Section 1- Proposal Provider Instructions 1.1 Overview of the RFP Consolidated School District 158 Overview Consolidated School District 158 is located in the northwest Illinois counties of McHenry and Kane, and serves the community of Huntley, and portions of Algonquin, Lake in the Hills, Union and Lakewood, Illinois. The district employs approximately 1,500 certified and non-certified staff, striving to provide a good mix of new and experienced teachers. Consolidated School District 158 serves approximately 8,900 students at 8 different schools in grades Pre-Kindergarten through 12.

Purpose of RFP

Consolidated School District 158 is seeking proposals from qualified consultants for technical assistance with development and implementation of an Arch Flash Hazard Study as recommended by National Fire Protection Association's 70E Standard for Electrical Safety in the Workplace. Your firm has been chosen as a possible candidate for this contract. As such, you are invited to submit a proposal. Notification of Candidate Consultants The submission of a response in no way obligates Consolidated School District 158 to any proposal provider. All consultants who submit Proposals will be notified of the results of the selection process. RFP Addenda In the event that modifications or additions to the RFP become necessary, consultants will be notified in writing.

Proposal Preparation Costs

The consultant will absorb all costs incurred in the preparation and presentation of the Proposal.

Supporting Documentation

All sales, supporting materials, and other documentation submitted with the Proposal will become the property of Consolidated School District 158 unless consultant states in writing at the time of submission.

Any changes in deadlines will be communicated to all consultants in writing. Any request for extension in deadline must be made in writing to Doug Renkosik via e-mail AND fax. Granting of the extension is at the discretion of Consolidated School District 158. We reserve the right to disqualify any consultant who does not comply with these deadlines.

Section 1- Proposal provider Instructions 1.5. Evaluation Process - Consolidated School District 158 will evaluate Proposals on the following (but not limited to) criteria:

• Proven ability to implement solutions in a similar environment.

• Depth and breadth of staff qualification offerings.

• Price in relation to value of technical services offered.

Consolidated School District 158 reserves the right to reject any or all Proposals, in whole or in part, with or without cause, even if all the stated requirements are met. In addition, Consolidated School District 158 may enter into negotiations with one or more entities simultaneously and award a contract without notification. At the sole discretion of Consolidated School District 158, proposal provider presentations may be requested prior to the award of the contract. Section 2 - Definition of Desired Services

2.2 Requested Scope of Services PART 1 - GENERAL 1.1 DESCRIPTION A. Provide an Arc Flash Hazard Study for the electrical distribution system by developing a comprehensive one-line diagram /

drawings(s) of the electrical distribution system & equipment from utility service and generators down to 208 volt electrical equipment and panels. This will require data gathering from all electrical equipment including but not limited to utility service entrance equipment, switchgear, switchboards, panel boards, motor control centers, PDUs, UPS, ATS's, apparatus and devices fed by transformers rated 125 kVA and above including impedance information, conductor size and length. The intent of the Arc Flash Hazard Study is to determine hazards that exist at each piece of electrical equipment shown on the one line drawing(s). All drawings delivered must be viewable in AutoCAD. District 158’s record construction and as-built drawings will be made available to the successful consultant for reference purposes, but all vendors are responsible for field verification of actual conditions.

B. The proposal shall state the total number of arc flash labels (locations with calculations) that will be furnished. Arc flash labels must be installed. The proposal will include creation of Arc Flash Hazard Warning Labels listing all data as shown on the Figure 1 example label below.

Arc Flash Hazard Study. Use of NFPA 70E Task tables is not allowed to determine Arc Flash Hazard Risk Category.

D. The Arc Flash Hazard Study shall consider all operating scenarios during normal conditions alternate operations, emergency power conditions, and any other operations, which could result in maximum arc flash hazard. The label shall list the maximum incidental energy calculated and the Scenario number and description on the label. The following is a list of operating scenarios: 1. Normal Utility Power 2. Emergency Generator Power 3. Bus Tie Breakers open 4. Bus Tie Breakers closed 5. UPS Power

E. Your proposal should be based on the number of electrical apparatus counts, based on the Device Count Summary Sheet in

Section 2, subsection 2.1 of this specification.

F. Provide the following data for the site electrical equipment:

1. Equipment Nameplate Data and Protective Device Settings for the following equipment shown in the table below. The database shall also have equipment database report listing the data below for each piece of equipment.

4. ID # of Equipment.

5. Time Current Curves, with protective device settings and comments on each curve.

G. The proposal for this study must state the following items:

1. Number of arc flash labels (locations) that will be produced & installed.

2. Identifying who will be accessing the electrical distribution equipment to collect the required data. Must include qualifications & resumes.

3. Name of person(s) or company who will be generating the one-line drawings with resume and number of years of drafting & AutoCad experience.

4. Who will be installing the labels.

5. Who will be providing on-site training to all Qualified workers, including trainer qualifications and outline of training materials.

6. Any exceptions to these specifications and requirements.

7. Terms and Conditions.

8. Estimated time (calendar days) to collect data.

9. Estimated time (calendar days) to generate one-line drawings.

10. Estimated time to (calendar days) generated warning labels.

11. Estimated time to (calendar days) install warning labels.

1.2 QUALIFICATIONS A. The consultant shall have the study prepared by qualified engineers. The Consultant shall be a registered Professional

Electrical Engineer who has at least ten (10) years of experience and specializes in performing power system studies. Must include license number, qualifications & resume.

B. Calculations for each Arc Flash condition shall be submitted. Section 2 - Definition of Desired Services

2.2 Requested Scope of Services (continued) 1.3 SUBMITTALS & GENERAL CONDITIONS

A. The consultant shall submit an electronic draft report for review before printing the arc flash hazard labels.

B. The draft report shall contain recommendations to reduce any arc flash hazard greater than arc flash Hazard Risk Category 2; and shall contain paper copies of the proposed labels.

C. The consultant shall electronically submit the final arc flash hazard study and arc flash warning labels at least 30 days prior to installation of labels on energized electrical equipment.

D. Submit one (1) hard copy of the study and (1) set of warning labels.

E. Your schedule shall include 30 calendar days for review and approve your submittals and recommendations. Work cannot proceed unless all submittals are approved or approved "as noted" beforehand.

F. Hearing protection, safety glasses and safety shoes must be worn at all times; and additional PPE worn when opening all electrical panels and equipment in the defined scope of work.

G. Consultants & vendor must sign & badge in at the security desk when visiting all locations.

H. All existing electrical equipment is energized and in constant operation.

I. There are no scheduled outages or maintenance shut downs.

J. Proposal shall include vendor provide all safety training and PPE for it’s staff as required to execute this scope of work.

K. Proposal shall include all overtime required to complete this work.

L. Consultant selected to perform this work shall comply with the criminal background check requirements for all consultant employees who will be working on this project on-site. The fees from the State of Illinois for the criminal background checks will be paid for by the School District's. The consultant's costs for personnel and travel to Woodstock, Illinois for the recording of finger prints shall be paid for by the consultant as a part of the project costs listed in the RFP response.

Vendor hereby represents, warrants and certifies that no officer or director of vendor has any knowledge that any employee thereof has been convicted of committing or attempting to commit any one or more of the following offenses set forth in the Criminal Code of 1961. 720 ILCS 5/1-1 et. seq., Sections 11-6 (Indecent solicitation of a child), 11-9 (public indecency), 11-14 (prostitution), 11-15 (soliciting for prostitute), 11-15.1 (soliciting for a juvenile prostitute), 11-6 (pandering), 11-17 (keeping a place of prostitution), 11-18 (patronizing a prostitute), 11-19 (pimping), 11-19.1 (juvenile pimping), 11-10.2 (exploitation of a child), 11-20 (obscenity), 11-20.1 (child pornography), 11-21 (harmful material), 12-15 (criminal sexual assault), 12-14 (aggravated criminal sexual assault), and/or those offenses defined in the “Cannabis Control Act”, 410 ICS 550/1, et. seq. (except paras. 704 and 705 of that Act), and/or those offenses defined in the “Illinois Controlled Substances Act”, 720 ILCS 570/100 et. seq., and/or any offense committed or attempted in any other state or against the laws of the United States, which if committed or attempted in this State, would have been punishable as one or more of the foregoing offenses. Vendor agrees that it shall not employ any person who has or may have direct, daily contact with the pupils of any school in the district, and for whom a criminal background investigation has not been conducted pursuant hereto, and further represents and agrees that all employees or applicants who may have direct contract with employees shall be required to furnish a written “Authorization for Criminal Background Information” on forms provided by the district authorizing the Board of Education to request a criminal background investigation of said person pursuant to Section 10-21.9 of the School Code of Illinois and to receive criminal history record information pursuant thereto to determine if the applicant has been convicted of committing or attempting to commit any of the criminal or drug offenses enumerated above. Vendor further agrees to submit with said authorization for any costs and expenses associated with the criminal background investigation. Vendor further represents, warrants and certifies that no employee or applicant with respect to whom the criminal investigation reveals any conviction for committing and/or attempting to commit any of the above enumerated offenses,

1.3 SUBMITTALS & GENERAL CONDITIONS (continued)

including the Sex Offender Registry Act, and the Statewide Child Murder and Violent Offender Against Youth Database shall be employed thereby in any position that involves or may involve contact with the students of the school district.

M. Proposals shall include a sample certificate of insurance from the provider. The insurance shall meet the following minimum requirements. The consultant who is awarded the work shall provide an insurance certificate which meets or exceeds the listed requirements as a part of the stated costs on the Price Breakdown sheet. The consultant shall maintain this minimum level of insurance coverage throughout the term of the project.

1.1. The successful consultant shall expressly bind himself/herself to defend and save the District harmless from all suits or actions of every name and description including Scaffolding Act Liability. Successful consultant shall carry insurance, in company or companies acceptable to the District, for Worker’s Compensation, Commercial General Liability, Professional Liability, and Automobile Liability.

1.2. The Insurance Coverage provided by the consultant shall be "per occurrence" type insurance. "Claims made" type insurance coverage will no be acceptable for this project.

1.3. Each consultant shall submit as part of the bid, a certification of insurance in force to meet the above specifications. That certificate shall name CONSOLIDATED SCHOOL DISTRICT 158 as an additional insured and shall state that all insurance listed above is primary. CONSOLIDATED SCHOOL DISTRICT 158 shall be notified 30 days prior to any material change in the insurance.

1.4. Workman's Compensation - Statutory - meet minimum requirements for State of Illinois work. 1.5. Comprehensive General Liability Insurance: The successful consultant shall maintain at all times during the contract

Comprehensive Liability Insurance (including Broad Form Property Damage) with limits for combined bodily injury and property damage of not less than $1,000,000 for each occurrence and not less than $1,000,000 aggregate.

1.6. District 158’s Protective Liability: The successful consultant shall maintain at all times during the contract District 158’s Protective Liability with limits for combined bodily injury and property damage of not less than $1,000,000 for each occurrence and not less than $1,000,000 aggregate.

1.7. Comprehensive Auto Liability Insurance: The successful consultant shall maintain at all times during the contract Comprehensive Auto Liability with limits for combined bodily injury and property damage of not less than $1,000,000 for each occurrence and not less than $1,000,000 aggregate.

1.8. Excess Liability: An umbrella policy is required with minimum lists of $5,000,000 per occurrence and will apply to both bodily injury and property damage. The umbrella policy shall apply over all primary coverage and limits of liability as listed in the preceding section. The umbrella coverage must be as broad as the primary policies and must be free of any restrictions that do not appear in the underlying policies.

1.9. Professional Liability: The successful consultant shall maintain at all times during the contract Professional Liability Insurance with limits of not less than $2,000,000 for each occurrence and not less than $4,000,000 aggregate.

1.10. Owned/Rented Equipment Insurance: The Consultant shall secure, pay for and maintain whatever Fire or Extended Coverage Insurance deemed necessary to protect the Consultant against loss of owned or rented capital equipment and tools, including any tools owned by mechanics, and any tools and equipment owned or rented by the Consultant. The requirement to secure and maintain such insurance is solely for the benefit of the Consultant; Consultant shall require same coverage of Subconsultant. Failure of the Consultant to secure such insurance or to maintain adequate levels of coverage shall not obligate District 158, or their agents and employees for any losses of owned or rented requirement. It is expressly understood and agreed that District 158 shall have no responsibility therefore, the Consultant secures such insurance the insurance policy shall include a waiver of subrogation clause as follows: “It is agreed that in no event shall this insurance company have any right of recovery against District 158 or their agents.”

1.11. Consultant’s Obligation: The procuring of the insurance required under this Contract shall be considered solely as securing Consultant’s obligation or liabilities assumed under the Contract. Consultant shall remain fully liable and responsible for all such obligations, whether or not the insurance provided by the Consultant is approved by District 158.

1.12. CERTIFICATE OF INSURANCE FORM The following wording must appear in the section, Description of Operations/Locations/Vehicles/Restrictions/Special Items:

Work cannot begin nor will any payments be issued until the Certificate of Insurance has been received with School District #158 being named as additional insured.

1.13. There will be no “Waivers of Subrogation” permitted on the insurance policy or contract between DISTRICT 158 and the Consultant. A copy of the Certificate of Insurance shall be furnished to the Director of Operations and Maintenance before the work begins.

PART 2 - EXECUTION

2.1 SHORT CIRCUIT STUDY

A. Before performing the Arc Flash Hazard Study, consultant must perform a short circuit study for the equipment shown on the one line diagram. Compare the calculated short circuit current to the equipment short circuit ratings. Verify that the equipment is properly rated for the available short circuit current.

2.2 PROTECTIVE DEVICE COORDINATION STUDY

A. Before performing the Arc Flash Hazard Study, consultant must perform a protective device coordination study for the equipment shown on the one line diagram. Determine the proper settings for the protective devices. For complex protective devices, create time current curves to determine the appropriate protective device sizes and settings. All recommendations for lowering incident energy must include modeling of the protective devices to insure that new settings or devices are properly coordinated.

2.3 ARC FLASH HAZARD STUDY

A. Perform the arc flash hazard study after the short circuit and protective device coordination study has been completed. B. The study shall be in accordance with latest applicable NFPA 70E, OSHA 29-CFR, Part 1910 Sub part S, and IEEE 1584

standards. The study must be performed using IEEE 1584 calculation for equipment rated 50 to 15 kV.

C. The study shall include a spreadsheet of all label data as shown on the example labels (Figure 1 & 2). D. Produce an Arc Flash Warning label as shown above in Figures 1 & 2. Also include the bus name and voltage. Labels shall

be printed in color and shall be moisture proof, adhesive backed. Labels for outdoor equipment shall be vinyl and UV resistant to avoid fading.

E. Produce Arc Flash Evaluation Summary Sheet Report listing the items below for the maximum calculated energy levels at

each location for each scenario. The versions of the report must be submitted – 1) sorted by Bus Name and 2) sorted by Hazard Risk Category. The reports shall list the following items: 1. Bus Name

PART 3 - ANALYSIS Analyze the short circuit, protective device coordination, and arc flash calculations and highlight any equipment that is determined to be underrated or causes an abnormally high incident energy calculation greater than Hazard Risk Category 2. Propose general methods and approaches to reduce the energy levels. Proposed major corrective modifications will be taken under advisement by the Engineer, and the Consultant will be given further instructions. PART 4 - REPORT The results of the power system study shall be summarized in a final report. The report shall include the following sections: A. One Line drawings w/ impedance - w/ (3) hard copies & (3) electronic copies (CD format).
