In 15-20% cases the bone ends of fractured bone do not unite and non union/ delayed union occurs.

It is equally common in all types of cases. Whether it is closed reduc�on, open reduc�on with screw plates or nailing. There are variety of causes such as clot forma�on, interposi�on of so� �ssue between ends of bone.

SURGIWEAR brings you HA powder to be injected between and around ends of broken bones.

in all cases of open or closed reduc�on. This will improve the environment around the fracture and increase chances of healing by providing an environment of Calcium & Phosphate ions the building blocks of bone. It will increase ac�vity of Osteoclasts and osteoblasts in the vicinity of fractured bone ends.

Closed injec�on of HA powder can be repeated a�er 7-20 days depending upon X-ray visualisa�on of bone healing ac�vity / callus. If callus is not visible a�er three weeks. Repeat injec�on of HA Powder is recommended.

The quality of SURGIWEAR HA powder is that it will not diffuse around and will remain coalesced as injected for prolonged period.

INTRODUCTIONConventional bone grafting is done with autologous bone graft from iliac crest, which is still the gold standard. However, it involves another operative procedure, longer operating time and more trauma to the patient. The donor site becomes painful after healing.

Surgiwear has developed xenograft in the name of G-Graft. It is natural Hydroxyapatite with natural collagen and with naturally occurring trabecular pattern. It is very useful for bone repair and replenishment. It can be used in weight bearing areas.

One should remember that results of use of G-Graft may be inferior to autologous bone.

DESCRIPTIONG-Graft is made of natural low crystalline Hydroxyapatite with collagen. It is available in form of granules, dowels and blocks. The shape can be changed by using Gigli saw and bone nibblers.

The information, given here, is not exhaustive and is for guidance only. The surgeon is best advised to use the method, which his own practice and discretion dictate to be best for the patient.

INDICATIONSG-Graft is indicated in almost all the places where a bone graft is required. Following are some of the indicative uses specialty wise.

Neurosurgery: Lumber & cervical fusion, vertebral reconstruction, cranial reconstruction and other types of reconstructions.

· Orthopedic Surgery: Lumber & cervical fusion, vertebral reconstruction, cranial reconstruction and other types of reconstructions, Cavity filling, osteotomy reconstructions, non union fracture and filling the gaps in cases of bone loss due to trauma.

· Plastic Surgery: Maxillofacial reconstructions, bony augmentation, hand surgery & cavity filling.

· Dental Surgery: Maxillofacial reconstruction surgery, periodontics, orthodontics and other areas requiring a bone graft.

· Other Specialties: Where a bone graft is required.

CONTRAINDICATIONSPresence of infection whether local or general, prohibits all kind of implantation procedures. Open wounds will not respond well with bone grafting surgeries.

Poor general health and metabolic deficiencies will not produce desired results.

Above mentioned contraindications are general. The surgeon must evaluate the individual patient for the risks involved.

WARNINGS Collapse of G-Graft is a possibility.Do not re sterilize G-Graft.Only blocks with weight bearing line should be used for weight bearing areas.The slot line on the block should be parallel with weight bearing axis of the body.

METHOD OF USEG-Graft implantation is an implantation procedure. It carries a greater risk of infection because of large surface area. It is advisable to take all precautions necessary for an implantation procedure. Some of the points are given below:a. It should be first procedure in the morning.b. There should be minimum number of people inside thetheatre (4persons). No scrub nursec. There should be minimum movement inside OT.d. Procedure should be as fast as possible 20-50 minutes.e. Use disposable mops only ( it is a must)f. Use disposable drapes.g. Use incise drapes on incision sites.h. Only 2-4 procedures in a day.i. The grafting material should be opened only just before implantation. j. Minimum handling of graft material.k. Use skin staplers for skin closure.After opening the site of implantation, obtain good hemostasis of the area. Scrap the walls of the cavity/defect. Remove all soft tissue from the cavity or defect. Harvest some bone and bone marrow from the same site by scraping and shallow drilling in the cavity. Incorporation of patients own bone and bone marrow provides BMP's for bone healing and improves results. Larger cavity size needs larger percentage of patients own bone. Fill gap with G-graft blocks or granules.Cover the gap with soft tissue. Gelatine foam sponge may be used to cover the granules and to prevent spilling of granules. Calcium phosphate cement (G-Bone CP Cement) may also be used to secure the granules and top the cavity.

procedure, as well as patients response, reaction or degree of intolerance to any foreign object implanted into the body.

Some complications specific to G-Graft are following

* Collapse of block: There must be secondry arrangements to prevent damage due to collapse of block

* Fracture of G-Graft

* Lack of integration

The medical literature is full of hazards and complications associated with use of Calcium Hydroxyapatite. Some of the major hazards are infection, extrusion of grafted material, serous discharge, serous collection, hemorrhage, skin erosion and migration.

The non ceramic Hydroxyapatite bone material has two components . The powder is made of tri calcium phosphate and tetra calcium phosphate. The fluid has different sodium and calcium salts. The two parts are mixed and putty like material is obtained. It can be used to fill the defects

After mixing it is usable for 3-5 minutes. Adding some drops of water can prolong usability but it may disturb ultimate strength. The hardening process continue and ultimate hardening may take 12-24 hours.

It can also be mixed with antibiotics for slow release of antibiotic at the site.

After tooth extraction a bony socket or alveolus is left. This socket is filled with blood clot. The clot later gets converted into bone.

However during conversion into bone the shape of alveoulus is not retained. It collapses and resorbed. The cheek side gets absorbed more deshaping the ridge. This collapsed dental ridge is not very suitable for dental implants or denture.

To preserve shape of alveolus, after extraction it must be filled with G-Bone synthetic or other bone graftingproducts from SURGIWEAR. During regeneration process of alveolus it prevents collapse and facilitate fast bone healing.

SURGIWEAR's G-Bone Synthetic SHAG31 is soft like sponge, after wetting. It can be filled into alveoulus without fear of spilling. Being soft it takes the shape of alveoulus.

In below x-ray, one root was filled with SHAG31. After 3 months new bone has formed.

DESCRIPTION

The SURGIWEAR “SYNTHETIC FABRIC PATCH” is designed to repair defects in soft tissues & re-enforcement of soft tissues such as DURAMATER, Peritoneal pleura etc. The patch is made of Poly Propylene (same as PROLENE Mesh). G-Patch-2 is made of Bio absorbable polymer Each pack contains one patch.

G-Patch is available in three sizes and custom sizes as well.

INDICATIONS

G-Patch is indicated for repair of Duramater, repair of peritoneum or places, where ever soft tissue repair is required.

CONTRAINDICATIONS

Presence of all kinds of infection (meningitis, ventriculitis, skin infection, bacteremia, septicemia and peritonitis etc.) whether local or general, prohibits all kind of implantation procedures. It is advisable to avoid implantation in such cases.

G-Patch is not a strong fabric. It should not be used in places where large tensile forces are expected. In such places stitch may not hold and it may give way and torn.

METHOD OF USE

Surgical Procedure

The implantation of G-Patch is a simple procedure. The patch is shaped and cut as per requirement. It is stitched with the tissue around the defect. Special care is required to use it as dura substitute. Water tight stitching is required to avoid leakage of CSF.

It should not be used without reinforcement in repair of peritoneum. The choice of exact procedure depends upon the training of neurosurgeon, customs in that hospital, country. Therefore the surgeon is best advised to use method, which his/her own practice and training dictate to be best for the patient.

G-Patch is supplied in three sizes. Custom sizes are also available. These are available sterile by EO gas in double peel open packing.

WARNINGS

This device has not been tested for compatibility with all drugs and therefore should be used with caution.

Occasionally, fibrous adhesions or fibrosis will bind the patch with the adjacent tissues. Tissues may adhere with it. This adhesion may cause harm. Inflammation and or foreign body reaction may occur in surrounding tissues and organs. Excessive tension may tear the patch.

PRECAUTIONS

Prior to surgery, prospective patients and or their representative should be informed of the possible complications associated with the use of this product.

The patch should be carefully secured to the surrounding tissue with ligatures in such a manner as to avoid cutting or tearing the patch.

Unique features of disposable drapes from SURGIWEAR

1. Almost all special drapes are made of 2 layers. Top layer is of non woven fabric and bottom layer is of plastic sheet. Top layer gives surgeon a comfortable feel and soft draping whereas the bottom layer gives complet protection from fluid penetration. Most of special drapes giveyou level 4 protection as per ANSI/AAMI standards and High performance critical area protectionas per Europen Union standards.

2. The thread used to sew the drapes is coated with water repellant coating. It resists passage of fluid from patient to surgeon's side & vice versa.

Normal absorbent thread absorbing ink and passing to other side.

3. Holes in drapes are cut using laser. Laser makes very clean cut holes without fraying of edges.

"Zero-F" MopZero Fiber Mop

Zero Fibre Mop

SURGIWEAR has brought an ultra new concept for surgeons “Zero-F Mop” for surgeries. Conventional surgical mops are made of multi layers of gauze stitched on sides or similar material. These mops are made of cotton fibre threads. During use cotton fibres (lint) are shed on the wound surface and on the organs touched by the mops. These fibres can lead to multiple types of problems such as nidus for infection, fibre granuloma, foreign body reactions, fever & other related problems. In “Implant surgery” these fibres can increase risk & complications associated with the implant. In Neurosurgery these fibres may cause epilepsy & other problems.

One may ask why these problems were not observed or noticed. There are always some post –operative problems. Some of these problems can be traced to these fibres inside the body. You will find the difference in results after use.

ADVANTAGES

* No loose fibres to shed. Therefore zero linting.

* Zero-F mop is smooth it will not damage surface of organs and tissue.

* Zero-F MOP is radio opaque. During the manufacturing process, a radio opaque thread containing Barium sulphate is added to make it radio-opaque. This quality will help surgeon to identify any lost sponges.

DESCRIPTION

Zero-F mop is the answer to the problems due to fibres. It is made of filament yarn and sides are bonded by a special process. Even the handle is made of filament yarn only. Stitching is also done with filament yarn thread. During use hardly any fibre will be shed, because there is no loose fibre to shed. The mop carries a radio-opaque marker too. So, the mops can be detected on x-ray. Zero-F mop has a very good water absorbency.

UEM SplintUniversal Emergency Splint

Universal Emergency Splint

Surgiwear has developed an Universal Emergency Splint “UEM Splint”.In emergency situations it is to be used to support almost any part of limbs in straight or bent position. It provides enough rigidity for easy transportation with minimum movement of injured limb. It is easy to carry in small pouch of 28 cm length and weighs around 200 gm.

UEM Splint has two main parts A & B. Part A is 28 cm long with three strips and three hook & loop fastener straps. Part B is 18 cm long with three strips and two hook & loop fastener straps. Part A & B are joined with a bendable strip & a gap of 5 cm. Two movable side arms 21 cm long are attached with part B. Straps are made of hook & loop fastener.

To apply in angular position (Bent position / flexed position) first of all bend the splint at the bendable portion. Then move the side arms to attach with other part of the splint. Then apply hook & loop fastener flaps around the limb.

To apply the splint in straight position, make the splint straight. Move the side arm to straight position & attach with the second part of splint. Then apply the straps.

To apply UEM Splint on elbow, first bend the splint at the bendable part. Place the splint behind elbow. Keep small part against arm & large part against forearm. First tighten straps1 & 5. Now attache side arms to back of strap 4 on both sides of elbow as shown in picture. Now apply straps 2 3 & 4.

To apply UEM Splint on knee joint, first bend the splint at the bendable part. Place the splint behind the legkeep small part against thigh and large part against leg. First tighten straps 1 & 5. Now attach side arms to back of strap no. 3 on both sides of knee as shown in picture. Now apply straps 2, 3 & 4.

To apply UEM Splint on ankle joint, first bend the splintat the bendable part. Place the splint behind the leg keep small part against foot and large part against leg. First tighten straps 1 & 5. Now attach side arms to back of strap no. 3 on both sides of knee as shown in picture. Now apply straps 2, 3 & 4.

To apply the splint in straight position on leg, make the splint straight. Move the side arms to straight position & attach with the second part of splint. Then apply the straps.

If required more than one UEM splints can be used incircumferential position. It will strengthen the splintingand will provide greater support.

If required more than one UEM splints can be used to increase the length. The design of splints is such that these will stick together to make it one long splint and will provide longer support.
