All serovars showed intense protein bands in the range from 20K to 45K. In the Western blot analysis, serum antibodies from calves infected with S. typhimurium (serogroup B) reacted with protein bands at the range of 17K, 24K, and 31K. The OMP of the two serovars S. dublin and S. enteritidis (both serogroup D1) reacted relatively similar in Western blot with the antisera collected from calves infected with their corresponding serovars. Two protein bands were characteristic for S.dublin and S. enteritidis, 14.4K and 24K. Only one protein band, 24K from the blotted OMP of S. anatum (serogroup E1) reacted with serum from infected calves infected with that serovar. Using the heterologous serum in the Western blot analysis gave weaker results than the homologous serum.

ELISA results detected the presence of serovar specific antibodies, S. typhimurium ELISA detected 10.9% and 4.3%, S. dublin ELISA  detected 7.3% and 2.6%, S. enteritidis ELISA detected 5.1% and 1.7%, while S. anatum ELISA  detected 2.9% and 0.9% of the serum samples collected from diarrhoic and apparently healthy calves respectively.  It could be concluded that Salmonella OMP were major immunogenic antigens that could be used in ELSA or Western blot to detect and monitor Salmonella infection in calves.

INTRODUCTIONSalmonella infections in calves continue to be a major problem worldwide. Substantial economic losses were manifested through mortality and poor growth of infected animals as well as the hazard of transmitting food poisoning to humans. Many outbreaks of salmonella infections has been reported world wide which encountered the most frequently isolated serovars as S. typhimurium, S. enteritidis, S. anatum S. newport, S. cerro, S. montevideo, S. agona and S. dublin which was considered the major host-adapted salmonella for cattle (Mitz et al. 1981, Konrad et al. 1994; Ritchie et al. 2001 and Veling et al. 2002).

The infection was always aggravated by poor hygienic conditions and inadequate nutrition and young calves were most susceptible to infection due to their immature immune responses, undeveloped microflora in their gastrointestinal echo-system and the permanent exposure to the source of  infection from the environment and their dams. Calves suffering from acute clinical salmonellosis may shed 108-1010 Salmonella/gram faeces and animals recovering from the infection may continue to be shedders of the organism for 2-12 weeks post infection. Shedding Salmonella by animals without clinical signs may be associated with chronic Salmonella infection (carrier), convalescence after acute infection or a recent starting colonization (Smith et al. 1989, Galland et al. 2000, Roy et al. 2001 and Radke et al. 2002).

Unlike the intermittent shedding of Salmonella in the faeces, antibody levels do not fluctuate greatly on daily basis. Sero-conversion was used in many countries as a tool for Salmonella surveillance and control in cattle, pig and poultry industry at slaughter to identify infected flocks as a regulatory procedures for food safety and security program. Application of ELISA can provide information about the infection status of the animal and the flock, moreover the repeated testing of animals differentiates those recently infected calves (increasing antibody titre) and those convalescent ones (decreasing titres) from salmonella carriers, which would have relatively constant titres (Smith et al. 1989; Nielsen et al. 1995 and Veling et al. 2002).

In order to control and stop the losses incurred by Salmonella in calves formalin-killed Salmonella bacterins has been used commercially in the field in many areas of the world (Wray and Sojka 1984; Roden et al. 1992), Yet, further investigations has to be conducted on the outer membrane protein (OMP) and their immunogenic potentials as a first step for potent vaccine production.

The objective of this study was to investigate the infection of calves with salmonella in certain localities, as well as to evaluate the antigenicity of the outer membrane proteins (OMP) by Western blot and ELISA as a preliminary phase for vaccine production.

MATERIALS AND METHODSSpecimens

A total of 253 faecal samples were collected from 137 diarrheic calves and 116 contact apparently healthy calves aged 1month-1year. At the same time 253 blood samples were collected from the previously mentioned animals in order to correlate the incidence of salmonella  to the sero-conversion of  each individual animal.

Samples were collected during the period of one year from June 2001 to June 2002 from calves aged (1month - 1year) at governmental and private farms in Giza, Kafr El-Sheikh and Dakahlea governorates.(buffaloe calves were not included in this study).

Specimens were transferred to the laboratory in ice box with minimum delay, where faecal samples were examined bacteriologically for Salmonella isolation and identification. Blood samples were allowed to clott, then centrifuged at 700xg for 15min. Sera were collected and stored in frozen aliquot’s until used in the serological tests.

Salmonella isolation and identification

Faecal samples were cultured into tetrathionate broth and Rappaport Vassiliadis broth, incubated at 37°C and 42°C respectively for 24hr. Loopful from these broth cultures were then streaked onto Mac Conkey and S.S agar plates, incubated at 37°C for 24-48hrs and Salmonella suspected colonies were identified morphologically, then biochemically by the API 20E Kit system (BioMereaux, France) and serologically according to the Kauffman-White Scheem by slide agglutination test using polyvalent and monovalent (O) and (H) antisera (Wellcome Research Laboratories, UK.) (Edwards and Ewing, 1972)

Preparation of the Salmonella outer membrane protein (OMP)

The four Salmonella enterica serovars were isolated (Typhimurium, Dublin, Enteritidis and Anatum) were cultured on tryptic soy agar  in Roux  flasks and incubated at 37°C for 24hr. Bacterial cells were harvested into 10mM HEPES buffer pH 7.4 and washed three times with normal saline. The bacterial cells were sonicated for 15 min. then

centrifuged at 1700xg for 20min. The sediment was discarded and the supernatant was centrifuged at 60,000xg and the pellet was dissolved in 2% sarkosyl in 10mM HEPES buffer and was left overnight at 4°C. The preparation was then centrifuged at 60,000xg for 1hr. and the pellet was dissolved in 10mM Tris HCl pH 7.2 and finally dialysed in 0.15MNaCl, 50mMPBS pH 7.4for 24hr. at 4°C.(Filip, 1973; Kudrna 1985; Brusnigina 1988) The protein content of these OMP preparations were then estimated according to (Lowry 1951).

Sodium Dodecyl sulphate-Polyacryamide Gel Electrophoresis (SDS-PAGE) of Salmonella OMP

The four previously prepared Salmonella OMP preparations (Typhimurium, Dublin, Enteritidis and Anatum) were screened for their OMP protein pattern by SDS-PAGE electrophoresis. The SDS-PAGE was performed in a vertical gel slap by the Laemmli discontinuous buffer system (Laemmli 1970). The gel slabs were 140 x 80 x 1.5mm in a vertical electrophoresis unit (Pharmacia). The stacking gel was 4% and the resolving gel was 12%. Equal volumes (25µl) of the different Salmonella OMP preparations and sampling buffer were vortexed for 15 seconds and heated in thermomixer for 5 min then loaded in the gel along with a molecular protein marker range from 14.4kDa to 94kDa. The electophoresis unit was adjusted at 60V for about 4hr. till the marker blue stain (bromothimol blue) in the sampling buffer reaches the end of the gel. The protein bands were fixed in 50% methanol, 7% acetic acid and 43% dist. Water, then visualised by Coommassie blue stain (Giovannaferro,1974 and Seleim 1999). 

 

Western blot analysis

Nitrocellulose membrane of 0.2µm porosity was used to transfer the electrophoresed proteins by the Western blot technique using Bio-Rad transfer unit containing transfer buffer pH8.3 (25mM Tris; 192mM glycine and 20% methanol. The transfer process was run under 60 V for 6hr. at 8°C. At the end of transfer process protein bands were checked on the nitrocellulose membranes by stainig with 1% amido black stain. On unstained nitrocellulose membranes, the unoccupied sites were blocked by blocking buffer (5% gelatin in phosphate buffered saline containing 0.01% Tween-20) (PBST) for 1hr at room temperature with agitation.  Membranes were then washed three times for 10min in PBST to wash away the excess of blocking buffer and finally left to dry at room temperature, then cut into strips. The blotted-OMP-protein strips for each Salmonella serovar were incubated with 1:100 diluted sera from calves that were positive isolation of the

corresponding serovar. The strips were then washed three times with PBST and incubated with rabbit anti-bovine IgG conjugated with the horse radish peroxidase for 1h at room temperature in dilution 1:2000 in PBS pH 7.4. The conjugate was then washed and the colour was developed by adding the substrate (30mg 4-chloro-1-naphtholdissolved, 10ml cold methanol, 30µl hydrogen peroxide in 50ml PBS pH7.4) (Zamora et al. 1999). 

Elution of Immunogenic proteins

All protein bands that reacted specifically in the Western blot reaction were eluted from each of the four salmonella serovar OMP preparations using Bio-Rad protein elution unit to be used in ELISA as coating antigens. Protein content was measured according to Bradford (1976).

ELISA Procedure

The ELISA was performed in polystyrene 96 well microtiter plates (Dyatech). A checkerboard titration was performed to determine the optimum antigen concentration and conjugate dilution. The plates were coated separately with 5ug/ml of each Salmonella eluted-OMP preparations in 100µl carbonate bicarbonate buffer pH9.6 and kept  for 2hrs at 37°C followed by overnight incubation at 4°C for antigen adsorption. Antigens which did not bind to the polystyrene plates were removed by washing three times with PBS pH 7.2-Tween 20 (0.05%) (PBST). Sites on the plates which were not occupied by the coating antigens were blocked by adding 100µl of 0.1% gelatine solution (blocking buffer) and allowed to incubate for 1hr. at 37°C. The plates were washed three times with the previously mentioned washing buffer to wash away the excess of the blocking buffer. 100µl calf sera diluted 1:100 in PBST  were then pipetted  in the ELISA plates and incubated at 37°C for 1hr. Plates were then washed again three times with PBST. Then 100µl rabbit anti-bovine IgG conjugated to horse radish peroxidase (Kirkegaard and Perry Laboratories) was added in dilution 1:3000 (in PBS containing 0.05%Tween-20). The colour was developed by adding 3,3,5,5-tetramethyl-benzidine (TMB) as substrate and the blue developed colour was then converted to yellow by addition of 100µl of 1M H2SO4 and the plates were read at 450nm. The positive control sera was used from animals that were identified as salmonella positive infected, while the negative control sera were collected from animals that had no history of infection and were proven negative for salmonella isolation. The OD values of serum samples greater than 0.32 for S. typhimurium ELISA, greater than 0.3 for S. dublin or S. enteritidis ELISA and 0.27 for S. anatum were considered positive, which is the mean value of the negative sera

plus two standard deviations (SD)  (John et al.1993; Galland et al. 2000).

RESULTSEpisodes of diarrhoea  in calves were reported in some governmental and private farms in Giza, Kafr El-Sheikh and Dakahlea governorates. Faecal samples collected from these calves as well as the contact apparently showed higher incidence of Salmonella 17.5% and 3.4% respectively. Four different Salmonella serovares were elucidated

namely, Typhimurium 7.3% and 1.7%, S. dublin 5.1% and 0.8%, Enteritidis 2.9% and 0.8% and Anatum 2.2% and 0% from diarrhoic and apparently healthy calves respectively (Table 1 and Figure 1). The antigenic structure of all the 28 Salmonella isolates were confirmed by the Kauffman-White Scheem (slide agglutination test) using polyvalent and monovalent (O) and (H) antisera (Table 1).

The OMP electrophoretic analysia of the four Salmonella serovars  was displayed in Figure 3 (a) The four serovars shared many protein bands as intense protein bands in the range from 20K and 45K was a general character of the four OMP preparations (common Salmonella antigen), while fainter bands were revealed at the higher molecular weight end (higher than 67K) aswell as at the lower molecular weight end (lower than 20K)  (Figure3-a). In the Western blot analysis, serum antibodies from calves infected with S. typhimurium (serogroup B) reacted with  protein bands at 17K, 24K, and 31K.

The OMP of   S. dublin  and S. enteritidis reacted  relatively similar in the Western blot reaction. Two protein bands were demonstrated,14.4K and 24K. as both serovars belong to serogroup D1 and the greater similarity in their somatic antigen structure, O:1, O:9 and O:12.

Only one protein band (24K) was reacting in the Western blot reaction of S. anatum blotted OMP  (serogroup E1)  and reacted specifically with serum from infected calves.(Figure 3-b).

ELISA results detected the presence of Salmonella antibodies in the serum of diarrheic calves in 24.1% of the animals (33 out of 137), while the rate of isolation of salmonella from these diarrheic calves was 17.5% (24 out of 137). In apparently healthy calves ELISA detected  the presence of Salmonella antibodies in their serum in 9.5% of the animals (11 out of 116), while the rate of isolation of salmonella from these apparently healthy calves was 3.4% (4 out of 116).(Table 1 and 2). There was some discrepancies in ELISA results with isolation results. Yet ELISA recorded higher incidence of salmonella infection with the four serovars , both in diarrhoic and apparently healthy calves.(Table 2 and Figure 2).

Istoricul imunizarii

In 1829, Charles Louis a descris simptomele care diferentiaza febra tifoida de alte febre si a facut legatura dintre simptomele clinice si leziunile interne observate la nivelul intestinelor, foliculilor limfatici mezenterici si al splinei (17, p.86).

Bretonneau in Franta si Smith in Statele Unite au pus in evidenta natura contagioasa a bolii (34, p.211).

In 1873, in Anglia, W. Budd a aratat ca materiile fecale sunt unul dintre factorii importanti de transmitere a febrei prin contaminarea apei (17, p.86).

In 1880, Eberth a identificat bacilul in tesuturile pacientilor (17, p.86), iar in 1884, in Germania, Gaffky a izolat si cultivat in cultura pura Salmonella typhi din splina pacientilor cu febra tifoida (34, p.211).

In 1896, Pfeiffer si Kolle in Germania si Wright in Anglia au realizat primul vaccin de uz uman compus din bacterii inactivate prin caldura si au demonstrat pe purcelusi de Guinea ca anticorpii specifici protejeaza de febra tifoida inoculata in cadrul testelor (34, p. 212). In acelasi an, Widal demostreaza ca serul convalescentilor cu tifoida este

capabil de a provoca aglutinarea cu bacteria salmonella, ceea ce a dat nastere termenului "agglutinins" si a testului serologic pentru diagnoza Salmonella typhi (dupa Felix si Widal) (17, p.86).

Wright a administrat vaccinul sau la doi ofiteri din Corpul Medical Indian, dintre care unul a ingerat bacilul tifoidic fara sa dezvolte boala (36, p.781). Wright a testat vaccinul pe 2.835 soldati voluntari (34, p.212) si, in ciuda unui anumit numar de reactii adverse, rezultatele au fost destul de incurajatoare pentru a putea garanta imunizarea trupelor care plecau in Africa de Sud in 1899 (razboiul boer). Cu toate acestea, Wright nu a putut vaccina mai mult de 14.000 de voluntari (36, p.5 si 781).

Opozitia fata de imunizare crestea in randul opiniei publice si a armatei si s-a ajuns ca un intreg transport de vaccinuri sa fie aruncat peste bord la Southampton. Consecintele au fost catastrofale pentru trupe. S-au raportat peste 58.000 de cazuri de febra tifoida printre soldatii din corpul de armata indian, iar 9.000 dintre acestia au decedat. Aceasta controversa a avut un ecou si in comunitatea stiintifica, odata cu duelul din British Medical Journal dintre Wright si statisticianul Karl Pearson. In final, Ministerul de Razboi a comandat testele necesare, ce au dus la concluzia ca vaccinul era eficace. Ca urmare, Wright a fost decorat. La sfarsitul primului razboi mondial, vaccinarea anti-tifoida, desi nu era obligatorie, devenise un obicei in randul armatei britanice (36, p.5 si 781).

Ca urmare a acestor experiente, alte vaccinuri cu bacterii celulare inactivate prin caldura si formol din celule intregi, au fost preparate, in mod special in Franta (17, p.100). Imunizarea sistematica a fost introdusa pentru prima oara in armata franceza in timpul primului razboi mondial (34, p.213).

Boala

Febra tifoida este o infectie acuta sistemica data de Salmonella typhi, care se manifesta prin stare de rau generalizat, anorexie, mialgie, febra mare (39-40°C), crampe abdominale, dureri de cap si hepatosplenomegalie (36, p.782). Formele mai severe au fost descrise ca avand disfunctii cerebrale, delir si stare de soc, perforarea intestinala cu hemoragie (17, p.89). Infectia se transmite prin ingerarea legumelor cultivate cu apa din canalizare, fructe spalate cu apa contaminata sau apa continand materii fecale contaminate (36, p.783).

Informatii epidemiologice la zi

In prezent febra tifoida se intalneste rar in tarile industrializate datorita masurilor de igiena si a imbunatatirilor constante in tratarea si distributia apei potabile (36, p.785).

In Franta (inclusiv teritoriile extraeuropene din America Centrala si de Sud sau Oceanul Indian), incidenta cazurilor confirmate de febra tifoida este estimata a fi 0,16 cazuri la 100.000 de persoane. Cu toate acestea, proportia reala de incidenta este mai degraba 1 la 100.000. In Guyana Franceza (America de Sud) ajunge la 12 la 100.000. In prezent, 70% din infectarile cu tifoida din tarile industrializate au loc in calatorii in afara regiunii (1).

Aceasta situatie contrasteaza puternic cu cea intalnita in tarile in curs de dezvoltare, unde febra tifoida este endemica si prezinta un mare pericol pentru sanatatea publica. Aproximativ 16 milioane de cazuri de febra tifoida sunt raportate anual in intreaga lume, dintre care 600.000 se dovedesc fatale (41).

In tarile in curs de dezvoltare, incidenta febrei tifoide este legata de conditiile de igiena si de riscul

contaminarii fecal-orale (36, p.785). Incidenta medie este intre 150 la 100.000 in America de Sud si 1.000 la 100.000 in anumite tari din Asia (41). In cele mai afectate regiuni, incidenta bolii atinge maximum printre copiii cu varste cuprinse intre 4 si 19 ani (41).

Numai in Indonezia se raporteaza o medie de 900.000 de cazuri anual, cu aproximativ 20.000 de decese. Subiectii cu varste intre 3 si 19 ani reprezinta 91% din cazuri (17, p.88).

In plus, incidenta tulpinilor de Salmonella typhi ce prezinta rezistenta multipla la antibiotice a crescut rapid din 1990, in special in subcontinentul indian si in Asia de Sud-Est (41).

Vaccinuri disponibile

Doua tipuri de vaccinuri sunt disponibile pe piata in prezent (16, p.258).

Primul dintre ele este un vaccin oral preparat din tulpini atenuate de Salmonella typhi Ty21a, un mutant al Salmonella typhi descris de Germanier si Furer in 1975 (17, p.104). Acest vaccin este inregistrat in peste 63 de tari din Africa, Asia, Europa, Australia si America de Nord (41).

Cel de-al doilea vaccin este parenteral, derivat din antigen capsular polizaharid purificat (36, p.788). Este inregistrat in 56 de tari din Asia, Africa, Europa, America de Sud si in Statele Unite (41).

Strategii de imunizare

Imunizarea impotriva febrei tifoide este recomandata de OMS tuturor persoanelor care calatoresc in tari cu conditii de igiena precare, in special in zonele cu endemic ridicata: Asia de Sud-Est, Africa, America de Sud (OMS). In Franta imunizarea este recomandata calatorilor in zonele endemice, incepand de la varsta de 5 ani (6).

Febra tifoida

Febra tifoidă este prezentă în majoritatea ţărilor din întreaga lume şi este foarte frecventă în ţările cu niveluri scăzute de igienă: se estimează că afectează aproximativ 16 milioane de oameni pe an, având drept consecinţă 600,000 de morţi (Ref 2 p 1770 ; 3 p 1/8).Această maladie a devenit rară în ţările industrializate datorită unei bune calităţi a infrastructurilor de îngrijire medicală.Vaccinarea este disponibilă printr-un vaccin poliosidic capsular. Vaccinul protejează împotriva infecţiei cu S.typhi, dar nu şi împotriva S.paratyphi A sau B. Necesită reinjectare la fiecare trei ani şi este administrată de la vârsta de 2 ani.

Maladia

Febra tifoidă este o infecţie bacteriană cauzată de Salmonella enterica serotipuri Typhi (Eberth bacillus) Paratyphi A, B sau C. Este o maladie bacteriană care invadează sângele prin sistemul limfatic mezenteric. Transmiterea este orală-fecală ca urmarea a ingestiei de alimente sau băuturi contaminate. Purtătorii cronici constituie, de asemenea, o sursă importantă de transmitere.

După absorbirea orală, bacteria trece bariera intestinală şi este răspândită prin sistemul limfatic în circulaţia sângelui. Endotoxina eliberata de microorganism joacă un rol important în implicarea viscerala a sistemelor nervos, digestiv şi cardiovascular.

Un diagnostic corect este oferit de culturile pozitive ale sângelui, în special în timpul primei săptămâni  înainte de începutul tratamentului antibiotic. Este mult mai dificil să interpretăm serologia (Widal-Felix). Această metodă demonstrează existenţa anticorpilor îndreptaţi împotriva antigenelor O şi H din ziua 8 utilizând o tehnică de aglutinare. Anticorpii anti-O dispar în 2 până la 3 luni, iar anticorpii anti-H persistă pentru câţiva ani (Ref 4, p 300).

Faza de incubare a maladiei durează între 7 şi 14 zile, şi, de obicei, trece neobservată, cu excepţia unui scurt episod de diaree la 24 până la 48 de ore după contaminare. După aceea, maladia progresează treptat pe o perioadă de o săptămână înainte de apariţia simptomelor clasice, cum ar fi disconfortul, febra până la 39-40º C, anorexia, mialgia, crampele abdominale, durerile de cap şi hepatomegalia şi splenomegalia. Este asociată cu somnolenţa, starea de apatie şi obnubilaţia pe timpul zilei cu insomnie pe timpul nopţii (starea tifoidă clasică).Diareea variază ca gravitate şi este întâlnită doar în două treimi din cazuri. În acest stadiu, semnele clinice scad rapid prin tratament între 2 şi 5 zile. Totuşi, sunt posibile anumite complicaţii şi poate apărea sângerarea în tractul gastrointestinal inferior sau perforarea intestinală cu peritonită. Sunt posibile, de asemenea, complicaţii miocardice asociate cu toxina, cu riscul unei insuficienţe cardiace sau a unei insuficienţe cerebrale, ca în cazul encefalitei.

Tratamentul standard implică administrarea antibioticelor corespunzătoare. Din punct de vedere istoric, cloramfenicolul a constituit un tratament de referinţă pentru febra tifoidă din anul 1948 (Ref 2 p 1772). Rezistenţa la acest antibiotic apare rapid, dar această rezistenţă a început să pună probleme de-abia din anii 1970. În prezent, tratamentul implică utilizarea medicamentelor active in vitro împotriva salmonellae, cu o bună pătrundere a ţesutului limfatic şi intracelular. La adulţi, antibioticul ales constă în fluorochinolone pentru aproximativ 7 zile. La copii în vârstă de sub 15 ani, pentru care fluorochinolonele sunt recomandabile, ceftriaxona este utilizată ca prim tratament (Ref 4 p 300 ; 6 p 30).

Prognoza pentru pacienţii aflaţi sub tratament este favorabilă în 95% din cazuri, iar maladia ia sfârşit fără sechele, însă poate avea drept consecinţă faptul că bolnavii vor fi purtători cronici pentru câteva luni.

Există o un număr estimativ de 16 milioane de cazuri noi în fiecare an în întreaga lume, cu 600,000 decese (Ref 6 p 27). Cea mai mare incidenţă în 2000 a fost de ordinul a 600 la 100,000 locuitori din Asia de sud-est (Ref 7 p 350).Febra tifoidă a devenit foarte rară în ţările industrializate datorită infrastructurilor de îngrijire medicală de bună calitate şi datorită unei atente monitorizări a alimentelor. În aceste ţări, majoritatea cazurilor sunt astfel importate de către turişti sau imigranţi.

Febra tifoidă este o infecţie bacteriană, iar rezervorul pentru atacul organismelor este strict uman: contaminarea se face prin fecalele pacienţilor care suferă de maladie sau ale purtătorilor sănătoşi. Transmiterea este în general oral-fecală şi implică ingestia apei sau alimentelor contaminate. Se poate produce, de asemenea, prin contact direct cu fecalele infectate sau prin ingestia alimentelor atinse de un purtător de organisme.

Purtătorii cronici joacă un rol important în transmitere: un pacient în aparenţă vindecat poate să fie purtător de salmonella, care este excretată în fecalele sale. Tifoida este o maladie a “mâinilor murdare”, iar linia de transmitere poate fi distrusă prin adoptarea măsurilor standard de igienă: spălarea mâinilor, dezinfectarea toaletei, lenjeria de pat şi dormitoarele pacienţilor. Pentru evitarea individuală, recomandarea, atunci când călătoriţi în regiunile endemice este să se evite consumul apei necontrolate şi consumul de alimente reci sau alimente cu origine necunoscută.

 

Privire de ansamblu asupra vaccinării şi strategiei de vaccinare

În 1896, Pfeiffer şi Kolle în Germania şi Wright în Anglia au conceput primul vaccin pentru uz uman, cuprinzând bacteria şi au demonstrat abilitatea anticorpilor specifici de a proteja porcii de Guineea împotriva unor doze de test. În acelaşi an, Widal a demonstrat că serul pacienţilor convalescenţi este capabil să inducă aglutinarea salmonelei, justificând termenul “agglutinins” şi diagnosticul serologic (Felix şi Widal) din S.typhi (Ref 1 p 1060).

Wright a administrat vaccinul unui număr de doi ofiţeri din Corpul de Armată Medicală din India, iar unul dintre aceştia a ingerat bacilul tifoidei fără să dezvolte maladia. Wright a evaluat atunci vaccinul pe 2,835 voluntari din armată. Datorită rezultatelor care au fost considerate suficient de încurajatoare, în ciuda frecvenţei efectelor adverse, decizia a fost să se vaccineze toate trupele care se îmbarcau pentru Războiul din Boer, din Africa de Sud (1899).După sfârşitul Primului Război Mondial, vaccinarea împotriva febrei tifoide, deşi nu era obligatorie, a fost larg răspândită în Armata Britanică. În prezent, vaccinarea este administrată printr-un vaccin capsular poliosidic care oferă protecţie pentru aproximativ 3 ani (Ref 6 p 31) şi este administrat intramuscular într-o singură injecţie. Un vaccin oral (de tipul Ty21a) este disponibil în anumite ţări (Ref 1, p 1065).Vaccinarea împotriva febrei tifoide este obligatorie pentru personalul de laborator şi este recomandat de OMS (Ref 3 p 6/8) pentru persoane care călătoresc în ţări cu condiţii de igienă

precare, şi regiunile specifice cu un nivel endemic foarte ridicat, cum ar fi Asia de Sud-est, Africa şi America Latină. Vaccinarea nu este recomandată pentru copii sub vârsta de 2 ani, deoarece răspunsul imun la vaccinurile poliozidice este prea slab.

Concluzii

Datorită dezvoltării unei rezistenţe multiple la antibioticele curente şi dificultăţilor existente în introducerea măsurilor de bază de igienă din numeroase ţări, lupta împotriva febrei tifoide este acum mai legată decât oricând de prevenirea prin vaccinare.

2.4.3. Febra tifoidă

Febra tifoidă este o boală infecţioasă acută şi transmisibilă, specifică omului, determinată de bacilul tific (Salmonella typhi) care pătrunde în sânge pe cale digestivă, afectând sistemul limfatic intestinal şi producând febră continuă, stare tifică, pete lenticulare, splenomegalie şi tulburări digestive.

Bacteria răspunzătoare pentru apariţia stării tifice specifice face parte din familia Enterobacteriaceae, genul Salmonella şi posedă trei tipuri de antigeni: O (somatic), H (flagelar) şi Vi (virulent) localizaţi pe suprafaţa corpului microbian, conferindu-i un înalt grad de rezistenţă faţă de mijloacele de apărare ale organismului uman sau mediul extern, unde poate supravieţui timp de 1-5 luni în sol, 2-3 luni în apă sau 25 zile în materiile fecale, în condiţii de întuneric, răcoare şi uscăciune.

Febra tifoidă este răspândită pe întregul glob, sub formă sporadică sau endemo-epidemică, în ultimele decenii înregistrând totuşi un declin continuu ca urmare a progresului serviciilor de igienă şi salubritate publică. Cu toate acestea, febra tifoidă continuă să reprezinte un mare pericol epidemiologic din cauza menţinerii surselor necunoscute de infecţie umană. Morbiditatea de gen este influenţată şi de sezon, în acest sens arătând că febra tifoidă prezintă o curbă epidemică sezonieră cu maximul în timpul verii şi toamnei, iar receptivitatea este generală, frecvenţa bolii scăzând treptat cu vârsta din cauza posibilelor efecte de imunizare dobândite de adulţi şi bătrâni prin tot felul de infecţii subclinice.

Principalul rezervor de infecţie este, în exclusivitate, omul care poate fi purtător de bacili tifici pentru o perioadă determinată (purtător temporar) când, în condiţii de convalescenţă, poate elimina bacili patogeni timp de 3 luni, prin fecale, sau pentru toată viaţa (purtător cronic), eliminând bacili tifici în mod continuu sau intermitent pe cale intestinală (10-11 bacili tifici pe gram de materii fecale) sau urinară (mai puţin frecventă, dar mai periculoasă). Febra tifoidă se transmite prin contact direct cu

bolnavii sau purtătorii de bacili tifici în condiţiile nerespectării unor norme minime de igienă personală şi sanitară sau cu ocazia unor deplasări turistice în zone contaminate (”tifoida de concediu”), precum şi prin contact indirect, prin intermediul muştelor sau consumului de alimente şi apă contaminate de purtători (în acest ultim caz amintind că epidemiile hidrice au un caracter exploziv fiind urmate de un ”evantai” de cazuri secundare de contact).

Poarta de intrare a infecţiei în organism este reprezentată de tubul digestiv, bacilii tifici fiind transportaţi, odată cu alimentele contaminate ingerate, în intestinul subţire, unde găsesc condiţii favorabile de multiplicare şi invazie a organismului prin intermediul căilor limfatice şi sanguine. Prin pătrunderea bacililor tifici în sânge se declanşează o stare de intoxicaţie generală, denumită stare tifică, determinată de endotoxina tifică cu efect neurotrop şi leucopenizant. Endotoxinele eliberate prin distrugerea bacililor tifici acţionează in situ asupra terminaţiilor nervoase simpatice, din regiunea mezenterică, rezultând tulburări vasculare, ulceraţii intestinale, meteorism, diaree etc., iar dacă acestea se resorb în sânge, atunci ele provoacă hipertermie şi stare tifică (intoxicaţia sistemului nervos central).

În mod schematic, febra tifoidă netratată cu antibiotice evoluează clinic în 4 faze distincte, curba de temperatură având aspectul unui trapez, în care latura ascendentă corespunde primei perioade a oscilaţiilor invazive, curba continuă cuprinde perioada de stare de 2 săptămâni din faza 2 şi 3, iar latura descendentă corespunde oscilaţiilor terminale din faza 4. În 4/5 din numărul total de cazuri, debutul bolii este lent, cu manifestări generale de febră care creşte constant în intensitate, cefalee persistentă, oboseală, ameţeli, dureri musculare, insomnie şi anorexie. Deseori, apare şi bronşita difuză, iar splina se măreşte constant. După această primă stare de instalare a bolii, care durează 5-7 zile, febra atinge valori maxime (39-400 C), devenind continuă (”febra de platou”), ceea ce înseamnă că starea tifică, corespunzătoare perioadelor 2 şi 3 din simptomatologia bolii, se află la apogeu. De cele mai multe ori, în această fază, apare delirul (bolnavii agitând mâinile după obiecte imaginare), cefaleea este maximă, tegumentul foarte uscat dezvoltă erupţii lenticulare (”rozeola tifică”), sistemul digestiv prezintă semne hiposecretorii, abdomenul este pronunţat şi dureros, splina şi ficatul se măresc, inima îşi diminuează sensibil activitatea, sângele îşi modifică formula leucocitară, respiraţia devine greoaie, iar sistemul nervos intens intoxicat determină hipoacuzie, apatie, adinamie şi indiferenţă, mergând în formele mai grave până la prostaţie şi stare confuzională, care trădează infecţii meningeale sau meningoencefalitice. Perioada de declin din ultima fază de evoluţie a bolii se caracterizează prin scăderea treptată a febrei (sub forma unor ample oscilaţii de la zi la noapte), accentuarea senzaţiei de foame şi intensificarea volumului şi frecvenţei urinare.

Evoluţia obişnuită a febrei tifoide poate fi scurtată prin tratamentul cu antibiotice, în acest caz starea de afebrilitate survenind la 4-10 zile de la tratament, dar în absenţa tratamentului, ea se poate prelungi până la 86-106 zile. Recrudescenţele manifestate prin perioade de revenire a febrei apar în faza de declin a bolii, înainte ca bolnavul să intre în afebrilitate, iar recăderile (recidivele) sunt posibile în primele 8-15 zile de convalescenţă în 9-12% din cazuri ca urmare a persistenţei unor focare de bacili tifici în zonele necrotice ale formaţiunilor limfatice, la adăpost de acţiunea anticorpilor şi antibioticelor. Reîmbolnăvirile de febră tifoidă prin noi infecţii sunt cu totul excepţionale deoarece în majoritatea cazurilor, se dobândeşte un anumit grad de imunitate. Rata mortalităţii se menţine în limite relativ scăzute (sub 1%), iar cazurile letale înregistrate se datorează mai mult unor complicaţii digestive (hemoragie şi perforaţie intestinală), cardiovasculare (miocardită şi flebită), hepatice (hepatită), respiratorii (bronhopneumonie), urinare (prostatită) şi neuronale (encefalită), decât manifestărilor tifice caracteristice. În general însă, frecvenţa tifoidei depinde de

vârsta indivizilor (copiii şi bătrânii contractând-o mai repede decât celelalte persoane), masivitatea infecţiei, starea de rezistenţă a organismului, precocitatea tratamentului ş.a.m.d., dar un rol deosebit în menţinerea sa sub control îl au măsurile profilactice permanente care urmăresc nu  numai identificarea, izolarea şi tratarea surselor de infecţie prin declararea şi spitalizarea obligatorie a bolnavilor, ci şi supravegherea şi prevenirea contaminării apei şi alimentelor sau protecţia masei receptive prin inocularea vaccinului antitifoidic, dar, mai ales, prin promovarea unei educaţii sanitare superioare.

.4.4.1. Paratifoida A, produsă de enterobacteria Salmonella paratyphi A, este mai răspândită în Orientul Mijlociu şi Îndepărtat, manifestându-se sporadic sau în epidemii, ca rezultat al contaminării apei potabile sau alimentelor. De la rezervorul uman de infecţie, ea se diseminează fie prin contact direct, fie prin contact indirect, prin consumul apei sau alimentelor infestate. Tabloul clinic al paratifoidei A este întru totul asemănător celui al febrei tifoide atât prin simptomatologia specifică (curba termică fiind aproape identică celei tifoide), cât şi prin severitatea evoluţiei patologice (cu aceleaşi complicaţii şi rată a mortalităţii).

Paratifoida B, datorată acţiunii bacilului Salmonella schottmülleri sau paratyphi B, este mai frecventă în sudul Europei şi Orientul Mijlociu şi prezintă unele particularităţi care o deosebesc de febra tifoidă obişnuită. În primul rând, debutul său este mai brusc, iar durata medie a bolii este mai scurtă (20-21 de zile), evoluţia sa fiind, de cele mai multe ori, uşoară, cu rare complicaţii intestinale şi mortalitate redusă. În al doilea rând, febra este neregulată, iar erupţia, când se produce, este mai bogată, asociindu-se cu herpesul peribucal. Tulburările digestive sunt dominate de anorexie, vărsături şi diaree, care este mai frecventă ca în febra tifoidă, iar starea tifică, în general, mai puţin evidentă, ceea ce înseamnă că posibilele complicaţii sunt procentual mai reduse.

Paratifoida C poate fi determinată de bacteriile din grupul Salmonella hirschfeldii sau paratyphi C şi Salmonella cholerae suis (care generează tipul tifoidic septicemic) şi este foarte răspândită în zona temperată a Asiei. Dintre salmonelele din grupul C, numai S.paratyphi C realizează forma clinică de paratifoidă, celelalte specii din acest grup declanşând septicemii severe. Forma tifoidică propriu-zisă se aseamănă cu o severă febră tifoidă, prezentând dese simptome de icter, dar spre deosebire de aceasta, în paratifoida C nu apar leziuni intestinale caracteristice. Cu toate acestea, evoluţia ei este foarte gravă, rata mortalităţii fiind destul de ridicată.

Diagnosticul pozitiv al paratifoidelor este sugerat de starea febrilă caracteristică, dar confirmarea lui nu se poate face decât prin examenul bacteriologic care trebuie să deceleze tipul paratific de agent patogen. În toate cazurile, tratamentul cu antibiotice îşi demonstrează eficienţa, dar în cazul paratifoidei C, acesta acţionează cu dificultate din cauza comportării variate faţă de antibiotice a salmonelelor din acest grup. În orice caz, profilaxia igienico-sanitară reuşeşte întodeauna să elimine riscul paratifoidelor.

Subsequent serological typing of the SMT isolates showed the following Salmonella serovars,the number of positive samples being given in parentheses: Typhimurium (12); Bredeney (10); Panama (5);Saint-paul (5); Minnesota (3); Mbandaka (2); Derby (1); Enteritidis (1); Orion (1) and Salmonella sp. collected in three swine farms were included in the study.Artificially contaminated samplesSix Salmonella-negative feed aliquots (25 g) were artificiallycontaminated with each 2 colony forming units (cfu/g), 20 cfu,or 200 cfu/g of Salmonella Typhimurium and submitted to bothstandard microbiological and polymerase chain reactionprotocols.Standard microbiological techniques (SMT)For pre-enrichment, 25 g of sample was inoculated into 250mL of 1% buffered peptone water (BPW) and incubated at 37ºCfor 18-24 h. For enrichment, 1 mL of BPW culture was transferredto 9 mL of Salmonella-selective Müller-Kauffmann tetrathionate(MK-TB; Merck) broth and incubated at 42ºC and a further 0.1mL added to 9.9 mL of Salmonella-selective Rappaport-Vassiliadis (RVB; Merck) and incubated at 37ºC for 24 h. Afterincubation, 1 mL of RVB was set aside for DNA extraction andPCR analysis and a loop from each of the broths was streaked,separately, onto a plate of Xylose lysine tergitol 4 agar (XLT4)and a plate of Brilliant green agar plus 4% (w/v) novobiocin,both of which were incubated at 37ºC for 24h. PresumptiveSalmonella colonies were characterized by biochemical assays(12), and somatic and flagellar antigen determination using slideagglutination and were also serotyped at a reference center(Fundação Osvaldo Cruz, Rio de Janeiro, Brazil). Theseprocedures were repeated for each sample. Pure cultures of S.Typhimurium and S. Enteritidis were used as positive controlsand Proteus vulgaris as a negative control, these strains beinginoculated into RVB and MK-TB and treated in the same way asthe samples.DNA extractionFor DNA extraction, 1 mL of a 18 h old RVB culture (describedabove) was centrifuged at 2000 g for 4 min and the pelletresuspended in 444 L of Tris-EDTA buffer (10 mmol l-1

Tris-HCl (pH 8.0) and 1 mmol l-1 EDTA) containing 30 L of50 mg mL-1 lysozyme (Pharmacia Biotech) and incubated at 4ºCfor 30 min, after which 25 L of 10% (w/v) SDS and 1.25 L ofproteinase K (20 mg mL-1, GibcoBRL) were added and thesolution incubated at 55ºC for 30 min. A 500 L volume ofphenol-chloroform (1:1), pH 8.0, was added and mixed for 5-10 sbefore centrifuging at 16000 g for 4 min, after which the liquidphase was once more extracted with phenol-chloroform andthen with chloroform only. The DNA was precipitated withsodium acetate and cooled isopropanol and incubated at -20ºCfor 30 min, centrifuged at 16000 g for 10 min at 4ºC, followingwhich the supernatant was removed and the DNA pellet washedwith 1 mL of 80% cooled ethanol and then resuspended in50 L of Tris-EDTA buffer and used immediately or stored at -20ºC. This procedure was repeated for each of the samples andthe positive and negative controls.Polymerase chain reaction DNA amplificationThe oligonucleotide primer set used was 139(5’GTGAAATTATCGCCACGTTCGGGCAA3’) and 141(5’TCATCGCACCGTCAAAG GAACC3’), amplifying a 284 bpfragment from the Sallmonella invA gene (9,17), this fragmentbeing common to all members of the genus Salmonella butabsent from other genera. performed using 2 L of the extracted DNA, 2.5 mmol l-1 MgCl2,10 mmol l-1 Tris HCl (pH 8.0), 5 mmol l-1 KCl, 0.2 mmol l-1 of eachnucleotide (GibcoBRL), 0.8 mol L-1 of each primer and 1 U ofTaq DNA Polymerase (Cenbiot Enzimas, Porto Alegre, Brazil) ina final volume of 25 L. Amplifications were carried out in aGeneAmp PCR System 2400 thermocycler (Perkin ElmerInstruments) using the conditions determined by Oliveira etal., 2002 (15) according to the following protocol: initialdenaturation at 94ºC for 5 min, followed by 35 cycles ofdenaturation at 94ºC for 1 s, primer annealing at 55ºC for 1 s andextension at 72ºC for 21 s; with a final extension at 72ºC for 7min. Amplification products were separated by electrophoresison 1.2% (w/v) agarose gels containing 5 g mL-1 of ethidiumbromide, a 100 bp DNA ladder (GibcoBRL) being used as amolecular weight marker.Statistical analysisMacNemar Test was performed on the data using Graphpad-Instat (1993). Sensitivity and predictive value rates werecalculated as previously described (20).RESULTSDetection of Salmonella in field samplesFifty-seven samples were Salmonella positive and 211negative using PCR-RVB and SMT. The PCR-RVB method andSMT both detected 39 (31.45%) positives from the 268 fieldsamples. Serological typing detected the following Salmonellaserovars in the 42 SMT Salmonella-positive samples, theDetection of Salmonella by PCR375number of samples positive for that serovar being shown inparentheses: Typhimurium (12); Bredeney (10); Panama (5);Saint-paul (5); Minnesota (3); Mbandaka (2); unidentifiedSalmonella serovar (2); Derby (1); Enteritidis (1); and Orion(1) (Table 1). S. Derby, S. Panama and S. Typhimurium weredetected in three SMT-positive samples that were negativeby PCR-RVB.Of the 268 samples tested, SMT resulted in 42 (12 LT, 6 IC,24 PSM) Salmonella-positive samples, only three of whichwere negative by the PCR-RVB, while the PCR-RVB methodgave 54 (17 LT, 10 IC, 25 PSM, 2 F) Salmonella-positive samples(Fig. 1), 15 of which were negative by SMT (Table 2).A sample was considered Salmonella-positive when eitherthe SMT or PCR-RVB gave a positive result, based on whichthe sensitivity of the SMT was 73.6% while that of the PCRRVBmethod was 94.7%. The negative predictive value (i.e. theconfidence in a negative reaction actually representing theabsence of Salmonella) was 98.6% for the PCR-RVB methodand 93.3% for SMT. The kappa comparability value betweenthe two sampling methods was 0.77.Salmonella was detected by PCR-RVB in all eighteen feedaliquots artificially inoculated with different concentrations ofS. Typhimurium. On the other hand, SMT failed to detectSalmonella in one aliquot inoculated with 2 cfu/g of S.Typhimurium.DISCUSSIONSince pork products are a significant source of humaninfection, the emphasis in the present study was to test a PCR

protocol for the detection of the genus Salmonella from pigsand pork products and to compare the new method with standardmicrobiological techniques (SMT).Table 1. Castagna et al.The PCR-RVB method for the detection of Salmonella atgenus level was tested in parallel with SMT using 268 fieldsamples, resulting in about 21% more samples being Salmonellapositiveby the PCR-RVB method than by SMT. The MacNemartest showed that there was a significant difference (p=0.0153)between the total number of SMT positive samples and thetotal number of PCR-RVB positive samples. These results agreewith a previous study (15) in which the same PCR-RVB protocolwas compared with SMT using poultry-related samples,although in the formerly case the PCR-RVB assay detected about128% more positive samples than SMT, possibly due to thedifferent nature and the amount of samples analyzed.The PCR-RVB assay had the advantage that it could beperformed in up to five days less than SMT, but the use of bothmethods could increase the number of positive samples. It hasbeen pointing that PCR with selective enrichment detected morepositive samples than the SMT, because the selective enrichmentdilutes PCR inhibitory substances and inhibits competitivemicroflora, which allows the target microorganism to grow thusincreasing the quantity of target DNA (18).Bacteria, which are naturally present in food and feedsamples, usually have reduced viability due to prolongedexposure to unfavorable conditions such as high saltconcentration, unsuitable pH values, freezing, and heating. The

combination of pre-enrichment and PCR offers the advantageof enhancing the sensitivity of the PCR method by increasingthe number of microorganisms from 106 to 109 cfu/mL during thepre-enrichment step, while reducing the negative influence ofthe complex food matrix by dilution of the sample (13). Usingthis methodology, there will be no increase in the DNA ofnonviable cells during the pre-enrichment step and the risk offalse-positives should be reduced.All of the 39 samples that were Salmonella-positive bySMT were also Salmonella-positive by the PCR-RVB method,which also detected 15 additional Salmonella-positive samples.We propose that the additional 15 Salmonella-positive sampleswere detected because the PCR assay was more sensitive thanthe SMT method. Studies formerly conducted (14,15,17)concluded that the primer set 139-141 is highly specific andthat the predicted amplicon is only generated by Salmonellastrains. By testing non-Salmonella genus, commonly presentin samples analyzed by PCR, only few strains that resulted innon-specific amplification of faint non-target-sized fragmentswere detected (14,17). Thus, the amplification of the gene invAhas been proposed as an international standard for Salmonelladetection (14).The failure to detect some Salmonella positive samples bySMT was possible related to the fact that isolates in these samplesproduced colonies lacking the characteristics of Salmonellacolonies leading to false-negative results (2). It could be alsorelated to the amount of Salmonella present on the sample. Inthe present study, when a low number (2 cfu/g) of Salmonellawas inoculated in feed aliquots (n= 6), SMT was not able todetect one of the positive samples, while PCR-RVB assays werepositive for all experimentally inoculated feed aliquots.The DNA of three Salmonella isolates (one of each serovarDerby, Panama and Typhimurium) was not detected by the PCRRVBmethod, these strains being isolated by SMT and identifiedbiochemically and by serotyping. Even considering that threestrains were not detected by the PCR-RVB method, the negativepredictive value of the PCR-RVB method tested was higher thanthat from SMT, conferring more confidence on negative resultsas compared to those produced by SMT. Moreover, the kappavalue indicated that there was a good comparability betweenboth tests (1). In two occasions (isolates of serovar Derby andPanama) the strain was isolated only from MK-TB. This may bedue to a higher sensitivity of these strains to the selective agentsin RVB, impairing their grow in RVB and resulting in the absenceof the target DNA for the PCR detection. On the other hand, theTyphimurium strain was isolated from the RV broth by the SMT,so we can conclude that the target DNA was present in thebroth tube. Previous studies (14,17) also reported a fewSalmonella isolates, which were not detected using the sameprimer set (139-141). In spite of some speculations that the geneinvA could be absent in some rare Salmonella strains (9), thefailure to detect this S. Typhimurium strain was probably morerelated to the presence of PCR inhibitors in the sample. Morerecently, the co-amplification of an internal amplification controlto indicate possible inhibitory substances derived from thesample was proposed (14). The inclusion of an internal control

may contribute to achieve a higher confidence on Salmonellanegative PCR-RVB results in pork.The increasing importance of pigs and pork as a major sourceof S. Typhimurium isolation in many countries including Brazil(3,5), emphasize the need of accurate diagnostic tools.
