Those woes put the Gulf of Mexico project over budget and behind schedule by April 20, the day the well erupted, destroying the Deepwater Horizon rig and killing 11 men.  Government investigators have yet to announce conclusions about what went wrong that day. The final step in the causation chain, industry engineers have said in interviews, was most likely the failure of a crucial seal at the top of the well or a cement plug at the bottom.  But neither scenario explains the whole story. A Wall Street Journal investigation provides the most complete account so far of the fateful decisions that preceded the blast. BP made choices over the course of the project that rendered this well more vulnerable to the blowout, which unleashed a spew of crude oil that engineers are struggling to stanch.  BP, for instance, cut short a procedure involving drilling fluid that is designed to detect gas in the well and remove it before it becomes a problem, according to documents belonging to BP and to the drilling rig's owner and operator, Transocean Ltd.  BP also skipped a quality test of the cement around the pipe -- another buffer against gas -- despite what BP now says were signs of problems with the cement job and despite a warning from cement contractor Halliburton Co.  Once gas was rising, the design and procedures BP had chosen for the well likely gave this perilous gas an easier path up and out, say well-control experts. There was little keeping the gas from rushing up to the surface after workers, pushing to finish the job, removed a critical safeguard, the heavy drilling fluid known as "mud." BP has admitted a possible "fundamental mistake" in concluding that it was safe to proceed with mud removal, according to a memo from two Congressmen released Tuesday night.  Finally, a BP manager overseeing final well tests apparently had scant experience in deep-water drilling. He told investigators he was on the rig to "learn about deep water," according to notes of an interview with him seen by the Journal.  Some of these decisions were approved by the U.S. Interior Department's Minerals Management Service, which has come under fire for what President Obama has called its "cozy relationship" with the oil industry. But in at least one case, the decision made apparently diverged from a plan MMS approved. MMS declined to comment.  Some of BP's choices allowed it to minimize costly delays. "We were behind schedule already," said Tyrone Benton, a technician who operated underwater robots and worked for a subcontractor. He said that on the day before the accident, a Monday, managers "hoped we'd be finished by that Friday . . . . But it seemed like they were pushing to finish it before Friday."  He added: "They were doing too many jobs at one time." Mr. Benton is suing BP and Transocean claiming physical injury and mental anguish.  BP acknowledges the well was running over budget but says it didn't cut corners. "Safe and reliable operations remain a priority regardless of how much a well is behind schedule or over budget," spokesman  Andrew Gowers wrote in an email.

 Some workers agree safety was paramount for both BP and Transocean. "Safety was their No. 1 concern.Protecting the environment was their No. 1 concern," said Darin Rupinski, a Transocean employee whose job was to help keep the rig in place. BP was drilling to tap an oil reservoir it had identified called Macondo, the same name as the cursed town inGabriel Garcia Marquez's novel "One Hundred Years of Solitude." As on many past projects, BP hired adrilling rig from Transocean, the largest deep-water driller. Workers from Transocean and other contractors

did most of the work, under the supervision of BP employees on the rig and in Houston. BP started working on the well in October, using a different rig. After three weeks natural gas got into thewell, called a "kick." That's not uncommon. But two weeks later a hurricane damaged the rig and it had to betowed to port for repairs. BP started again in January, this time with Transocean'sDeepwater Horizon, a warhorse rig that hadworked for BP for years. BP filed a new drilling permit with federal regulators.  According to a company document seen by the Journal, BP approved spending $96.2 million and about 78days on the well. The target time was much less -- about 51 days. By April 20, the well was in its 80th day,owing to delays such as one that had begun on March 8. That day, workers discovered that gas was seeping into the well, according to drilling reports from the rigreviewed by the Journal. Workers lowered a measuring device to determine what was happening, but when

they tried to pull it back up, it wouldn't budge. Engineers eventually told them to plug the last 2,000 feet of the then-13,000-foot hole with cement and continue the well by drilling off in a different direction. The episode took days to resolve, according to drilling reports, not counting backtracking and re-drilling.Each extra l day cost BP about $1 million in rig lease and contractor fees. Other problems arose. The rock was so brittle drilling mud cracked it open and escaped. One person familiar with the matter estimates BP lost at least $15 million worth of the fluid. Still, by mid-April, the well seemed a qualified success. BP was convinced it had found a lot of oil. Untilengineers in Houston could make plans to start pumping it out, the workers on the nearly complete well, in astandard practice, would plug it and temporarily abandon it. One of the final tasks was to cement in place the steel pipe that ran into the oil reservoir. The cement wouldfill the space between the outside of the pipe and the rock, preventing any gas from flowing up the sides. Halliburton, the cementing contractor, advised BP to install numerous devices to make sure the pipe wascentered in the well before pumping cement, according to Halliburton documents, provided to congressionalinvestigators and seen by the Journal. Otherwise, the cement might develop small channels that gas couldsqueeze through. In an April 18 report to BP, Halliburton warned that if BP didn't use more centering devices, the well wouldlikely have "a SEVERE gas flow problem." Still, BP decided to install fewer of the devices than Halliburtonrecommended -- six instead of 21. BP said it's still investigating how cementing was done. Halliburton said that it followed BP's instructions, andthat while some "were not consistent with industry best practices," they were "within acceptable industrystandards." The cement job was especially important on this well because of a BP design choice that some petroleumengineers call unusual. BP ran a single long pipe, made up of sections screwed together, all the way fromthe sea floor to the oil reservoir. Companies often use two pipes, one inside another, sealed together, with the smaller one sticking into theoil reservoir. With this system, if gas tries to get up the outside of the pipe, it has to break through not justcement but also the seal connecting the pipes. So the more typical design provides an extra level of protection, but also requires another long, expensive piece of pipe. "I couldn't understand why they would run a long string," meaning a single pipe, said David Pursell, apetroleum engineer and managing director of Tudor, Pickering, Holt &Co., an energy-focused investmentbank. Oil major Royal Dutch Shell PLC, in a letter to the MMS, said it "generally does not" use a single pipe. BP's Mr. Gowers said the well design wasn't unusual. BP engineers "evaluate various factors" to determine

what design to use for each well, he said. Despite the well design and the importance of the cement, daily drilling reports show that BP didn't run acritical, but time-consuming, procedure that might have allowed the company to detect and remove gasbuilding up in the well. Before doing a cement job on a well, common industry practice is to circulate the drilling mud through thewell, bringing the mud at the bottom all the way up to the drilling rig.

 This procedure, known as "bottoms up," lets workers check the mud to see if it is absorbing gas leaking in. If so, they can clean the gas out of the mud before putting it back down into the well to maintain the pressure.The American Petroleum Institute says it is "common cementing best practice" to circulate the mud at leastonce. Circulating all the mud in a well of 18,360 feet, as this one was, takes six to 12 hours, say people who've runthe procedure. But mud circulation on this well was done for just 30 minutes on April 19, drilling logs say, notnearly long enough to bring mud to the surface. This decision could have left gas at the bottom of the well. When workers poured in cement to seal thesides, that gas would have been pushed up the outside of the well. Expanding as it rose, it would havereached the top, where it either would have pushed against a massive seal on the ocean floor or might havegone even higher and reached the bottom of the pipe connecting the well to the drilling rig. BP's Mr. Gowers said the amount of time spent circulating mud is "one of many parameters consideredwhen designing a successful cement job." He said BP's investigation is ongoing. Three offshore engineers the Journal asked to review the drilling reports all pointed to the failure to circulatethe mud completely as a serious mistake. Robert MacKenzie, a former oil-industry cementing engineer nowat FBR Capital Markets, said, "If you have any worries about gas, if you have any worries about getting agood cement job, you should definitely do it." BP also didn't run tests to check on the last of the cement after it was pumped into the well, despite theimportance of cement to this well design and despite Halliburton's warning that the cement might not sealproperly. Workers from Schlumberger Ltd. were aboard and available to do such tests, but on the morning of  April 20, about 12 hours before the blowout, BP told Schlumberger workers their work was done, accordingto Schlumberger. They caught a helicopter back to shore at 11 a.m. BP told the Journal Tuesday that the tests weren't run because they were needed only if there were signs of trouble in the cement job, and the work seemed to go smoothly. But the same day, BP officials told

congressional investigators there were signs before the disaster that the cement might have beencontaminated and that some cementing equipment didn't work properly, according to a memo from twoCongressmen. The mood aboard the rig on April 20 was upbeat. The work was nearly done, and workers were eager to putthe troublesome well behind them. Some saw indications that managers wanted to wrap up quickly. Kevin Senegal, a subcontractor employeewho cleaned tanks, said he was told to be ready to clean two tanks on a coming shift instead of the usualone. "To me it looked like they were trying to rush everything," he said.  A disagreement broke out on the rig on April 20 over the procedures to be followed. At 11 a.m., workers for the half-dozen contractors working on the rig gathered for a meeting. Douglas Brown, Transocean's chief mechanic on the rig, testified Wednesday at a hearing in Louisiana that a top BP official had a "skirmish"with top Transocean officials. The Transocean workers, including offshore installation manager Jimmy Wayne Harrell, disagreed with adecision by BP's top manager about how to remove drilling mud and replace it with lighter seawater. Mr.Brown said he heard Mr. Harrell say, "I guess that is what we have those pinchers for," referring to a part of the blowout preventer that would shut off the well in case of an emergency. BP won the argument, said Mr. Brown, who is a plaintiff in a suit against BP and Transocean. Mr. Harrelldeclined Journal requests for comment.  A little after 5 p.m., to check the well's integrity and whether gas was seeping in, rig workers did what iscalled a "negative pressure test." It was supervised by a BP well-site leader, Robert Kaluza. His experiencewas largely in land drilling, and he told investigators he was on the rig to "learn about deep water," accordingto Coast Guard notes of an interview with him. BP declined to comment on his experience.

  A lawyer for Mr. Kaluza said he "did no wrong on theDeepwater Horizon." The test initially strayed from the procedure spelled out in BP's permit, approved by the MMS, according tothe Coast Guard interview with Mr. Kaluza. When the first test results indicated something might be leaking,workers repeated the test, this time following the permitted procedure. The second time, pressure rosesharply, with witnesses saying that the well "continued to flow and spurted," according to notes gathered byBP's investigators that were reviewed by the Journal. BP denies violating its MMS permit.

 Well-control experts say it's clear gas was leaking into the well, most likely through the seal at the top butpossibly through the bottom or even through a collapsed pipe. Earlier this month, BP lawyers told Congress the test results were "inconclusive" or "not satisfactory." OnTuesday, according to the Congressmen's memo, BP said it saw signs of "a very large abnormality." Just two things then stood between the rig and an explosive mixture of gas and oil. One was the heavydrilling mud. The other was the blowout preventer near the sea floor. But the BOP had various problems,among them some leaking hydraulics. By 8 p.m., BP was satisfied with the test and had enough confidence to proceed. It was this that may havebeen "a fundamental mistake," a BP official told congressional staffers Tuesday, according to the memofrom two members of Congress. Following BP's instruction, Transocean workers turned to replacing the mud with seawater, according toCoast Guard interviews with Mr. Kaluza and Donald Vidrine, the top BP official on the rig. Removing the mudkeeps it from polluting the sea but also means there's less weight to hold down any gas. BP's plans for the well, approved by the MMS on April 16, called for workers to remove the mud beforeperforming two procedures designed to make sure gas couldn't get into the well. The first called for installing a giant spring to lock the seal at the top of the well in place after removal of themud. There's no evidence in rig-activity logs the spring was ever installed. If gas was coming up the sides of the well, pushing against the seal, this spring would have helped prevent leakage. Second, BP opted to remove the mud before placing a final cement plug inside the well. In documents presented to Congress, BP has hypothesized that gas could have gotten into the inside of thepipe through a failure of the cement at the bottom of the well. BP was planning to set a second, backupcement plug in the well before declaring its work done.

 But workers began removing mud before setting this plug, leaving little to prevent any gas inside the pipefrom rising to the rig. That plan was approved by the MMS on April 16, according to the permit reviewed bythe Journal.  A spokeswoman for the Interior Department, of which the MMS is a part, said it was "looking at everything,from what happened on the rig that night and the equipment that was being used to the safety, testing andbackup procedures."  About 9:45, the seawater and remaining mud began to head back up the pipe. Witnesses say they saw mudshooting out of the derrick like water from a firehose. A worker on the rig floor made a frantic call to BP's Mr.Vidrine, who had gone to his office, according to his interview with the Coast Guard. Transocean workers raced to tame the well. Nothing worked. This was no ordinary gas kick. It was far moreferocious.

 Workers rushed to hit the emergency button to activate the blowout preventer's clamps and detach the rigfrom the well, according to witness accounts. They were too late. Gas flowing out found an ignition source,and an explosion rocked the rig. Well No. 60-817-44169 was beyond control and on its way to becoming infamous. -- - Vanessa O'Connell, Jeffrey Ball, Douglas A. BP Relied on Cheaper Wells --- Analysis Shows Oil Giant Used 'Risky' Design More Often Than MostPeers By Russell Gold and Tom McGinty

1,310 words19 June 2010The Wall Street JournalJ A1English(Copyright (c) 2010, Dow Jones & Company, Inc.) In recent years, oil giant BP PLC used a well design that has been called "risky" by Congressionalinvestigators in more than one out of three of itsdeepwater wells in the Gulf of Mexico, significantly moreoften than most peers, a Wall Street Journal analysis of federal data shows. The design was used on the well that exploded in the Gulf of Mexico on April 20, killing 11 workers andcausing America's worst offshore oil spill. The only other major well design, which is more expensive,includes more safeguards against a natural-gas blowout of the kind that destroyed theDeepwater Horizon.  A Journal analysis of records provided by the U.S. Minerals Management Service shows that BP used theless costly design -- called "long string" -- on 35% of itsdeepwater wells since July 2003, the earliest datethe well-design data were available. Anadarko Petroleum Corp., a minority partner of BP's in the destroyedwell, used it on 42% of its deepwater Gulf wells, though it says it doesn't do so in wells of the type drilled byBP. Both companies used the design much more often, on average, than other major Gulf drillers. Out of 218deepwater wells in the Gulf drilled since July 2003, 26% used long-string design. It derives its name from itsuse of a single, long "string" of pipe from the sea floor to the bottom of the well. Other big drillers use long-string design less frequently than BP, according to the Journal's data analysis.Royal Dutch Shell PLC used long string designs on 8% of its wells and Chevron Corp. on 15%. Australianfirm BHP Billiton PLC used long string on 4% of its wells. The insight into BP's record comes amid fierce pressure on the oil giant and its partners, who share billionsin liability in the accident. Anadarko blasted BP Friday in a statement by Chief Executive Jim Hackett, who

said: "The mounting evidence clearly demonstrates that this tragedy was preventable and the direct result of BP's reckless decisions and actions."  A long-string design is cheaper because a single pipe runs the length of the well and can be installed in onestep. But it also can create a dangerous pathway for natural gas to rise unchecked outside the pipe. The alternative, known as liners, is seen as safer because it has more built-in places to prevent oil or gasfrom flowing up the well uncontrolled. "There are more barriers, and the barriers are easier to test," saysGene Beck, an engineer and professor at Texas A&M University.  A BP spokesman said long string is widely used and is a perfectly acceptable design, particularly in areaswhere other wells have been drilled and the geology is well understood. "There is nothing inherently unsafeabout long strings," says BP spokesman Andrew Gowers. BP's chief executive, Tony Hayward, told acongressional panel Thursday that "the long string is not an unusual design in the Gulf of Mexico, as Iunderstand it." Long-string wells are made of a continuous length of steel, which makes the well sturdier over time -- a pointMr. Hayward alluded to in his testimony, noting that the design decision had "to do with the long-termintegrity of the well." The Minerals Management Service signed off on BP's long-string plan for the Horizonwell, he added. "It was approved by the MMS," Mr. Hayward said.  Anadarko says it doesn't use long-string design for drilling exploration wells in unfamiliar areas. Thecompany also says it only uses long strings in lower-pressure wells. The well BP was drilling with theDeepwater Horizon was an exploration well, and was well above normal pressure. As minority partner, Anadarko says it didn't have a role in deciding the BP well's design. "It's not that long strings are unsafe, but they have to be under the right conditions," says Darrell Hollek, Anadarko's vice president of gulf operations.

 The other method, known as "liner tieback," is more complex and costlier. First, a section of pipe called aliner is placed at the bottom of the well and cemented into place, creating an extra barrier to prevent naturalgas from rising to the surface. Typically, another pipe is connected to the liner to create a pipe to thesurface. Chevron's head of North American exploration and production, Gary Luquette, said Chevron typically avoidslong-string design because it offers fewer layers of protection. "You can make choices early on to cut costs,

slim down your project to make it economic today and have dire consequences down the road, or you canbuild in that reliability and philosophy of dependability up front and save yourself a lot of headaches in thefuture," Mr. Luquette said. In an internal BP email released by a Congressional committee, a BP drilling engineer in Houston toldcolleagues that the long-string design "saves a good deal of time and money."  A letter to BP's CEO, Mr. Hayward, by two Democratic Congressmen ahead of his testimony to their committee on Thursday, said the choice of the long-string design for the Horizon well was one of fivedecisions BP made that posed a trade-off between cost and well safety. Drilling experts say the long-string design can be riskier than liner-tieback, particularly for high-pressurewells. "It was a safe and accepted method, but it is not the most conservative method. The mostconservative would be to make sure there is not a straight shot [for gas] up to the surface, that you cementeverything in place," says Greg McCormack, director of the University of Texas at Austin PetroleumExtension Service.

  According to an internal BP well-planning document seen by the Journal, the company expected the wellbeing drilled by the Deepwater Horizon to be high-pressure at least as far back as January. Well-control experts and congressional investigators agree that the well design, by itself, didn't doom theDeepwater Horizon. The design created a pathway for gas to flow up the well, potentially pressuringequipment near the floor of the Gulf. But the well would have been secure if the cement plug at the bottom of the hole had held. Congressional investigators Monday criticized BP for using a long string on that well. "The decision," says aletter from Congressmen Henry Waxman (D., Calif.) and Bart Stupak (D., Mich.), "appears to have beenmade to save time and reduce costs." Using a liner design would have cost an additional $7 million to $10million, according to an internal BP estimate released by the congressmen. The congressmen's letter noted that a long string creates only two barriers to prevent gas from flowing up

the well: cement at the bottom of the well, and a large seal where the well reaches the sea floor. The liner-tieback design adds two more barriers: the cement around the liner and a mechanical seal that attaches theliner to the pipes. Marvin Odum, president of Shell's U.S. operations, said Shell doesn't use a long string for high-risk wells.Shell said many of the times it used long strings indeepwater wells, either it hadn't encountered highpressures in the well, or the well was in an area where Shell had drilled and was comfortable with theconditions. "When it is a high-pressure, deepwater well, we only have one way of doing that way, and that iswith a liner tieback. Period," he says. -- - Ben Casselman contributed to this report.

statement from the company. Problems had plagued the well for weeks before April 20. Brian Morel, the young engineer who wrote theemail that morning, had earlier called it a "nightmare well" to BP colleagues, according to an email releasedby investigators. Workers had lost drilling tools in the well, fought off intrusions of explosive natural gas, run far behindschedule and over budget.

 But the sun rose over calm seas that day and offered the promise that the nightmare would soon be over. Workers had finished drilling the well 11 days earlier and had lined it with steel and cement. The job wasclose enough to completion that workers were already "worrying" about the next job, Mr. Morel later said,according to notes taken by BP investigators after the accident. Mr. Morel's attorney declined to comment for this story. On Tuesday, Mr. Morel declined to testify before a federal panel, citing his rights under the Fifth Amendment. Before the Deepwater Horizon crew could leave for another job, there was a final step: The well needed tobe tested to make sure the cement and steel locked together, preventing any gas from leaking in andcausing a fire or explosion.

 If the well passed, giant cement plugs -- as long as a football field -- would be inserted. The well then couldbe temporarily abandoned until BP was ready to tap its oil and gas. Despite its importance, this "negative" test -- how to do it, how to interpret it -- is basically left to the discretionof rig workers. And different rigs have different procedures. Normally, workers on the rig remove about 300 feet of mud below the blowout preventer and replace it withseawater. Mud holds down any gas that leaks into the well. So companies usually test a well fully to makesure it is sealed against any influx of gas before removing too much of the mud. But BP engineers in Houston, including Mr. Morel and his colleague Mark Hafle, had decided to set thecement plug much deeper than usual and remove 10 times as much mud as is normal before running thetest. It was unusual, but BP says it changed the procedure in order to avoid damage to a key seal. Ronald Sepulvado, the top BP manager who was on shore that day with his phone switched off, was askedunder oath by the Interior Department-Coast Guard panel in July if he had ever run a negative test where somuch mud had been removed. "No, ma'am," replied Mr. Sepulvado. Had he ever heard of BP doing so anywhere? "No, ma'am." BP had asked federal regulators for permission to use a deeper plug on April 16, and received approval after only 20 minutes. But Transocean workers and contractors aboard the rig later said that they weren'tinformed of the change until the morning of April 20. The decisions to pull out so much mud perplexed Robert Kaluza, BP's day-shift manager on April 20. "Don'tknow why -- maybe trying to save time," he later told BP internal investigators, according to notes from thatconversation reviewed by the Journal. "At the end of the well sometimes they think about speeding up." Mr. Kaluza has declined to testify before either Congress or the federal panel, citing the Fifth Amendment.BP has denied cutting corners to save time and money. BP said the notes are only investigators'

interpretation of Mr. Kaluza's comments. Jimmy Wayne Harrell also found the directive unorthodox. A 54-year-old from small-town Mississippi, Mr.Harrell had worked for Transocean almost his entire adult life. He was the most senior of the 79 Transoceanworkers aboard the rig that day.  At a daily 11 a.m. meeting in the rig's cinema room, Mr. Kaluza told everyone about BP's plan. Mr. Harrellprotested. "All these plans kept changing," Mr. Harrell later testified. Mr. Harrell and Mr. Kaluza argued about thenegative test, according to one witness.

 "This is how it's going to be," Mr. Kaluza said, according to sworn testimony from one witness, and Mr.Harrell "reluctantly agreed." In sworn testimony, Mr. Harrell denied arguing with Mr. Kaluza. He said he just wanted to make sure that anegative test was performed and that Mr. Kaluza agreed. But his lawyer, Pat Fanning, said that Mr. Harrellalso told Mr. Kaluza he didn't want to remove so much mud before running the negative test and wasoverruled. Mr. Kaluza couldn't be reached for comment for this story.

 "It was BP's well, they were paying for it. BP gave the marching orders," Mr. Fanning said. Not long after the meeting, a helicopter carrying a group of executives -- two each from BP and Transocean --landed on the rig for a tour. Mr. Harrell said he spent almost the entire rest of the day either showing them around or in his office. By 5 p.m., Transocean workers had removed much of the mud and started the pressure test, according to atimeline of events prepared by BP. It didn't go well. Pressure built up unexpectedly, and no one was sure why. Workers in the rig's central"drilling shack," a type of control room, struggled to interpret the readings. In walked Mr. Harrell with thevisiting VIPs. Mr. Harrell stayed behind as the tour moved on, but he didn't think the problem was serious. He orderedanother worker to tighten down a valve at the top of the blowout preventer -- the device that is supposed topinch off the well in the event of disaster -- that prevented mud above from leaking down. This seemed to resolve the problem. Mr. Harrell testified that he was satisfied with the test results and wentback to the visiting executives. It was the last time there is any record of Mr. Harrell, the rig's most experienced leader, setting foot on thedrill floor. His lawyer says he wasn't distracted by the visiting executives and that the crew could have askedfor his help at any time, but never did. Mr. Harrell's second-in-command, Randy Ezell, stuck around the drill shack for a few more minutes, but soonhe also left to return to the visiting executives. He later testified to the joint Interior Department and CoastGuard panel that if it hadn't been for the tour, he would have stayed longer to deal with the situation. With Mr. Harrell gone, the argument continued. Wyman Wheeler wasn't convinced everything was all right.Mr. Wheeler was the dayshift toolpusher, the man who supervised the drilling crew for 12 hours each day. "Wyman was convinced something wasn't right," Christopher Pleasant, another Transocean worker, later recalled in testimony. Mr. Wheeler couldn't be reached for comment. But Mr. Wheeler's shift wrapped up at 6 p.m. on April 20. His replacement, Jason Anderson, came on dutyand had his own interpretation of the test, according to Mr. Pleasant. Mr. Anderson, 35, had worked on the rig since it left the shipyard in 2001.  A burly former high school linebacker, Mr. Anderson had earned the respect of his fellow rig workers, andnow he assured them that the pressure readings weren't unusual. Mr. Kaluza decided to check with Donald Vidrine, an experienced BP manager who was due to relieve Mr.Kaluza at 6 p.m. The two BP men conferred for an hour, with Mr. Vidrine peppering Mr. Kaluza with questions. Mr. Vidrinewasn't satisfied. "I wanted to do another test," he said, according to the notes of BP's internal investigationseen by the Journal. Workers performed the test again, but this time the results were even more perplexing. One smaller tubethat led up from the well showed no pressure, a sign that the well was stable. But gauges on the main pipedid show pressure, according to BP's preliminary investigation. The two pipes were connected and should have had the same pressure. It wasn't clear what was going on inthe well. One possibility, put forward by engineers who have studied the events subsequently, is that the

smaller pipe was clogged, interfering with pressure readings. Finally, about 7:50 p.m., Mr. Vidrine made a decision, according to Mr. Pleasant. He turned to Mr. Kaluza,his colleague, and told him to call BP engineers in Houston and tell them he was satisfied with the test, Mr.Pleasant said. Mr. Vidrine, through his attorney, declined to comment. Over the next two hours, there were other signs the well was slipping out of control. For one, more fluid wasflowing out of the well than was being pumped in, according to electronic data reviewed by investigatorsafter the explosion. But none of the Transocean workers monitoring the well caught these signs. Investigators from the federalpanel have said Transocean workers may have struggled to monitor the well because they were performingother work at the same time.  Around 9 p.m., the meeting of the executives wrapped up. A few of them went up to the bridge, including PatO'Bryan, BP's recently appointed vice president of drilling in the Gulf of Mexico. The rig's captain showed them a training simulator -- a video game that allowed the crew to practice keepingthe giant Deepwater Horizon in position during severe weather. Mr. O'Bryan, 49, had earned a Ph.D. at Louisiana State University decades earlier on how to measure gas

escaping into a well. Now, gas was escaping in, unchecked, and Mr. O'Bryan was on the bridge -- standingaround the simulator with the rig's captain. Mr. Ezell, the second-in-command, was lying in bed, watching television with the lights out, when his phonerang, he testified before the panel in May. He glanced at his clock. It was 9:50 p.m. "We have a situation," said Steve Curtis, an assistant driller, on the other end of the phone, according to Mr.Ezell. "Randy, we need your help." Mr. Ezell got up, put on his clothes and went to get his hard hat asalarms rang. Before he could reach it, the first of two massive explosions ripped through the rig. In the following few minutes, Messrs. Anderson and Curtis were killed. Mr. Wheeler was badly injured. Theblowout preventer failed to pinch off the well. And most of the other men who made the crucial decisions of  April 20 were fleeing for their lives.

Inspectors Adrift in Rig-Safety Push --- Outgunned by Industry and Outmatched by Job, Agency Lags By Leslie Eaton, Stephen Power and Russell Gold

1,955 words3 December 2010The Wall Street JournalJ A1English(Copyright (c) 2010, Dow Jones & Company, Inc.) NEW ORLEANS -- Seven months after the Deepwater Horizon exploded in the Gulf of Mexico, the troubledfederal agency that oversees offshore drilling has been revamped, renamed and given a new leader with amandate to turn what critics called an industry lapdog into an effective watchdog. But there's at least one big change the agency hasn't made: fixing its deeply flawed inspection program. As ithas for four decades, that program sends inspectors armed with little more than checklists and pencils intothe Gulf to ensure the safety of more than 3,500 oil platforms and drilling rigs.  A Wall Street Journal examination finds that these inspectors have been overruled by industry, underminedby their own managers and outmatched by the sheer number of offshore installations they oversee.Inspectors come into the job with little or no hands-on experience in deep-water drilling, learning as they go. The small cadre of 55 or so inspectors, who are largely checking hardware, get good marks for reducingworkplace injuries on rigs and platforms. But safety experts say the main causes of major accidents arealmost always human error, not the mechanical failures that inspectors focus on. Inspectors aren't lookingfor signs of systemic safety problems -- poor decisions, cutting corners, muddled responsibilities -- thatinvestigators are linking to the Deepwater Horizon explosion. The agency "thought all we needed to do was to go through the motions," says Perry Jennings, a former inspector who is now president of the New Orleans chapter of the American Federation of GovernmentEmployees, which represents rig inspectors. "And, guess what? It's come back to bite them." On Thursday, the staff of the presidential commission looking intoDeepwater Horizon issued a scathingcritique of how the U.S. oversees offshore drilling. It faulted the government for skimping on money for the

agency, called inspector training "inadequate and unacceptable" and recommended a wholesale change inthe way the agency approaches safety regulations. Michael Bromwich, the one-time federal prosecutor recruited by the Obama Administration to revamp theMinerals Management Service, says that most inspectors were diligently doing their jobs. But, he acknowledges, the effort was inadequate: "As operations offshore became more sophisticated, our inspection regime did not keep pace." How to improve the agency is a matter of dispute. Republican lawmakers caution that throwing more moneyinto the agency wouldn't be a silver bullet and are likely to resist a major funding bump. Mr. Bromwich saysthe agency, now called the Bureau of Ocean Energy Management, Regulation and Enforcement, has teamsof employees and consultants working to redesign the inspection program, and hopes to have a plan byearly next year. While the causes of the Deepwater Horizon disaster remain in question, investigators are pointing to a

complex series of human errors, including the design of the well and the failure to recognize problems withcement that was supposed to keep natural gas from flowing into the well. Crucial safety barriers, such as afinal cement plug, were not installed or were removed; workers aboard the rig misinterpreted key tests andfailed to notice warning signs. No one knows if a more robust and sophisticated inspection program could have detected these problems or prevented the explosion. But there is broad agreement among safety experts that a massive overhaul isneeded to create the kind of inspection program that can help avoid such disasters in the future. "They need to concentrate more on safety management systems and safety culture as opposed to just onequipment," says Ken Arnold, an engineer who served on an advisory panel that recommended changes tothe inspection program back in 1990. "That is a very hard thing to do."

 Offshore drilling has changed substantially since the government inspection program was created followinga spill off the coast of California in 1969. Oil company engineers onshore can monitor what happens on therigs and platforms, minute by minute. High-tech equipment and subsea robots allow rigs to float above a wella mile or more under the surface and to drill in freezing temperatures and high pressure into once-unreachable rock formations. Inspecting, however, has stayed much the same.

 Inspectors are mostly former oilfield workers without college degrees, have little formal training and aren'trequired to pass any certification tests; they are expected to learn their craft by shadowing more experiencedpeers. They have almost no direct experience in the specialized field of deep-water drilling, and, duringoffshore inspections, have no access to technology more advanced than a pocket calculator. Failed inspections seldom result in fines, which tend to be low to begin with. Last year, inspectors wrote up2,298 violations offshore, but only 87 were referred on for possible fines, according to an Interior Departmentreport. The agency levied civil penalties in 20 cases last year, totaling $919,000 -- less than the cost of oneday's drilling aboard the Deepwater Horizon. Inspectors can shut down operations for safety reasons, which can cost energy companies far more than thefines. But to do so, inspectors need permission from their managers -- and some former inspectors complainabout having been overruled. The agency's inspection manpower for the Gulf of Mexico is concentrated in five district offices stretching400 miles along the coast from New Orleans to Lake Jackson, Texas, south of Houston. Much of inspectors' time offshore is spent reviewing paperwork and copying data -- such as how much oil isbeing pumped or dates on which equipment was tested -- by hand from printouts and logs. They make surefirst-aid kits are on board, and that any open holes in the deck have railings to prevent falls. They are also supposed to check safety equipment such as gas alarms and watch them being tested by rigworkers "as conditions and time permit." Many inspectors use a laminated check-list of potential violations tolook out for -- the one for drilling includes almost 150 items. Many inspections are relatively brief. The last inspector to visit theDeepwater Horizon, Eric Neal, spent justtwo hours on April 1 aboard the high-tech 32,600-ton, multi-level installation. That was about the sameamount of time a visiting group of executives would take to tour the rig on April 20, the day of the disaster. The 33-year-old Mr. Neal had visited the rig only once before, with his father, who was training him and is

also an inspector. He told federal investigators that he felt more qualified to inspect the installations thatproduce oil and gas than he did drilling rigs, which in the last decade have become increasingly high-techand complex as they have moved into deeper water.  As part of cost-cutting measures in the 1990s, the agency started requiring inspectors to be generalists,rather than specializing in specific types of installations like they used to. Mr. Neal declined to comment, and the agency would not make him or other inspectors available for interviews. "In the beginning the job was to make sure people were performing in safe conditions," says CynthiaThompson, who started out as a technician at the agency in New Orleans more than 25 years ago and saysshe became its first female inspector in 1997. "I didn't go out to be quick, I'd go out to be thorough." Offshore workers sometimes resisted her efforts, she says; many energy companies give out bonuses totheir workers when inspectors don't write up any violations. "I'd tell them, 'I don't live here, you do,'" says Ms. Thompson, who retired at the end of 2004 after beinginjured in a helicopter accident. Inspectors and their managers have a long history of being courted -- and pressured -- by industryexecutives and low-level rig workers, many of whom went to the same high schools, attend the samechurches and cheer for the same little-league teams as the inspectors, former inspectors have said. Thecoziness also has been noted in reports by the Interior Department's inspector general. When Mr. Bromwich arrived at the revamped agency in June, he vowed to tackle "the reality andappearance of" conflicts of interest at the agency. He set up an internal investigations unit to root out

misconduct, and began requiring inspectors to disqualify themselves when they have personal or family tiesto a company. Inspectors on day trips offshore now have to bring their own lunches, and are no longer allowed to accept free food aboard rigs. The agency is also developing a system to track violations and ensure they're corrected, and has askedCongress to increase civil penalties. Next year, offshore operators will be required to have detailed safetyplans that the agency can review and enforce. The agency recommended this policy almost two decadesago but never implemented it.

  A new training program is being designed for inspectors, whose number has risen to 58, with seven newhires pending. The plan is to add a total of 30. Mr. Bromwich has gone on trips to five colleges along the Gulf Coast to try to recruit engineering students. One obstacle: The agency is restricted by federal salary guidelines, and so can't offer competitive pay toattract highly educated inspectors. Salaries for most inspectors start at $47,448. Engineering-schoolgraduates command far more when they go to work for the energy industry. This year's graduates of Louisiana State University's petroleum engineering school, for example, were offered an average of $94,000for industry jobs. Some safety experts, including members of the presidential panel investigating the oil spill, have suggestedthat the agency borrow an approach adopted by the United Kingdom after a 1988 platform explosion killed167 people. The approach, known as "safety case," puts the burden on companies to prove to regulators that they have

identified risks and developed procedures to cope with them -- before the companies can receive permissionto drill. Inspectors and auditors are supposed to keep an eye on whether the company is doing a good jobpolicing itself, and whether any deficiencies are being communicated to senior executives. Mr. Bromwich has said that adopting this approach wholesale would be disruptive for years. Either way, paying for any changes is likely to be a problem. Congress has given the agency $365 million for the current fiscal year, about $23 million more than last year's budget, but well short of the $100 millionboost that the Obama administration had sought. The administration has proposed finding more money by upping the fees that companies pay to fundinspections -- an idea that has drawn the industry's opposition. Some industry leaders, including Rex Tillerson, the chief executive of Exxon Mobil Corp., say they wantcompetent regulators. The industry's powerful lobbying group, the American Petroleum Institute, says it

supports giving the agency more money, but that the funding should come out of the billions of dollars theindustry already pays in taxes and royalties. Key congressional Republicans are skeptical about increasing the agency's budget at a time of cost cutting."The question I have is, what specifically are these inspectors going to do and how it will ensure we haveproduction?" says Rep. Doc Hastings, a Washington Republican who is likely to become chairman of theHouse Natural Resources Committee. 9 December 2010The Wall Street JournalJ A1English(Copyright (c) 2010, Dow Jones & Company, Inc.) The oil industry has said the Deepwater Horizon rig catastrophe was a unique event, the result of anunprecedented series of missteps that are unlikely to be repeated. The recent history of offshore drillingsuggests otherwise. In the months before and after the rig exploded and sank, killing 11 and spilling millions of barrels of oil intothe Gulf of Mexico, the industry was hit with several serious spills and alarming near-misses, some of themstrikingly similar to what happened aboard theDeepwater Horizon.  A blowout off the coast of Australia left oil flowing into the Timor Sea for weeks. An out-of-control well in theGulf of Mexico dislodged a 4,000-pound piece of equipment on the deck of the Lorris Bouzigard drilling rig asworkers scurried to safety. A gas leak in the North Sea aboard a production platform came within a roguespark of a Deepwater Horizon-scale disaster off the coast of Norway. Data from regulators around the world suggest that after years of improvement, the offshore-drillingindustry's safety record declined over the past two years. The Wall Street Journal reviewed statistics from four countries with large offshore oil industries and modernregulatory systems: the U.S., Great Britain, Norway and Australia. (A fifth, Brazil, declined to make its dataavailable.) Each country uses different approaches to measure losses of well control or spills, but they reveala similar trend. In the U.S. portion of the Gulf of Mexico in 2009, there were 28 major drilling-related spills, natural-gasreleases or incidents in which workers lost control of a well. That is up 4% from 2008, 56% from 2007, andnearly two-thirds from 2006. Taking into account the number of hours worked on offshore rigs, the rate of these incidents rose every year from 2006 to 2009.

 The U.K.'s Health and Safety Executive counted 85 serious oil and gas releases in the 12 months endedMarch 31, up 39% from a year earlier. Taking into account the number of hours worked offshore, the rate of incidents was the highest since 2004-05. In Norway, companies had 37 oil and gas releases and "well incidents" in 2009, according to the country'soffshore regulator. That is up 48% from 2008 and is the highest level since 2003. Norway's rate of incidentsper man-hour rose 42% in 2009, to its highest level since 2005. In Australia in the first half of this year, there were 23 oil spills, gas releases and incidents in which oil or gasentered a well, threatening a blowout, according to Australia's National Offshore Petroleum Safety Authority.That is almost as many as the 24 such incidents in all of 2009. The incident rate, accounting for hoursworked, has more than doubled since 2005. "Why does the safety performance of the offshore industry seem to be deteriorating?" asks Jane Cutler, aformer oil executive who now heads Australia's offshore regulator. Her answer: "People can forget to be

afraid." The American Petroleum Institute, which represents the U.S. oil industry, cautions against reading too muchinto the statistics. "Drawing sweeping conclusions from a limited analysis of this data is simply not valid," theinstitute said in a statement. In the past, API experts have pointed to other data that indicate the industry'srecord is improving, such as a steady decline in the volume of oil spilled each year. "I do not believe there is an outbreak or a rash of accidents," says Lee Hunt, president of the International Association of Drilling Contractors, a trade group. "When they do happen, there is generally a high level of competency demonstrated in resolving the situation successfully."

Still, oil companies that drill around the world are re-examining their procedures, in large part out of fear thata mistake could lead to their own costly disaster. BP Plc has estimated that the Gulf spill will cost thecompany and its partners, which owned the blown-out well, $40 billion. There are various possible explanations for the recent spate of problems. Investigations into theDeepwater Horizon and some other recent incidents have pointed to the industry's difficulties finding and retainingenough experienced workers, its struggles to balance safety priorities with profit demands, and occasionallapses in the face of lax regulation. These challenges have become more pronounced as oil companies

continue to push the limits of technology and experience in deeper water, harsher environments and morecomplex oil reservoirs, the investigators say. Nevertheless, investment in deep-water oil is speeding up. Drilling there is critical to sating the growingglobal thirst for fuel. The potential payoffs -- shareholder profits for oil companies, tax revenue, jobs andenergy independence for governments -- are too big to stop offshore development. The Obama administration ended its six-month moratorium on deep-water drilling in October, one monthahead of schedule, although it has reversed plans to expand drilling into new areas. The European Unionbriefly considered its own moratorium, but backed down under pressure from member nations that saidexisting regulations were sufficient. "Onshore oil production is decreasing. Shallow water is decreasing. Deep water is the only place that isincreasing," says Rafael Sandrea, an industry consultant. "That is why everyone is heading in that direction." Since the Deepwater Horizon blowout, industry and regulators have promised to take steps to prevent

another such disaster. New rules are being developed for the Gulf of Mexico. In the U.S. and Europe, newrapid-response systems are in the works to contain deep-water oil leaks. The industry's faith in its ability to safely develop oil and gas from facilities at sea is largely unshaken. TheDeepwater Horizon "was an isolated incident," says Erik Milito, a senior official at the American PetroleumInstitute. "We do not believe there is a systemic failure across the industry." The industry points to a lengthy track record -- 50,000 wells drilled in the Gulf of Mexico without acatastrophe similar to what unfolded earlier this year. But some experts say such statistics mask the increasing challenge of offshore drilling. With each passingyear, the wells are getting more complex because the easy drilling locations already were tapped. Most of those 50,000 wells were easy compared to theDeepwater Horizon's well. David M. Pritchard, a petroleum engineer and consultant, studied a database of 5,000 Gulf of Mexico wells

since 1993. The data is compiled by industry to allow companies to compare their performance against their peers. The wells are indexed by drilling difficulty. Mr. Pritchard looked for wells that were at least as complexas the well the Deepwater Horizon was trying to drill. He found 43. "What is the real risk of occurrence of a catastrophic failure? Is it one in 50,000, or is it now one in 43?" asksMr. Pritchard. He says he worries the industry is in "total denial" about the risks it faces. The American Petroleum Institute says it is looking at Mr. Pritchard's research as part of a wide-rangingexamination of deep-water well-design issues. It expects to issue recommendations next year. The Deepwater Horizon isn't the only offshore oil facility to run into problems recently. Several lower-profileincidents illuminate the industry's growing struggle to manage deep-water risks. On April 19, 2009, almost exactly a year before theDeepwater Horizon exploded less than 20 miles away,another Gulf drilling rig was rocked by a violent blast. Noble Corp.'s Lorris Bouzigard rig was working in morethan 2,000 feet of water when a bubble of explosive gas raced up the well, pushing ahead of it heavy drilling

fluid and knocking out of place a two-ton piece of metal, according to a subsequent federal investigation. Rigworkers recalled a "deafening roar" and struggled to regain control as other crew members rushed tolifeboats. The workers were able to trigger a massive set of valves called the blowout preventer, shutting off the well,and the gas dispersed without igniting. No one was injured. Federal investigators concluded that, as was the case with theDeepwater Horizon, the rig's crew didn'timmediately detect gas entering the well, in part because computer screens in the control room weren'tconfigured to show problems clearly. Moreover, investigators said, workers for the well's owner, LLOGExploration Co., had made a last-minute procedural change, eliminating a step that might have cleared gasout of the well.

 Bob McMann, vice president of drilling for Covington, La.-based LLOG, says the company accepts most of the investigators' findings, including its workers' failure to follow the planned procedure. He says the incident"wasn't even close" to being a major disaster, and credited workers' training and skill with resolving thesituation quickly. Noble declined to discuss the incident. Several months later, on the other side of the world, workers failed to contain another big problem. On Aug.21, 2009, a Thai company drilling in the Timor Sea 400 miles west of Darwin, Australia, lost control of an oil

well being drilled from its Montara platform. The operator, a unit of state-controlled PTT PCL, had completed drilling a 12,450-foot well in March 2009and temporarily shut it down. PTT returned in August and began work to connect the well so it could produceoil.  A few hours after the well was reopened, it burped about 50 barrels of oil and an undetermined amount of gas. After alarms died down, work resumed. Two hours later, a major blowout occurred. The well spilledseveral hundred barrels a day of oil into the sea for six weeks. A fire melted the drilling rig and platform,causing an estimated $150 million in damage.  An Australian government report on the incident made public last month criticized PTT for "widespread andsystemic" shortcomings. The blowout, it said, was "an accident waiting to happen." Investigators said thatlocal regulators were too cozy with PTT and didn't engage in rigorous oversight, a charge similar to oneleveled at U.S. regulators in the aftermath of theDeepwater Horizon spill.  Another similarity between the PTT and BP spills: Workers missed a key opportunity to discover the problemusing a pressure test. In the case of the PTT blowout, workers didn't run a pressure test that could haveshown them the well was leaking, according to the government investigation. In the case of theDeepwater Horizon, workers did run such a test, but misinterpreted the results, in what both BP and outsideinvestigators have described as a crucial mistake. Ms. Cutler, the Australian federal offshore regulator, blames the PTT accident on "incompetent personnel,staff and contractors." PTT has acknowledged "deficiencies" and promised a new commitment to safeoperations, according to a statement. On May 19 of this year, a month after theDeepwater Horizon accident, with the industry under world-widescrutiny and the BP well still gushing, another major offshore disaster was narrowly avoided. This occurredaboard the Gullfaks C producing platform off the coast of Norway, a country considered to have highstandards for safe and environmentally responsible offshore exploration.  A 16,000-foot well being drilled from the platform encountered a series of well-control problems, includingtwo serious gas influxes, but the operator, Statoil ASA, pressed on. An internal Statoil report from November says its workers didn't notice when pressure on the pipe increased, a clear sign of trouble. Eventually, the pipe burst and heavy drilling mud began leaking out of the well into underground rocks. Theloss of mud left less weight inside the well, allowing oil and gas to force their way up toward the surface.Fortunately, the oil carried solids along with it, plugging up the holes in the pipe and stopping the flow on itsown. "It is as close as you can be to an explosion," says Goril Tjetland, a former Statoil petroleum geologist whonow works with the Bellona Foundation, a Norwegian environmental-advocacy group. Norway's offshore regulator, the Petroleum Safety Authority, concluded that Statoil hadn't assigned workerswith the right experience to the project, which involved a particularly difficult type of drilling. "Only chance . . .prevented the incident from developing into a major accident," the regulator concluded in a report. Alarmedby a rise in such incidents, the regulator noted that "limited competence may be a contributing factor."

 The industry and its backers point out that none of the three incidents rival theDeepwater Horizon in scopeor damage. In fact, they say, the mishaps show how in most instances, procedures and technology in placesaved the day or mitigated the damage. Some experts say the spate of accidents shows that theDeepwater Horizon wasn't an isolated event, andthat by describing the disaster as a "low probability, high consequence event," the industry has blinded itself to the likelihood that such accidents will recur. "This accident was bound to happen," says Nancy Leveson, a Massachusetts Institute of Technologyprofessor and engineering safety expert who has studied theDeepwater Horizon disaster. "It might not

The Gulf Oil Spill: There Was 'Nobody in Charge' --- After the Blast, Horizon Was Hobbled by aComplex Chain of Command; a 23-Year-Old Steps In to Radio a Mayday By Douglas A. Blackmon, Vanessa O'Connell, Alexandra Berzon and Ana Campoy

Flames were spreading rapidly, power was out, and terrified workers wereleaping into the dark, oil-coated sea. Capt. Curt Kuchta, the vessel's commander, huddled on the bridge withabout 10 other managers and crew members.  Andrea Fleytas, a 23-year-old worker who helped operate the rig's sophisticated navigation machinery,suddenly noticed a glaring oversight: No one had issued a distress signal to the outside world, she recalls inan interview. Ms. Fleytas grabbed the radio and began calling over a signal monitored by the Coast Guardand other vessels.

 "Mayday, Mayday. This is Deepwater Horizon. We have an uncontrollable fire." When Capt. Kuchta realized what she had done, he reprimanded her, she says. "I didn't give you authority to do that," he said, according to Ms. Fleytas, who says she responded: "I'msorry."  An examination by The Wall Street Journal of what happened aboard theDeepwater Horizon just beforeand after the explosions suggests the rig was unprepared for the kind of disaster that struck and wasoverwhelmed when it occurred. The events on the bridge raise questions about whether the rig's leaderswere prepared for handling such a fast-moving emergency and for evacuating the rig -- and, more broadly,whether the U.S. has sufficient safety rules for such complex drilling operations in very deep water. The chain of command broke down at times during the crisis, according to many crew members. They report

that there was disarray on the bridge and pandemonium in the lifeboat area, where some people jumpedoverboard and others called for boats to be launched only partially filled. The vessel's written safety procedures appear to have made it difficult to respond swiftly to a disaster thatescalated at the speed of the events on April 20. For example, the guidelines require that a rig worker attempting to contain a gas emergency had to call two senior rig officials before deciding what to do. One of them was in the shower during the critical minutes, according to several crew members. The written procedures required multiple people to jointly make decisions about how to respond to"dangerous" levels of gas -- a term that wasn't precisely defined -- and some members of the crew wereunclear about who had authority to initiate an emergency shutdown of the well. This account of what happened aboard the rig at the time of the explosions, which killed 11, is based oninterviews with survivors, their written accounts, testimony to the Coast Guard and internal documents of rigoperator Transocean Ltd. and well owner BP PLC. In written responses to the Journal, Transocean said that the time between the first sign of trouble and thecatastrophic explosion was too short for the crew to have done anything to effectively prevent or minimizethe disaster. The company also said the rig's chain of command was in place and "did not hinder responsetime or activity."  At a Coast Guard hearing on Thursday, Jimmy Wayne Harrell, the top Transocean executive on the rig,acknowledged under questioning that a split chain of command on the platform could lead to "confusion" butit didn't hinder emergency response. At the same hearing, Capt. Kuchta said that communications had notbeen a problem. BP declined to comment on anything that happened April 20.

 In the minutes before the Deepwater Horizon exploded, almost no one on board realized that serioustrouble was brewing, other than a few men on the drilling floor -- the uppermost of three levels on themassive structure. The sea was as still as glass. A cool wind blew faintly from the north. Capt. Kuchta was hosting two BPexecutives on board for a ceremony honoring the rig for seven years without a serious accident. Nearly 20 men, many of them close friends, were operating the drilling apparatus, which already had boredthrough more than 13,000 feet of rock about 5,000 feet deep in the Gulf of Mexico. No alarms had soundedthat day signaling gas on the platform.  At about 9:47 p.m., workers all over the rig heard a sudden hiss of methane gas. Methane is often present inthe ground in and near reservoirs of crude oil, and managing the threat is a regular part of drilling. Within two minutes, pressure caused by gas in the well pipe had spiked dramatically, drilling recordsindicate. A torrent of methane gas struck the rig. Power failed throughout the vessel. "Everything started jumping up and down and rocking us," said Kevin Senegal, 45, a tank cleaner, in an interview. Out on the water, 40 feet away, a 260-foot supply ship called the Damon B. Bankston was tethered to the rigby a hose. That ship's captain said in an interview that he saw drilling "mud," which is used as acounterweight to gas in the well, flying out of the drilling derrick like a "volcano." He radioed the bridge of theDeepwater Horizon. He was told there was "trouble with the well" and the Bankston should move 150

meters back. Then the channel went silent. Micah Sandell, a 40-year-old with a wife and three children, watched with alarm from the rig's gantry crane, amassive device that moved across the main deck on a track. He radioed his crew to move away from thederrick. Down on the deck, Heber Morales, 33, a former Marine from Texas, turned to the worker beside him. "Oh,man. That's not good," he said. The two moved away from the derrick. Up in the crane, Mr. Sandell saw another worker on the deck, assistant driller Donald Clark, a 48-year-oldformer soybean farmer from Newellton, La., bolt for a set of stairs leading for the area where workers werefighting to control the well. Ms. Fleytas, one of only three female workers in the 126-member crew, was on the bridge monitoring therig's exact location and stability. Briefly, all the equipment went black, then a backup battery kicked on. Sheand her coworkers checked their monitors, which indicated no engines or thrusters were operational.

Multiple gas alarms were sounding. One of the six huge engines that kept the floating platform stable wasrevving wildly. No methane had been detected on the Deepwater Horizon before the massive gas jolt. So no "Level 1" gasemergency -- according to Transocean safety regulations, when "dangerous" levels of gas are detected inthe well -- had been declared, according to crew members. That meant the crew had gotten no general alertto prepare for trouble and no order to shut down anything that might ignite the gas. The rig's regulations state that in the event of such an emergency, the two top managers -- on April 20 theywere BP's senior person on the rig, Donald Vidrine, and Transocean's installation manager, Mr. Harrell --were to go to the drilling floor and evaluate the situation jointly. But once the gas hit, neither was able to getto the area. Transocean says the rig's chain of command and safety standards were followed and worked effectivelyunder the circumstances. Mr. Harrell didn't return phone calls. BP said Mr. Vidine was unavailable tocomment.

 When the pressure in the well spiked suddenly, the drilling crew had limited options and little time to act.Jason Anderson, a 35-year-old "toolpusher" who was supervising the crew on the oil platform's drilling floor,tried to divert gas away from the rig by closing the "bag," a thick membrane that surrounds a key part of thedrill mechanism. That didn't work. Four emergency calls were made from the rig floor to senior crew members in the moments before the blast,according to a BP document reviewed by the Journal. One went to Mr. Vidrine, according to notes about astatement he gave the Coast Guard that were reviewed by the Journal. The rig worker, who isn't identified inthe notes, told him the drilling crew was "getting mud back," a sign that gas was flooding into the well. At thatpoint, Mr. Vidrine rushed for the drilling floor, but already "mud was everywhere," he told the Coast Guard.

  At about 9:50 p.m., Stephen Curtis, the 40-year-old assistant driller working with Mr. Anderson, called therig's senior toolpusher, Randy Ezell, who was in his sleeping quarters, according to a statement given by Mr.Ezell to the Coast Guard. Mr. Curtis said that methane was surging into the well and workers were on the verge of losing control. Two rig workers who later discussed the matter with Mr. Ezell said he was told that Mr. Anderson was goingto trigger the blowout preventer, a 450-ton device designed to slice the drill pipe at the ocean floor and sealthe well in less than a minute. If triggered in time, it might have been enough to prevent the explosions, or atleast limit the scale of the disaster, say some drilling experts. Mr. Ezell prepared to go to the drilling floor,according to his statement. Seconds later, the methane ignited, possibly triggered by the revving engine. That set off an explosion thatblew away critical sections of the Deepwater Horizon, sheared off at least one engine, set large parts of therig on fire and allowed oil to begin spewing into the sea. Mr. Curtis, an ex-military man who enjoyed turkey hunting, and Mr. Anderson, a father of two who wasplanning to leave the Deepwater Horizon for good at the end of his 21-day rotation, almost certainly werekilled instantly, according to other workers. So was veteran driller Dewey Revette, 48, from State Line, Miss. Six men working nearby also died. Theyincluded 22-year-old Shane Roshto and Karl Kleppinger, Jr., 38, from Natchez, Miss., and Mr. Clark, the

assistant driller who had rushed to the stairs to help out. Dale Burkeen, a 37-year-old Mississippian who operated the rig's tall starboard crane, had been trying to getout of harm's way when the blast hit. It blew him off a catwalk, other workers say, and he fell more than 50feet to the deck, where he died.  A series of detonations followed. The motor room was wrecked. Steel doors were blown off their hinges. Thewheel on one door flew off and struck a worker. Crew members were hurled across rooms, leaving manywith broken bones, gashes and serious burns. When he heard the first explosion, toolpusher Wyman Wheeler, who was scheduled to go home the nextday, was in his bunk. He got up to investigate. The second blast blew the door off his quarters, breaking hisshoulder and right leg in five places, according to family members. Other workers scooped him up andcarried him toward the lifeboat deck on a stretcher. The explosions knocked gantry-crane operator Mr. Sandell out of his seat and across the cab. As he fled

down a spiral staircase to the deck, another explosion sent him into the air. He fell more than 10 feet, thengot up to run. "Around me all over the deck, I couldn't see nothing but fire," he said in an interview. "Therewas no smoke, only flames." He ran for the lifeboat deck. From the bridge, Chief Mate David Young ran outside to investigate and to suit up for firefighting. After heencountered only one other crew member in gear, he returned to the bridge. Crew members say nosignificant firefighting efforts were undertaken. "We had no fire pumps. There was nothing to do but abandonship," said Capt. Kuchta, in testimony at a Coast Guard inquiry on Thursday.  As workers poured out of their quarters, many found their routes to open decks blocked. Ceiling tiles andinsulation were blown everywhere. In some areas, fire-suppression systems were discharging carbondioxide. Stairways were gone.  According to many workers, most crew members didn't get clear direction from the bridge about what to dofor several minutes. Finally, the public-address system began to blare: "Fire. Fire. Fire. Fire on the rig floor.This is not a drill."

 Many crew members couldn't reach their designated assembly areas. Scores scrambled instead toward theonly two accessible lifeboats, which hung by cables 75 feet above the water on one side of the rig. Eachenclosed and motorized boat could hold about 75 passengers. "The scene was very chaotic," said worker Carlos Ramos in an interview. "People were in a state of panic."Flames were shooting out of the well hole to a height of 250 feet or more. Debris was falling. One craneboom on the rig melted from the heat and folded over. Injured workers were scattered around the deck. Others were yelling that the rig was going to blow up."There was no chain of command. Nobody in charge," Mr. Ramos said.

 "People were just coming out of nowhere and just trying to get on the lifeboats," said Darin Rupinski, one of the operators of the rig's positioning system, in an interview. "One guy was actually hanging off the railing. ... People were saying that we needed to get out of there."  At one point, a Transocean executive was standing partly in the lifeboat, helping injured workers off the rigand telling Mr. Rupinski not to lower the boat yet. Rig workers piling in were shouting for him to get the boatdown.

 "There had to be at least 50 people in the boat, yelling, screaming at you to lower the boat," Mr. Rupinskirecalled. "And you have a person outside saying, 'We have to wait.'" Terrified workers began jumping directly into the sea -- a 75-foot leap into the darkness. Mr. Rupinski radioedthe bridge that workers were going overboard.  A Transocean spokesman said the company hasn't yet been able to determine exactly what happened in thelifeboat loading area. Capt. Kuchta and about 10 other executives and crew members, including Ms. Fleytas, were gathered onthe bridge, which was not yet threatened by fire. When word reached the bridge that workers were jumping,Ms. Fleytas's supervisor issued a "man overboard" call. The Bankston, now positioned hundreds of feet from the burning rig, picked up the call. Officers on that

vessel had seen what appeared to be shiny objects -- the reflective life vests on rig workers -- tumbling fromthe platform into the water. The Bankston put a small boat into the water and began a rescue operation. Messrs. Vidrine and Harrell, the two highest ranking executives, appeared on the bridge. Mr. Vidrine later told the Coast Guard that a panel on the bridge showed that the drilling crew, all of whom were dead bythen, had already closed the "bag," the thick rubber membrane around a section of the well. But the emergency disconnect, which would sever the drilling pipe and shut down the well, had not beensuccessfully triggered. Some crew members on the bridge said the disconnect needed to be hit, and ahigher-ranking manager said to do so, according to an account given to the Coast Guard. Then another crewmember said the cutoff couldn't be hit without permission from Mr. Harrell, who then gave the OK.  At 9:56 p.m., the button finally was pushed, with no apparent effect, according to an internal BP document. Mr. Young, the chief mate who had left the bridge to survey the fire, told Capt. Kuchta that the fire was"uncontrollable," and that everyone needed to abandon the rig immediately, according to two workers on the

bridge. Under Transocean safety regulations, the decision to evacuate was to be made by Capt. Kuchta and Mr.Harrell. Capt. Kuchta didn't immediately issue the order, even though at least one lifeboat had already pushed away,according to several people on the bridge.  At the Coast Guard hearing Thursday, several crew members said they weren't certain who issued theabandon ship order or whether one was ever given. Capt. Kuchta didn't return calls seeking comment, but inhis testimony said it was obvious to all by that time that the crew should evacuate.  Alarmed at the situation, Ms. Fleytas recalled in the interview, she turned on the public-address system andsaid: "We are abandoning the rig." Capt. Kuchta told everyone who remained on the bridge to head for the lifeboats, according one person whowas there. One boat was long gone. When they reached the boarding area, the second was motoring away, accordingto several witnesses. Ten people were left on the rig, including Mr. Wheeler, the injured toolpusher, who waslying on a gurney. The deck pulsed with heat. The air was thick with smoke, and the surface of the water beneath the rig --covered with oil and gas -- was burning. Crew members attached a 25-foot life raft to a winch, swung it over a railing and inflated it. Mr. Wheeler was lifted in and several others climbed in with him.

 As the raft began descending, Ms. Fleytas jumped in. The remaining people on the rig, including Capt.Kuchta, leapt into the Gulf. Once the life raft reached the ocean, it didn't move, even as fire spread across the water. Some hanging onto its sides thought the heat of the rig was creating a draft sucking the craft back in. Terrified, Ms. Fleytasrolled out of the raft into the oil-drenched water. "All I saw was smoke and fire," she recalled. "I swam away from the rig for my life."

 Minutes later, the rescue boat from the Bankston plucked Ms. Fleytas and several others from the water.The crew of the small boat saw that a line attached to the life raft was still connected to the burning rig. "Cut the line," yelled one Bankston crew member. Another passed over a knife, the raft was cut free, and thelast survivors were towed away from the fire. All told, the Bankston rescued 115, including 16 who wereseriously injured. A Transocean spokesman says that the fact that so many survived "is a testament to theleadership, training, and heroic actions" of crew members. The crew of the Deepwater Horizon watched from the deck of the Bankston as the drilling platform burnedthrough the night.
