2. Lessons Learned

2.1 Electrical Shock

Division : TPDDL

Date of Incident : 5th Nov’15

Type of incident : Fatal

Incident Time :18:20 Hours

Location : HT 518-25/19, Hari Enclave, Zone 518, Dist Mangolpuri

Injury Nature : Electrical Shock

Description of Incident:

A message was received in Zone at around 18:25 hrs regarding the accident of a lineman at Hari Enclave Pole No. HT 518-25/19. It was observed that Lineman went for attending HT jumper burnt complaint on said location. He attended the complaint on his own after switching off the HT panel, without any PTW, supervision or creating proper Safety Zone. He got electric shock due to back charging. Later he was taken to Sanjay Gandhi Hospital, Mangolpuri where he was declared brought dead.

EVENTS LEADING TO INCIDENT: 1. The Victim Late Sh Mahender (Competency Level: Silver) was engaged with Breakdown

team from morning shift. 2. Shri Durgesh , ZSO joined shift at around 17:20 hrs. 3. As the numbers of interruptions were higher due to rain Shri Durgesh, asked for

additional gang from AMC Supervisor, Shri Anil. 4. The victim was asked to do overtime by the BA supervisor and Foreman. 5. He was allocated an alternate Helper, Shri Nandlal . 6. The duty ZSO, Shri Durgesh, was informed about the additional gang by AMC

supervisor. 7. One Official had instructed Shri Mahender to attend the pending HT complaint of Hari

Enclave.. The said Official also informed Sh Durgesh that Mahender has been sent to the mentioned location and to keep track of it.

8. Sh Jairam , LM was also instructed to attend a RO at P-2 Blk and a HT complaint at P-1 Block(clear from the fact that work was done at this location, post accident, under supervision and with PTW) on his own.

9. Sh Mahender came down from first floor of office with RMU handle and asked Sh Nandlal to come along to site near Geeta School.

10. Sh Mahender and Sh Nandlal reached the site, where Sh Mahender scaled the fencing (he was not having the keys of the substation) and entered the E3 outdoor Substation, operated the CG Lucy RMU between 18:15 to 18:30 hrs (as deduced from feeder loading report). Shri Durgesh called up Late Sh Mahender, who informed that he was attending HT jumper repair fault in Hari Enclave.

11. They then reached HT518-25/19, which had a tee off towards two DTs through HT ABC. The main line was bare OH. There one thimble of HT ABC was found burnt.

12. Sh Mahender did not possess any neon tester or discharge rod (as he is not supposed to carry out HT operations on own).

13. He went on a building and threw chain on the outgoing bare OH side. No chains or crocodile clamps were placed on the remaining two sides of tee section. The LT MCCB of the two transformers on tee section were neither switched off nor earthed. The GO of the same were also not disconnected.

14. Ladder was then placed on HT518-25/19. Sh Mahender put on helmet, safety belt and safety rope and went up the ladder. There he secured himself with the pole. Gloves though available were not used.

15. He asked Sh Nandlal to pass on the knife for doing work, when Sh Nandlal noticed some sparking in earthing braid of HT ABC which he identified as accidental back charging.

16. He asked Sh Mahender to take the gloves and suddenly Mahender came in contact with HT ABC, got electrocuted and fell unconscious.

EVENTS AFTER INCIDENT:

1. Going through the DT meter reading of the two DTs located on the tee off , it is evident that there was a voltage indicated at the mentioned duration which is preceded by a sustained period of a condition of one phase HT jumper being disconnected ( as was the case at site). This confirms back feeding on the circuit.

2. Sh Nandlal with local help reached office where he informed another ZSO Sh Naresh and Foreman Sh Surjeet who switched off the supply of o/g supply of Jalebhi Chowk 1 feeder from Zonal office , between 18:30 and 18:45 hrs ( as deduced from feeder loading report).

3. ZSO Sh Durgesh received call from TO at 18:23 hrs and from Security Control at 18:27 hrs informing about the accident. He went to the site of accident with ZSO Sh Naresh Kumar in the latter’s vehicle along with other Zonal personnel

4. A separate team comprising of Sh Kishan Lal, Sh Sanju, Sh Purshottam and Sh Jassu was dispatched to E7 S Stn for attending an area complaint, for which PTW No 3014146 ( from 18:38 to 19:15 hrs) was taken by Sh Durgesh although he was not at the work site of the stated PTW.

5. LM Mahender was rescued from pole, taken to Sanjay Gandhi Hospital, where he was pronounced brought dead.

OBSERVATIONS

1. It is to be noted that during the time of accident three teams were working on HT complaints but supervisor was not available at any of the sites. Further, the way in which Sh Mahender and other LM were sent to work on HT circuits independently by multiple ZSOs proves that the violation of PTW is not restricted to a single person or a particular time or circumstances, but a prevalent practice .

2. The AEC identified glaring loopholes in PTW process adherence in the Zone.

3. It was also noted that many complaints which are registered in Walk in and not entered in OMS system are allocated to Linemen pose a serious threat to the adherence to working norms and guidelines. Further, it was also noted that even for area complaints the call is made to B/D LM by TOs whereas the only communication in such cases is to be given to ZSO.

ANALYSIS

Investigation team investigated the incident sequentially and the root causes were identified accordingly.

1. Complete violation of PTW norms.

2. Working without authorized TPDDL supervisor.

3. Ineffective Safety Zone creation leading to electrocution by back-charging.

4. Considerable number of complaints beyond the ambit of OMS system.

5. Back-charging due to multiple sources of supply being available in the vicinity.

Root Cause Analysis Human Factors: 1.1 Violation by Individual – The victim was not supposed to work individually in HT complaint. 1.3 Violation by Supervisor – Victim was instructed to work individually without supervision 1.4 Operation of Equipment without authority- RMU switched off without any PTW or supervision 3.1 Lack of knowledge of Hazards Present-The victim tried to work without creation of safety Zone oblivious to the hazard of Back charging Physical Factors 5.7 Inadequate isolation of equipment– Safety Zone not created. No Neon tester or discharge rod used to check supply status. System Factors: 8.2 Inadequate leadership – The anomalies existing in practice should have been identified and amended earlier 14.4 Inadequate enforcement of PSP (Inadequate monitoring of work) – Supervisor was not present at site 15.8 Incorrect Instructions: Direct instructions were given to the victim by supervisor/s to breach basic PTW norms Recommendations 1. Clear instruction to be issued to each ZSO personally to abide by PTW norms and ensure

complete creation of safety Zone in all B/D, Mntc, Project or any work involving HT network

and take a written confirmation/ certification from each ZSO for the same.

2. Clear instruction to be issued to each Lineman personally to abide by PTW norms and not to

work on any HT circuits without physical supervision of ZSO/ZRDMO and take a written

confirmation/ certification from each Lineman for the same.

3. Competency Level of all ZSOs to be revisited and sessions of Safety Zone creation to be

disseminated to all ZSOs and retests to be conducted.

4. All complaints received through walk in are to be logged in OMS by TOs and no Lineman

will work in field without any system generated number or written instruction (in case of non

system generated works like Tyco Box replacement, etc).

5. Regular analysis of PTWs taken vis a vis dip/drop in feeder loading to be done and any

discrepancy to be treated as a disciplinary issue/dereliction of duty on part of duty ZSO/

Shift in charge.

6. Test for providing Rule 3 Authorization to be revisited.

7. No premises are to be given LT supply from multiple sources.

Office Safety Recognition

MEPL Nellore

Heavy rains & flooded situation

The plant was inundated with flooded water due to

continuous heavy rains. People from Field Hostel

were partially evacuated for a day and they have

been placed in Nellore.

The Flood Emergency Situation was well

managed and the restoration of the plant was

done in time with collective effort of the site team.

A Life Saving Experience - 17.11.2015

One of the firemen and one of the security men who were caught in between the flooded

waters, on their way back home, were rescued and brought back to the plant
