By signing this form, I ____________ give permission to be filmed, interviewed

and anything else in relation to the completion of the production of this music

video. I give permission for my picture to be taken and used in the continuing

production and advertising of the project. I also grant permission for my name

to be shown in the credits of the video as well as in news/ information in

relation to the video.

I declare that I will not press charges or take any legal actions against

_____________ if they use my name, performance or appearance as a part of

their A-Level media coursework or any further use of the music video or

production in future endeavours.

By signing this form, I understand that I am agreeing to the above conditions

and will work to the best of my ability for the production, making myself

available if/when necessary. I further acknowledge that by agreeing to this I
