Work Rehabilitation Process -Related Injury Flowchart

Administer first aid and take employee to a doctor

Employee report’s injury to supervisor/principal who gets data input on SMS (or complete relevant Workplace Health & Safety Forms for district, regional and central

office staff)

Employee and Supervisor/Principal complete and lodge WorkCover claim forms. Should be lodged with WorkCover via your region within 20 business days of the injury occurring. Relevant leave forms should also be submitted.

If the injury results in more than 5 days time off work or requires modifications to duties/workplace the supervisor/principal notifies a Rehab & RTW Coordinator.

The Rehab & RTW Coordinator contacts the injured employee and arranges a time for an initial interview.

Initial interview conducted by Rehab & RTW Coordinator

Rehabilitation file prepared by Rehab & RTW Coordinator

Maintain-at-work or Return-to-work program formulated by the Rehab & RTW Coordinator in conjunction with the employee, supervisor and the treating medical practitioner. This should be signed by all relevant parties before the employee commences the program.

Progress is monitored regularly by the Rehab & RTW Coordinator throughout the rehabilitation

period.

A case closure report is prepared by the Rehab & RTW Coordinator. This should be done upon the conclusion of rehabilitation. A clearance medical certificate is obtained for all

employees who are cleared to return to work with no continuing restrictions. An occupational rehabilitation survey should also be given to the employee to help determine

the effectiveness of rehabilitation interventions.
