The Need for O.R. Turnover Solutions (Cont’d.)

These infections related to medical care can be devastating and even deadly. HAIs are the most frequent adverse event in healthcare. Hundreds of millions of patients are affected by HAIs worldwide each year, leading to significant mortality and financial losses for health systems. According to the World Health Organization, of every 100 hospitalized patients at any given time, 7 in developed countries and 10 in developing countries will acquire at least one HAI.

Figure 1 Prevalence of HCAI in developed countries*

Figure 2 Prevalence of HCAI in developing countries*

HAIs are categorized by the following four types of infection, listed in order of prevalence:

1. Catheter-associated urinary tract infections2. Surgical site infections3. Bloodstream infections4. Pneumonia

HAIs account for 37,000 deaths in Europe, 8,500-12,000 in Canada, and 99,000 in the U.S. annually. This equates to approximately 270 deaths per day in the U.S. alone. In Europe, the annual financial losses due to HAIs are estimated at approximately €7 billion in direct costs only and reflect 16 million extra days of hospital stay. In U.S. hospitals, annual direct medical costs of HAIs range from $28.4 to $33.8 billion.

Surgical Site Infections (SSI)Globally, the numbers associated with SSIs in acute care facilities are staggering:

United States• SSIs occur in 2% to 5% of patients undergoing inpatient surgery• Approximately 500,000 SSIs occur each year• SSIs lead to an average of 7-10 additional hospital days of stay• With an SSI, there is a 2-11 times higher risk of death• The additional cost per SSI ranges from $3,000 to $29,000 depending

on the type of procedure and the infecting pathogen• SSIs account for $10 billion annually in additional healthcare expenditures

United Kingdom• The estimated direct costs for a patient who develops a surgical site

infection are between €2,265 and €2,518• SSIs accounted for 14% of HAIs• Nearly 5% of patients who had undergone a surgical procedure were

found to have developed an SSI

Europe• Incidence of SSI ranges from 1.5% to 20%• SSIs contribute greatly to the economic costs of surgical procedures

– estimated range: €1.47-19.1 billion

australia• The Australian Council for Safety and Quality in Health Care

suggests that 2% to 13% of patients suffer from SSIs• SSIs increased length of stay by six days at a cost of A$12,419 to

A$40,940 per patient• Total costs for SSIs are estimated at A$268 million per year• SSIs account for 7,000 deaths annually

Canada• Minor post-surgical infections add an additional $5,000 to $10,000

per case, while major procedures (e.g., orthopedic or cardiac) increase the patient cost of care by well over $100,000 per case.

SANDEL O.R. Turnover SolutionsThe development of the SANDEL O.R. Turnover Solutions was driven by two main goals: 1. Improve patient outcomes, and 2. Remove costs associated with operating room turnover.

• According to the CDC, there are 51 million surgeries conducted in acute care hospitals in the U.S. each year, along with another 50 million conducted in surgery centers; each procedure carries the same risk of a SSI occurring

• According to the Australian government in 2010–11, more than one-quarter of all hospitalizations involved surgery; there were 1 million surgeries in public hospitals and 1.4 million in private hospitals

• According to the Canadian government 2.2 million surgeries occur each year

• Information on Surgical Procedures in European Countries is represented in the chart below:

table 4. Selected surgical procedures, inpatient cases per 100,000 population, European countries, 2010 (or nearest year)

SanDEL O.R.turnover Solutions

Note: Some of the variations across countries are due to different classification systems and counting methods.1. Procedures per 100,000 females2. Procedures per 100,000 males3. Procedures per 1,000 live births4. In Belgium, Germany and Slovenia, differences in counting method result in a large over-estimation for certain

procedures (e.g., coronary angioplasty and coronary bypass).Source: OECD Health Data 2012

The cost improvement benefits of using a SANDEL STAT-PAC:• Decreased turnover time – SANDEL STAT-BLOC™ impervious linens significantly

reduce the time needed to clean the O.R. table after each case – all needed supplies are in one package

• Virtually eliminate the cost of nonsterile reusable linens, with the exception of blankets

• Virtually eliminate the abuse of linens, such as using sheets for positioning or for drying the floor, etc

• Extend the life of the O.R. table mattress by protecting them from blood and fluids with impervious linens

• Standardization allows a repeatable process for every procedure

The safety benefits for patients and staff with SANDEL STAT-PAC:

• SANDEL STAT-BLOC linens have an absorbent, quilted layer that is skin-friendly to the patient, helping prevent skin breakdown

• The impervious SANDEL STAT-BLOC linens provide a barrier between the patient’s skin and the O.R. table mattress, preventing any transmission of bacteria or any germicide used to clean the mattress, which could cause caustic burns to the patient’s skin

• Decrease the risk of back injuries with SANDEL STAT-PAC hampers, which allow bags to be removed from below the hamper ring; this avoids lifting the heavy bags up and over the hamper

Improved surgeon satisfaction with SANDEL STAT-PAC:• Provides a demonstrable, repeatable process improvement and standard of care• Improved O.R. turnover time• Improved infection prevention practices in the O.R.

Antimicrobial TechnologySANDEL STAT-BLOC™

The proprietary, patent pending, SANDEL STAT-BLOC antimicrobial linens offer an added layer of protection in the clinical environment. While there is no patient contact with the table sheet’s antimicrobial layer, microbes are stopped below the linen’s surface, keeping them away from the patient’s skin. This antimicrobial barrier helps prevent the colonization and migration of bacteria and pathogens most often associated with SSI, including MRSA.

SANDEL STAT-BLOC antimicrobial film, created using nanotechnology, is laminated to a super absorbent wicking layer and a skin-friendly, hydrophilic, non-woven patient contact layer. STAT-BLOC antimicrobial disposable linens work continuously to help prevent the proliferation of harmful bacteria on and in the linens providing an added level of protection against HAI. Infection protection is key in producing superior surgical outcomes. The O.R. table can be a hot spot for potential infections; STAT-BLOC linens can protect patients by providing the cleanest surface possible for every surgical procedure.

The antimicrobial layer is treated with triclosan, a widely accepted antimicrobial ingredient known for its safety and antimicrobial efficacy. Triclosan has been used in skin care products worldwide for more than 30 years. It provides immediate, broad-spectrum antimicrobial activity to properly formulated topical preparations. In the US and other countries around the world, triclosan has been used in personal hygiene products since the 1960s.

Today, triclosan is found in a wide array of professional and consumer products internationally including hand soaps, surgical scrubs, and shower gels, as well as healthcare personnel hand washes. Proven to be effective against MRSA (Staphylococcus aureus), Escherichia coli and CRE, triclosan, along with the other key features and benefits of the SANDEL STAT-PAC solution, makes it the most effective system on the market.

The linens are also offered without an antimicrobial coating if required.

Antimicrobial Technology (Cont’d.)

Wound ContaminationReusable linens can be a contributing factor for SSIs. The linting and pilling on re-usable linen poses not only a problem of quality control for laundry managers, but an issue for surgeons concerned over the potential for surgical wound infections. Recommended practices, from industry organizations, for the selection and use of surgical gowns and drapes clearly identifies that barrier materials used for surgical gowns and drapes should be as lint-free as possible. SANDEL STAT-BLOC™ linens reduce lint and create a cleaner and safer Operating Room.

SANDEL STAT-BLOC Table Linens• Disposable, lint-free, latex-free, antimicrobial table linen• Three layers of protection

• Soft, quilted, breathable, hydrophilic layer: Allows for fluid absorption; eliminates wrinkling which can lead to skin breakdown and decubitus ulcers

• Super absorbent layer: Strong concentration of Super Absorbent Polymers (SAP) for maximum fluid control

• Impervious, antimicrobial layer: Prevents bacteria from colonizing on the O.R. table; non-slip material helps reduce patient sliding

Lift Sheet and butterfly style lift sheet are available. Strong, soft and absorbent, allowing fluids to migrate into the absorbent table sheet below.

SANDEL STAT-BLOC Armboard Covers• Disposable, antimicrobial • Dual belt-loop design, straps for added safety• Straps are secured to the armboard covers to prevent from falling• “Fold- and-lock” closure

SANDEL STAT-BLOC Pillow and Headrest Covers• Disposable, antimicrobial covers designed to prevent bacteria from colonizing on

pillows and headrests• “Fold- and-lock” closure

All SANDEL STAT-BLOC products are also available without an antimicrobial coating.

Disposable Safety Straps• Disposable to minimize HAIs• Fits around all areas of the body, including thighs and legs

SANDEL STAT-MOP™The SANDEL STAT-MOP™ is economical, fast, and compliant with O.R. standards. A fresh, clean, mop will always be available to clean each room after each procedure to help prevent cross contamination. Ansell does not offer cotton mops as they introduce lint into the O.R. environment.

• A disposable, non-linting mop head designed specifically for the O.R.• More absorbent than traditional cotton, rayon and microfiber mops• Helps prevent cross-contamination• 100% post-industrial material & constructed of less material, resulting

in less waste for disposal• Meets AORN and other global guidelines for using a clean mop

between each case• Rayon and microfiber mops also available

Cost analysis considerations to convert to SANDEL STAT-PAC™Turnover time: find out what amount of time is required to fully clean and turn an O.R. after a procedure, preparing it for the next patient. (This does not include the setup of equipment trays, supplies, sterile equipment draping, etc. for the next procedure.)

• If reusable, cloth linens are currently used to dress the O.R. table and clean the O.R., the acquisition costs, laundry costs, and replacement costs should be considered • Find the cost/pound/kg of laundry (from the in-house or outside laundry service)

• Find the approximate weight of the used linens (wet and heavy) as they leave the O.R.

• Impervious, absorbent, disposable table linens will decrease turnover time and require less labor for cleaning

• Identify how the reusable linens are being used: i.e., sheets for the table sheet and lift sheet; pillow cases or towels to cover the armboards; towels or washcloths to wipe and clean the room

• If reusable mops are being used, find their wet weight and approximately how many times they can be reused before replacement

• Consider the number of times reusable linens can be used before it must be replaced (due to damage)

• Consider the number of bags used for the current method vs. the need for the SANDEL STAT-PAC method, i.e., reduced linen bag needs, red bag waste reduction initiative, etc

• Consider if the current method involves double-bagging. SANDEL STAT-PAC bags do not require double bagging

• Are reusable table straps (patient safety straps) being used? Consider their acquisition costs, cleaning costs, and replacement costs

• Are reusable arm straps being used? Consider their acquisition costs, cleaning costs, and replacement costs

• Consider what is being used as patient positioners – foam positioners, rolled blankets, rolled towels, etc

• Table Linens: Looks high quality, thicker and more substantial; very high coefficient of friction: tacky surface. Not as absorbent (contains little SAP - Super Absorbent Polymer); Non-laminated (unsecured) top layer, which can wrinkle under patient’s skin and lead to skin breakdown; the ends of the sheet are not closed, allowing SAP to escape into the environment (“snowing”); airborne SAP becomes potential contaminant in surgical wound

• Lift Sheet: Soft, absorbent, 34” x 66”; 40” x 80”; and butterfly style available. Rips easily by hand lengthwise (on 34” edge)• Great for competitive demonstration purposes – creates doubt of sheets ability

• Armboard: Low coefficient of friction, for ease of application; Standard armboard comes with one (1) strap, not attached in any way (can fall to floor when table is being set up). Not antimicrobial.

• Pillow case and Headrest: Not antimicrobial• Patient Safety Strap: Only one available; narrow• Bags: For all colored bags: Draw cord – resin (poly) is very poor quality, with no

‘stretch’ (bags do not pass the ‘thumb test’); very brittle and easily punctured; the cord is thin and acts as a razor when the bag is cinched, thus allowing the cord to come out of the bag. Draw cord and draw tape bags are only offered clear

• Mop heads - no competitive edge across the board• Solidifier: No advance branded solidifier; must purchase from outside supplier

Ecolab• Table Linen: Same as Advanced Medical, except ends

are sealed (no snow); lift sheet does not tear and is not as tacky as Advanced Medical. Not as much SAP (Super Absorbent Polymer). Sliding top layer; unsecured and blousy, which could lead to patient shifting and/or wrinkled fabric under patient’s skin, which could lead to compromised skin integrity

• Armboard: Straps are narrower; offer a belt loop style; higher price points

• Bags: Same as Advance Medical – except the drawstring is not skinny – overall poor quality. Clear draw tape bag is opaque (prevents visualization of contents)• Red biohazard kick-bucket bags are quality bags• Clear kick-bucket bags are quality bags

• Mops: Cotton mop produces a significant amount of lint, rayon mops are of good quality, microfiber mops are offered in various options and are of good quality

• Solidifier: They use their own brand• Patient Strap is the same as our large and regular straps• Offer: Encompass OR Environmental Hygiene Program and absorbent floor pads

and prep pads

Competitors (Cont’d.)

Medline Medline leads with their standard packs and price is very competitive. They also offer a customized pack which is more expensive and will not be competitive in the market against our packs

• Table Linen: Quilted. Not as much SAP (Super Absorbent Polymer); does not have a high COF (Coefficient of Friction). Is acceptable but not great

• Lift Sheet: Hydrophobic. Prevents fluid from getting to SAP in table sheet to be absorbed there; as a result the skin of the patient remains wet. Size is 40” x 60” which is too short for arm-tucking (vs. our 66”) or for obese patients

• Armboard: Different material from table sheet; armboard straps are loose and unattached; thin straps (not narrow); grey color

• Bags: Very good bags, high quality and 2.5 mil thickness; offer both draw cord and draw tape which are as good as our bags; good quality kick-bucket bags

• Mops: Offers poor quality cotton mops which produce unwanted lint; also offer rayon and microfiber mops
