Consumer Redemption Process

By Mail: Consumers must submit claims by mail in one mailing envelope containing (1) original, dated sales receipt or a leg-ible copy of the original dated sales receipt from a participating retailer dated between November 23, 2015 and December 25, 2015, (2) redemption form, completely and legibly fi lled out and (3) the original UPC & IMEI codes for the device taken from the packaging (or a legible copy of the codes).

Online: Consumer can enter their claim at http://www.lgphonerebates.com/holidayrebate and upload (1) the original, dated sales receipt or a legible copy of the original dated sales receipt from a participating retailer dated between November 23, 2015 and December 25, 2015 & (2) the original UPC & IMEI codes for the device taken from the packaging (or a legible copy of the codes).

Consumer Redemption Rules:

End-user consumer purchasers age 18 or older who purchase qualifying new LG G4 or LG V10 models at eligible Verizon Wireless Locations and participating Verizon Authorized Dealers or www.verizonwireless.com as a resident in the 50 U.S. or D.C. during the Program Period will be eligible to receive the specifi ed LG Visa Prepaid Card by mail-in rebate. Parts and Accessories do not count towards this promotional offer.

Except as otherwise required by law, this promotional offer is limited to one LG Visa Prepaid Card by mail-in rebate per person per household on each qualifying new LG G4 models purchased between November 23, 2015 and December 25, 2015 (the “Program Period”). Purchase date is determined by invoice. Multiple purchases of qualifying new LG G4 or LG V10 models during the Program Period must appear on the same sales receipt. This program is subject to availability of the LG G4 device. No rain-checks are allowed.

This promotional offer is not combinable with any other LG prepaid card or mail in rebate promotional offer. Purchased merchandise must not be returned. Promotional offer void where prohibited, taxed or restricted bylaw. LG Visa Prepaid Card offer claims must be postmarked no later than January 8, 2016. Omission of the sales receipt or other requested infor-mation may delay or stop the processing of this promotional offer. Duplicate submissions of offi cial form or cash register receipt(s) will not be accepted. No group or organization requests will be honored. Requests for fulfi llment to PO Boxes will not be accepted.

Redemption form and receipt become the property of LG upon submission, and will not be returned.
