Social Work This program is offered for 7 hours of social work continuing education through co-sponsorship of the University of Pittsburgh’s School of Social Work, A Council of Social Work Education-accredited school and, there-fore, a PA pre-approved provider of social continuing edu-cation. These credit hours satisfy requirements for LSW/LCSW, LPC and LMFT biennial license renewal.

PURPOSE/TARGET AUDIENCE

The purpose of this conference is to provide healthcare providers with updates and current practices in diabetes treatment and prevention. The target audience is physicians, nurses, social workers and dietitians.

OBJECTIVES

Describe the importance of evidence-based clinical practice guidelines in the treatment of persons with diabetes.

Recognize basic behavioral health principles applied within the ongoing management of diabetes.

Discuss technology and tools available to patients and providers.

Explain how insulin pump therapy can promote proactive management behaviors in patients.

Describe the Population Health Pathways Hub Model.

Define the role of nutrition in the diabetes-stroke relationship.

Identify the controllable lifestyle behaviors that increase the risk of a person with diabetes to have a stroke.

Review the current guidelines for appropriate therapy in patients with diabetes.

Describe the stepwise approach to drug therapy for type 2 diabetes.

Define health literacy best practices for healthcare settings.

Identify the 10 attributes of health literate organizations.
