• It is a graphical way of showing and analyzing real causes behind a problem or effect.
• Created in 1943 by professor Kaoru Ishikawa of Tokyo University and adopted by Dr.W.Edwards Deming.
1. Define the key effect of the problem.
2. Identify the main cause category bones.
3. Brainstorm the causes for each main bone.
4. Start drawing the diagram.
5. Add lower level causes of identified causes.
6. Check the logical validity of each cause/effect chain and check for completeness.
7. Prioritize the causes to address and plan the next actions.
