    1 PURPOSE

    It is the duty of JGC and all its Subcontractors to protect and maintain the health of all employees, and ensure that they are suitable and medically fit to undertake the duties assigned.

    JGC and its Subcontractors will utilize a health assessment technique, based on careful appraisal of the possible health risks to which the individual will be exposed and the provision of suitable and adequate medical / first aid facilities and personnel throughout the Construction Phase of the Dolphin Project.

    A health screening assessment will be conducted for all JGC and Subcontractor employees before employment on site, and at regular intervals as recommended by OGP Report No. 6.46/228, Health assessment of fitness to work in the E&P Industry, dated November 1995.

    JGC will provide a Medical Centre and First Aid facilities within the construction site, and will operate these facilities in accordance with the State of Qatar Labour Law 2004 (Article 104), and the Ras Laffan Industrial City Guidelines for the Establishment and Operation of First Aid Stations. This on-site Medical Centre will provide an important facility for persons who need immediate medical treatment for the purpose of preserving life, or minimizing the consequences of an injury, until they can be transferred to a hospital.

    2 SCOPE

    This Medical, First Aid & Health Assessment Procedure has been developed in accordance with the following laws, regulations and guidelines:

    State of Qatar Labour Law 2004.

    RLC Guidelines for the Establishment and Operation of First Aid Stations within Ras Laffan Industrial City.

    OGP Report No. 6.46/228, Health assessment of fitness to work in the E&P Industry, November 1995.

    This Medical, First Aid & Health Assessment Procedure will apply to JGC and its Subcontractors, in all work locations, throughout the Construction Phase, and will:

    Provide a system of health management that ensures the Project workforce:

    - Are aware of all health hazards and control measures available to them.

    - Are fit to carry out their assigned tasks.

    - Suffer no detrimental long-term effects to their health or well-being.

    - Have a good understanding of health risks and improved health.

    Ensure that the ability of employees to carry out their assigned duties is not impaired by the use of illegal substances or the misuse of legal drugs, medicines or alcohol.

    3 OBJECTIVES

    Through implementation of this Medical, First Aid and Health Assessment Procedure, JGC is determined to achieve the following objectives.

    3.1 Medical & First Aid

    Identify possible health risks associated with planned activities and the working / resting environment.

    Comply with local government legislation and industry standards / best practices.

    Liaise with RLC to provide the best possible medical support for all site personnel.

    Provide medical and first aid facilities to rapidly and effectively treat personnel taken ill or injured.

    Maintain a group of trained and qualified Advanced First Aiders and professional medical staff.

    Train employees in basic first aid.

    3.2 Health Assessment / Health Hazard Identification

    Competent personnel will conduct health hazard identification, investigating all work related activities, to identify the potential for exposure to the following health hazards:

    Physical.

    Chemical.

    Psychological.

    Biological.

    Environmental.

    A Health Risk Impact Assessment, based on the Health Hazard Identification, will be conducted (to determine the risk and frequency of exposure) and documented, and the findings communicated to all employees involved. This Health Risk Impact Assessment will:

    Be performed by competent, employer approved, health professionals.

    Maintain employee confidentiality.

    Comply with local legislation and industry standards / best practices.

    Ensure that potential health hazards associated with work related activities and environment are identified.

    Ensure that risks of personal exposure, related to degree and frequency, are determined.

    Ensure that controls are implemented to reduce health risks to ALARP.

    4 ROLES AND RESPONSIBILITIES

    4.1 JGC Site Management

    The JGC Site Manager will hold the ultimate responsibility for ensuring that this procedure is effectively communicated to, and implemented by, all JGC and Subcontractor employees throughout the Construction Phase.

    Assisted by the JGC Site HSE Manager, the JGC Site Manager will ensure the following:

    All employees are screened, and certified medically fit for the purpose employed, by an approved physician, prior to working on site.

    Suitably and sufficiently equipped medical facilities, staffed with medical and health professionals, are provided for all site personnel during all operational hours.

    Medical and health professionals remain available at all times.

    Suitable and adequate first aid facilities and equipment are provided and identified by each Subcontractor.

    Sufficient number of trained Advanced First Aiders are provided and identified by each Subcontractor.

    Employees are trained in basic first aid.

    Employees are informed of medical and first aid arrangements, location of medical and First Aid stations, and identification of Advanced First Aiders.

    4.2 Subcontractor

    JGC is responsible for ensuring that each participating Subcontractor implements the following:

    Ensuring that all employees are medically screened, and certified medically fit for the purpose employed, by an approved health professional.

    Providing adequate medical and first aid facilities and equipment during the course of work.

    Ensuring that these medical and first aid facilities are regularly inspected, and that associated supplies and equipment are replenished in a timely manner.

    Ensuring that a sufficient number of trained Advanced First Aiders are engaged and identified throughout the course of work.

    Posting notices and erecting signage to keep employees informed of the first aid arrangements, including the location of equipment, facilities and trained Advanced First Aiders.

    Ensuring that all Advanced First Aiders remain contactable during normal working hours.

    4.3 Medical Team

    The Medical Team and its make up will be decided following the Health Risk Impact Assessment. In the interim, the Site activity risk assessment will identify the requirements for interim support.

    The roles and responsibilities of this Medical Team will include the following:

    Providing stabilization and treatment for occupational injuries and related illnesses.

    Providing health assessment to employees.

    Providing treatment to occupational injuries and related illnesses.

    Relating patients to the RLC Medical Centre or Government Hospital when the need arises.

    and potential causes of injury.

    Maintaining confidentiality with respect to all medical records.

    Maintaining a first aid log, and the medical files of each and every employee assessed or examined.

    Maintaining all medical facilities, equipment and medicines, and ensuring that regular inspections are performed and stocks replenished in a timely manner.

    Providing post-examination, post-assessment or post-treatment assistance, as necessary.

    Providing quick and effective medical response in the event of a medical emergency.

    Conducting random drugs / alcohol tests, as required.

    Conducting welfare related Occupational Health checks.

    Monitoring hygiene standards throughout the site, particularly in accommodation, kitchens, etc.

    Providing Medical and Hygiene training / education to site personnel.

    Monitoring the effectiveness of the control measures and evaluating the need for improvement.

    Remaining contactable after normal operational hours.

    4.4 Advanced First Aiders

    For the purpose of this procedure, Fist Aider will refer to a person who has received training and is certified, with current valid certificate, by a first aid training centre that is certified/approved by the local government.

    JGC is required to appoint suitably qualified and trained Advanced First Aiders during their course of work. JGC will also ensure that their subcontractors implement to the same standards. These appointed Advanced First Aiders will be easily identified by the general workforce by either an armband or the wearing of a vest with first aid insignia.

    The responsibilities of these Advanced First Aiders will include the following:

    Attending training and refresher training to ensure that they remain certified.

    Providing first aid treatment to any injured person when called upon.

    Maintaining a first aid log of persons treated.

    Reporting treatment provided to the JGC Site HSE Manager and Medical Doctor.

    Responding immediately to a medical emergency when called upon.

    Inspecting first aid facilities, and ensuring that equipment and supplies are replenished in a timely manner.

    Remaining contactable during normal operational hours.

    Participating in Incident / Accident Investigations, as required.

    5 HEALTH IMPACT ASSESSMENT

    Generic health hazards, and associated risks and mitigation measures, have been identified and recorded in Appendix 8 of the Hazard Identification & Risk Analysis Procedure & Log, RLO-0000-FPR-50113.

    A detailed health impact risk assessment, as discussed in Section 3.2 (Health Assessment / Health Hazard Identification), will be conducted, prior to major project construction activities, by the JGC Medical Team.

    6 MEDICAL & FIRST AID FACILITIES

    During the initial Construction Phase activities, Site Preparation and Temporary Facilities construction, JGC and its Subcontractors will assess the capability and quality of the Medical Facility available in RLC, as part of the health impact risk assessment. Based on the assessment, JGC will implement the recommendations as approved by Dolphin.

    Use of the RLC Medical Facility shall not exempt Subcontractors undertaking construction activities from providing First Aid facilities, equipment and Advanced First Aiders.

    JGC and its Subcontractors will provide Medical and First Aid facilities in accordance with the following requirements.

    6.1 Medical & First Aid Personnel

    6.1.1 JGC Medical Team

    The JGC Medical Team will initially comprise 1 Medical Doctor, 1 Paramedic, 1 Nurse and 1 Ambulance Driver. Following the results of the Health Risk Impact Assessment, JGC will review these requirements and take appropriate action to ensure its adequacy.

    The Medical Team will report directly to the JGC Site HSE Manager.

    The Medical Team will be mobilized, prior to the start of Construction activities, to ensure that the Health Risk Impact Assessment and its findings are implemented prior to any of the identified high hazard activities taking place. The resumes of all members of the Medical Team will be submitted and reviewed by Dolphin, prior to their engagement by JGC.

    The Medical Team will execute their duties and responsibilities in accordance with Section 4.3 of this procedure, and will operate the Medical Centre in accordance with RLC Guidelines for the Establishment and Operation of First Aid Stations within Ras Laffan Industrial City.

    The Medical Team will operate the Medical Centre in accordance with the Operational Hours listed in Section 6.2.1.

    6.1.2 Advanced First Aiders (JGC and Subcontractor)

    JGC will ensure a minimum of 1 trained and suitably qualified Advanced First Aider for every 50 employees. However, due to the high risks associated with construction activities, the environment and the distance between work faces, the results of the Health Risk Impact Assessment will be used to decide the final minimum manning levels for Advanced First Aiders.

    All appointed Advanced First Aiders will have received internationally recognized and proper training from a training centre certified and approved by the Local Government, and will hold with him a current certificate issued by this approved organization.

    Appointed Advanced First Aiders will execute their duties in accordance with Section 4.4 of this procedure.

    All Subcontractors are required to inform their workforce, JGC and Dolphin of the names and contact numbers of their appointed Advanced First Aiders by posting their photos, names and contact number on notice boards, bulletin boards and security guard station/post, as well introducing / identifying them in daily toolbox meetings.

    6.2 Medical & First Aid Facilities

    6.2.1 Medical Centre

    6.2.1.1 Registration of Medical Centre with RLC

    Prior to establishing and operating each Medical Centre, JGC will register with RLC no later than 1 month prior to the intended operation of the facility, in accordance with RLC Guidelines for the Establishment and Operation of First Aid Stations Within Ras Laffan Industrial City.

    JGC will complete, and return to RLC, the application form Registration to Establish and Operate a First Aid Station provided in Appendix 1.

    Once RLC has reviewed and endorsed the Registration, a First Aid Station Certificate of Registration (as provided in Appendix 2), with a validity period of 1 year, will be issued. This Certificate of Registration will be prominently displayed in the Medical Centre at all times during operation.

    During the operation of the registered Medical Centre, the JGC appointed Medical Doctor is required to submit, to RLC Medical Centre, a daily report in the format provided in Appendix 3.

    In the event that the registered Medical Centre is required to operate beyond the 1 year validity period granted by RLC, JGC will return the First Aid Station Certificate of Registration to RLC, at least 2 weeks prior to its expiry date, for renewal/extension.

    6.2.1.2 Location

    JGC will establish and operate 2 Medical Centres throughout the Construction Phase of the Project.

    1 Medical Centre will be located in the Accommodation Camp and the other in the JGC Temporary Facilities Area. The location of these Medical Centres is shown in the Temporary Facilities Plan, RLO-0000-GPG-00521, in drawings H-000-1651-102, H-000-1651-103 and H-000-1651-206.

    These Medical Centres will be prominently identified and marked with a Red Cross / Crescent and the wording Medical Centre.

    These Medical Centres will include a list of emergency contact telephone numbers for all JGC key personnel, the available emergency services of all nearby hospitals, and a map showing the nearest route to these hospitals.

    6.2.1.3 Operation

    The JGC Medical Centres will be operated in accordance with the requirements of the Guidelines for the Establishment and Operation of First Aid Stations Within Ras Laffan Industrial City and any recommendations from the Health Risk Impact Assessment.

    The JGC Medical Centres will be manned and operated by the JGC appointed Medical Team in accordance with the following operational hours:

    Saturday to Thursday

    0700 hours to 1200 hours - Medical Centre in JGC Site Office (Laydown Area).

    1300 hours to 1800 hours - Medical Centre in JGC Site Office (Laydown Area).

    1900 hours to 0700 hours - Standby / On-Call in Accommodation Camp.

    Friday & Public Holidays

    0700 hours to 1200 hours - Standby / On-Call in Accommodation Camp Medical Centre.

    1300 hours to 0700 hours - Standby / On-Call in Accommodation Camp.

    6.2.1.4 Facilities & Equipment

    It is anticipated that both Medical Centres will be equipped with the following equipment, however, the actual requirements will be determined by the Health Risk Impact Assessment and JGC Medical Doctor following his mobilization to site.

    Thermometer.

    Cardiac defibrillator.

    An ambulance will operate in accordance with the Medical Centre operational hours and will be equipped with the following:

    Portable cardiac defibrillator.

    Mobile stretcher.

    Medical oxygen and accessories.

    Intravenous stand and accessories (including saline solution).

    Ambu bag.

    First aid kit with splints.

    6.2.2 First Aid Station & Kit

    Each Security Guard Station/Post within the Construction Area (Plant Area & Tank Farm Area) and Accommodation Camp will be equipped with a First Aid Kit suitable for trauma response. These Security Guard Stations/Posts will also be known as First Aid Stations, and will be prominently identified with a Red Cross / Crescent.

    These First Aid Stations will, as a minimum, be equipped with the following equipment and accessories:

    A first aid kit suitable for trauma response.

    Mobile stretcher (canvas or foldable type).

    7 HEALTH SCREENING ASSESSMENT

    All employees of JGC and its Subcontractors are required to undergo a pre-employment health screening assessment, performed by their designated health practitioner, within 2 weeks from arrival in Qatar. For Local Nationals, this health screening assessment must be performed at least 2 weeks before they are assigned to the Dolphin Project Site.

    Based on the OGP Report S6.46/228, Assessment of Fitness to work in the E&P industry, November 1995, the health assessment for all new employees is categorized into 3 levels:

    Level 1 - Nurse-based health assessment.

    Level 2 - Physician-based health assessment.

    Level 3 - Additional Investigation.

    Appendix 4 provides a Health Assessment Matrix based on country of domicile and occupation.

    7.1 Level 1 (Nursed-Based Health Assessment)

    A Level 1 health assessment is required by all new employees, regardless of occupation.

    Each employee is required to complete a health questionnaire and undergo basic clinical examination. This will be performed by a nurse / paramedic working under the supervision of the designated Physician.

    7.1.1 Health Questionnaire

    The designated Physician shall prepare a health questionnaire that will contain, as a minimum, the following information:

    Full name, residing and home address, date of birth, and sex.

    Full name of next of kin and contact number.

    Occupation.

    Past medical and dental history.

    Detailed occupational history.

    Family medical history (parents and siblings).

    Current medical conditions / complaints.

    Known allergies.

    Current medication taken.

    Immunizations received (type, date of last booster).

    Lifestyle (smoking, alcohol intake, exercise, etc.).

    7.1.2 Basic Clinical Examination

    Immediately after reviewing the health questionnaire, the nurse / paramedic will perform the following basic clinical examination on the employee:

    Height.

    Weight.

    Blood pressure.

    7.2 Level 2 (Physician-Based Health Assessment)

    A Level 2 Health Assessment requires the designated Physician to review the Level 1 Health Assessment, interview the employee and perform any physical examination as appropriate. The Level 2 Health Assessment may also be performed in the following situations:

    Abnormalities detected during Level 1 Health Assessment.

    Due to the occupation involved.

    As a result of the living and working environment.

    To comply with legislative requirements.

    If any significant abnormalities are detected during the Level 2 Health Assessment, the employee will be required to undergo additional investigations in the form of a Level 3 Assessment.

    7.3 Level 3 Health Assessment (Additional Investigation)

    The Level 3 Health Assessment, also known as Additional Investigations, is based largely on the results of Level 2 Physician-Based Health Assessment. Level 3 Additional Investigation may also be required in the following situations:

    Due to the occupation involved.

    As a result of the living and working environment.

    To comply with legislative requirements.

    The above criteria will determine the types of additional investigation (listed below) required to be performed.

    7.3.1 Eye Testing

    Eye testing will include the following:

    Color vision.

    Testing of visual fields and visual depth.

    Peripheral vision.

    Night vision (where applicable, especially for drivers).

    7.3.2 Hearing

    Hearing test will include:

    Audiometry.

    7.3.3 Laboratory Tests

    7.3.3.1 Blood Test

    Testing for blood borne diseases will form part of this assessment.

    Further urine tests may be required if the initial dip test results were abnormal.

    7.3.4 Radiological Investigation

    7.3.6 Electrocardiogram (ECG)

    An ECG should not be mandatory for a basic health assessment. A resting ECG is not a predictive test but can be considered useful by the examining physician if clinically indicated.

    In the case of cardiological problems, the advice of a specialist is recommended.

    7.3.7 Drug Test

    Drug testing should respect a chain of custody and will include all or some of the following:

    Amphetamines.

    Barbiturates.

    Benzodiazepines.

    Cannabinoids.

    Cocaine.

    Methadone.

    Methaqualone.

    Opiates.

    Phencyclidine.

    7.3.8 Alcohol Testing

    Testing for chronic and acute alcohol intake can be performed through a Breathalyzer (breath alcohol concentration) or a blood test (blood alcohol concentration).

    7.3.9 Psychological Assessment

    The examining physician may recommend, if indicated, a referral for psychological assessment.

    7.3.10 Testing of Physical Fitness

    The examining physician may recommend that an appropriate test of physical fitness be carried out.

    7.4 Frequency of Health Assessment

    Health Assessment will be performed in accordance with the Health Assessment Matrix provided in Appendix 4 for all employees and the various occupations.

    Thereafter, a periodic assessment will be performed at least once every 3 years, but this will vary further depending upon:

    Type of proposed employment and duty.

    Work environment (geographical risk, exposure to travel and/or health risk).

    Local government legislation.

    Past medical history.

    Employees age.

    As defined in the OGP Report S6.46/228, Assessment of Fitness to work in the E&P industry, the world is divided into the following geographical areas:

    Low health risk area (North America, Western Europe, Japan, Australia, New Zealand).

    High health risk area (remaining countries of the world).

    7.5 Health Status Reporting

    Upon the completion of a Health Assessment, the designated Physician will report the status of an employees medical fitness to his Employer.

    The results of the health assessment provided to the Employer shall be limited to the classification of the employees fitness according to the following categories:

    Fit to work.

    Fit with restriction.

    Temporarily unfit to work.

    Unfit for specific occupation.

    Unfit to work.

    The health assessment reports of JGC and Subcontractor employees will be submitted to the JGC Site HSE Manager before they are assigned to commence working in the construction site.

    8 MONITORING & REPORTING

    Occupational related injuries and illnesses will be monitored, reported and investigated in accordance with the HSE Incident / Accident Investigation and Reporting Procedure, RLO-0000-FPR-50104. This process includes investigations to identify immediate and root causes of any occupational related injury and illness.

    Details of First Aid cases and absence from duties will also be monitored.

    9 FIRST AID TRAINING

    All appointed Advanced First Aiders must have completed a training course and hold a valid First Aid Certificate from a locally approved Training Centre.

    Advanced First Aiders will attend a refresher-training course once every 3 years or earlier (i.e. before the expiry of the First Aid Certificate).

    In addition to the Appointed Advanced First Aider, it is JGCs intention to train as many employees as possible in providing basic first aid treatment necessary to facilitate the preservation and/or the containment of an injury to an injured person in the construction site until he/she receives professional treatment.

    11 DOCUMENTATION & REVIEW

    The main Medical Centre in the Accommodation Camp will keep both computerized and hard copy data of medical records, as well as information on outpatient cases and work related injury cases.

    The Medical Team will provide the following information to the JGC Site HSE Manager on a monthly basis:

    Number of outpatient cases, based on each type of illness.

    Number of work related injuries.

    A breakdown on the type of injuries, parts of the body injured, etc.

    Number of days per injured case expected out of work.

    The JGC Medical Centre will keep an approved register that will contain, as a minimum, the following information:

    Full name, age and occupation of employee.

    Short description of the injury / illness and its cause.

    Nature of the work in which the employee was engaged at the time of sustaining the injury or becoming ill, and the date and time that it occurred.

    Outcome of the case, i.e. stating whether the employee returned to work, or was sent home / back to the camp, to a physician or to hospital, and means of transportation where applicable.

    These registers will be retained by JGC for a period of not less than 3 years from the date of the last entry.
