Abstrak dan Pendahuluan
Abstrak
Tujuan. Penelitian ini bertujuan untuk memperkirakan risiko hot flashes relatif terhadapmenopaus alami dan untuk mengevaluasi asosiasi kadar hormon, variabel perilaku, dan
variabel demografis dengan risiko hot flashes setelah menopaus.
Metode. Kami melakukan penilaian tahunan dari 255 wanita yang premenopause pada
awal dan mencapai menopause alami dalam 16 tahun masa tindak lanjut.
Hasil. Prevalensi sedang / berat hot flashes meningkat setiap tahun premenopause,
mencapai puncak 46% dalam 2 tahun pertama setelah periode menstruasi terakhir (FMP).
Hot flashes menurun perlahan-lahan setelah menopause dan tidak kembali ke tingkat
premenopause sampai 9 tahun setelah FMP. Mean (SD) durasi sedang / berat hot flashes
setelah FMP adalah 4,6 (2,9) tahun (untuk setiap hot flashes, 4,9 [3.1] tahun). Sepertigadari perempuan pada 10 tahun atau lebih setelah menopause terus mengalami sedang /
berat hot flashes. Perempuan Afrika-Amerika (obesitas dan tidak obes) dan perempuan

Pendahuluan
Hot flashes secara luas diterima sebagai kardinal dan gejala yang paling merepotkan
menopause dan berpengalaman untuk beberapa derajat oleh sebagian besar perempuansebagai tahun akhir reproduksi mereka. [1,2] Terapi hormon terus menjadi pengobatan
utama untuk pengelolaan medis hot flashes. Namun, risikonya, yang diidentifikasi dalam
Perempuan percobaan Health Initiative, [3] sekarang membatasi penggunaannya untuk
mengatasi gejala menopause untuk "periode terpendek atau waktu" ("tidak melebihi 3
sampai 5 tahun" dalam pedoman dari Amerika Utara menopause Society). [4]
Bukti empiris mendukung bahwa anjuran 3-5tahun untuk terapi hormon untuk
pengelolaan hot flashes kurang bermanfaat. Penelitian ini dimotivasi oleh penelitian

pada beberapa perempuan, tetapi yang tidak konsisten mengidentifikasi prevalensi sedang
/ berat hot flashes setelah masa akhir menstruasi (FMP) atau risiko untuk hot flashes yangmelampaui 5 tahun setelah FMP. Dalam meta-analisis studi terutama cross-sectional,
hampir setengah dari perempuan melaporkan hot flashes 4 tahun setelah FMP, dan 10%
melaporkan gejala 10 tahun setelah FMP. [5] Dalam sebuah penelitian terhadap wanita
Australia, durasi rata-rata hot flashes dari onset gejala untuk belajar endpoint adalah 5,2
tahun, [6] tetapi durasi setelah menopause, yang merupakan penanda kunci untuk
manajemen medis, tidak teridentifikasi. Sebuah survei lebih dari 8.000 wanita Latin-
Amerika menunjukkan bahwa lebih dari 60% melaporkan gejala vasomotor 12 tahun
setelah menopause. [7] Kami sebelumnya melaporkan bahwa total durasi median hot
flashes adalah 10,2 tahun ketika diperkirakan dari onset gejala dalam reproduksi akhir
tahun melalui transisi menopause. [8] dalam penelitian tersebut, identifikasi calon hot
flashes dalam transisi menopause dini kontribusi yang kuat untuk durasi panjang mereka.Namun, banyak peserta tidak berkembang melampaui menopause, dan durasi hot flashes
setelah FMP, yang merupakan periode yang paling umum untuk manajemen medis, tidak
baik ditandai.
Data sekarang tersedia untuk memeriksa prevalensi dan risiko dari hot flashes pada
tahun-tahun pascamenopause. Penelitian ini memperkirakan prevalensi hot flashes terkait
dengan FMP dan dievaluasi faktor risiko hot flashes yang terus lebih dari 5 tahun setelah
FMP. Kami juga meneliti apakah faktor-faktor risiko diprediksi kelanjutan pendek atau
panjang hot flashes (yaitu, lebih dari 3-5 y) setelah FMP. Titik cutoff untuk waktu setelah

Metode
Peserta studi
Studi ini mengevaluasi 255 wanita di Penn ovarium Aging Study yang mencapai
menopause alami selama masa tindak lanjut 16 tahun (1996-2012). Kami hanya
memasukkan peserta yang mencapai menopause alami untuk mengatasi tujuan utama
kami memperkirakan risiko hot flashes terkait dengan FMP. Perbandingan variabel
penelitian pada awal antara sampel dan sisanya dari kelompok yang tidak diamati untuk
mencapai menopause alami selama penelitian (n = 181) menunjukkan tidak ada
perbedaan yang signifikan, dengan pengecualian usia, yang lebih tua dalam kelompok

Variabel Studi
Variabel hasil primer, sedang / berat hot flashes, dilaporkan oleh para peserta pada setiap
periode follow-up, menggunakan daftar gejala menopause divalidasi tertanam dalamkuesioner wawancara terstruktur. [11] Pada setiap tindak lanjut, pewawancara bertanya
apakah panas berkedip atau berkeringat di malam hari terjadi pada bulan lalu, ditanya
apakah mereka terjadi pada tahun lalu, dan meminta mereka untuk menilai keparahan
mereka (0, none; 1, ringan, 2, sedang, 3, berat). Kalinya dalam beberapa tahun dievaluasi
dalam kaitannya dengan FMP. The FMP diidentifikasi setelah 12 bulan atau lebih tidak
ada perdarahan menstruasi dan ditetapkan sebagai waktu 0 untuk setiap wanita untuk
memungkinkan evaluasi longitudinal hot flashes setiap tahun sebelum FMP (sampai,
tetapi tidak termasuk, waktu 0) dan dari FMP seterusnya. Penghentian sedang / berat hot
flashes didefinisikan sebagai tidak sedang / hot flashes parah dilaporkan selama minimal

sama sebagai hasil sekunder.
Estradiol dan follicle-stimulating hormone (FSH) diukur dengan radioimmunoassay di
Translational Research Center Klinis dan dari University of Pennsylvania, menggunakan
Coat-A-Count kit komersial (Siemens Healthcare Diagnostics, Tarrytown, NY). Sampel
darah dikumpulkan pada setiap kunjungan studi (menyediakan maksimum yang mungkin
dari 28 sampel per wanita), disentrifugasi, dan dibekukan dalam aliquot pada -80 C. Tes
dilakukan dalam batch, yang termasuk empat kunjungan per peserta, untuk mengurangi
variabilitas dalam-wanita akibat kondisi assay. Semua tes dilakukan dalam rangkap dua
dan diulang jika nilai-nilai berbeda oleh lebih dari 15%. Interassay dan intra-assay
koefisien variasi kurang dari 5%.

Generalized untuk tindakan berulang digunakan untuk memperkirakan asosiasi bivariabel
dan multivariabel dari variabel penelitian dengan sedang / berat hot flashes. Estimasi
varians untuk uji statistik pada koefisien regresi yang disesuaikan dengan pengamatanberulang dari setiap peserta menggunakan persamaan estimasi [16] Semua model yang
disesuaikan dengan waktu, yang didefinisikan dalam tiga segmen relatif terhadap FMP:.
(1) premenopause-setiap tahun sebelum FMP (sampai, tetapi tidak termasuk, waktu 0);
(2) dini postmenopause-FMP (waktu 0) untuk kurang dari 6 tahun setelah FMP; dan (3)
terlambat postmenopause-6 tahun atau lebih setelah FMP (lihat definisi titik cutoff
bawah). Variabel kategori tiga tingkat ini termasuk dalam masing-masing model. Untuk
setiap peserta, total durasi hot flashes dihitung dari laporan pertamanya sedang / berat hot
flashes laporan diamati terakhirnya sedang / berat hot flashes pada tindak lanjut.
Pengamatan hot flashes pada penilaian terakhir yang tersedia dianggap disensor. Hot
flashes juga disensor pada waktu penggunaan dilaporkan hormon, kehamilan, atau
menyusui di semua model. Sampel ini tidak memasukkan pengamatan histerektomi atauooforektomi bilateral sejauh hanya wanita yang mencapai menopause alami dimasukkan,
seperti dijelaskan di atas. Model diulangi untuk mengevaluasi titik cutoff sekunder pada 3
tahun atau kurang setelah menopause dan memiliki hasil yang hampir identik.
Semua kovariat didefinisikan a priori. Setiap kovariat ditambahkan tunggal untuk model
dasar dengan waktu dan interaksi dengan waktu. Semua kovariat berpotensi waktu
bervariasi diperlakukan seperti itu dalam pemodelan. Kovariat yang terkait dengan hot
flashes pada P " 0.20 dimasukkan dalam model multivariabel untuk menentukan
kontribusi independen dari kovariat untuk hasil hot flashes. Inklusi dalam model
multivariabel akhir dipandu oleh apakah masing-masing variabel tetap signifikan secara
statistik pada P " 0,05 atau diubah asosiasi yang signifikan lainnya dengan lebih dari15%. Interaksi hipotesis dengan waktu diperiksa untuk BMI, ras, dan hormon. Tidak ada
tiga arah interaksi memenuhi kriteria inklusi.
Titik cutoff untuk awal dan akhir postmenopause diidentifikasi dari perkiraan hot flashes
pada setiap tahun relatif pascamenopause dengan FMP (Gambar 1). Titik cutoff pada
tahun 6 setelah FMP memiliki penurunan gejala terbesar dari FMP (dengan ukuran
sampel yang handal) dan konsisten dengan titik cutoff antara awal dan akhir
postmenopause yang disarankan dalam revisi JERAMI + 10 pementasan. [9] sekunder
titik cutoff pada tahun 3 setelah FMP memiliki pertama (tapi kecil) menurun dari
perkiraan hot flash puncak dan konsisten dengan titik cutoff di postmenopause awal
disarankan dalam JERAMI + 10.

digunakan dalam analisis: premenopause, setiap tahun sebelum periode menstruasi akhir

Kadar hormon dimodelkan menggunakan transformasi log alami untuk mengurangi
pengaruh nilai-nilai yang besar. Mean subjek dari dua langkah hormon diperoleh pada
setiap periode penilaian yang digunakan dalam analisis. Hormon dimasukkan secara
terpisah dalam model multivariabel karena korelasi yang tinggi. Dalam model eksplorasi,
laju perubahan untuk setiap hormon (kemiringan) sebelum FMP digunakan untuk
memprediksi durasi lampu kilat panas setelah FMP. Tingkat perubahan dihitung untuk
setiap wanita sebagai garis regresi linear untuk semua titik dalam kisaran linear yang
diamati sebelum FMP, seperti yang dijelaskan sebelumnya. [17] Odds rasio (OR) untuk
hormon disajikan untuk setiap satu unit (SD ) perubahan log hormon.
Analisis eksplorasi lanjutan pendek dibandingkan panjang hot flashes setelah FMP
termasuk 158 peserta yang sedang / berat hot flashes selama minimal 1 tahun setelah
FMP. Kelompok kelanjutan pendek termasuk peserta yang sedang / berat hot flashes
diamati untuk mengakhiri dalam 3 tahun follow-up interval. Peserta yang tindak lanjut
yang berakhir sebelum 3 tahun dan dilaporkan sedang / berat hot flashes pada
pengamatan terakhir mereka dikeluarkan karena itu tidak mungkin untuk menentukan
penghentian sedang / berat hot flashes dalam interval 3 tahun bagi para wanita. Juga, kita
dikecualikan peserta yang memiliki post-FMP tindak lanjut yang cukup untuk secara
akurat menentukan penghentian hot flashes dalam 3 tahun pertama setelah FMP. Peserta
yang telah sedang / berat hot flashes selama lebih dari 3 tahun setelah FMP terdiri
kelompok kelanjutan panjang.
Perhitungan kekuatan statistik yang dihitung dengan menggunakan STATA versi 12
(Stata, College Station, TX). Asumsi didasarkan pada data di kohort dan termasuk risiko

35% dari hot flashes untuk kelompok terpajan (ringan atau tanpa hot flashes), sedikitnya
delapan pengukuran berulang per wanita, tipe I #error 5%, dan within- sebuah korelasi
perempuan 0,4 untuk 80% kekuatan untuk mendeteksi OR sebesar 1,93 untuk faktor
risiko dengan prevalensi rendah 20%. The dideteksi OR 1.76 untuk faktor risiko denganprevalensi 30% dan bahkan lebih rendah untuk faktor risiko dengan prevalensi yang lebih
besar dari 30%. Untuk tujuan eksplorasi kami mengidentifikasi prediktor kelanjutan
pendek dibandingkan panjang sedang / berat hot flashes, sekitar 35% dari 158 perempuan
diklasifikasikan dalam kelanjutan subkelompok singkat, seperti yang didefinisikan oleh
moderat / hot flashes parah yang berakhir dalam waktu 3 tahun setelah FMP . Kami
memeriksa faktor risiko yang subjek-tingkat dengan prevalensi 20% sampai 50%
dikaitkan dengan kelanjutan pendek dibandingkan dengan kelanjutan lama setelah
menopause. Kami berasumsi bahwa 25% wanita memiliki kelanjutan pendek hot flashes
pada kelompok tidak terpapar, dengan asumsi yang sama seperti di atas. Berdasarkan data
yang tersedia, ada 80% kekuatan untuk mendeteksi OR sebesar 3,6 sampai -2,77, masing-
masing, dalam analisis eksplorasi. Kekuatan lebih besar untuk faktor risiko terus-menerus.
Semua analisis dilakukan menggunakan paket SAS 9.3 statistik (SAS Inc, Cary, NC). Uji
statistik dua sisi, dengan P "0,05 dianggap signifikan.
Hasil
Contoh Deskripsi
Dua ratus lima puluh lima peserta diikuti selama 16 tahun, dengan 15,1 laporan hasil rata-
rata per peserta dan 1.336 orang-tahun masa tindak lanjut setelah periode menstruasiterakhir. Selama tindak lanjut, 203 perempuan (80%) melaporkan sedang / hot flashes
parah, 44 perempuan (17%) memiliki hot flashes hanya ringan, dan 8 perempuan (3%)
melaporkan tidak ada hot flashes. Mean (SD) usia adalah 42,16 (3,38) tahun pada awal
dan 51,47 (3,32) tahun di FMP; 49% adalah Afrika Amerika dan 51% berkulit putih ().

Prevalensi Hot Flashes dalam Kaitannya dengan FMP
Prevalensi puncak sedang / berat hot flashes sebesar 46% dan terjadi dalam 2 tahun
pertama setelah FMP (Gambar 1). Sepuluh tahun sebelum FMP, prevalensi adalah 16%
dan meningkat menjadi 32% pada tahun sebelumnya FMP. Prevalensi sedang / berat hot
flashes menurun perlahan-lahan setelah tahun-tahun puncak. Kembali ke tingkat
premenopause tidak terjadi sampai kira-kira 10 tahun setelah FMP, ketika prevalensi
(32%) adalah sama dengan tahun sebelum FMP.
Prevalensi puncak setiap hot flashes (ringan, sedang, atau berat) adalah sekitar 74% dan
juga terjadi dalam 2 tahun pertama setelah FMP. Sepuluh tahun sebelum FMP, prevalensi
adalah 32% dan meningkat menjadi 58% pada tahun sebelumnya FMP. Kembali ke
tingkat premenopause tidak terjadi sampai 5 sampai 9 tahun setelah FMP, ketika
prevalensi apapun hot flashes adalah sekitar 58%, prevalensi yang sama seperti pada
tahun sebelumnya FMP.

Durasi Hot flash Setelah FMP
Dalam subkelompok dari 182 perempuan yang dilaporkan sedang / berat hot flashessetelah FMP, mean (SD) durasi hot flashes setelah FMP adalah 4,6 (2,9) tahun. Mean
(SD) durasi setiap hot flashes (ringan, sedang, atau berat, n = 230) setelah FMP hanya
sedikit lebih panjang 4,9 (3,1) tahun. Sepuluh persen (25 dari 255) dari peserta tidak
memiliki hot flashes setelah FMP. Total rata-rata (SD) durasi sedang / hot flashes parah
dari onset awal untuk diamati endpoint hampir dua kali lebih lama sebesar 8,8 (4,4)
tahun. Total rata-rata (SD) durasi setiap hot flashes adalah 10,2 (4,2) tahun.

Sebuah interaksi yang signifikan antara BMI dan ras (P = 0.01) dalam kaitannya dengan

Prediktor Pendek Versus Kelanjutan panjang dari Hot Flashes Setelah FMP

Diskusi
Durasi rata-rata sedang / berat hot flashes setelah FMP lebih dari 4,5 tahun dalam

Risk of Long-term Hot Flashes After

Abstract and Introduction
Abstract
Objective. This study aims to estimate the risk of hot flashes relative to natural
menopause and to evaluate the associations of hormone levels, behavioral
variables, and demographic variables with the risk of hot flashes after menopause.
Methods. We performed annual assessment of 255 women who were
premenopausal at baseline and reached natural menopause within 16 years of
follow-up.
Results. The prevalence of moderate/severe hot flashes increased in each
premenopausal year, reaching a peak of 46% in the first 2 years after the final
menstrual period (FMP). Hot flashes decreased slowly after menopause and did
not return to premenopausal levels until 9 years after the FMP. The mean (SD)duration of moderate/severe hot flashes after the FMP was 4.6 (2.9) years (for any
hot flashes, 4.9 [3.1] y). One third of women at 10 years or more after menopause
continued to experience moderate/severe hot flashes. African-American women
(obese and nonobese) and obese white women had significantly greater risks of
hot flashes compared with nonobese white women (interaction, P = 0.01). In
multivariable analysis, increasing follicle-stimulating hormone levels before the
FMP (P< 0.001), decreasing estradiol (odds ratio, 0.87; 95% CI, 0.78-0.96; P=
0.008), and increasing anxiety (odds ratio, 1.05; 95% CI, 1.03-1.06;P< 0.001)
were significant risk factors for hot flashes, whereas higher education levels were
protective (odds ratio, 0.66; 95% CI, 0.47-0.91;P= 0.011).
Conclusions. Moderate/severe hot flashes continue, on average, for nearly 5 yearsafter menopause; more than one third of women observed for 10 years or more
after menopause have moderate/severe hot flashes. Continuation of hot flashes for
more than 5 years after menopause underscores the importance of determining
individual risks/ benefits when selecting hormone or nonhormone therapy for
menopausal symptoms.

Hot flashes are widely accepted as the cardinal and most troublesome symptom of
menopause and are experienced to some degree by most women as their
reproductive years end.[1,2]
Hormone therapy continues to be the primary
treatment for the medical management of hot flashes. However, its risks, whichwere identified in the Women's Health Initiative trials,
[3] now limit its use for
menopausal symptoms to "the shortest period or time" ("not to exceed 3 to 5
years" in the guidelines of The North American Menopause Society).[4]
Empirical evidence supporting the recommended 3- to 5-year hormone therapy for
management of hot flashes is lacking. This study was motivated by previous
studies, which suggested that hot flashes continued for more than 5 years in some
women but which did not consistently identify the prevalence of moderate/severe
hot flashes after the final menstrual period (FMP) or the risks for hot flashes that
extended beyond 5 years after the FMP. In a meta-analysis of primarily cross-
sectional studies, nearly half of the women reported hot flashes 4 years after the
FMP, and 10% reported symptoms 10 years after the FMP.[5] In a study ofAustralian women, the mean duration of hot flashes from symptom onset to study
endpoint was 5.2 years,[6]
but the duration after menopause, which is a key marker
for medical management, was not identified. A survey of more than 8,000 Latin-
American women indicated that more than 60% reported vasomotor symptoms 12
years after menopause.[7]
We previously reported that the total median duration of
hot flashes was 10.2 years when estimated from symptom onset in the late
reproductive years through the menopausal transition.[8]
In that study, the
prospective identification of hot flashes in the early menopausal transition
contributed strongly to their long duration. However, many participants had not
progressed beyond menopause, and the duration of hot flashes after the FMP,
which is the most common period for medical management, was not well
characterized.
Data are now available to examine the prevalence and risks of hot flashes in the
postmenopausal years. This study estimated the prevalence of hot flashes in
relation to the FMP and evaluated risk factors for hot flashes that continued more
than 5 years after the FMP. We also explored whether these risk factors predicted
a short or long continuation of hot flashes (ie, more than 3-5 y) after the FMP. The
cutoff points for time after the FMP were guided by the data and provided
empirical support for recent revisions in early- and late-stage postmenopause that
were presented in the Stages of Reproductive Aging Workshop (STRAW) + 10
staging of reproductive aging.[9]
Methods
Study Participants
The study evaluated 255 women in the Penn Ovarian Aging Study who reached
natural menopause during a 16-year follow-up period (1996-2012). We included
only participants who reached natural menopause to address our primary aim of

estimating the risk of hot flashes in relation to the FMP. Comparisons of study
variables at baseline between the sample and the remainder of the cohort who
were not observed to reach natural menopause during the study (n = 181) showed
no significant differences, with the exception of age, which was older in the studygroup at baseline (42.2 vs 40.4 y,P< 0.001).
The full cohort of 436 women was randomly identified by telephone digit dialing
in Philadelphia County, PA, using stratified sampling to obtain equal numbers of
African-American and white women, as previously described.[10]
At enrollment,
all women were premenopausal, had regular menstrual cycles of 22 to 35 days for
the previous three cycles, were aged 35 to 48 years, and had intact uterus and at
least one ovary. Exclusion criteria at cohort enrollment included current use of
any hormonal or psychotropic medications, alcohol or drug abuse, major
psychiatric disorder in the past year, pregnancy or breastfeeding, uncontrolled
hypertension, and serious health problems known to compromise ovarian
function. The institutional review board of the University of Pennsylvaniaapproved the study, and all participants provided a written informed consent form.
Study Design
After cohort enrollment, follow-up assessments were conducted for 16 years at
intervals of approximately 9 months in the first 5 years and then annually, with a
2-year gap between assessments 10 and 11. Study data were collected at two in-
home visits, which were timed to the early follicular phase of the menstrual cycle
(days 2-6) in two consecutive menstrual cycles, or approximately 1 month apart in
noncycling women for 14 assessment periods. Assessments 15 and 16 were
conducted by telephone interview. The study was described to participants as a
general women's health study. Trained research interviewers obtained menstrual
dates, structured interview data on overall health, blood samples for hormone
assays, and anthropometric measures. Participants completed a set of validated
self-report measures to assess health and other behavioral measures of the study at
each assessment period.
Study Variables
The primary outcome variable, moderate/severe hot flashes, was reported by the
participants at each follow-up period, using a validated menopausal symptom list
embedded in the structured interview questionnaire.[11]
At each follow-up, the
interviewer asked whether hot flashes or night sweats occurred in the past month,asked whether they occurred in the past year, and asked them to rate their severity
(0, none; 1, mild; 2, moderate; 3, severe). Time in years was evaluated in relation
to the FMP. The FMP was identified after 12 months or more of no menstrual
bleeding and was designated as time 0 for each woman to allow for a longitudinal
evaluation of hot flashes each year before the FMP (up to, but not including, time
0) and from the FMP onward. Cessation of moderate/severe hot flashes was
defined as no moderate/severe hot flashes reported for at least 1 year. Any hot

flashes (mild, moderate, or severe) were evaluated in the same manner as a
secondary outcome.
Estradiol and follicle-stimulating hormone (FSH) were measured byradioimmunoassay in the Clinical and Translational Research Center of the
University of Pennsylvania, using Coat-A-Count commercial kits (Siemens
Healthcare Diagnostics, Tarrytown, NY). Blood samples were collected at each
study visit (providing a possible maximum of 28 samples per woman),
centrifuged, and frozen in aliquots at $80C. Assays were conducted in batches,
which included four visits per participant, to reduce the within-woman variability
resulting from assay conditions. All assays were performed in duplicate and
repeated if values differed by more than 15%. Interassay and intra-assay
coefficients of variation were less than 5%.
Other covariate selections were based on previously determined associations with
hot flashes and the aims of this study: current age, age at the FMP, race (self-reported African American or white), body mass index (BMI !30 kg/m
2,

were also censored at times of reported hormone use, pregnancy, or breast-feeding
in all models. The present sample included no observations of hysterectomy or
bilateral oophorectomy inasmuch as only women who reached natural menopause
were included, as described above. The models were repeated to evaluate asecondary cutoff point at 3 years or less after menopause and had nearly identical
results.
All covariates were defined a priori. Each covariate was added singly to the basic
model with time and to the interaction with time. All potentially time-varying
covariates were treated as such in modeling. Covariates that were associated with
hot flashes at P" 0.20 were included in multivariable models to determine the
independent contribution of the covariates to the outcome of hot flashes. Inclusion
in the final multivariable models was guided by whether each variable remained
statistically significant atP"0.05 or modified other significant associations by
more than 15%. Hypothesized interactions with time were examined for BMI,
race, and hormones. No three-way interactions met the inclusion criteria.
The cutoff points for early and late postmenopause were identified from estimates
of hot flashes at each postmenopausal year relative to the FMP (Figure 1). The
cutoff point on year 6 after the FMP had the largest symptom decrease from the
FMP (with a reliable sample size) and was consistent with the cutoff point
between early and late postmenopause suggested in the revised STRAW + 10
staging.[9]
The secondary cutoff point on year 3 after the FMP had the first (but
smaller) decrease from the peak hot flash estimates and was consistent with the
cutoff point in early postmenopause suggested in STRAW + 10.

Hormone levels were modeled using natural log transformations to reduce the
influence of large values. The subject mean of the two hormone measures
obtained at each assessment period was used in the analysis. Hormones were
entered separately in multivariable models owing to their high correlation. In the
exploratory model, the rate of change for each hormone (slope) before the FMP
was used to predict hot flash duration after the FMP. The rate of change was
calculated for each woman as the linear regression line for all points in the
observed linear range before the FMP, as previously described.[17] Odds ratios(ORs) for the hormones are presented for every one-unit (SD) change in log
hormone.
An exploratory analysis of short versus long continuation of hot flashes after the
FMP included 158 participants who had moderate/severe hot flashes for at least 1
year after the FMP. The short continuation group included participants whose
moderate/severe hot flashes were observed to end within the 3-year follow-up
interval. Participants whose follow-up ended before 3 years and reported
moderate/severe hot flashes at their last observation were excluded because it was
not possible to define the cessation of moderate/severe hot flashes within the 3-
year interval for these women. Also, we excluded participants who had post-FMP
follow-up that was insufficient to accurately determine the cessation of hot flashes
within the first 3 years after the FMP. Participants who had moderate/ severe hot
flashes for more than 3 years after the FMP composed the long continuation
group.
Statistical power calculations were computed using STATA version 12 (STATA,
College Station, TX). Assumptions were based on data in the cohort and included
a 35% risk of hot flashes for the unexposed group (mild or no hot flashes), at least
eight repeated measures per woman, a type I !error of 5%, and a within-woman
correlation of 0.4 for 80% power to detect an OR of 1.93 for risk factors with a
low prevalence of 20%. The detectible OR is 1.76 for risk factors with a
prevalence of 30% and is even lower for risk factors with prevalence greater than30%. For our exploratory aim of identifying predictors of short versus long
continuation of moderate/severe hot flashes, approximately 35% of the 158
women were classified in the short continuation subgroup, as defined by
moderate/severe hot flashes that ended within 3 years after the FMP. We
examined which subject-level risk factors with prevalence of 20% to 50% were
associated with short continuation compared with long continuation after
menopause. We assumed that 25% of women had short continuation of hot flashes

data, there is 80% power to detect ORs of 3.6 to $2.77, respectively, in the
exploratory analysis. Power is greater for continuous risk factors.
All analyses were conducted using the SAS 9.3 statistical package (SAS Inc,
Cary, NC). Statistical tests were two-sided, withP"0.05 considered significant.
Results
Sample Description
Two hundred fifty-five participants were followed for 16 years, with 15.1 mean
outcome reports per participant and 1,336 person-years of follow-up after the last
menstrual period. During follow-up, 203 women (80%) reported moderate/severe
hot flashes, 44 women (17%) had only mild hot flashes, and 8 women (3%)
reported no hot flashes. The mean (SD) age was 42.16 (3.38) years at baseline and
51.47 (3.32) years at the FMP; 49% were African American and 51% were white (
).

Prevalence of Hot Flashes in Relation to FMP
The peak prevalence of moderate/severe hot flashes was 46% and occurred in the
first 2 years after the FMP (Figure 1). Ten years before the FMP, the prevalence
was 16% and increased to 32% in the year before the FMP. The prevalence of
moderate/severe hot flashes decreased slowly after the peak years. Return to
premenopausal levels did not occur until approximately 10 years after the FMP,
when the prevalence (32%) was the same as that in the year before the FMP.
The peak prevalence of any hot flashes (mild, moderate, or severe) was
approximately 74% and likewise occurred in the first 2 years after the FMP. Ten
years before the FMP, the prevalence was 32% and increased to 58% in the year
before the FMP. Return to premenopausal levels did not occur until 5 to 9 years
after the FMP, when the prevalence of any hot flashes was approximately 58%,
the same prevalence as in the year before the FMP.
Hot Flash Duration After FMP
In the subgroup of 182 women who reported moderate/ severe hot flashes after theFMP, the mean (SD) duration of hot flashes after the FMP was 4.6 (2.9) years.
The mean (SD) duration of any hot flashes (mild, moderate, or severe; n = 230)
after the FMP was only slightly longer at 4.9 (3.1) years. Ten percent (25 of 255)
of the participants had no hot flashes after the FMP. The total mean (SD) duration
of moderate/severe hot flashes from initial onset to observed endpoint was nearly
two times longer at 8.8 (4.4) years. The total mean (SD) duration of any hot
flashes was 10.2 (4.2) years ( ).

FSH had a strong interaction with time in relation to hot flashes (P < 0.001),
indicating that the risk of hot flashes increased by 63% with each 1-SD increase in
log FSH before the FMP (OR, 1.63; 95% CI, 1.45-1.83; P < 0.001) in
multivariable analysis ( ). The association of FSH with hot flashes was not
significant after the FMP (ie, early postmenopause stage and late postmenopause
stage at !6 y), when further FSH increases were moderate and stabilized at
postmenopausal levels (Figure 2). Estradiol was evaluated separately in the
multivariable model and had no significant interaction with time. Each unit (SD)
decrease in log estradiol during the study interval resulted in a 14% increase in the
risk of hot flashes (OR, 0.86; 95% CI, 0.77-0.96;P= 0.008).

Predictors of Short Versus Long Continuation of Hot Flashes After FMP
We explored whether risk factors measured before the FMP predicted a short or
long continuation of hot flashes after the FMP. The cutoff point was at 3 yearsafter the FMP, as defined above. Only race remained a significant predictor after
adjusting for all other variables in the model in this subgroup analysis. African-
American women were more than twice as likely as white women to belong to the
long-term group (OR, 2.17; 95% CI, 0.99-4.78; P = 0.05). Contrary to our
expectation, the duration of hot flashes before the FMP did not predict a short or
long continuation of hot flashes after the FMP (OR, 1.05; 95% CI, 0.93-1.17;P=
0.38).
Discussion
The mean duration of moderate/severe hot flashes after the FMP was more than
4.5 years in this population-based sample that was followed for 16 years. The
prevalence of hot flashes peaked in the year after the FMP, but the mean
prevalence of moderate/severe hot flashes did not return to its level before the
FMP until approximately 10 years later. When the total duration of hot flashes
was considered, including time before the FMP, the mean duration of
moderate/severe hot flashes was nearly 9 years. These findings are consistent with
other recent reports of the duration of hot flashes[5-8,18]
and add further evidence
that many women may experience hot flashes considerably longer than several
years after menopause.
Hormone levels were significantly associated with hot flashes during the study
interval. It is noteworthy that increasing FSH levels were strongly associated withhot flashes before the FMP but not in the early or late postmenopausal stages. We
speculate that the strong interaction between FSH and hot flashes was attributable
to the large increase in FSH levels that occurred before the FMP (ie, the first time
interval in the study), when FSH levels approximately doubled (Figure 2),
whereas there was much less increase in FSH levels after the FMP as FSH
gradually stabilized at postmenopausal levels. Estradiol had a weaker association
with hot flashes during the study interval, but the pattern of estradiol across time
seemed to be notably similar to its pattern in the detailed hormone studies by
Burger et al[19]
and Randolph et al.[20]
We observed that the largest decrease in
estradiol levels occurred approximately between year $2 and year +3 around the
FMP, which Randolph et al[20]
identified as a significant decrease. These findings
add support to their conclusion that the hormone patterns around the FMP were arelatively consistent process in ovarian aging and were not associated with age at
the FMP. Further studies might determine whether remission of hot flashes occurs
after this observed decrease in estradiol levels as estradiol levels stabilize at
postmenopausal levels.
The association of race with hot flashes was consistent with previous studies that
found African-American women to be more likely to report hot flashes.[21-25]

A limitation of the study is that current users of hormone therapy and women who
had had surgical menopause were not included; further studies identifying
associations between these conditions and hot flash duration are needed. Our
evaluation of risk factors for a short or long continuation of hot flashes after theFMP was conducted in a smaller subgroup with limited power; thus, results
should be interpreted with caution. A number of women continued to report hot
flashes at study endpoint, and it is possible that longer follow-up would increase
the prevalence and risks of hot flashes after the FMP. Although we evaluated
multiple risk factors known to be associated with hot flashes, other variables may
be important. Our findings are based on a population-based cohort of African-
American and white urban women who were in generally good health with no
current hormone use; thus, they may not be generalizable to all perimenopausal
women.
The primary strengths of this report are the evaluation of hot flashes in relation to
time before and after the FMP during a 16-year period. All participants werepremenopausal at baseline and reached menopause during the study, which
provided a clear identification of menopause with minimal recall bias. The
population-based sample was randomly identified and stratified to have similar
numbers of African-American and white women for analysis of racial
associations. The repeated measures of hot flash reports and hormones were
concomitant, and the multiple samples for hormone assays were collected in the
early follicular phase in menstruating women for consistent assessments.
Conclusions
This study shows that women can expect hot flashes to continue, on average, for 4to 5 years after the FMP, and that some women will experience moderate/severe
hot flashes for more than 10 years after the FMP. The risk of hot flashes is
modified by clinically evaluable risk factors, including race, obesity, anxiety, and
education levels. Because the natural duration of hot flashes can exceed the
duration (3-5 y) recommended for hormone therapy, it is not surprising that
women may experience a return of hot flashes after discontinuing hormone
therapy, although this requires further studies. The findings point to the
importance of individualized treatmentconsidering the potential duration of hot
flashes together with increasing evidence of effective treatment options[4,37]
when managing postmenopausal women.
