A. Approve the Standard Services Agreement (Procurement Contract No. 21725) with PrimaryDiagnostics, Inc. (Principal: Andrew Kobylinski; Location: San Francisco), to provide software forCOVID-19 vaccine appointment scheduling system and related technical support, for the contractperiod of 2/1/21 - 4/30/21, in the amount not to exceed $500,000;

B. Adopt a Resolution to waive the County’s competitive procurement process for PrimaryDiagnostics, Inc. with respect to this contract due to the urgent need for this service and uniqueaspects of the software platform;

C. Delegate Authority to the Agency Director, or designee, to execute agreement, subject to approval asto form by County Counsel and submit an executed copy to the Clerk of the Board for filing;

Approve the Third Amendment to the Standard Services Agreement (Procurement Contract No.15764) with Alameda Alliance for Health (Principal: Scott Coffin; Location: Alameda) to continueto provide administration of a Care Management Service Bundle to Medi-Cal eligible high users ofmultiple systems and homeless individuals as part of Alameda County Care Connect, extending theperiod by 12 months from 7/1/17 - 12/31/20 to 12/31/21, increasing the amount from $8,684,669 to $9,363,269 ($678,600 increase); and

B. Authorize the Social Services Agency Director, or designee, to submit Project RoomKeyexpenditure claims to the State for reimbursement; and

 19. Public Health Department - Adopt a Resolution of Local Support authorizing the filing of an application to
