Request for CAV (Certification, Authentication, Verification) Requirements A. Two (2) sets of certified true copies of the following: I. Graduate II. Special Order (S.O.) or Certificate of S.O. exemption of courses with PAASCU Accredited status Transcript of Records Diploma Related Learning Experience (for BSN graduates) 2 x 2 (two pcs. - latest picture) III. Undergraduate Transcript of Records Certification of enrollment and number of units earned 2 x 2 (two pcs. - latest picture) B. One (1) pc. of documentary stamp C. Accomplished Application Form (blank form can be secured from the Office of the Registrar) D. CAV fee of P40.00 Authentication Procedure 1. Applicant files application with complete requirements . 2. AQ transmits the application to CHED. The applicant may submit the application personally to CHED however; he/she cannot get the "Claim Stub" on the day of filing. The applicant needs authorization letter from the Registrar. 3. CHED transmits the application to the Department of Foreign Affairs (DFA) through DFA's official courier. 4. Applicant may come back to the Office of the Registrar 3-5 days after date of filing for the claim stub. 5. Applicant or authorized representative will go to DFA, Manila after seven (7) working days from date of filing at CHED. DFA releases authenticated documents (with Red Ribbon) upon payment of appropriate fee/s. Authentication Fee: (Php 75.00 per document for a maximum of five (5) copies)

Authentication Procedure

1. Applicant files application with completerequirements.
