Part A should be completed during the Initial Planning Meeting which takes place prior to Medical Needs Service provision commencing.
Part B should be completed/updated at each review stage (typically every 6 weeks).

How is the current provision working? Are there any proposed amendments?
Update the timetable in Part A, using a different colour to show changes
E.g. 02.03.20 All going well; S is enjoying the E-Learning and would like another 2 subjects (Science and Geography). The HLSA session is valued, and S would like an additional weekly visit. School have been in contact every week to drop off History work; S is also doing French via the school app.21.04.20 As above – the extra 2 subjects have been too challenging, Geography will be withdrawn for the next 6 weeks.
Would the young person benefit from a reintegration programme for return to school, HLSA session moved to school and/or consideration of Alternative Provision alongside MNS?
02.03.20 – no, S is not ready for that yet.21.04.20 – Yes; school agree that S can attend the small group in school Weds and Fri mornings for the next 4 weeks with K. March – we will then assess this. S has said she would like to be able to visit a care farm; Mr Jones is looking into this.
Are there any updates or changed guidance from health professionals that need to be considered?
02.03.20 – yes; S has now had her first CAMHS appointment and that went well.

How many HLSA sessions accessed (out of how many) and length of sessions accessed
02.03.20 –-5/6 sessions attended and these have gone well. 1 session missed due to a stomach bug.21.04.20 - 12/12 sessions attended – going well.
Any issues with engagement? Does further advice from health need to be sought?
02.03.20 - no issues with engagement21.04.20 - as above, the extra E-Learning subject was too much, and this is being withdrawn.
Note access to E-Learning and amount of work submitted. Is further support from the E-Learning company required?
02.03.20 - 2 subjects English & Maths initially offered – going well. Would like Science and Geography also. All assignments submitted and positive feedback received. S also completing French work via the school Show My Homework app. 21.04.20 - Geography being withdrawn. No extra support needed.

If it appears likely that the young person may continue to be unwell and require further Medical Needs provision, please advise the family that they will need to seek updated health guidance closer to the agreed end date and that this needs to be from a health professional who has physically seen the young person recently.
If the young person is under the care of a specialist service e.g. CAMHS, guidance should be from this service rather than a GP. The Medical Needs Service is able to receive guidance for continuations via telephone and/or email if this is easier for the health
