Children who are 5 years old by September 30 can start kindergarten this fall. But first, they need to be registered. To register your child, make sure to bring everything listed in the chart below. After you register, bring your rising kindergartner to the Children’s Museum of Richmond and your child will receive free admission and a surprise. (This offer is for the Saturday after registration only.)

✓ You will need to take the following with you to your child’s elementary school:

1. Child’s certified original birth certificate

2. Child’s Social Security number*

3. Proof of address*

4. School Entrance Health Form. Go to your licensed medical provider who can help you complete all three parts and will sign the form for you. This is not done at school.**

Part I – Health Information Form

Part II – Certification of Immunization

Part III – Comprehensive Physical Examination Report completed within the 12 months prior to the date the child first enters kindergarten

5. Photo ID for parent or legal guardian (in some school systems)

You will be able to receive information that will help your child get ready for the first day of kindergarten. You’ll learn when to bring your child in for a tour of the school or if there are special summer activities that you might otherwise miss.

Your child’s school can better prepare for classroom space, staffing and materials.

Collecting your registration paperwork can be a challenge. By registering on time, there will be no surprises. If you’re missing a requirement, you’ll be guided to resources that can help you complete the process in time for your child to begin school with his classmates.
