthe complaint and the response, the Executive Director de-termines that a complaint is not related to the standards orpolicies, the complainant is so advised in writing with a copyto the school or college, and the matter is treated as re-solved.Anonymous complaints pertaining to accreditation mattersare retained and, depending on circumstances, may or maynot be forwarded to the school or college involved, depend-

ing somewhat on the severity of the complaint. This decisionis made by the Executive Director. Where a complainant hasthreatened or filed legal action against the institution involved,the ACPE will hold complaints in abeyance pending resolu-tion of the legal issues and the complainant is so advised.

If the Executive Director finds a complaint to be extremelyserious in nature charging egregious conduct that may war-rant adverse action by the Council, or involves an interpreta-tion which the Executive Director believes should be madeby the Council, the complaint will be submitted to the Coun-cil for determination at the next regular meeting. Extraordi-nary remedies available for complaints covering extremecases are set forth above.

The ACPE has an obligation to respond to any complaintswhich may be lodged against it by any institution, student,faculty or third party in respect to the application of ACPE’sstandards, policies and procedures where the complainingparty is directly affected thereby. Any such complaint shallbe submitted in writing. The Executive Director shallpromptly determine the facts surrounding the issues andshall attempt to resolve the matter in consultation with thePublic Interest Panel established pursuant to Article V ofthe ACPE By-Laws. Complaints which cannot be resolved bythe Executive Director shall be considered and resolved atthe next regular meeting of the Council. The time frame forresolution is generally within six months.

Technical Standards for Doctor ofPharmacy ProgramAll students entering into the Doctor of Pharmacy Programmust meet all of the eligibility requirements for admission.Because of the nature of the program, in addition to the ac-ademic requirements of the program, several additional re-quirements have been set in place to assure that studentsreceive the highest quality educational experience while si-multaneously assuring the safety of their patients and thegeneral public. Final consideration regarding compliancewith all technical standards is based upon patient safety,which is a primary focus within the pharmaceutical educa-tion curriculum and process. All students entering into the Doc-tor of Pharmacy Program must have:

1. An up-to-date record of immunizations on file with theFDU Office of Student Health Services.

Because students will be practicing in a number of differentpublic health settings, immunization is of critical importanceto prevent the spreading of certain communicable diseases.For a list of required immunizations, you may consult the as-sistant dean for student affairs or a member of ExperientialEducation.

2. A successful criminal background check. All students must authorize the school to obtain a criminalbackground check prior to admission into the school. Allstudents may be subject to additional background checks asper the requirements of the school or its experiential affilia-tion contract requirements. Experiential practice sites mayrequest results of background check(s), which are to beprovided by the student. A student’s eligibility to enroll in,or disqualification from, the program will be made on an in-dividual basis. No single type of offense will be used as an au-tomatic rescission of acceptance or reason for dismissal.However, the school may rescind an offer of admission tothe program if an applicant’s background history check re-veals that the applicant has committed a serious offense. Se-rious offenses include, but are not limited to, felonies, drugoffenses, forgery, assault, abuse and neglect.

3. Regular access to transportation. As part of their program, students will regularly travel off-siteto practice experiences throughout New Jersey and the re-gion. For this reason, it is essential that a student have regu-lar, on-demand access to transportation to complete thepractice experience component of the program.

4. Health care insurance. All students are required to possess and maintain a healthcare insurance policy during their enrollment in the pro-gram.

Additionally, due to the interactive, critical and technical nature ofpharmacy, there are several additional criteria that a candidate mustmeet in order to complete the classroom, laboratory and practical as-pects of the program.

1. Sensory and ObservationalStudents must be able to observe demonstrations and con-duct exercises in a variety of areas related to contemporarypharmacy practice, including, but not limited to:

• Monitoring of drug response and preparation of special-ty-dosage forms;

• Observation of demonstrations and experiments in thebasic and pharmaceutical sciences, medical illustrationsand models;

• Observation of patients accurately at a distance and closeat hand, noting nonverbal as well as verbal signals;

• Vision-related observation including, but not limited to,visualizing and discriminating findings on monitoringlaboratory and diagnostic tests; reading written and illus-trated material; discriminating numbers and patterns as-sociated with diagnostic and monitoring instruments andtests; observing the activities of technical staff operatingunder their supervision; reading information on a com-puter screen and small print on packages or package in-serts; distinguishing shapes, colors, markings and othercharacteristics of small objects (e.g. different dosage

forms); and competently using instruments for monitor-ing drug response; and

• Utilization of other sensory modalities for physical assess-ment and dispensing, and other forms of practice.

2. Mobility

Students must have the capacity to:Demonstrate fine and gross motor skills and coordinationfor the purposes of preparing, compounding and adminis-tering medications; interacting with and caring for patientsand operating equipment essential to the practice of pharmacy including, but not limited to, compounding pre-scriptions, filling prescriptions, counting prescription medications, administering medications, preparing intra-venous products and administering intramuscular and sub-cutaneous injections. The student must be able to conduct aphysical assessment of a patient by palpation, auscultationand other diagnostic maneuvers. Other motor activities in-clude performing first aid and/or cardiopulmonary resusci-tation in the clinical setting.

• Show an ability to function and maneuver within a practicesetting, demonstrating the necessary dexterity, stamina,stability and mobility while standing, sitting and walking(either alone or assisting patients in need of support);

• Move or position a patient in a manner that facilitatesphysical assessment or diagnosis;

• Utilize computer-based information systems with ade-quate motor function to perform data entry and analysis;

• Transport himself or herself to off-site settings and expe-riential locations in a timely manner; and

• Respond promptly to urgencies within the practice set-ting requiring rapid and accurate preparation of appro-priate emergency medication and preparation of sterileintravenous medications.

3. CognitionAll students must be able to perform the following, but notlimited to:

• Assess, measure, calculate and analyze and process rele-vant data and facts for the purposes of problem solvingand decision making;

• Concentrate for extended periods of time, often in thepresence of distractions and high levels of volume;

• Demonstrate the ability to multitask in a stimulating andstressful environment;

• Show the ability to solve problems involving scientificprinciple and observation with proficiency and accuracy;

• Calculate appropriate dosages for a variety of patient-specific conditions;

• Retain and recall critical information in an appropriatemanner; and

• Interpret graphs or charts describing biologic, economicor outcome relationships.

4. BehavioralStudents within the program must:

• Possess the emotional health and stability to fully utilizetheir intellectual abilities, exercise good decision-makingand sound-judgment skills at all times, and develop effec-tive relationships with patients and inter-professional col-leagues;

• Demonstrate the ability to work focused and poised inemergency situations coping and adapting to changingenvironments and displaying flexibility and function inthe face of uncertainties inherent in the academic andclinical environments for the safety of their patients andthe general public;

• Respond effectively to constructive criticism and evalua-tion of performance;Respect and adhere to the American Pharmacists Associa-tion Oath of a Pharmacist and code of ethics stated therein;

• Always maintain a level of professionalism and respectfor faculty, colleagues and patients; and

• Recognize and demonstrate respect for differences inculture, values and ethics among patients, faculty, peers,clinical and administrative staff and colleagues.

Enrolled students are expected to maintain and demon-strate all technical standards through their program and incourse work, interaction with peers and faculty and in theirintroductory and advanced pharmacy practice experiencesas referenced in experiential syllabi.

The ability to maintain the technical standards is a separatecomponent from academic progression; therefore maintain-ing a passing grade point ratio is not sufficient.

Enrolled students who are unable to meet one or more ofthe foregoing standards are subject to action such as remedi-ation, counseling or dismissal from the school.

A student with a medically diagnosed disability may functionin the program as long as the condition is under sufficientcontrol to allow meeting the above listed standards with orwithout reasonable accommodation. Academic Intervention Any student who fails an exam or assignment within a didac-tic course is encouraged to attend the course instructor’s in-person or virtual-office hours within 10 days of the return ofa failing grade. The student will work with the faculty mem-ber to assemble a plan of action for remediation that may in-clude, but is not limited to:

1. One-on-one meetings with the faculty member to reviewtopics of difficulty and go over the assignment in question;

2. Referral to campus support services for help in areas ap-propriate to its purview (including writing help, basicmath skills and some foundational science assistance);and

3. Development of a study plan to supplement course workwith additional assignments, review and resources.

Any student failing a midpoint or final evaluation within apractice must contact the assistant dean for experiential edu-cation within one week of receipt of the failing grade.

Academic LoadDue to the academic cohort nature of this program, stu-dents must maintain full-time academic standing in relationto the progression of the doctor of pharmacy degree. Full-time standing will be defined on a semester-by-semesterbasis.

Academic load exceptions will only be given through writ-ten approval from the dean of the School of Pharmacy &Health Sciences or the Student Affairs and Academic Pro-gression Committee.

Academic Probation*Good academic standing is defined as achieving cumulativegrade point ratio (GPR) of 2.75 or above at the end of anysemester and/or remediation period.

The Student Affairs and Academic Progression Committee re-views the progression of all students in the pharmacy program.

1. A student will be placed on academic probation whenone or more of the following occurs:• The student has failed course(s) or remediation for acourse.• The student’s cumulative grade point ratio (GPR) fallsbelow 2.75.

2. During the time a student is on academic probation, thestudent must meet with the assistant dean for student affairsand/or other deans or their designee(s) to discuss theircourse of action for returning to good academic standing.

3. Any student who is placed on academic probation or re-quired to meet with the Student Affairs and AcademicProgression Committee, must meet with the School ofPharmacy & Health Sciences financial aid counselor todiscuss any possible financial aid consequences.

4. A student may be removed from academic probationwhen the specific cause for being placed on academicprobation has been removed, that is:• The student passes the course that was previouslyfailed; or• The student’s GPR is at or above 2.75; or• For either cause, the student complies with any addi-tional requirements from the assistant dean for studentaffairs and/or other deans or their designee(s).

Attendance PolicyStudents are expected to attend — on time — each andevery pharmacy course in which they are enrolled, as atten-dance and participation are essential to academic progress.Individual instructors may include attendance and class par-ticipation in the determination of the final grade. Each in-structor will announce his/her grading policies at thebeginning of the semester in each didactic course, makingclear the weight to be given to attendance and participationin grade determination.

Students are expected to attend — on time — each andevery exam. It is the responsibility of the student to notifythe instructors or the School of Pharmacy & Health Sciencesbefore the beginning of the exam if unexpected circum-stances prevent the student from attending the exam ontime. In case of medical emergency, the student must notifythe instructors and course coordinator as soon as possible,but it must be within 24 hours of the exam. The studentsmust provide documentation within 72 hours of the exam.Otherwise, no make-up exam or course remediation will beprovided to the student. If a student is allowed to take make-up exam(s) after final exams, there may not be an opportu-nity for remediation, due to scheduling, in the event thatthe student does not successfully complete the course.

In addition, students should refer to the course syllabi forclarification of attendance and professionalism policy for allexperiential courses.

Students must register for courses each semester no laterthan the deadline set by the Office of Enrollment Services,which is posted on the Academic Calendar found online atwww.fdu.edu/pharmacy.

Only registered students are permitted into the classroom —children, guests, pets (except service dogs), etc., are specifi-cally excluded unless written permission is given by the assis-tant dean of student affairs and programmatic effectiveness.

Candidacy for a DegreeA Declaration of Candidacy form, accompanied by an appro-priate fee, must be filed in the Office of Enrollment Servicesno later than eight months preceding graduation. Filingdates must be obtained from the Office of Enrollment Serv-ices. Degrees are awarded September 1, February 1 and theday of commencement in May.

Course RemediationDue to the nature of the program, it is critical that studentsdevelop a level of mastery within a course of study prior tohis/her progression within the program. Any student whodoes not receive a grade of C or higher for a course will beissued an “Incomplete” grade. The student will then be of-fered the opportunity to register for an independent studycourse (one credit, billed at a per-credit rate). If a studentdeclines a course remediation, they will receive a grade of“F” and be required to re-enroll in the course the next academic year when it is offered. This may result in thedelay of graduation by one year. No student will be allowedto remediate more than one course per semester. Remedia-tion will take place prior to the start of the following aca-demic semester. Students are limited to a maximum ofthree remediation courses throughout the course of theDoctor of Pharmacy Program.

Faculty will determine the format and content of the reme-diation course of study to assist students in the clarificationof the course material within the respective course.

Students in need of remediation who are scheduled to par-ticipate in IPPEs or APPEs during designated remediationtimeframes will be contacted by the experiential educationstaff to discuss site reassignment. Such reassignments are atthe discretion of experiential education and based on siteavailability.

At the end of the remediation period, students will be givenan assessment to determine their mastery of the course ma-terial. If a student completes the assessment with a passingscore, the “Incomplete” grade for the course will be convertedto a C. A student will receive a letter grade for a one-credit, in-dependent study course. If a student does not show proficien-cy to pass the course, both the “Incomplete” grade and theindependent study course grade will be converted to an Fand the student will be required to re-enroll in the course.

If the course failure is an IPPE or APPE, there will be no re-mediation provided. Inability to achieve milestone assess-ments may require additional IPPE or APPE course work todemonstrate competency. If the student fails an APPE course,the student will be referred to the Student Affairs and Aca-demic Progression Committee immediately. The studentmust reenroll in the appropriate IPPE or APPE course dur-ing the next academic year, and will be charged at per-creditrate, provided that the student remains in good academic

standing. Failure of an APPE rotation will result in delayedgraduation.

Course Requirements, LateAssignments and ExaminationsEach student is responsible for meeting the requirements asdescribed in each course syllabus distributed at the begin-ning of each semester.

Cumulative Grade Point RatioThe student must maintain a cumulative grade point ratioof 2.75 (on a 4.00 scale) throughout the program. Studentprogress is evaluated at the end of each semester.

Degree Requirements1. Students must complete all required credits for the phar-

macy program. 2. A cumulative GPR of at least 2.75 is required for gradu-

ation from the School of Pharmacy & Health Sciences. 3. A graduate program of study must be completed within a

period of seven years from the time the student first reg-isters in the pharmacy program.

Dual Degree Programs Admissions Requirements 1. Students interested in applying for a dual degree must

have a cumulative grade point ratio (GPR) of 3.00 orhigher after the spring semester of P1 year and meetthe GPR and testing requirements of the graduate program.

2. Applications for dual-degree programs will only beconsidered for admission at the end of the spring semester of P1 year. After this time, no applications for dual degrees will be accepted.

Master’s Degree Requirements1. Students must complete all required credits for the

program at Fairleigh Dickinson University other thanthose for which transfer credits have been awarded.

2. A cumulative GPR of at least 2.75 is required for gradu-ation from all colleges except the Silberman College ofBusiness, the School of Public and Global Affairs andcorporate and organizational communication program(Maxwell Becton College of Arts and Sciences) and theHenry P. Becton School of Nursing and Allied Health’sDoctor of Nursing Practice (DNP) program, which re-quire 3.00. Students who receive two or more C gradeswill be warned, and if evidence of definite improve-

ment in their academic performance does not ensue,they will be asked to withdraw from graduate study.

3. A graduate program of study must be completed with-in a period of five years from the time the student firstregisters for graduate study. Exceptions to the five-yearlimitation must be requested in writing to the dean orthe director and the student’s major department chair-person or school director. The Silberman College ofBusiness has a seven-year limitation.

Maintenance of Matriculation Status1 In order to maintain matriculation status, students

must register consecutively for the fall and spring se-mesters. If consecutive registration is not maintained,students must reapply to the Admissions Office. It isrecommended that graduate students who find it nec-essary to absent themselves from a major semester atthe institution register for matriculation maintenance,thereby retaining active status. This process also pro-vides the opportunity for the students to avail them-selves of the use of various University facilities, i.e.,recreational facilities and the libraries.

2. The appropriate graduate department will be notifiedif a student is no longer eligible to participate in thedual-degree program.

3. If a student withdraws from or changes their dual de-gree, there may be an impact on the student’s originalgraduation date.

4. If a dual-degree student does not complete the master’sdegree at the same time they complete the Doctor ofPharmacy degree, they will pay the per-credit tuitionrate of the graduate program.

Early AlertStudents are expected to monitor their own progress andacademic standing in the courses they take. This will en-able them to assure their own compliance with program re-quirements and their maintenance of acceptable grades.Whenever a student feels the need for academic guidance,he/she is encouraged to contact the instructor of the par-ticular course.

Post-Midterm GradingStudents have the ability to access their grades using Web-Advisor following the midterm examination period. If a student is in jeopardy of failing two or more courses, thestudent will be contacted by the assistant dean for studentaffairs and/or other deans for an in-person meeting to discuss strategies for success and remediation.

Elective Course PlacementAt the end of each academic year, students must meet minimum requirements, which ensure students are on

track for successful completion of the Doctor of Phar-macy program. Minimum requirements include success-ful performance in the Pharmacy Curriculum OutcomesAssessment (PCOA) and a grade of C+ or higher in selectcourses listed below. Minimum criteria for each academicyear are detailed below.

Minimum requirements at the end of each academic year are:

1. P1 year: satisfactory PCOA performance and coursegrade of C+ or higher in select core pharmacy courses(PHRM6102, PHRM6103, PHRM6104, PHRM6211 andPHRM6212)

2. P2 year: satisfactory PCOA performance and coursegrade of C+ or higher in select core pharmacy courses(PHRM7105, PHRM7106, PHRM7107, PHRM7108 orPHRM7120)

3. P3 year: satisfactory PCOA performance

Students who do not achieve minimum requirements at theend of P1 year may be ineligible for the dual degree andlose their ability to select elective course work during theirP2 didactic curriculum. Students who do not achieve mini-mum requirements by end of P2 year lose their ability toselect a portion of their elective course work during theirP3 didactic curriculum. Students will be placed into elec-tive courses designed to strengthen skills and knowledgenecessary to meet objectives and core tenets of the pharma-cy program. Students who fail to achieve minimum criteriaat the end of P3 year will lose ability to select their experi-ential elective course work.

Grade AppealsPlease refer to the Student Grade Appeals/Procedure lo-cated in the Academic Regulations section of the FDU Stu-dent Handbook. The Student Handbook can be found onlineunder Academic Resources at www.fdu.edu/pharmacy/current-students.

Grading PolicyLetter grades will be assigned by course instructors (coordi-nators) based on students’ performance. Grading policy isnoted in the syllabus of each course.

Progression in the Pharmacy ProgramStudents must achieve a grade of C or higher in order to passa course. Students who fail to achieve a minimum cumulativegrade point ratio (GPR) of 2.75 at the end of any semesterand/or remediation period will be placed on academic pro-bation until they achieve a cumulative GPR of 2.75 or higher.Students placed on academic probation are not consideredto be in good academic standing, and thus, are not permittedto enroll and participate in IPPE or APPE courses. Due to po-tential impact on overall GPR, students who fail a course runthe risk of jeopardizing their progression in the Doctor of

Pharmacy Program. A course can only be repeated once. Acumulative GPR of 2.75 must be attained by all professionalgraduate students prior to graduation.

A student will be subjected to academic review from thepharmacy program if he/she fails to:

1. Maintain a cumulative GPR of 2.75,

2. Attend class or

3. Maintain continuous registration.Any student not achieving a letter grade of C or higher intwo or more didactic courses by the end of each semesterwill have their academic performance reviewed by the Stu-dent Affairs and Academic Progression Committee and benotified of the committee’s recommendation on progres-sion in the pharmacy program

Fairleigh Dickinson University, through its various facultiesor appropriate committees, reserves the discretionary rightto suspend or dismiss any student from the University forfailure to maintain a satisfactory academic record, accept-able personal behavior, acceptable standards of academicconduct, acceptable standards of practice in a clinicalagency or satisfactory standards of health.

Suspension or Termination from thePharmacy ProgramA student may be suspended or terminated from the Doctorof Pharmacy Program if the student is not in good academicstanding at the end of any semester. Action of suspension ortermination will be determined following evaluation of in-class performance and a determination by the Student Affairsand Academic Progression Committee. If a student is not seento be progressing through the program toward successfulcompletion, he/she may be suspended or terminated fromthe program following a formal hearing with this committee.

Any student whoenrolls in the program knowing that he or she will not sub-mit proof of the required screening or immunization as-sumes the risk that he or she may be unable to complete theprogram and obtain the degree. Individuals with questionsor concerns about their ability to meet these standardsshould contact the Experiential Education staff.

Background Check/DrugScreening/Student PharmacistProfessional Liability InsuranceAffiliated hospitals and other health care organizations oper-ating under the Joint Commission require criminal back-ground checks and/or drug screenings for all individualswho have direct contact with patients, including pharmacystudents placed in IPPE and/or APPE rotations at these

sites. In addition, other experiential sites may require back-ground checks and/or drug screenings as per their affilia-tion agreements with the University.

Enrolled students in the pharmacy program must satisfacto-rily complete a criminal background check and/or drugscreening from approved vendors, prior to participating inscheduled IPPEs and/or APPEs. Background checks anddrug screens are performed on at least an annual basis, butmay also be required at other times as requested in writingby the University or affiliated site depending upon the spe-cific requirements of those sites. Students are responsible forall associated costs.

All drug screenings and background checks are conducted byan approved vendor, and results are forwarded to ExperientialEducation. Once received, Experiential Education will notifyaffiliated sites of completion. Students with disqualifying ad-verse findings and/or drug screening results will be advised ofthe outcome. Discrepancies and findings that may deter phar-macy program progression relative to experiential affiliationsite contracts will be discussed with the student by a School ofPharmacy & Health Sciences designee. Experiential practicesites may request results of drug screening(s) and/or back-ground check(s), which are to be provided by the student.

Students are also required to obtain student pharmacist pro-fessional liability insurance, at their own expense, for the du-ration of their time in the program. Students will not be ableto participate in IPPEs or APPEs until drug test, backgroundcheck and proof of student pharmacist professional liabilityinsurance requirements have been met as determined by Ex-periential Education staff in conjunction with experientialsite affiliation contracts. Please contact Experiential Educa-tion for additional information regarding backgroundchecks, drug screenings and student pharmacist professionalliability insurance.

Clinical Site Precautions for the Ebola VirusOut of an abundance of caution, the School of Pharmacy &Health Sciences has put into effect a series of safeguards toassure its students’ safety during their clinical experiences.In the event that a student is working at a hospital where anill patient presenting symptoms and risk factors of Ebola isbeing triaged or treated, pharmacy students will be asked toimmediately contact Experiential Education, and the stu-dent(s) may be asked to leave the hospital or other clinicalsite immediately, provided that he/she has had no directcontact with the patient. CDC protocols and precautions willtake precedent and be enacted.

If the student has been assessed to have no exposure risk, heor she will be asked to do the following:

• Communicate with his/her supervisor to assure that thereis no immediate risk or potential exposure concerns.

• Contact Experiential Education by telephone and/oremail and await further instructions about the situationand the potential need for adjustment of the site experi-ence or reassignment to another clinical site to completethe practice experience.

• As instructed by Experiential Education, either return tothe site to continue work or gather his/her belongingsand leave the site until cleared to return.

CompensationStudents will not accept payment for work performed duringIPPEs/APPEs.

DefermentIn the event of unforeseen circumstances during APPE rota-tions, a request to defer an APPE rotation may be submittedto Experiential Education. It must be emphasized that a de-ferment of an APPE rotation will result in a delayed gradua-tion and conferment of diploma.

HIPAA/OSHA/CPR TrainingStudents are required to have CPR certification training,HIPAA training and OSHA training prior to beginning theirpractice experiences. FDU School of Pharmacy & Health Sci-ences provides all trainings to students during P1 studentorientation. Recertification of CPR training occurs duringthe P3 year. The CPR certification must be the AmericanHeart Association BLS for Healthcare Providers (CPR andAED) Program. The training must be done in-person, notonline. No other programs are accepted. If a student missesany training sessions it is his/her responsibility to schedule,pay for and complete one or all trainings prior to participat-ing in an IPPE or APPE.

LicensureIntern LicensureIf a student participates in an IPPE/APPE outside of NewJersey, it is the student’s responsibility to determine if an in-ternship license is required to participate in the desiredstate. If an internship license is required, it is the student’sresponsibility to obtain and maintain the license throughoutthe out-of-state experience. If the intern license expires or isrevoked, the student will be withdrawn from the IPPE/APPEand will receive a failing grade.

Professional LicensureIf a student is planning to become licensed outside the stateof New Jersey, it is his/her responsibility to contact the re-spective state’s board of pharmacy for state licensing require-ments and to develop a plan to meet those requirements.

Post Exposure Prophylaxis (PEP)In the event that a student or faculty member is exposed to areportable confirmed or suspected communicable disease, asreferenced in the New Jersey Administrative Code 8:57, dur-ing their participation in the program, the affiliated site willnotify the student as being an individual who may have beenin contact with the case during the infectious period of dis-ease and thus exposed to that disease. The student and fac-ulty may receive appropriate Post Exposure Prophylaxis(PEP) through the site’s Employee Health Department orEmergency Department at no cost to the individual studentand faculty. The University’s Student Health Services Direc-tor will be notified if there is a suspected or confirmed com-municable disease report involving the students and facultyof the University.

Site PlacementSites are assigned through the experiential software, incor-porating student preferences and program requirementsinto a lottery algorithm. The majority of IPPE and APPE ex-periential sites are located within a 90-mile geographic driv-ing distance from the FDU School of Pharmacy & HealthSciences. Having a site located close to where a student livesdoes not guarantee that student’s assignment to the site. Allstudents must have on-demand access to transportation. Sitelocations are not changed due to a student’s inability to trav-el to an assigned site.

In order for students to have a diverse experience, they arenot allowed to request site preferences or be placed at a sitewhere they currently work or intern, within the same depart-ment previously worked or interned or be assigned to a pre-ceptor who is or was their supervisor. In addition, in order toavoid any conflicts of interest, a student cannot be assignedto a preceptor who is a relative.

At any time during the course of the year, preceptor and siteplacements are subject to change, by site or school request,due to unforeseen circumstances. In the event this occurs,the student is notified and presented with an alternative siteand/or preceptor so as not to compromise academic pro-gression.

Due to the complexity of site scheduling, all scheduling is-sues must be handled by the Experiential Education staff.Under no circumstances should students contact a precep-tor or site directly in an attempt to arrange or change apractice experience.

Students may be subject to additional health clearance requirementsset by a particular experiential site.

Inthe event remediation is needed, previously granted specialrequests are not honored.

Students in need of remediation of a P1 or P2 course aretentatively placed in the last available timeframe offered forthe corresponding experiential course to ensure didacticcompetencies are met prior to participating in an IPPE.Students in need of remediation of a P3 course are not per-mitted to begin the APPE sequence until all didactic re-quirements are fulfilled. This may result in postponement

of one or more APPE rotations and a delayed graduation.Upon successful completion of the remediated course, stu-dents are placed at an available site as determined by Expe-riential Education staff.

Students who fail one or more midterm exam(s) of a didac-tic course are tentatively placed in the last available time-frame offered for the corresponding experiential course.Subsequent placement is contingent on successful comple-tion of the course.

Students failing more than one didactic course will beplaced on academic probation and will not be eligible forexperiential placement until they are determined to be ingood academic standing.
