Summer Camp lunches are even more delicious than ever

before! Each day a different restaurant will be catering in food

for families who choose to purchase lunch! Parents can

choose daily from Ledo’s Pizza, Chick-Fil-A, NY Pizza, Jason’s

Deli, and Chubby’s Tacos! Each restaurant offers a variety of

different options at a fee of $5 per day! Your hungry camper

will be sure to find something they will love!

EXTREME CAMP FORM

*A registration fee of $25 (non-refundable) is required to enroll for summer camp. *A deposit of $325.00, which will pay for your final week of camp, must be paid at the time of registration (non-refundable). *By paying the registration fee your child will get a camp t-shirt, water bottle, and cinch bag during their first week. *Weekly payments are due on the first day of the week by 9:00am. After 6:00pm on the second day, a $25 late payment fee will beassessed.

*Since all camp programs are prescheduled, any changes (additions/deletions) to originally selected programs must be made by FRIDAY-TWO WEEKS IN ADVANCE of requested change. All requests for cancellations made after this deadline will not be accomodated and pa yment will be required.

***PLEASE NOTE: Summer Camp will be CLOSED on Monday, June 12 & Tuesday ,July 4, in observance of Independance Day. Complete both sides of form, attach a copy of your health insurance card & payment, and return to front desk .
