Resident Guide & Information Pack
Welcome to Hillbrook Grange! We trust your time with us will be happy and that you will
feel comfortable and well cared for.
We have produced this information guide to our Home - Hillbrook Grange - to help persons
who are thinking of applying for admission and for those who already live in the Home in
order that they know what to expect from us and the services which we provide.
We would like you to read carefully the information you find in this Guide. Please keep this
information in a safe place so that you can refer to it as necessary.
If you need help to read or understand any part of this guide or require a large print version,
please do not hesitate to ask a member of staff or the Registered Manager. An audio version
of this guide will be available shortly.
If there are changes to the Home, we will also seek to update the information in the Guide so
that you are kept fully informed of what is happening.

The Home
Originally established in 1953, the Home was managed by the Bramhall and Woodford Old
People’s Welfare Committee, a charity established to provide residential care for the people
of Bramhall and Woodford. Following incorporation in 2012, Hillbrook Grange is now a
private company limited by guarantee, and remains a registered charity. The Care Home
provides the highest standard of residential care for 41 older people aged 65 and over,
including older persons who might
require care for a Dementia type
illness.
The main building – the Old House
- is late Victorian with two
extensions one built in the 1960’s
and the other completed in 2013.
The Home is situated in seven acres
of beautiful grounds with seating,
pathways and wooded areas. It is
within walking distance of
Bramhall village and local train and
bus services. There is ample car
parking within the grounds.
There are 41 bedrooms in the Home (13 of which have en-suite facilities) and two of these
rooms are fully equipped for wheelchair users. There are three lounges, a quiet
lounge/library and a large dining room. The communal bathrooms are fitted with bath hoists
and there are grab rails in all of the toilets. The Home has two floors and there are two
passenger lifts.

Living at Hillbrook
Hillbrook Grange aims to promote the privacy, dignity and independence of our residents,
whilst encouraging them to maintain social contact with family and friends both inside and
outside of the Home.
At Hillbrook Grange, we value each individual who comes to live here. We welcome
applications from persons aged 65 years and over who may be in need of social and personal
care, either as a permanent resident; or in need of short stay/respite care, and who enjoy the
opportunity to share the richness and diversity of their experiences.
We can assure everyone who comes to live at the Home that they will be treated with respect
and dignity and that they will receive the care and support they need in line with their
assessed requirements and their individual wishes. Additionally, we seek to ensure that
everyone is kept safe and free from harm by assessing and managing the risks to peoples’
safety and well-being.
We actively encourage prospective resident’s family and friends to visit the Home prior to
completing the application process.
We require that the needs of every applicant be comprehensively assessed. The Home carries
out its own assessments of self funding Service Users using our trained, experienced staff.
The Home’s Care Management team give careful consideration to each applicant which will
include consulting staff and outside professionals as appropriate. Depending on vacancies, we
will usually seek to make a decision on the offer of a placement within seven days following
completion of the application form. Where there are no current vacancies, it is possible to
place an applicant on the waiting list; however their needs would have to be re-assessed prior
to admission. The Home will confirm all such decisions in writing.
Application
To make an application you should:
• Contact the Registered Manager by letter, telephone, fax or email.
• Request a brochure and information pack
• Complete an initial enquiry form with the senior carer on duty
• Visit the Home and talk to residents, and perhaps stay for a meal
• Complete all the relevant sections in the Admission Application Form
• Have your needs assessed
• Be given a decision within seven days of completing the application procedure.
The Care Plan
Both permanent and short stay Service Users will have an individual Care Plan.
For prospective permanent residents only, the two week assessment stay will enable you and
the staff to get to know each other and will help you to make known your wishes and
preferences.
You will have a key-worker allocated to you and this person will be responsible for your Care
Plan. Your Care Plan will be regularly reviewed and you will have the opportunity to discuss

your care with your allocated key worker who will co-ordinate your Care Plan with the staff.
You may also choose to have a relative or friend involved in this process, but nothing will be
discussed with them without your permission.
An Annual Review will also take place.

All of our staff have undergone a rigorous Criminal Records Check.
To assist you further, all staff wear name badges and their role may be identified by their
uniform.

Social contact and Activities
You are encouraged to maintain social contact with friends and to continue to participate in
any activities which you might have enjoyed before coming in to Hillbrook Grange.
At Hillbrook, you may
participate in regular keep-fit
classes, chair zumba, bingo,
quizzes and puzzles, craft
sessions, coffee mornings,
parties for celebratory and
commemorative occasions,
as well as other forms of in–
house entertainment.
Regular outings also take
place. These might involve
canal boat trips as well as
visits to garden centres,
shopping malls, the seaside,
and the theatre.

We also provide a weekly shop from which residents can purchase confectionery, greetings
cards, stamps and other essentials.
Religious services are conducted by visiting clergy on a monthly basis.
Hairdressing*
This service is provided weekly, but of course you are welcome to request that your own
hairdresser come to the Home at a time convenient to you.

Chiropody* The senior in charge of the Home can arrange chiropody visits for you. Alternatively, you
may wish a relative of friend to take you to your own chiropodist.

If your local Doctor is located outside of the Home’s catchment area, then you or a family
member should ensure that you are registered with a local Doctor from one of the above
mentioned medical centres prior to admission.
District Nurses
The Bramhall Health Centre District Nursing Team supports the Home. If you are in need of
a District Nurse visit, please speak to the senior on duty who will arrange this for you.
The District Nurses usually visit on Tuesday and Friday.
Hospital
Stepping Hill Hospital is the nearest general hospital.
Library
The local community library provides books, which are exchanged fortnightly. In the Home’s
library, there are books which you may take to read and return when you have finished.
Telephone
A public pay telephone is situated in the main corridor adjacent to the Reception foyer. If you
prefer, you can arrange at your own cost to have a telephone installed in your bedroom.
Visiting
Friends and family may visit at any time. There are no restrictions on visiting times unless at
the specific request of the resident.
Complaints and Protection
We are committed to ensuring the safety and well-being of our residents at all times. There
are policies and procedures in place to ensure the protection of our residents from abuse. Any
evidence or report of abuse will be investigated promptly and appropriate action will be
taken.

A policy is in place, which precludes staff from assisting in the making or witnessing of
residents’ wills. Accordingly, a family member, solicitor or a friend must deal with matters
relating to advice on finance and personal insurance.
Fees
The Home accepts self-funding as well as Local Authority funded residents. The fees are
maintained within the rates advised by Stockport Metropolitan Borough Council and are
payable 4 weeks in advance by bank standing order mandate. For Service Users who are fully
funded, there are no top up fee charges. Top up fees representing the difference between the
weekly fee and the Local Authority contribution will be met by the resident’s family.
A contract is issued on the resident’s admission to the Home.
Day-to-day running of the Home
The Registered Manager has the qualifications and experience necessary to run the Home and
to meet the aims and objectives set out in the Statement of Purpose, under the overall
responsibility of the Home Committee, a sub group of the main Board.
The Home Committee comprising the Chairman, two directors, the Registered Manager, and
the General Manager oversee the running of the Home and ensure that staff are appropriately
trained in the care of older people.
Service Users’ money
Service Users are expected to control their own finances, except where they do not wish to do
so; in which event a small sum of money may be kept in the safe. All transactions are
recorded and safeguards are in place to protect the interests of Service Users.
Safe Working Practices
Policies and procedures exist to ensure the Health and Safety of residents, staff and visitors to
the Home. All staff are trained in Health and Safety issues and the majority of staff are
qualified first aiders. There is a Health and Safety committee in the Home.

Individual accommodation, furniture and fittings
All Service Users have a well appointed bedroom to ensure their comfort and privacy.
Of the forty one bedrooms in the Home, thirteen have en-suite facilities. Two of those rooms
have been specifically equipped to accommodate those who might require wheelchair
assistance. All bedrooms have a comfortable bed, curtains, mirror, overhead lighting and
bedside lamp. There is also an easy chair, bedside cabinet, chest of drawers, wardrobe, and a
small folding table. Each room has a safe for money and valuables. A lockable medication
cabinet is also provided for those residents who self-medicate. (Please note that the senior on
duty on the day of your admission will complete a self-medicating risk assessment with you
in order to promote your rights to independence). Call bells are situated in each bedroom so
residents may summon 24/7 assistance. All doors are lockable and residents may have a key
if they wish.
Laundry services
The Home’s laundry service will provide you with clean bedding, towels, and face cloth. If
you would like to have your personal items washed by the laundry staff, please ask the senior
on duty.
In order to assist staff, it would however be helpful if you could have you personal items
labelled with your name.
Action in the event of a fire
There is a fire notice affixed to the back of each bedroom door giving instruction on what to
do in the event of fire. The fire alarm system is tested weekly and Service Users will be
informed when this is due to take place.

Quality Assurance
The latest survey suggested that residents and their relatives enjoyed the warm homely
atmosphere of the Home and that Service Users enjoyed living here. They made helpful
suggestions about improving the social activities, which are now being implemented.
To ensure high quality standards within the Home:
• We seek to involve residents and their families in the development of the Home by
consulting with them and keeping them fully informed of changes.
• We regularly seek the views of residents on the quality of the services we offer, so
that we can continue to improve them and address any deficiencies.
• The management team regularly review the operations of the Home with a view to
their continuous improvement.
• We provide staff with regular supervision, and training and development.
• We take complaints very seriously and we aim to address and to resolve them
amicably and in a timely fashion.
• The Home scored a top rating with the CQC during their most recent unannounced
inspection in May 2013. A copy of their report is available on request or directly
from the CQC website.
We welcome feedback. Accordingly, Quality Assurance questionnaires are freely available
within the Home and residents are free to complete these at any time.

This Guide has been prepared to assist Service Users on admission to the Home. Please do
not hesitate to speak to a member of staff, if there is something about which you are unsure or
if there is something you need. The Registered Manager is available in the Home five days
per week while the Chairman and the General Manager are generally present three days a
week; and all will be happy to assist you with any queries you might have.
