Here’s how it works: If you or anyone in your immediate family encourages a friend to choose a Koelsch Community, and we don’t already have a relationship with them, please submit their name using the form below. Print, fill out, and drop off the form at any Koelsch Community. The referral must move in and stay for 30 days. This is what you’ll receive: A check for $2,000. Each additional referral increases by $500 per referral (ie. second referral earns $2,500, third referral earns $3,000 and so on). Upon confirmation that someone you referred has chosen one of our Communities, you will be notified on how to receive your check. Thank you for being part of our family and for your continued trust in us. Move-ins using the Medicaid program are not eligible.
