Exclusion List Screening

• Exclusion Lists on two U.S. government sites must be checked

1. www.oig.hhs.gov – print the final screen and retain screenshot

2. www.sam.gov - click “save to pdf” and retain pdf• Notify Intuition immediately if you believe you have an

individual on one of the lists• Provide proof of screening from both sites when

requested


First: Go to the OIG Website: www.oig.hhs.gov

Step 1:Click Exclusions Database

Step 2:Enter the first and last name and click Search.

Step 3:If no results are found:

Print the web page and retain as proof of screening.

If results are returned: Follow the on-line instructions to complete the screening. Print the final web page and retain as proof of screening.

Notify Intuition immediately if a member of your team is on found on this list.


Next: Go to the SAM Website: www.sam.gov

Step 1:Click Search Records


SAM Website Screening (continued)

Step 2:Click Advanced Search – Exclusion



Step 3:Click OK



Step 4:Select either Single Search or Multiple Names.



Step 5:Select Individual.

Enter the first and last name(s).Click Search.



Step 6:If no results are found:

Click “Save PDF” and retain as proof of screening.

If results are returned: Repeat the screening with additional clarifying information to complete the

screening. Click “Save PDF” and retain as proof of

screening.

Notify Intuition immediately if a member of your team is on found on

this list.
