O-D-A FAILURE INVESTIGATION TRAINING COURSES & INDEPENDENT FAILURE INVESTIGATORS

Our highly acclaimed 3-day Observe-Deduce-Analyse (O-D-A) Failure Investigation training courses are delivered on-site to leading companies across all sectors of industry world-wide. The O-D-A course covers the modes of failure of materials and, through the application of the O-D-A technique, which embraces RootCause Analysis and Forensic Engineering, determines the Root Cause of Failure of mechanical hardware. The course entails over 50% hands-on practical syndicate exercises, involving critical examination and analysis of failed hardware; the delegates then summarise their findings and give short presentations to their course colleagues. Also included is a practical FMECA exercise and, case studies on Human Factors andRepair Faults that have lead to in-service failures. We organise the syndicates so delegates are from differentwork areas, experience and disciplines - design, reliability, stress, production, failure investigation, quality, etc- resulting in additional benefits such as cross-fertilisation of ideas, teamwork and networking opportunities.The O-D-A course is suitable for a minimum of 6 to 8 delegates (2 tutors). However, our experience shows that the ideal delegate number is 9 to 12 (3 tutors) and the maximum is 13 to 16 (4 tutors). Each syndicate is tutored by a highly experienced professional engineer, thereby ensuring each delegate’s full interactive involvement.
