Members of the Children’s Hospital Oakland medical staff and community physicians can directly admit most clinically stable patients to Children’s between the hours of 8 a.m. and 8 p.m. Follow the steps below:

1. Call 510-428-3601

2. Provide Preliminary Patient Information to Our Hospitalist•Name,age,gender•Diagnosis(andpossibleneedforisolation)• Patient’schiefcomplaint• Presentingsignsandsymptoms• Yourinitialplanofcare

3. Admit LocationThe direct admit hospitalist will let you know whether your patient will be admitted to the main hospital or the Summit unit. If bed availability is uncertain,thedirectadmithospitalistwillnotifythenursingsupervisor,whowill call you back with updated bed information as soon as possible.

4. Care ManagementIfyouareonChildren’sMedicalStaff,butpreferthatyourpatientbemanagedbythehospitalistserviceduringtheirinpatientstay,letusknow.

5. Direct your patient’s family to the hospital• If admitted to the main hospital:TellthefamilytobringtheirchilddirectlytotheAmbassadorDeskinthemainhospitallobby,andasktheambassador to notify admissions of their arrival.

• If admitted to the Summit unit:ThefamilyshouldgodirectlytotheSummit unit (See map on page 144) to be admitted.

Children’s utilizes a centralized fax referral system to streamline the outpatientspecialtycarereferralprocess.Toaccessthestandardreferralform,gotowww.childrenshospitaloakland.org,andclickon“ReferaPatient.”

To submit a non-urgent outpatient referral, please follow these steps:1.Completethestandardpatientreferralform,andincludeallrequested

patient information. 2.Includeauthorization(ifrequired),medicalrecordnotes,pertinentoperativenotes,andpertinentdiagnosticimagingorlaboratoryresults.

Cardiac ServicesThePediatricCardiacCenterprovidesacompleterangeofdiagnostic,therapeutic,interventional,surgicalandfollow-upservicesforchildrenwithalltypesofheartdisease.Wecareforpatientsfrombirththroughadolescence,andintotheirearly20s,withtransitionalservicesavailableforadults.Acomprehensivefetalassessmentprogramcompletestheservicesofferedbythecenter.

EightpediatriccardiologistsandthreepediatriccardiacsurgeonsleadtheCCS-approvedcenter,withsupportfromadedicated,speciallytrainedteam,includingasocialworker,clinicalnursespecialist,nurseclinician,techniciansandofficestaff.Thecentergeneratesmorethan7,000outpatientvisitseachyearacrossninediffer-entlocations,andperformsabout200cardiacsurgeriesand200cardiaccatheteriza-tionseachyear.ThecardiologistsandcardiacsurgeonsworkcloselywithChildren’sanesthesiologists,neonatologistsandintensivists.

Fourpediatriccardiologistsrunourfetalconsultservice,performingover900fetalechocardiogramseachyear.Thepediatriccardiologistworkscloselywiththefamilyandobstetrician/perinatologisttocoordinateprenatalcareandthetimingandloca-tion of delivery. Transfer to Children’s Hospital is facilitated as indicated.

Inaddition,thePediatricCardiologyMedicalGroup–EastBaynowoffersHealthyHearts,amultidisciplinaryprogramdesignedtopreventandtreatchildhoodobesityandrelatedillnesses,suchasdiabetes,heartdiseaseandhighbloodpressure.HealthyHeartsstaffoffersculturallysensitive,individualizedfamilysupportandreferralstocommunityprogramsdesignedtoencouragehealthylifestylechanges.(See Healthy Hearts on page 41 for more information about the program.)

Wecommonlyreceivereferralsforchildrenwithlymphadenopathy,splenomegalyandabdominalmassestodeterminetheiretiology.Ourprogramincludesamultidisciplinarytumorboard(TB)comprisedofoncologists,surgeons,pathologists,radiologists,radiationtherapistsandbonemarrowtransplant physicians. •Bone tumors: The division provides advanced treatment for all bonetumors,includingosteogenicsarcoma,Ewing’ssarcomaandprimitive neuroectodermal tumors(PNETs)ofallstages.Our multimodality approach may includechemotherapy,radiationandsurgerydesignedtopreservelimb function. An orthopaedic oncologicsurgeonisonstafftoperform the initial biopsies and thenthedefinitivesurgeryforallthese children.

•Central nervous system tumors: (See Neuro-Oncology Program on page 48.)

•Germ cell tumors: The division provides comprehensive care for bothbenignandmalignantgermcell tumors.

•Histiocytic disorders:Usingprotocols developed by the Histiocyte Society and other experts,thedivisionprovidestherapyforClass1(Langerhanscellhistiocytosis),Class2(virus-associatedhistiocytosissyndrome,familiallymphohistiocytosis),andClass3(malignanthistiocytosis)disorders.

•Kidney tumors: We provide sophisticateddiagnosisandstagingofkidneytumors,includingcytogeneticandmolecular classification. We provide specific therapy for Wilms tumorandotherraremalignantkidney tumors such as renal cell

carcinoma,clearcellsarcoma,malignantrhabdoidtumorandpoorly differentiated sarcomas.

•Leukemia/Lymphoma: The division provides comprehensive care for children and adolescents with both acute lymphoblastic (ALL)andacutemyelogenous(AML)leukemia,andfortheless common chronic childhood leukemias. Available treatments includestate-of-the-artsalvagetherapy for relapsed patients offeredthroughourassociationwith the Therapeutic Advances inChildhoodLeukemia(TACL)consortium,comprisedofagroupofinstitutionsthattogetherdevelopphaseI/IIclinicaltrialsofnewdrugsandnoveldrugcombinations in which there are pre-clinical data that support testingthesenewapproachesinrecurrent childhood leukemia. The division also provides a full spectrum of treatment options forchildrenwithbothHodgkindiseaseandnon-Hodgkinlymphomas. We offer a variety of transplantation options for those patients for whom it is indicated.

•Liver tumors: The division providestreatmentformalignant(hepatoblastoma,hepatocellularcarcinoma)andbenignlivertumors.

•Neuroblastoma: The division treatspatientswithlow-,intermediate-,andhigh-riskdisease.Thisincludesprovidingbone marrow transplantation when appropriate,aswellasprovidingexperimental therapy to relapsed patients.

Our treatment facilities include a 12-bedinpatienthematology/oncologyimmunecompromisedHEPA-filtered unit with private rooms that serves as our bone marrow transplant unit,a14-bedadjoiningunit,anda20-beddaytransfusionandchemotherapy unit that also houses ourpheresisprogram.Patientsare seen in the Outpatient Clinic Monday to Friday.

We provide comprehensive outpatientoncologycareincludingchemotherapy administration in our Children’s Hospital Oakland day hospitalclinic,aswellasinclinicsinNapa,Nevada,andWalnutCreek.Housingisavailablelocallyforparentsduringtheirchild’sinpatienttreatment.

•Red cell disorders: The division provides consultative services for the evaluation and treatmentofanemiaofanycause,includingcongenitalanemia,nutritionalanemia,anemiasecondarytoenzymedefects,orhemoglobinopathiesormembranedisorders,andsecondaryanemiacaused by another illness or infection. We also evaluate patients with inherited bone marrow failuresyndromesandcongenitalhemochromatosis. When indicated,thedivisionwillprovideongoingcareforthesepatients,includingreferralforbonemarrowtransplantation when appropriate.

• Iron disorders: The division provides consultative services and treatment for patients with irondeficiency,especiallythosepatients who appear refractory to oralironreplacementtherapy,andcongenital(neonatalorjuvenile)oracquired(transfusional)hemochromatosis.

NeonatalFollow-UpProgramservices include neurodevelopmental pediatricassessment,referrals,healthfollow-upandcasemanagement.ChildrenmustmeetCCSHighRiskInfantFollowUp(HRIF)criteriaand have been hospitalized in an intensive care nursery. Services are billed to insurance and are supported byCCSandChildren’sHospital&Research Center Oakland. A HRIF Programisrequiredforallhospitalsthat have CCS-approved neonatal intensive care units. Referrals to the NeonatalFollow-upProgramareaccepted from community providers if the child meets HRIF criteria and was hospitalized in an Intensive Care Nursery

(PFT):Completetestingincludingflowvolumeloops,lungvolumes(plethesmographyornitrogenwashout),diffusingcapacityandairway resistance is available and usually can be performed on childrenasyoungas6yearsoldthroughyoungadult.

• Impulse Oscillometry Spirometry (IOS): IOS provides asimplemethodofanalyzingcomponents of reactive airways disease,asinasthma,inchildrenwhoaretooyoungorunabletoperform traditional pulmonary functiontesting.Typically,childrenasyoungas3yearsoldcan complete the procedure.

• Infant Pulmonary Function Testing (IPFT):Usingamildsleepmedication,infantsasyoungas a few months old can be tested todeterminelungdevelopment
