morethanaweekbeforereceivingyourattention.Youweregivenoralinstructionsthatnothingwastobestackedontopofthefilecabinets.Asecondreminderwasissuedtwoweekslatertocleanoffthetopofthefilecabinets,anditincludedspecificinstructionsastowherethematerialsshouldbeplaced.Bothofthesedirectiveshavebeenignored.

1. Amemorandumgiventoyouasahighpriorityitemwasnottranscribedforoveroneweek.Whenitwastranscribed,thefirstandseconddraftscontainednumerouserrors,manyofwhichwereofatotallyunacceptablenaturegivenyourexperienceandjoblevel.
2. Inlookingthroughyourtodofile,therewerenumerousitemswhichIhadgiventoyouto
sendtootherpeoplethatweremorethanoneweekoldandnotyetprocessed.
3. Youeitherdoublescheduledorfailedtoindicatethatyouhadscheduledmeetingsformeuponthreeoccasionscausingmetomissmeetings.
4. Youhavebeennotedtohavespentanexcessiveamountoftimeonpersonalphonecalls.

MEMORANDUMTO: MaryJonesFROM: TomDoe SupervisorDATE: April21,20__SUBJECT: SuspensionforPoorWorkPerformanceOnJanuary8youreceivedaninitialwrittenwarningandonFebruary1youreceivedasecondwrittenwarningwhichpointedoutseveralseriousdeficienciesinyourworkperformance.Ihavebeenmonitoringyourwork,andyouhavenotmadesufficientimprovementintheareaswediscussed.Immediateandsustainedimprovementisnecessaryinthefollowingareas:
1. Youhavebeenmakingfrequenttypingerrorsinvolvingthespellingofcommon,nonmedicalterms.Revisionshaveoftenbeenreturnedwithadditionaltypingerrorswhichwerenotpresentontheoriginalletters.
2. Intypingdictations,youarenotusingthedictionaryandothersourcesofreferencetocheck
thespellingofwords.Asaresult,yourerrorrateonsomedictationshasbeenashighas30wordsperpage.Again,correctedversionsstillcomebackwithseriouserrors.
3. AsIpointedout,youseemtohaveaprobleminconcentratingonyourworkwhichhasbeen
compoundedbytoomanyincomingandoutgoingpersonalphonecalls.Suchcallsshouldbelimitedtoemergencysituationsinthefuture.
