A sample of the SERVICE/PARTS PUBLICATIONS ORDER FORM is shown on page 6, with details of how to fill out each area on pages 8 and 9. Additional copies of the order form are available through DDS Distribution Services by ordering publication number 3629. For your convenience a reproducible order form can be found at the end of this catalog. When placing an order, be sure the following information is correct and complete: Missing Information may result in the delay of your order.

Note: Please enter one order by fax, phone or mail to avoid duplication. If confirmation of receipt of order is required, note on original order.

ORDERING INFORMATION

III. SHIPPING:

1. DDS Distribution Services is committed to a once a week shipping schedule. Orders received on Tuesday will be shipped on Thursday of the same week. NOTE: Exception to the “in by Tuesday; out by Thursday” shipping cycle will be made of orders of an

Emergency nature. There will be a $20.00 rush order charge assessment for each request and Freight cost usually increases appreciably.

2. Orders for all publications will be combined into one shipment, thereby reducing Freight cost significantly.

Orders will be shipped to the attention of the PART/SERVICE MANAGERS. Should you want to designate an Individual at your location who will be responsible for the disbursement of the content of the DDS Distribution Services shipment, communicate your request to the EH Representative at DDS Distribution Services.

Each shipment will contain a shipping record. It will detail among other things the contents and the name of the individual who placed the order.

IV. BACKORDERS:

EUCLID-HITACHI will make every effort to keep the inventory at DDS Distribution Services fully stocked so that backorders are kept at a minimum. However, requests for very old publications may require a waiting period since they are not stored in high quantities. Should this occur, you will be notified via the shipping record, which will be inside carton number one of every shipment. Backorders are kept on file and shipped as stock becomes available.

V. TRANSPORTATION:

DDS Distribution Services will choose the most direct and inexpensive transportation means that provides Proof of delivery.

Alternate methods, such as next day air can be requested, however, please keep in mind that air freight Is expensive and all freight charges are paid by you.

VI. INVOICING:

Once a month you will receive an E-H invoice for the total month’s shipment activity.

VII. INQUIRIES:

1. Office hours are from 8 AM to 4:30 PM, Monday thru Friday.

2. Ask for the EUCLID-HITACHI Representative.

3. When checking on the status of an order, reference the dealer code and publication

Number.

4. When reporting a discrepancy on a shipment or on an invoice, please reference the shipping record number.

SERVICE/PARTS PUBLICATIONS ORDER FORM: 1. Invoice To – This box should contain the BILL TO address for each order.

2. Dealer No./Dealer Code – List your Dealer Code on this line for billing purposes.

3. Ship To – This box should contain the address where the order is to be shipped. If it is the same as the Invoice To Address – enter Same.
