    This letter is formal notice that the BBB Industries, LLC facility located at 2325 W. Charter Way, Stockton, CA 95206-1135 is being permanently closed, with associated worker terminations beginning no earlier than May 17,2014. The entire plant is being closed.

    Worker terminations related to this plant closing are scheduled to take place in stages beginning no earlier than May 17, 2014 and the entire facility will be permanently closed between June 1,2014 and June 13, 2014. There are no bumping or transfer rights and the affected workers are not represented by a union.

    Attached hereto are the job classifications of the positions to be affected, the names of the workers currently holding the affected positions, and the anticipated schedule for terminations. There are two hundred forty-one (241) affected workers . 1 The affected workers are in the following general classifications:

    3 Customer Service

    9 Maintenance

    1 BBB Industries contracted with Full Steam Staffing for contract workers and those workers who are employed by Full Steam (not BBB Industries) are included on the listing with an * for clarity.

    We have reviewed the accompanying special-purpose Statement of Assets of the San Joaquin County Treasury as of December 31, 2013, in accordance with the Statements on Standards for Accounting and Review Services (SSARS) issued by the American Institute of Celtified Public Accountants (AICPA). All information included in the County Treasury's Statement of Assets is the representation of the management of the San Joaquin County Treasurer's Office.

    A review consists principally of inquiries of Treasury personnel and analytical procedures applied to financial data. It is substantially less in scope than an audit in accordance with generally accepted auditing standards, the objective of which is the expression of an opinion regarding the financial statements taken as a whole. Accordingly, we do not express such an opinion.

    The accompanying special-purpose Statement of Assets was prepared in compliance with Government Code Section 26920(a) as discussed in Note I.c and not intended to be a presentation in conformity with generally accepted accounting principles.

    Based on our review, we are not aware of any material modifications that should be made to the accompanying special-purpose Statement of Assets in order for it to be in conformity with the basis of accounting described in Note 1.

    This report is intended solely for the information and use of the Board of Supervisors, Treasury Oversight Committee, Treasury management, and Auditor-Controller. It is not intended to be, and should not be, used by anyone other than these specified parties.

    1. Summary of Significant Accounting Policies

    a. Financial Reporting Entity

    In accordance with Government Code (GC) §27000, the Treasurer of the County of San Joaquin (County) has the fiduciary responsibility of receiving and keeping safely "all money belonging to the County and all other money directed by law to be paid to him and apply and pay it out, rendering the account as required by law".

    According to GC §53607, the County Treasurer, as delegated by the Board of Supervisors (BOS) and legislative body of other agencies, has the authority and responsibility to invest or reinvest funds so deposited with the County Treasury. This authority and responsibility are renewable annually and were first formalized by County Ordinance Section 2-2951 in March 1996. The most recent renewal was made on April 30, 2013 (Board Order B-13-321).

    Pursuant to GC §27130 to §27137, the BOS has established a County Treasury Oversight Committee to review and monitor the Treasury's Investment Policy. The Committee membership includes the Treasurer-Tax Collector, Auditor-Controller, Assistant County Administrator, Assistant Superintendent of Schools, and a non-County representative.

    The Pool is not registered with the Securities and Exchange Commission (SEC) as an investment company.

    b. Treasury's Investment Strategy

    The County Treasury's investment strategy for the Pool is to maintain the principal and to provide sufficient cash to meet disbursement needs. The cash flow needs of the Pool members are monitored daily to ensure that sufficient liquidity is maintained to meet the needs of participants. The County Treasury's basic investment strategy is to buy and hold, to a designated maturity, high quality fixed income investments.

    The investment pool participants are classified into two major groups, "Superior Court, Schools and Local Board Districts" and "County Funds". The Superior Court is governed by the State's Administrative Office of the Courts, and the Schools and Local Board Districts represent ail school districts and special districts governed by local boards within the County of San Joaquin. These entities are either required by law or have chosen to maintain funds with the County Treasurer.

    County Funds include all of the County's operating and trust funds, the County's clearing (holding) accounts, and special districts funds controlled by the County BOS. The Un-apportioned Interest Earnings Trust Fund, a County holding fund, is presented separately on the financial statements to provide the accounting of the operational results of the pool investment.

    c. Financial Statement Presentation

    The accompanying special-purpose Statement of Assets was prepared for the purpose of presenting the total assets of the Treasury pursuant to GC §26920(a) and is not intended to be a presentation in conformity with generally accepted accounting principles. Accordingly, note disclosures required by Governmental Accounting Standards Board Statement No. 40, Deposit and Investment Risk Disclosures. an amendment to GASB Statement No.3, are not presented, since such disclosures are not required by GC §26920(a).

    investigation into the alleged Work Place Violence filed against Mr.

    Lee purportedly by one (1) of his co-workers.

    Additionally, I sent you a formal Request for Information dated

    February 14, 2014 and your response was that you had to check

    with others before providing me the information I requested. It has been approximately two (2) weeks since you responded to my

    formal Request for Information. You have not communicated with

    me concerning my request since you received my letter.

    You informed the Union and Mr. Lee of the Violence in the Work

    Place Complaint on December 30, 2013. As stated then, we welcome your investigation and interview of Mr. Lee. As of now, it

    has been over two (2) months since noticing Mr. Lee and I of the

    charge and of your investigation. As stated when you infonned us

    of the investigation, "We welcome your investigation." We also

    shared with you our concerns regarding the timeliness of the

    alleged charge and our concern has only deepened. In response to

Also attached are minutes for the November and December 2013 HSCDC Board of Directors meetings (Attachment 2); the January 2014 quarterly progress repOlt which discusses HSCDC' s progress towards program goals and monitoring strategies (Attachment 3); monthly cash advances issued to HSCDC from the County (Attachment 4) and the Auditor-Conh'oller' s Year-To-Date Expenditure Report for County Head Start costs (Attachment 5).

    1. Call to Order: The regular meeting of the Head Start Child Development Council, Inc. Board of Directors was called to order at 12;07 PM by Jacqueline Flowers, Board Chairperson. 2. Roll Call: Roll call established a quorum.

    Present: Jacqueline Flowers, Jamie Balocchi, Bertha Cooley, Dr. Everett Low Absent: Elaine Saculla, Susan Rodriguez, Claude Brown

    3. Approval of Minutes: The minutes of the regular meeting held October 25, 2013 were presented for review.

    A motion was made by Jamie Baiocchi to accept the Minutes of the Regular Meeting of the Board of Directors held October 25, 2013 as presented. The motion was seconded by Bertha Cooley. Motion carried.

    4. Recognition of Guests and Visitors: Jesus Garcia was introduced as a guest. It was noted that Mr. Garcia has submitted an application to serve on the Board of Directors. 5. Public Comments: No public comments were made. 6. Board of Directors' Comments: Board members made introductions and gave brief background information for guest, Jesus Garcia. Mr. Garcia then explained briefly his background and interests, noting that he served as a member on the HSCDC Delegate Policy Committee for three years, as well as a member on the SJC BOS Head Start Policy Council for 3 years. 7. Executive Director's Comments: Gloryanna Rhodes, Executive Director, updated Board members on the recent follow-up visit made by OHS Region IX pertaining to the Federal review findings in the 2013 Onsite Monitoring Review. It was noted that the two findings in Fiscal were cleared. Other areas reviewed during the follow-up visit were discussed. 8. Communications:

    8.1 HHS/ACF/SJC 80S HS: A handout was given regarding the Grantee's acceptance of the Quality Improvement Plan submitted by HSCDC. The update on the 2013 Federal Review Findings/Follow-up Review by OHS was discussed in Agenda Item 7: Executive Director's Comments.

    8.2 SJC 80S HS: Gloryanna Rhodes, Executive Director, reported that the Grantee agency did grant retroactive approval of the two Chevrolet Cargo Vans to HSCDC. A copy of the approval letter was included for reference. 9. Old Business

    9.1 Delegate Policy Committee Composition and Formation: Presented by Gloryanna Rhodes, Executive Director. A simplified handout detailing the DPC composition and formation was presented for Board members' review. It was noted that the included handout will be used to train the Delegate Policy Committee. Comments were received and addressed.

    A motion was made by Dr. Everett Low to approve the Delegate Policy Committee.Composition and Formation as presented. The motion was seconded by Bertha Cooley. Motion carried. 9.2 Leases

    9.2.1 Lease Renewal: Presented by Gloryanna Rhodes, Executive Director. This item was previously reviewed at the October 25,2013 Board meeting and tabled. It was noted that the lease renewal date for Creative Head Start is December 15, 2013. It was also reported that talks with the

    Executive Director of Creative Child Care, Inc. (CCCI) are ongoing, in regard to transferring the name on the lease to CCCI. A great deal of discussion ensued. Questions were raised and discussed.

    A motion was made by Dr. Everett Low to approve the extension of the lease for the Creative Head Start facility with a 90 day provision; if, at 90 days, Creative Child Care, Inc. has failed to transfer the lease into their name, this item will come back to the Board for further review. The motion was seconded by Bertha Cooley. Motion carried. (Jamie Baiocchi abstained from vote.)

    10. New Business 10.1 Migrant FV 2013-2014 Budget: Presented by Mario Beltran, Director of Finance. It was

    noted that the primary reason for this budget revision is to include the partnership with EI Concilio, which was previously approved by the Board. Questions were raised and discussed.

    A motion was made by Bertha Cooley to accept the Migrant FY 2013-2014 Budget Revision #1 as presented. The motion was seconded by Jamie Balocchi. Mation carried. 10.2 Contracts and Leases

    10.2.1 Contracts 10.2.1.1 RFP - Receive Bids from Outside Executive Consulting Firms:

    Presented by Stephanie Powell, Organizational Development Analyst. All advertised listings of the notice of RFP were explained. Only one bid was received, which was distributed for Board members review. Gloryanna Rhodes, Executive Director, made a note that in future the agency would like to advertise outside of the local community as well as throughout San Joaquin County. Board members agreed and suggested that in future the agency also target Stanislaus County and even Sacramento County.

    The bid received from St. Clair HR Consulting was reviewed thoroughly. Questions were raised and discussed. The meeting was then called into closed session.

    The meeting returned to open session. A motion to accept and retain the services af St. Clair HR Consulting was made

    by Jamie Baiocchi. The motion was seconded by Bertha Cooley. Motion carried. 10.2.2 Leases

    10.2.2.1 Lease Renewals: Presented by Mario Beltran, Director of Finance. A handout detailing the lease renewals for California Street and Gianone was included for reference. Discussion ensued regarding ADA compliance at Gianone. The Board Chair requested that the renewal for Gianone be tabled until January.

    A motion was made by Dr. Everett Low to approve the lease renewal for California Street as presented. The motion was seconded by Bertha Cooley. Motion carried.

    11. Staff Reports 11.1 Program Progress & Enrollment Reports: Report received and reviewed. 11.2 Financial Report: Presented by Mario Beltran, Director of Finance.

    11.2.1 Credit Card Expenditures: Credit card statements along with backup documentation were reviewed by Jamie Baiocchi, Vice Chairperson.

    11.3 Personnel Report: Verbal report given by Gloryanna Rhodes, Executive Director. 11.4 Food Services Report: Report received and reviewed.

    A motion was made by Jamie Balocchi to approve the extension of the Senior Meals Program with San Joaquin County for one month in January 2014. The motion was seconded by Bertha Cooley. Motion carried.

    Progress Report January 2014

    The following is a report of HSCDC's progress towards meeting Program Goals:

    GOAL # 1: School Readiness HSCDC staff will enhance parents/guardians in their roles as first teachers of their children; increase their skills in advocating for their children's education in the community and promote life-long learning.

    Objective 1.1 HSCDC staff will support parents in their roles as first teachers of their children by partnering with them in the development of educational goals, objectives, and strategies that support children's learning and strengthen parents' advocacy skills. Objective 1.2 HSCDC will support families' connections to the community including local schools, and community-based agencies that support life-long learning. Objective 1.3 The 2014-2015 program budget includes funds to support training including but not limited to: curriculum planning, parent leadership skills, parent advocacy, transition planning, intentional learning, school readiness activities.

    Progress Towards Goal Baseline data was gathered from the TS Gold assessment system on all children's current developmental levels including their performances on measurements of HSCDC's School Readiness Goals. In addition, education staff and Program Supervisors are generating reports which will enable them to develop plans for groups of children. Education Content Area staff are in process of aggregating agency-wide data for analysis.

    Parents received information during instructional home visits regarding: the importance of their involvement in their children's education; opportunities for volunteering in the HSCDC program; the screening and assessment process; the home-school relationship; parents' roles in planning to address their children's individual needs. In addition, information on methods of advocating for their own needs and that of their families was provided. Teaching staff and parents shared information on individual children's current functioning levels in all domains including School Readiness tasks and are in process of developing individual child plans.

    GOAL # 2: Family Engagement HSCDC staff will support families as lifelong learners, facilitate long-term family stability, and promote strong connections to peers and community through a cadre of family services staff that possesses theory, knowledge, and experience in developing goal-directed relationships with families Objective 2.1 HSCDC staff with knowledge, skills and experience will support families in reaching their goals by enhancing healthy self-reliance through identifying family aspirations, building on existing family strengths, partnering in setting and working towards life/educational/career goals and encouraging the use of relevant community resources. Objective 2.2 HSCDC staff, that are both knowledgeable and skilled, will support families in: identifying and enhancing peer/community connections through the development of family support groups to enhance relationships with children and peers; and through memberships in parent advocacy/leadership groups within the community.

    Objective 2.3 The 2014-2015 program budget includes· funds to support training including but not limited to: family engagement, relationship development, goal-setting, working with families living in poverty, building partnerships, leadership skills, advocacy, and readiness activities.

    Progress Towards Goal Parents are supported in developing long-term family stability in relationships, housing, food, employment, health care through strong connections to peers and community. HSCDC facilitates this through an assessment process which enables parents to identify their own needs, strengths, aspirations and set meaningful goals for themselves and their children. The assessment and planning process enables parents to see progress towards attaining their goals.

    Progress towards HSCDC's family engagement goals focused on enhancing staff knowledge and skills to support parents through changes in structure, job title, scope of responsibility, and span of control. This resulted in integration of family services and education under Program Supervisors; and an integrated model of service delivery to families. Staff received training on family engagement and school readiness outcomes. In addition, a cadre of family services staff are participating in a Family Development Credentialing program. A mental health professional provided consultation and training on an as needed basis to family services staff working with high needs families. Family Services staff have met with nearly 80% of HSCDC parents to identify needs, strengths, and develop goals. Of these parents, nearly 90% have attained some or all of their short-term goals.

    The HSCDC Registered Nurse provided health trainings to all education staff throughout the County on signs, symptoms, treatment of asthma and the use of EPI-Pens.

    GOAL #3 Family Well-Being HSCDC staff will support parents/expectant families in promoting the health, well-being and development of their families including pregnant women, infants, toddlers and preschool and school-age children through health/nutrition education that supports positive lifestyle practices.

    Objective 3.1 HSCDC staff will support parents/expectant families to identify the connections between the success of their children in school, the well-being of the family and healthy life practices including: regular prenatal and postnatal care; breastfeeding; regular medical and dental care; balanced nutrition; regular physical exercise; routine health practices; and disease prevention activities. Objective 3.2 HSCDC staff will support increased connections to and collaborations with community agencies involved in supporting and providing: nutrition education; physical activities; prenatal and postnatal care; education on healthy lifestyles for pregnant women and families with young children. Objective 3.3 The 2014-2015 program budget includes funds to support training including but not limited to: health practices, disease prevention, nutrition, prenatal and postnatal care, breastfeeding.

    Progress Towards Goal Health education was provided to parents through instructional home visits, curriculum activities that promote positive lifestyle practices, the Expectant Families curriculum, and indiVidual consultations. Multidisciplinary teams with health, education and mental health expertise were available to consult and plan with parents of children with moderate to severe health/mental health/behavioral issues. Parents identified as needing medical and/or dental homes were provided lists of community providers and supported as needing in obtaining insurance coverage and finding appropriate local providers.

    Pregnant women received information on the importance of prenatal and postnatal care, the value of breastfeeding and wellness planning for their child.

    During the period from October through January, HSCDC Registered Nurse and education staff met with parents to develop specialized health plans for 43 children with acute and chronic health conditions to ensure their health and safety in the classroom. In addition, 19 newborn visits were conducted to provide information to parents on the importance of well-baby care.

    January 2014 Quarterly Report

    EDUCATION: The December 2013 Monthly Program Progress report indicated that 83.2% of currently enrolled Head Start children had completed screenings (developmental, behavioral, sensory) and 86.5% of currently enrolled Early Head Start children had completed screenings. The 45 day requirement for programs opening September 2013 has not yet been reached.

    Assessments of HSCDC were completed in November of 2013.     HEALTH AND NUTRITION: During the period from October 2013 through January 2014, HSCDC Registered Nurse and education staff met with parents, Program Supervisors and site staff to develop specialized health plans for

    43 children with acute and chronic health conditions to ensure their health and safety In the classroom. Conditions included: hypothyroidism, MRSA, hydrocephaly, heart conditions, diabetes, cancer, seizure disorders, etc. In addition, 19 newborn visits were conducted to assess the well-being of the mother and newborn and provide information to parents on the importance of well-baby care, breastfeeding, postpartum depressions, environmental risks, etc.

    The Registered Nurse provided training to newly-hired family services workers Supervisors on policies and procedures, the Periodicity Chart, timelines, community resources and documentation requirements. A series of trainings was implemented in November and December for teaching staff on signs and symptoms of asthma, use of inhalants, and EPI Pen usage

    MENTAL HEALTH: During the period from October through January, the Health/Mental Health Department received 46 HS children and 2 EHS referrals on children displaying moderate to severe behavioral difficulties, Year-to-date number of referrals of HS children is 46, EHS children is two.

    HSCDC currently has collaborative arrangements with Sacramento State University, California State University Stanislaus, National University, and San Francisco University for the placement oftrainees and interns. HSCDC currently has three MSW Interns, two of whom are Spanish-Speaking. Each of the Interns is providing services for 16 hours a week. Training and supervision has been initiated on a

    weekly basis.

    The Mental Health Liaison has facilitated ongoing conflict resolution sessions with six site teams. The Liaison continues to provide consultation and support to Specialist staff on issues related to general classroom management, when to refer a child for mental health assessment, and education staff training needs.

    Child/Family Resources staff continues to be available for weekly drop-in consultation for family services, education and Specialist staff.

    During the period from October 2013 through January 2014, five Quality In itiative trainings and six Day, Part year education at five sites for education

    School Readiness trainings were provided by Program Supervisors for Part-staff. Mentor Teachers conducted Literacy Make-it and Take-It workshops staff.

    The Program Development Coordinator has rovided monthl CDA trainin p y g for Teacher Associates in preparation for staff credentialing. Eight of ten sessions have been completed.

    Family Services and Home Visiting staff were provided training on strategies to work with challenging

    families.

    Program Supervisors were provided training on revised procedures for the ROMA assessment process and Family Partnership Agreements. This was done in the "Train the Trainer model" with the expectation that the Program Supervisors would train line staff.

    Five informational sessions were offered to Family Services Workers and Home Visitors on the process for obtaining a Family Development Credential. Thirty nine partiCipants indicated interest. The agency is in the process of identifying which participants would be in Cohort 1 and Cohort 2. The credential is offered in cohorts of 25 participants.

    The Information Technology Department provided training to administrative, management, resource staff, Home Visitors, Program Supervisors and Coordinators on poliCies and procedures for

    technology use, the Google mail, drive, and calendar.

    HEAD START CDC, INC. Monitoring Report

    SUMMARY REPORT Presented to Grantee For 121312013

    This report covers Sel)tel'l1b,~r monitoring and corrective actions that occurred during

    Level 1- Ongoing Monitoring by line staff and their supervisors: Program Supervisors oversaw the Daily-7 for health and safety, and the Pre-Opening checklist for full year and part year classes. Level 2- Monitoring by Special Assignment: 89 ERSEA files were monitored for eligibility and documentation. Management reports of current enrollments and waitllsts continued to be provided to Program Managers to assist in their monitoring of recruitment, enrollment, & replacement of participants in funded slots. Level 3- monitoring was conducted In December for a sample of EHS and preschool classes, including file review, file review for Expectant families, Child Environment, and Home base Home Visit I

    • Improvements in monitoring tools were implemented: 4 monitoring tools were revised: Child Environment, File Review Child/Family, File Review Expectant Families, and the Home Base Home Visit Checklist. The tools were developed following consultation with HSCDC content experts, and review of the October 2013 issuance of the 2014 OHS Monitoring Protocol.

    3. Issues/ Arcas of Needed Improvement • The Pre-opening checklists for part day classes that opened in September were submitted along with

    requisitions for repairs and materials. The most frequently cited Items were: 1.) Need to add items to disaster kit; 2.) Need for new bark In outdoor play areas; 3.) Need to post schedules in classrooms.

    • ERSEA eligibility review of 89 files indicated some discrepancies between application forms, ChlldPlus input and eligibility documents.

    • File reviews for level 3 had the highest number of findings for education indicators (56) family services (19), and Health (12). The Child Environment Tool had the highest number of findings regarding the need to individualize lesson plans for each child and to obtain parent input regarding lesson plans. The need for documentation of parent input was a common finding between all tools used including Home Base visit observation.

    4. Targets for possible Follow-up Training or Monitorin9 • ERSEA technical assistance has been made available to HSCDC to continue into the next quarter of

    reporting. Changes In monitoring methods, development of corrective action plans, and implementation of CAPs are anticipated.

    HSCDC distributes noncompliance reports to management corrections. The corrections will be tracked and reportable. The monitoring results and reporting for this period will be included in the review of data for the upcoming 2014 self-assessment.

    • Discussion and identification of systemic causes behind recurring monitoring findings during
