Incident Description
An employee was conducting an inspection on a faulty overflow slurry sump pump
when he placed his left hand into a belt guard to check the conditions of the belt.
Whilst in the process, his left fingers were caught between the pulley and the drive
belt when the pump rotated backwards induced by the backflow of water in the
discharge line.
The injured person (IP) sustained an amputating injury to the tip of his left hand 4th
distal finger. IP was put on restricted duties.
Findings
1. Complacency by person doing the task
2. Inadequate positive isolation process
3. Poor inspection method used for the task
4. Non compliance to site Safe Work Procedures
5. Lack of hazard and risk identification
6. Lack of proper PPEs
7. Poor communication between the Operator and the IP.

Key Learnings
1. Do Take 5 / conduct Risk Assessment with all team members to identify the
risks and put in place effective controls before executing tasks
2. PPEs are the last line of defence so ensure to wear them before starting
work.
3. Following site procedures will prevent incidents.
4. All team members to participate in Take 5 job planning and personnel to
speak up and stop unsafe work practices when aware of risks not being
managed.

Incident Description
Employee was assisting to help pack up and clear the drill site.
Whilst picking up and attempting to place a heavy-duty step stool on top of the
Morooka, the safety step slid and caught the employees left thumb jamming it
between the metal Morooka safety rail resulting in a laceration.
Findings
1. Lack of communication - Injured person (IP) did not communicate with drill
supervisor to assist with the lift.
2. Lack of planning - Injured Party( IP) did not assess task properly and plan
the lift (No Take 5).

Summary
• TPP tails NAPP of -19.2 kg H2SO4/t to 14 March exceeded the -30 kg
H2SO4/t target while the 30DMA value meets the set target.
• Both 30DMA and 90DMA sulfur are within the ≤1% target
• The ANC/MPA data for the dredge sand is not available for March due
to lockdown of Bige operation since 2 March.
• The Alternative ANC Method project is 90% complete at the end of
March. Only 15 limestone samples remains to be sampled, and 24
samples to be dispatched to EGI for comparative analysis. The new
ANC method is intended to address the Permit requirement.
• A draft proposal from Coffey/Tetra Tech to provide training for staff to
undertake dieback and river navigability survey was received. OTML
review was completed and final proposal is expected soon.
• Field component of dieback and river navigability survey deferred from
2020 to Q1 2021 was affected by travel restriction on the consultant.
CEPA was notified of the situation and they have accepted the situation.
• CEPA is working on the Terms of Reference and work program to do a
compliance audit on OTML. Due to COVID-19 situation, the visit is
deferred to July 2021
• Domestic drinking water sampling for 19 an 26 March have not been
completed due to the lockdown
