1. INTRODUCTION 1.1. Purpose of Request for Proposal (RFP) The purpose of this RFP is to appoint Field Agency for conducting the fourth round of Community Behaviour Tracking Survey (CBTS) 1.2. Background of the survey Uttar Pradesh is India’s most populous state, with approximately 200 million people, and with weak health infrastructure and poor health outcomes. While significant improvements in health and development outcomes over the past decade point to the impact of government interventions, coverage and quality continued to be significantly below the levels required to achieve the state’s ambitious health and development targets. There is a tremendous opportunity to improve the state execution capacity to enhance the efficiency, effectiveness and equity in health and development. This is the basis upon which the Bill & Melinda Gates Foundation (the foundation) was invited to provide techno-managerial assistance to the Government of Uttar Pradesh (GoUP). Subsequently, a Technical Support Unit (TSU) was set up in November, 2013, by a consortium consisting of University of Manitoba, India Health Action Trust, JSI Research and Training Institute and Engender Health, against the Memorandum of Cooperation (MoC) signed by the foundation and GoUP in December 2012. The TSU’s mandate is to support the Government of India and Government of Uttar Pradesh in the implementation of RMNCH+A. Since its establishment, the Uttar Pradesh Technical Support Unit (UP TSU) is supporting the GoUP to improve the key RMNCH+A outcomes. The TSU provides techno-managerial support to meet/improve those outcomes. One of the important components of techno-managerial support is to provide real-time concurrent data on critical maternal and child health outcomes. Since inception, IHAT-UP TSU has implemented three rounds of community level surveys in 25 high priority districts (HPDs) and results are being used by the government in different review mechanism. In order to start next round of survey within a short time period IHAT-UP TSU wishes to hire a third party agency who is having adequate experience in these type of activities and located in Uttar Pradesh. In brief IHAT-UP TSU needs the following for immediate hiring, followed by ~7 days training and then starting field work from December 2016. The CBTS in high priority blocks: The first round of the CBTS has been completed in 100 blocks of the 25 HPDs. The second round of the CBTS has been completed in 20 selected blocks of the HPDs. The third round of the CBTS has been completed in 25 HPDs (125 blocks). Now, the IHAT- UP TSU is planning to roll out the fourth round of the CBTS in same 20 blocks covered in CBTS 2. The basic purpose of this survey is to monitor health outcomes in the poor performing blocks. Within the block, the survey will be conducted in the selected ASHA catchment areas which will be treated as Primary Sampling Units (PSUs). This survey will provide the block level estimates in the selected blocks. It is planned to roll out the CBTS 4 in high priority blocks from December 2016. 1.3. Specific Objectives and Scope of the survey

As mentioned the field staff will be engaged for the next 3-4 months in continuation. Some of the main features of the survey are

Survey design: The design of the survey is finalised. IHAT will share the design with the hired agency.

Use of mobile based application: This round of survey will be conducted in mobile based application. Mobile based application will be provided by IHAT before rolling out the survey to hired agency.

Training of the survey team: Before rolling out the survey, the survey team will be thoroughly oriented on the design, objective, importance, and tools of the survey. The investigators will be trained on mobile based application and pre-testing will be done. The training logistics and management will be done by FA.

Field monitoring and data quality assurance: FA will be overall responsible for field monitoring and data quality assurance, however the IHAT team will be monitoring the survey in terms of data quality. A protocol will be established and implemented to reduce the potential non-sampling errors. The feedback given by IHAT has to be incorporated. Any deviation & sub-standard data has to be compensated by the FA as per guidance of IHAT.

To maintain the protocol given steps will be followed:

o Standardization of the interview method – how to ask the questions and how to record the responses – through an interviewers’ manual that will be used for training the field research teams.

o Back checks: During the field data collection period in each block, the field supervisor will visit at least 2 completed/ongoing PSUs per interviewer and verify the following:

Whether the interviewer had visited the PSU at the reported date. Whether the interviewer had visited the selected ASHA area. Whether the interviewer has covered all the households within the boundary of ASHA area – this will

be done through visiting at random the households in the periphery of the area. Whether the interviewer has missed any eligible respondent or interviewed any ineligible respondent

in the ASHA area. IHAT Team verify the geocodes for the location of interview.

If there has been error in any of the above criteria, the data collection in that PSU or replaced PSU will be conducted by FA, without any additional cost to IHAT.

IHAT will conduct the feedback meeting periodically with the field team. In the meeting IHAT team will discuss and present key findings related with progress of the survey and data quality issues. For the data quality assurance following analysis will be done and discussed with the field team:

Average number of household covered per day per investigator. Average time spent in a day in the field. Target vs. achievement of the required sample in each PSUs. Consistency in recording skip patterns. Average time taken to complete an interview.

IHAT team will also discuss the field experience and challenge in the meeting and appropriate action will be taken accordingly. The first feedback meeting will happened after 3 days of data collection and after that on weekly basis. All logistic arrangements for the meeting has to be done by FA.

1.4. Geographical Coverage The survey will be implemented in selected 20 blocks of the HPDs of Uttar Pradesh. IHAT will share the block details with the hired field agency. 1.5. Sampling Design The design is finalised. IHAT will share the design with the hired field agency. Around 45555 respondents (of all 4 groups) will be surveyed from 20 blocks and around 575 respondents of each survey group from a block. The sample size may varies across the groups and across the blocks.

If required additional PSUs need to be covered to achieve required sample size. 1.6. Survey Implementation Plan The survey implementation plan will be shared by IHAT in the beginning of Survey. The mid-course change in the plan needs to be accommodated. 1.7. Data Collection Procedure The female field investigators will interview the respondents of different survey groups moving across the ASHA catchment area/village by using mobile based application. One investigator will be completing one ASHA area within stipulated time. 2. SCOPE OF WORK The proposal should be prepared on the basis of the following scope of work. 2.1 The Field Agency will be required to appoint the following manpower to conduct the CBTS-4:

The list of investigators and supervisor will be provided by IHAT. FA has to be ensured that at least 80% of field investigator and 60% of supervisor have experience in previous round of CBTS. Any violation needs to be approved by TL M&E or person designated by him. Final selection will be done by IHAT. The IHAT recommendation related to recruitment and termination will be implemented by FA.

2.2 Training of Field Team by IHAT /FA The training will be organised by FA but will be facilitated and monitored by IHAT. The training manual and agenda will be finalised by IHAT. The resource persons for this training will be IHAT survey specialists and other trainer identified by IHAT. 2.3 Major contents of the Schedules

Sample Schedule is enclosed as per Annexure- D. 2.4 Transportation For effective and efficient implementation of the field operation, FA must provide one vehicle to each survey team (consisting of 6-7 members).

2.5 Salary and DA for Field Team The agency must pay at least the minimum salary and DA to field team.

2.6 Layers of Monitoring and Supervision FA has to place at least two-tier monitoring and supervision mechanism to maintain the data quality. Quality supervisors will ensure the data quality through supervision and monitoring; also provide hand hold support whenever needed. Team coordinators will coordinate the field team, supervise and monitoring the data collection. The feedback given by the IHAT has to be completed within 24 hrs. 2.7 Team Composition The FA will provide adequate number of investigators, quality supervisors and team coordinators to complete the survey within

the timeline. The field staffs (field investigators and team coordinators) will be divided into teams and each team will consist

of 6-7 staffs (5-6 field investigators and 01 team coordinator). Field implementation plan and deployment of field staffs will be

mutually decided by IHAT and Field Agency during training. 2.8 Supplies and Equipment The requisite tools, other equipment’s such as mobile handsets without SIMs (to be self-arranged by FA), mobile charger & power bank, and consumables (medical kit, bowel, Mid-Upper Arm Circumferences – MUAC tape) will be provided by IHAT. The FA will be held responsible for any damage resulting due to the careless handling of the equipment. Unavailability of such items will be considered as quality compromise and has financial implication.

2.9 Informed Consent Before undertaking the survey in any household, FAs must ensure that informed consent is read to each eligible respondent to take their prior consent before their responds.

2.10. Establishment of Field Office of Field Agency FA appointed for this survey is expected to have a regular functional office equipped with electricity, IT infrastructure, and means of communication (fax, landline telephone, internet, etc.). This office should be functional during the entire duration of the contract. The head office should have a 24 hrs inbound call services to answer any calls from community related to survey reported by community or data collectors.

2.11. Data Management FA will be responsible for collection and uploading of data to the IHAT server on daily basis. FA will ensure that there will be

no data loss/damage before successfully submission to server. Any failure needs to be compensated by FA

3. ROLES AND RESPONSIBILITIES OF FIELD AGENCY (FA) The selected FA will be required to sign a contract with IHAT for implementing CBTS-4 in the blocks.

The principal responsibility of the FA is to carry out the survey fieldwork in the sample/assigned PSU till achievement of desired block wise sample size and group wise sample. 3.1. Overall Roles and Responsibilities Assigned to Field Agency

concerned: Monitoring and operation of the field activities

o Back-check, spot-check and quality ensure FA should strictly adhere to the timeline of the survey failing which a penalty would be imposed. FA are required to ensure that each interview is done only after obtaining the informed consent of the respective

respondent. FA will also ensure that investigators keep all the information strictly confidential. FA are required to extend full cooperation to the monitoring personnel from the IHAT. A set of field check lists will be produced by IHAT to assess quality of data collected by the Field Agency. FA is required to

comply with the feedback and reorient the investigators if needed. FA must make provision and pay for group insurance to all the field staffs. At any stage, after signing the contract, FA is strictly prohibited from making any sort of subcontracting of any of the survey

activities. IHAT reserves the right to take appropriate legal action against FA if any such incidence is noticed or reported. IHAT will be providing instructions/suggestions from time to time to maintain the quality of data. These instructions must

be strictly followed by FA for the smooth conduct of the survey. IHAT reserves the right to terminate the contract or to take appropriate action against FA if found not adhering to the survey

protocols or roles and responsibilities or instructions/suggestions provided by IHAT at any stage of the execution of the survey.

4. IMPLEMENTATION SCHEDULE/TIME LINE FOR DATA COLLECTION

The CBTS 4 fieldwork would be completed within a period of 04 months after completion of training. The detailed deliverables

along with time line are given in the table below:

The training will be a residential training therefore FA should identify a

venue for training where accommodation for the candidates and 4-5

halls for training are available. The arrangement has to be certified by

IHAT. They should also ensure that training should continue from 9:00

am to 7:00 pm each day without any interruption. The entire training

logistics such as training venue, training halls, projectors, white boards,

markers, accommodation and food for the trainees, vehicle for the field

training etc. will be arranged by the FA. The field training will be a demo

of actual field.

All the Schedules and manuals in bilingual will be provided by IHAT with

sufficient quantity before starting the training.

2 Training of the recruited

candidates

The recruited candidates along with some extra waitlisted candidates

will be trained thoroughly on the tools before starting the field work. The

training will be organized in small groups (25-30 candidates in each

groups). Duration of the training will be of 7 days which includes about

5 days of class room training and 2 days of field training. The training

will be organised by the FA but will be facilitated and monitored by

IHAT. The training manual and agenda will be finalised by IHAT. The

resource persons for this training will be IHAT survey specialists and

other trainers identified by IHAT. In case the training is extended for 1-

2 days, IHAT shall borne the additional cost and the FA shall provide the

required services.

3 Sharing of list of selected

candidates

Final selection of the candidates will be done based on their

performance during the training. After completion of training, the FA

will share final list of selected candidates (field investigators, team

coordinators, cluster coordinators, and quality supervisors) to IHAT.

The candidates appeared in the list will only do the survey, no additional

candidates (out of list) will be allowed to work. For any change, the FA

will inform and take prior approval of IHAT.

4 Submission of Progress

Report

The FA must submit a progress report weekly.

6. PENALTY CHARGES

a) In case of any violation of the deliverables the IHAT will charge a penalty of maximum 15% (including 5% performance guarantee) as described below;

I. For each phase of payment, 5% amount will be deducted as performance guarantee, which will be paid along with the last phase of payment (8th phase) after deducting the penalty, if any

II. Maximum / Additional 10% of penalty will be charged against each phase/deliverables wherever delay incurred by the FA.

7. RISKS OF ENGAGEMENT

a) In case of data loss or damage or manipulation of data or of poor quality of data; fresh data will be re-collected by FA without any additional cost from IHAT

b) Cost of any lost/fully damaged asset of IHAT (mobile handsets without SIMs, mobile charger & power bank, etc.), will be recovered from FA on book value.

c) Repairing cost of IHAT’s assets (as described in point b above) during tenure of contract will be borne by FA. d) Payment will be done in phases after ensuring that pre-decided proportion of data collection is complete and data

quality standards are met.

8. GUIDELINE FOR SUBMITTING PROPOSALS

8.1. Technical Proposal

Items to be included in the proposal are as follows:

a. Organizational Profile

I. Name and address of the Organization. II. Year of establishment.

III. Legal status of the Organization – Whether Organization is registered under society Registration ACT in India or is an autonomous body or a Limited company or partnership firm, etc., and details thereof (e.g., name(s) of partners, Managing Directors, Chief Executive Officers, key persons)

IV. Principal nature of activities undertaken. V. Organizational structure and names of personnel, their titles, and curriculum vitae including nature of appointment and

duration with the organization of the key personnel proposed to be involved in this survey VI. Communication details of the organization: mailing address, telephone and fax numbers, email address, etc.

VII. Organogram of the project

b. Survey Experience

I. Area of specialization of Field research/survey as stated in pre-qualification criteria (enclose copy of the papers, letter of engagement, etc.)

II. The geographical coverage (State/UT) of surveys conducted by FA. III. List of large-scale demographic and/or health and/or social sector surveys conducted by the Organization (including CBTS,

DLHS, NFHS, AHS, CES, CE-NRHM, etc.) with information on geographical area covered. IV. Names and addresses of sponsoring or funding agencies for whom the earlier surveys were conducted. (Indicate key

person’s name and contact details of sponsoring/ funding agencies). V. Were the surveys completed in time? If not, reasons thereof.

VI. The cost of each survey conducted should be submitted separately in a tabular form with documentary evidence. VII. Documentary evidence of experience of conducting at least one large-scale demographic and/or health surveys in the

last three years.

c. Financial Status of the Organization (For the last 3 financial years)

I. Total revenue and expenditures of the organization. II. Latest copy of the certified Audited Annual Accounts in support of the Financial Status.

8.2. Financial Proposal

I. The financial quotes should cover the entire cost of the household survey, training of survey staff, travels & allowances, provision of vehicles for each team, all resource costs etc.

II. The total cost quoted should be inclusive of all taxes including service tax.

8.3. Submission of Proposal

The agency shall submit all bid documents in a sealed envelope. Bid must consist of the following:

a. Eligibility criteria as per the criteria given in Annexure - A of this document

b. Technical proposal, super scribing on the top right hand side of the envelope as “Technical Bid for CBTS-4”.

c. Financial proposal super scribing on the top right hand side of the cover as “Financial Bid for CBTS-4”

d. The envelopes containing the Technical and Financial Proposals are to be enclosed in another envelope, clearly super scribing the, “Bid for Hiring Field Agency for Conducting CBTS-4 in Uttar Pradesh”.

All the pages of the Financial Proposal shall be duly signed by the authorized signatory of the Agency in ink before submission.

Corrections, if any, shall be countersigned. The Agency should submit the Bid in the prescribed Performa Annexure-C only, failing which the offer shall be summarily rejected.

The last date for submission of complete proposal with all supporting documents (by hand, by post or by courier) is 19th December, 2016 up to 3.00 PM. Any proposal received after the prescribed time will not be entertained. IHAT will not be responsible for any loss or postal delay.

8.4 Evaluation of the proposal for FA:

An evaluation committee of IHAT, would examine both the technical and financial bids based on the details provided in the Invitation for proposal for those agencies who are short listed as per the eligibility criteria for technical evaluation.

Selection of FAs

FAs are required to score a minimum of 60 points out of 100, (60%) to be qualified in the technical proposal. Technical Evaluation on the basis of:

FAs have to submit financial bid in separate sealed envelope. The evaluation committee reserves the right whether financial bid should be opened or not.

A clear detailed financial proposal (Annexure-C) carrying unit cost for each and every head item of the survey must be submitted. An ad-hoc figure for the cost of survey will be summarily rejected at the time of financial proposal opening.

For the final selection, Quality and Cost Based Selection (QCBS) criteria will be adopted. The weightage for technical and financial proposal will be 60 and 40 percent, respectively.

Points obtained by the Field Agency for both Technical (60%) as well as Financial (40%) scores would be clubbed for the final selection. Total Score = 0.60 x Technical Score + Lowest Financial Bid/Financial Bid of the Field Agency x 0.40

9. General Terms & Conditions

a) The proposal, along with all the correspondence and documents exchanged by the Agency and IHAT, shall be written in

the English language.

b) IHAT reserves rights to amend any of the tender conditions or apart thereof before the last date for the receipt of the

tender, if necessary.

c) The field agency should furnish an Earnest Money Deposit (EMD) of Rs. 300,000/- by means of DD/BG on any

Nationalized Bank / Scheduled Bank payable in favour of the “India Health Action Trust”. The tenders received without

the Earnest Money Deposit will be summarily rejected. The Earnest Money Deposit will be refunded after the completion

of the entire process. No interest will be payable by the IHAT on the Earnest Money Deposited.

d) The earnest money deposit will be refunded to the unsuccessful tenderer by registered post at the expiry of the period

of validity of tender or the entrustment of the work to the successful tenderer whichever is earlier. e) No Proposal shall be accepted unless it is properly sealed. f) The Financial Proposal will be evaluated only when a Field Agency has qualified the technical proposal. g) The proposal will be valid for 30 days from the date of submission; IHAT will make its best effort to elect Field agency

within this period. h) The agency is advised to attach any additional information that is considered necessary to establish its capabilities. No

further information will be entertained after submission of application unless it is required by IHAT. The IHAT, however,

reserves the right to call for additional information and clarification on information submitted by the agency. i) Proposals must be received by IHAT, at the following address :

j) The data, schedules, reports and other material used by the agencies during the implementation of the survey shall remain the property of the IHAT. The Agency will not be allowed to use this information in any forum, national or international, without the explicit permission given in writing by the IHAT.

k) IHAT reserves the right to cancel the RFP at any stage without citing any reason.
