REPORTING OF INDIAN MEDICAL LITERATURE IN THE INDEX MEDICUS AND THE EXCERPTA MEDICA Surveys the growth of Indian medical societies and periodicals. Gives a quanti- tative measure of the output of Indian medical literature. Studies the extent of coverage and speed of repor t i ng of Indian medical literature in th", Index medicus and the Excerpta medica using the Biblio- graphy of scientific publications of South and South East Asia as the basis. Coverage of periodicals as a whole and problems relating to seepage and scatter of Indian medical literature are the other aspects dealt with. INTRODUCTION The histor y of medicine in India can be traced as far back as the Vedic period. The several systems of medicine practised in ancient and medieval India have given us a rich knowledge. The contributions of medical prodigies like Caraka, Susruta, Vagbatta etc. have made a great impact on the growth of medical science in India in the subsequent times. It was only in the beginning of the Seventeenth century, Western medicine got emplanted into our country. From about Eighteenth century, the study and practice of medicine began to be placed on a better scientific knowledge. The establishment in 1784 of the Asiatic Society of Bengal marked the beginning of scientific research in India. Along with other branches of knowledge, biological sciences received the due attention of the Asiatic Society. The s~bject of medicine was originally included under the activities of the biological science s ',branch of the Society and it was only in 1906, a seperate branch for medicine was created. The year 1823 is yet another landmark in the history of medicine in India. The interest of the Government in promoting medical research may be said to have taken a . concrete shape in 1911, when the Indian Council of Medical Research was established. The development of medical societies since the last quarte r of the 18th century to the first quarter of the 20th century has been quite exhaustively dealt with by Neelarneghan [7]. The learned societies devoted to the subject of medicine appears to be greater in number as compared to that of other discipline s, The study of medicine is now receiving considerable attention. The number of medical institutions engaging in higher education and research is very large. The publication of the Central Bureau of Health Intelligence [5] gives an idea of the facilities available now for higher medical education in India. Along with the development of the scientific study of medicine, the output of medical literature in India has shown to be increasing proportionately. The output of lite rature in medicine has been higher considering the literature being published in other scientific subjects. So also among the Indian scientific and technical periodicals, medicine accounts for the highest number. Not only the medical periodicals are larger in number, but also they seem to have certain regularity in their publication. Neelameghan has surveyed the evolution of Indian medical periodicals and eoctet ie s upto 1920 [7]. Appendices 1 and 2 in this paper bring the data up-to-date. While Neela- meghan, in his publication, has given the data under various c%"oss-sections, only arr alphabetical listing of the societies and chronological listing of the periodicals have been attempted in this paper. INTRODUCTION

The hi stor y of medicine in India can betraced as far back as the Vedic period. Theseveral systems of medicine practised inancient and medieval India have given us arich knowledge. The contributions of medicalprodigies like Caraka, Susruta, Vagbattaetc. have made a great impact on the growthof medical science in India in the subsequenttimes. It was only in the beginning of theSeventeenth century, Western medicine gotemplanted into our country. From aboutEighteenth century, the study and practice ofmedicine began to be placed on a betterscientific knowledge.

1 and 2. In Table I, the number of contri-butions on medicine appearing in pe riodicals,as reported in the Bibliography of scientificpublications of South and South East Asia(hereafter referred to as the Bibliography)for the year s 1956 to 1964 is given. It maybe seen that the output of Indian periodicalliterature in medicine has shown a steadyincrease through the years.

In Table 2, the number of books onmedicine, which have been noticed by theIndian national bibliography during theyears 1958 to 1964 is given. Indian medicalbooks are mostly on the subjects of AyurvedaHomoeopathy, Naturopathy, Unani, Gemolo-gy etc. They are also of varying standards.Books on Sexology, Birth Control and PublicHealth also appear in a good number.

Scope of the Study

The present study on the reporting ofIndian medical lite rature in the Index mediclEand the Excerpta medica is yet another in theseries of studies being made by us on thereporting. of Indian scientific literature inmajor international abstracting and indexingservices [6,9,10, II, 12]. Exhaustivenessand speed of reporting of Indian medicalliterature in one international indexingservice and in one international abstractingservice are examined in this paper.Coverage of Indian periodicals as a whole inthese two services is also studied. Theproblems of seepage and scatter of Indianme dical literature which have re levance tothe extent of coverage of literature ininternational abstracting and indexingservices are the other aspects dealth within this study.

Methodology

This study has been made basing theIndian contributions on medicine, noticed inthe Bibliography in the year 1962. Reportsand theses have not been taken into con si de r a-t ion , In checking the entries of the l'; blio-graphy with the Index medicus and the---Excerpta medica.a few difficulties have beenmet with. The annual index of the Indexmedicus does not provide any indication asto the issue number or month of the Indexmedicus wherein a particular contributionhas been noticed. As a result, each

monthly issue of the Index medicus has to bescanned individually to locate the relevantent~ie ~. The .Index medicus, being an indexingpe r Iodtc a.l nottc e s the contributions rapidly.As it has been observed that more than 99.5per cent of the contributions are coveredwithin a time = Iag of 6 months, search forIndian contributions in this service has beenlimited upto June 1963 issue. All the issuesof the Index medicus from January 1962 toJune 1963 have been checked to locate theIndian contributions.

Checking of Indian contributions in theExcerpta medica presented yet anotherproblem. The Excerpta medica which usedto be published in 21 sections till 1962increased the number of sections to 25 there-after. Each section has its own separateannual index. In orde r to locate the entrie s ,as many as 75 indices covering three yearshad to be checked. Since the annual indicesof various sections for the year 1964 wasnot readily available to us for consultation,the index of each issue of the differentsections for the year 1964 had to be checked.As this procedure was found to be time-consuming, an alternative method wasattempted. In this method the entries ofIndian contributions were divided first intothe subjects of the various sections of theExcerpta medica and then checked in therespective sections. In this method also,difficulties were met with in the case ofinter-disciplinary titles. For such titles,all the possible sections where they arelikely to be reported, had to be scanned.It was feared that in this method, someentries might not be traced at all. It. thenleft us no other alternative than to check allthe sections of the Excerpta medica fromJanuary 1962 to June 1964 for locating theIndian contributions. For this purpose77,277 entries of 1962, 85,063 e nt r ie s of1963 and 48,278 entries of 1964 i.e.210,618entries in total of the Excerpta medica hadto be checked. The search in the Excerptamedica was upto June 1964, as it has beennoticed that about 98.5 per cent of the Indiancontributions are covered within a time-lagof 18 menths. It is likely that some of theent r ie s of the Bibliography of 1962 mighthave been covered in the Excerpta medicabeyond June 1964. but it may be reasonablyassumed that this would be a negligiblepercentage.

2 EXTENT OF REPORTING

Table 3 gives the periodical-wise distri-bution of the contributions noticed by theBibliography in 1962 and their coverage inthe Index medicus and the Excerpta medica.It may be seen from this table that theIndex medicus and the Excerpta medica havecovered 38.5 per cent and 13.5 per centrespectively of Indian medical literature ofthe year 1962. The coverage of Indianmedical literature in both the servicesoffers scope for improvement. It may alsobe pointed out here, that the coverage ofIndian literature in the Excerpta medicais found to be poorer in re lation to theabstracting services of other subjects exa-mined in our earlier studies [6,9,10,11,12].

3 QUICKNESS IN REPORTING

The quickness in reporting of Indian medi·ca1 literature in the two services has alsobeen studied and the data have been presen-ted in Table 4. The time -lag is determinedby calculating the number of months elapsedbetween the indexing of the article in theBibliography and the appearance of itsabstract or index in the respective services.In the Excerpta medica,the abstracts ofinter-disciplinary articles have been givenin all the relevant sections. As a result,the time •.lag in the coverage of one and th esame article varies from section to sc cti on,In such cases, the least time-lag has beentaken into account. The comparison ofspeed of reporting between an abstractingservice and an indexing se rvice is notequitable for obvious reasons. The speed ofreporting in the Excerpta medica, anabstracting service, cannot be comparedequitably with the speed of reporting in theBibliography, an indexing service. There-fore, in the pre sent study one indexingse rvice and one abstracting se rvice we retaken for studying the time-lag in relationto the Bibliography. The Bibliography hasbeen taken as the base, because it has beenthe only secondary publication in Indiacovering Indian scientific literature tillDecember 1964. The Bibliography used tocover mainly original contributions and with-in 1-2 months after their publication. Itmay also be reasonably expected that mostof the contributions reported in the Biblio-graphy are worth coverage by internationalabstracting and indexing services.

From Table 4, it may be seen that themaximum time -lag in reporting in the caseof the Index medicus is 10 months whe reasin the case of the Excerpta medica it is 23months. Within a time-lag of six months theIndex medicus has covered B18 contributions(9B.5 per cent) of the total number of contri-butions covered. Of the total 293 contribu-tions covered in the Excerpta medica,119contributions (40.6 per cent) have beencovered within a time-lag of 6 months; 141contributions (48.1 per cent) have beencove red within 7-12 months; that is within thetime-lag of 12 months 260 articles (88.7 percent) have been covered; within the time-lagof 18 months, 288 articles (98.3 per cent.)have been covered.

The Index medicus has been able toprovide a faster service, whereas theExcerpta medica. notwithstanding the factthat it is an abstracting service. has toconsiderably improve the speed of reportingof Indian literature.

4 COVERAGE OF PERIODICALS

Out of the 77 periodicals (vide Table 3)covered by the Bibliography, i7 periodicalswere regularly indexed in the Index medicus.From yet another 9 periodicals, selectedreview articles were inde xe d in the Indexmedicus in 1962. Though the coverage inIndex medicus of Indian medical periodicalsas such is poor, a perusal of the list ofperiodicals covered in the Inr'ex medicus inthe years 1962 and 1963 shows that efforts

'are being made to cover more and moreIndian medical periodicals. In these twolists, it has been stated, that selectedreview articles from 3 more periodicalsi ,e. Indian journal of dermatology andvenereology, Journal of the AnatomicalSociety of India and Maharashtra medicaljournal will be covered from 1963; two moreperiodicals i,e. Indian practitioner andJournal of the Indian medical profession willbe regularly covered from 1963; Calcuttamedical journal will be regularly coveredfrom 1964; Antiseptic and Patna journal ofmedicine will be regularly covered from1965. From Table 3, it appears that Bombayhospital journal, Indian journal of anaesthesiaIndian journal of surgery, Indian medicalrecord, Journal of the Indian PediatricSOCIetY, Journal of J J Group of Hospitals

and Medical Co l le qe , Journal of post~raduate medicine ..and Punjab medicaljournal carry a large number of contribu-tions. The Index medicus may cover theaforesaid journals in order to ensure amore comprehensive reporting of Indianlite rature. Those journals whe refrom onlyselected articles are being covered now mayalso be fully covered by the Index medicusin future.

Out of the 77 periodicals, the Excerptamedica has covered 37 periodicals. Fromthe list of periodicals covered in theExcerpta medica in 1964, it appears thatBombay hospital journal, Indian journal ofmicrobiology, Indian medical forum,Indian medical record, Journal of obstetricsand gynaecology of India, are being coveredby them. But from Table 3, it is seen thatthe contributions from these periodicalswhich were noticed in the Bibliography in1962 were not covered in the Excerpta ra ,

medica. It is possible that the Excerptamedica has started covering these periodicalsfrom 19':'4 only. The periodicals, Armedforces medical journal (India), Indian journalof anae sthe s ia , Journal of post graduatemedicine, lvlaharashtra medical journal,Punjab medical journal are not covered bythe Excerpta medica. These periodicalspublish quite a good number of papers. TheExcerpta medica may cover these periodicalsalso to provide an exhaustive coverage ofIndian medical literature. It also appearsfrom the list of periodicals covered in theExcerpta medica in 1964 that the periodicals,Eastern anthropologist, Indian heart journal,Indian journal of de rmatology, Indian journalof psychiatry, Indian journal of radiology,Indian journal of social work, Law quarterlyreview, Journal of correctional work,Journal of genetics, Journal of MysoreMedical Association, Journal of obstetricsand gynaecology, Journal of rehabilitationin Asia, Oriental archives of opthalmology,Proceedings of the All India OpthalmologicalSociety, Samiksha have been covered by theExcerpta medica, but they have not beencovered by the Bibliography.

5 DEPTH OF COVERAGE OF PERIODICALS

For examining the depth of coverage ofperiodicals, only those periodicals whichhave been covered in all the three serviceshave been taken into account. The number

.oi such periodicals in found to be 14. Thefindings are presented in Table 5.

From Table 5, it appears that the depth ofcoverage of Indian periodicals in the Excerptamedica is rather low. This is supportedalso by the data given in Table 3 wherefromit may be seen that less than 25 per centof the contributions are covered in the caseof 18 periodicals; less than 50 per cent ofthe contributions are covered in the case of8 periodicals; less than 75 percent of thecontributions are covered in the case of3 periodicals; and cent per cent of the contri-butions are covered in the case of only oneperiodical. One pos sible reason for thislow percentage in the depth of coverage ofperiodicals in the Excerpta medica may bestricter criteria for selection of contributionsas it is an abstracting service. But thisreason does not appear to hold good when weexamine the standard of the some of theIndian contributions cove red in the Excerptamedica. The paper" Integrated generalhealth survey in Indian by S C Seal publishedin the Indian journal of public health (Vol. 6No.l,pp.1-7) has not been reported in theBibliography on account of its popularnature, whereas it has been covered in theExcerpta medica (Sec 17, Vol. 9, Se r , No. 24).There are also numerous instances whereimportant contributions reported in theBibliography, have not been covered in theExcerpta medica. The coverage of thepaper s published in the Indian journal ofmalariology (Vol. 16 Nos.l & 2, March, June1962) is examined here by way of illustration.

It may be mentioned that all the abovepapers excepting the last one have beenreported by the Index rne di cus , While thepapers under S. Nos. 1 and 3 are of similarnature, only the paper under S. No.1has been covered in the Excerpta medica.It is also not comprehensible as to why thepapers under S.Nos.4-7 have not beencovered at all.

The difficultyof international abstracting and indexingservices, like the Excerpta medica and theIndex medicus in covering the periodicalsfalling under the above categories is under-standable. These periodicals publish occas-sionally one or two pepers on a particulardiscipline and international abstracting andindexing services devoted to particulardiscipline can not be expected to receive alarge number of such periodicals of alienand peripheral subjects and to scan themregularly. In order to further demonstratehow scattering of literature in differentcategories of periodicals brings aboutdeficiency in reporting by the internationalservices, the coverage of Indian rnedi ca lliterature published in four diff e r e ntcategories of periodicals - 1 core periodicals2 periodicals relating to broad subjects;3 periodicals relating to border-line subjectsand 4 periodicals relating to alien subjectshas been studied. The findings are givenin Tables 6 and 7.

From Table 6, it is seen that out of thetotal number of contributions covered inthe .L3ibliography, 93.5 per cent are from coreperiodicals, 4. 0 per cent from the periodicalsof broader subjects, 0.2 per cent from theperiodicals or border-line subjects and2.3 pe r cent from the periodicals on aliensubjects. It may be pointed out that Indianmedical literature is concentrated mainlyin core periodicals pertaining to thesubjects of medicine, and only t. 5 per centof the literature is likely to escape thenotice of the international services onaccount of seepage and scatter of literature.This aspect may perhaps require furtherdetailed study as an earlier paper by uswith regard to Indian mathematical literature[11] presents a different trend.

The Index medicus has covered 38.1per cent of the total literature, of which37.0 per cent of the contributions belongto core periodicals and 1. I per cent ofcontributions to the periodicals of broadersubjects. The Excerpta medica has covered

13.5 per cent of the total literature, ofwhich 13.2 per cent of the contributionsbelong to the core periodicals and 0.3 percent of contributions belong to the periodi-cals of broader subjects.

A study of the different categories ofperiodicals carrying medical contributionshas also been made and the data is presentedin Table 7. From a m orig the periodicalscarrying medical contributions covered bythe Bibliography in 1962, 62.3 per centbelong to the core subject of medicine,14.3 per cent belong to broader subjects,4.0 per cent belong to border-line subjectsand 19.4 per cent belong to alien subjects.

This servicehas not been also covering any of theperiodicals belonging to subjects alien tomedicine.

The above findings show that internationalabstracting and indexing services devoted toa particular subject disc ipl.iue are at adisadvantage to ensure exhaustive coverageof literature for the reason that articles ofa particular subject discipline, appear notonly in core periodicals, but also inperiodicals of broader subjects, border-linesubjects and alien subjects, and as such areliable to escape notice.

National abstracting and indexing servicescan however ensure exhaustive coverage andact as a feeder service to internationalabstracting and indexing services, solvingthe problems of seepage and scatter. Asregards Indian scientific literature, theIndian science abstracts being published byInsdoc since January 1965, not only ensuresbibliographical control of Indian scientificliterature but also provides a feederservice to international abstracting andindexing services. Other national abstract-ing services such as the Indonesian abstractsJapanese periodicals index - Naturalscience s, Pakistan science abstracts,Roumanian science abstracts - Naturalscience_s may also be cited as examples.
