Integrated Care: A Roadmap for the Future Workforce Building Momentum for Change in Graduate Education

With funding from the Zellerbach Family Foundation, the Greater Bay Area Mental Health & Education Workforce Collaborative (Regional Partnership) is pleased to announce a one-day conference on Behavioral Health-Primary Care Integration on June 6th, 2013 at the California Endowment Conference Center Oakland. Partnering with CalSWEC, and the CalMHSA Integrated Behavioral Health Project, this event is targeted towards graduate level faculty in local social work, MFT, psychology, counseling and nursing programs, along with public mental health leaders. One of the keynote speakers includes Alexander Blount, EdD, Director, Center for Integrated Primary Care, Professor of Family Medicine and Psychiatry, University of Massachusetts Medical School. The conference will also include panel presentations on current Bay Area integration efforts, along with a multi-disciplinary academic panel. The goal of this conference is to educate and build momentum for graduate level training programs to support the behavioral health workforce needs of the public system. The California Innovations Summit: The Triple Aim as a

Framework for Improving the Health of Clients with Complex Mental Health, Substance Use and Physical Health Conditions

May 22-23, 2013 At the recent Thirteenth Annual Behavioral Health Information Management Conference and Exposition, “Addressing the Needs of Mental Health, Alcohol, and Other Drug Programs”, Dale Jarvis quoted Don Berwick, former CMS Administrator, as saying,

“Our nation is at a crossroad. The care we have simply cannot be sustained. It will not work for health care to chew even more deeply into our common purse. If it does, our schools will fail, our roads will fail, our competitiveness will fail. Wages will continue to lag, and, paradoxically, so will our health. The choice is stark: chop or improve.”

Health care improvement is defined by improving the quality of the care, the health of populations, and reducing costs associated with care. While we know what the Triple Aim is, the question for systems responsible for improved care, health, and reduced costs for clients with complex mental health, substance use, and physical health conditions is how do we achieve it? The California Innovations Summit: The Triple Aim as a Framework for Improving the Health of Clients with Complex Mental Health, Substance Use and Physical Health Conditions will be two days of exploring how to work collaboratively in California to achieve the Triple Aim. Coordinated and integrated care is key and significant change among all involved in health care is needed and happening across the state. Roles between clients, their families and providers are changing to allow for person-centered care and shared decision-making. Systems level leadership is placing an increased emphasis on collaboration, shared goals, and redesign principles. Health plans are introducing payment reform and incentivizing coordination and improved care. Provider teams of primary, substance use, and mental health care are focusing on changes in service delivery design, clinical practices, client self management, and clinical information systems.

Keynote speakers at the Summit that will help to further inform Triple Aim efforts in California include Edward H. Wagner, MD, MPH, Director (Emeritus), The MacColl Center for Health Care Innovation, Group Health Research Institute Senior Investigator; Jurgen Unützer, MD, MA, MPH, Professor of Psychiatry, University of Washington, Director, AIMS Center, CiMH Care Integration Collaborative Co-Chair; and Dale Jarvis, CPA, Dale Jarvis & Associates, LLC. Day One of the Summit will introduce five panel presentations on innovative care coordination and integration models that highlight key changes and strategies for Triple Aim achievement with complex clients from across the state. Day Two of the Summit consists of small group discussions that will shape recommended next steps to improve the field’s shared understanding of the key themes from innovative integration models. Day Two will also encourage development of key partnerships that will advance the integration of care in California. The Summit will publish conference proceedings documenting recommendations and action plans to inform state and local policy and practice.

Working Well Together Peer Certification Summit May 17, 2013 in Sacramento

On May 17, 2013 county mental health directors and administrators, state and county Workforce Education and Development (WET) leaders and staff and county and county-contracted Peer Support leaders and providers - including adult consumer, youth, family member and parent/caregiver Peer Specialists - will convene in Sacramento to provide final input on recommendations to the State of California regarding the certification of peer providers working in public mental health and integrated health care settings. This day-long summit is being conducted by Working Well Together (WWT), under the statewide technical assistance center contract with the Office of Statewide Health Planning and Development (OSHPD), and is supported by WWT’s mission to ensure public mental health agencies are prepared to recruit, hire, train, support and retain multicultural clients, family members, parents and caregivers as employees in the public mental health system.

Why Peer Support?

Studies of drop-in centers have shown that they are associated with high satisfaction and increased quality of life, enhanced social support and problem-solving (Mowbray & Tan, 1993). One study found case management services plus a peer specialist counselor were associated with enhanced quality of life, fewer major life problems, and greater gains in social support for those receiving such services than for those receiving case management services without a peer (Felton et al., 1995). A study of a one-to-one peer support program for persons with co-occurring mental health and substance abuse problems found that program participants had fewer crisis events and hospitalizations, improved social functioning, greater reduction in substance use, and improvements in quality of life compared to a non-matched comparison group (Klein, Cnaan, & Whitecraft, 1998).

Why Certification?

Statewide certification provides a number of benefits. Those benefits include the formal validation of the role of peer providers; standardization of quality of services; provides information on competencies to employers; provides scope of practice that consumers, family members and parent/caregivers benefit from and can be attached to the ability to bill Medi-Cal for services provided.

Federal Support for Certification

In 2007, the Centers for Medicare and Medicaid Services (CMS) disseminated a set of guidelines by which states could develop Peer Support Providers and Peer Support Services. Guidelines require minimally a state plan to: train and certify/credential Peer Providers; address the supervision of Peer Providers; and ensure care coordination in the context of a comprehensive and individualized plan of care.

WWT Peer Certification Accomplishments

WWT accomplishments to date concerning Peer Certification includes the research and development of a report on existing Peer Certification Standards nationally and across the state, entitled “Certification of Consumer, Youth, Family and Parent Providers: A Review of the Research” (WWT Report); conducting five regional meetings with stakeholders and topic experts to identify and report on efforts, and barriers towards certification in California as reported in “Certification of Consumer, Youth, Family and Parent Peer Providers: A Summary of Regional Stakeholder Meeting Findings”;; and conducting the upcoming statewide meeting with key stakeholders and topic experts to review, identify, report on current efforts, barriers and finalize next steps as recommendations to State.
