Cataract

Cataracts are an opacification ofthe crystalline lens in the eye. Earlystages cause reduce light levels and scatter. Advanced stages cause(preventable) blindness.

Cataract extraction with intraocularlens implantation is the most widelyperformed surgery in the US.

Harold Ridley

Ridley noticed that shards from the canopy (PMMA)of RAF pilots lodge in their eyes did not get rejectedby the body’s immune system. Proposed making animplantable lens out of the material.

Lens Injection

Lenses are rolled up and injected through the smallincision. They unroll as theyexit the injector. The surgeon then adjusts the IOL positionto ensure proper placementinside the capsule and behind the iris.

Presbyopia

Your ability to accommodatereduces steadily with age.Typically, you don’t noticethe effects until it affects yourability to read comfortably. This is presbyopia.

Multifocal IOLs

Near

Far Multifocals come in a variety of forms, but the basic concept is to have two different powers simultaneously on asingle lens. One power allows for distance vision, while the other allowsfor near vision.

Variations in zone size, number ofzones, power distribution, progressives, aspherics and diffractives all exist.

Refractive Multifocal Optics• A refractive lens has alternating zones

of varying refractive power.

• The zones of a refractive multifocal lens are typically the same size and width and act as independent annular refractive lenses.

-1

• Triangular steps of transparentmaterial.

• Light diffracts from the stepsand is primarily split intothree beams.

• Angle of step shifts the amountof energy in each order.

Full Aperture Diffractive IOLs

3M Diffractive Pharmacia Multifocal

• Step heights set so that roughly 40% in near focus and40% in distance focus.• Relative energy does not change with pupil size.• Remainder of light diffracted into other orders.

Apodized Diffractive Lens

+1 0

• Reduce blaze angle towardslens periphery.

• Apodization causes a shift inthe relative amounts of energyin each focus.

Phakic IOLsPhakic IOLs are currently in trials. This is a means of implanting a lensin the eye to correct for refractive error.

Concerns:Damage to corneal endothelium causescorneal opacitiesContact with crystalline lens causescataracts.

Accommodation

Relaxed ciliary muscle pulls zonules taut an flattens crystalline lens.

Constrict ciliary muscle releases tension on zonules and crystalline lens bulges.

Page 22: Recent Advances in Intraocular Lenses Jim Schwiegerling, PhD Ophthalmology & Vision Sciences Optical Sciences.

Accommodating IOLs

Accommodating IOLs are based on a mechanical translation of the lens within the eye. As the ciliary muscle contacts, it presseson the hinged haptics and causes the lens to move forward.

Summary

• IOL implantation is a routine surgery.

• Recent advances in design and materials seek to optimize optical performance. Aspheric and Achromatic design emerging.

• Primary side effect of conventional IOLs is lack of accommodation.

• Multifocal refractive and diffractive design provide simultaneous vision. Always a trade-off.

• Holy Grail !!! Accommodating IOL. Still needs work, but lots of people exploring it.
