2 Trigger Elimination at Two Week Post Enrollment for 2nd Quarter 2012

During the second quarter, 32 children/seniors enrolled in the program received in-

home environmental assessments to aid them in reduction of in-home allergens and

irritants.

The children/seniors also received education on outdoor environmental

allergens/irritants, such as air pollutants, smog, wildfires, weather changes

and pollen.

Program staff provided allergen proof mattress and pillow case covers to aid

in the reduction of asthma triggers.

Two week post enrollment follow-up surveys via phone calls indicated that 32

parents successfully limited their children’s exposure to asthma triggers.

Best Practices

Collaborative activities included participation in health fairs, community presentations,

outreach to senior citizen centers, coalition meetings, and other community events.

Referrals received from community collaboration (2nd quarter) 75

Air Quality and Chronic Disease Education Activities:

Senior Adult Activities: In an effort to spread awareness of the effect of poor air quality on senior adults, 55 years and older, the following activities were accomplished during this reporting period:

3 asthma/air quality presentations were provided at various senior centers and health organizations through Riverside County.

Staff participated in 3 senior health fairs.

Active living senior retirement communities and senior centers were targeted for Asthma Program Services throughout Riverside County. Informational packets were distributed to site Directors/Coordinators.

Media Promotion:

346 quarter sheet flyers were created and distributed for advertising air quality presentations to active living senior populations.

Staff Development: Staff participated in various trainings and webinars to increase their proficiency as asthma educators. Refer to Attachment III for media promotion articles and IV for staff development activities.

Inland Empire Asthma Coalition

The 2rd quarterly Inland Empire Asthma Coalition (IEAC) meeting was held on November 14, 2012 at the Molina Healthcare Regional Office in Colton. Pulmonologists, asthma nurses, lung health educators, school nurses, and other community health representatives were among the professions represented at the meeting. Agencies and organizations in attendance included:

o County of Riverside DOPH Asthma Program o American Lung Association of California o San Bernardino County Unified School District o Molina Healthcare of California o San Bernardino County DOPH CARE Program o San Bernardino County Department of Public Health o Community Hospital of San Bernardino o Riverside Unified School District – Head Start o Kaiser Permanente – Fontana o Arrowhead Regional Medical Center Breathmobile o Loma Linda University Medical Center o Western Riverside Council of Governments o Inland Empire Health Plan

A strategic plan to conduct activities for Asthma Awareness Month (May 2013) is in the planning stage.

The Coalition convenes quarterly with an e-newsletter distributed to members one month prior to each meeting. Meetings are held from 1:30 PM - 3:00 PM at varied locations.
